4.Pathological laughing with syncope and occipital hypoperfusion as an unusual late effect of pontine infarct
Neurology Asia 2010;15(2):179-183
Occipital lobes are not usually implicated in the current proposed pathways of pathological laughter.
We present a case of occasional pathological laughter associated with pontine infarct. Brain SPECT
revealed hypoperfusion of bilateral occipital lobes in addition to an underlying abnormal regulation
of the cortico-pontine-cerebellar pathway. The cause of bilateral occipital hypoperfusion was thought
to be due to vertebrobasilar stenosis in our patient. The co-existence of bilateral occipital lobe
hypoperfusion in our patient suggest that occipital lobes may also be involved in the generation of
pathological laughter.

Postural orthostatic tachycardia syndrome (POTS) is characterized by orthostatic intolerance, a
presentation of autonomic dysfunction which is frequently observed in the patients with multiple
sclerosis (MS). Nevertheless, to date, there have been a few studies focusing on POTS in MS patients
and the underlying pathomechanism. We present a 28-year-old woman with relapsing-remitting opticspinal
multiple sclerosis who also suffered POTS and discuss the possible pathomechanism of POTS
in MS.

BACKGROUND:Stem cel transplantation has been shown to have many advantages in treatment of diseases. Its application can treat various diseases in many systems such as nervous system, immune system and endocrine system. Its advantage is incomparable. These are al credit to the special abilities, including easy sample colection, strong self proliferative ability, wide differentiation range, repair of injured tissue and immunoloregulation. OBJECTIVE:To explore the clinical effects of stem cel transplantation in treatment of nonunion. METHODS:A computer search was performed in CNKI database for articles concerning stem cel transplantation for nonunion published from 2005 to 2014. The key words were stem cels, nonunion in Chinese. 244 articles were analyzed, and typical literatures were further compared. RESULTS AND CONCLUSION:Stem cels have the abilities of self-renewal and differentiation. Their source is not limited, and the sample colection is convenient. Stem cels have smal injury to the donor, do not have immunogenicity, and have fewer complications. The cost is low. Stem cels are easily isolated and cultured, can proliferate infinitely, and provide a pathway for severe bone injury such as nonunion, bone defects and comminuted fracture. The deep study of stem cels creates a new direction for nonunion treatment, and has achieved certain results in clinical trials.

Objective To investigate the clinical efficacy of warm needling moxibustion in treating irritable bowel syndrome of liver depression and spleen deficiency type and explore the mechanism of its action. Methods Ninety-two patients with irritable bowel syndrome of liver depression and spleen deficiency type were randomly allocated to treatment and control groups, 46 cases each. The same acupoints were selected in the two groups. The treatment group received warm needling moxibustion and the control group, conventional acupuncture. The abdominal overall symptom grading score was observed before and after treatment. The clinical therapeutic effects were compared between the two groups. Results The total efficacy rate was 95.6% in the treatment group and 77.8% in the control group; there was a statistically significant difference between the two groups (P<0.05). In the two groups, there was a statistically significant difference in the abdominal overall symptom grading score at the end of treatment and two months of follow-up compared with before treatment (P<0.05). There was a statistically significant difference in the Hamilton Depression Scale score at the end of treatment and two months of follow-up between the treatment and control groups (P<0.05). In the control group, there was a statistically significant difference in the abdominal overall symptom grading score at two months of follow-up compared with the end of treatment (P<0.05).Conclusion Warm needling moxibustion is an effective way to treat irritable bowel syndrome of liver depression and spleen deficiency type.

Objective To investigate the function of SD rats Schwann cells on the growth and differentiation of adipose-derived stem cells(ADSCs) when they are co-cultured in Transwell culture system. Methods The ADSCs harvested from SD rats were divided into 3 groups. Group A: ADSCs and Schwann cells were cultured in Transwell indirect co-culture system. Group B: ADSCs were induced by β-BME and BHA. Group C: a contrast group. The morphology of ADSCs was observed and immunohistology was pedormed. Results ADSCs of group A and group B were partly differentiated to the cells which had long neurite, and had a positive result in the NF staining, but negative in the GFAP staining. Group A shows no great difference in neurite length compared with group B(P < 0.05), and no great difference in cell quantity compared with group C (P < 0.05). Conclusion Schwann cells of SD rat have a growth supportive and neuronal differentiation inducing effect on ADSCs.

Objective To transduct with human telomerase reverse transcriptase(hTERT) to Schwann cells via electransfection technique to prolong the life span and enhance the proliferative capacity of Schwann cells. Methods The hTERT RNA was derived from esophagus cancer tissue and pcDNA3.1-hTERT vector was built. With electransfection technique, we tmnsfected vector into ADSCs.The PCR,TRAP-PCR, and PI-annexin V were tested to prove the expression of hTERT in ADSCs.Results After transduction the hTERT RNA in Schwann ceils, TRAP-PCR test, PI-annexin V dying and S100 dying was positive. Conclusion Telomerase catalytic subunit(hTERT) is transduct into Schwann cells, and it can to enhance the prolifertative capacity.
