1.Clinical significance of increased plasma IGF-1 in type 2 diabetic patients with malignant tumor

Objective To test plasma insulin‐like growth factor 1(IGF‐1) level in type 2 diabetic patients with malignant tumor , and make comparison with diabetic patients and normal patients .To discuss the significance of detection of plasma IGF‐1 level in type 2 diabetes in screening early malignant tumor .Methods Plasma IGF‐1 level were determined in type 2 diabetes among malig‐nant tumor group ,type 2 diabetes ,and normal control group ,and statistical comparison was made between the three groups .Results the plasma levels of IGF‐1 of type 2 diabetes mellitus ,type 2 diabetes mellitus with malignant tumor were significantly lower than normal group(P<0 .05) .And the plasma level of IGF‐1 in type 2 diabetes mellitus with malignant tumor group was obviously high‐er than that of type 2 diabetic group(P<0 .05) .Plasma IGF‐1 ,FBG ,HbA1c and 2 HBG level are risk factors for tumor .The level of FIns is a protective factor for tumor .Conclusion Type 2 diabetes is closely related to the malignant tumor ,and the plasma levels of IGF‐1 is a risk factor for tumor .
2.Relationship of nuclear transcription factor-?B/interleukin-8 with angiogenesis and prognosis of gastric cancer

Objective To disclose the implication of nuclear transcription factor(NF)-?B and interleukin(IL-8) in the angiogenesis and prognosis of gastric cancer. Methods The expressions of NF-?B, IL-8 and microvessel density (MVD) in 41 samples of gastric cancer were detected by immunohistochemistry. Results NF-?B and IL-8 were positive in 28 samples (68.3%) and 12 samples (29.3%) respectively. NF-?B, but not IL-8, was related to MVD significantly( P =0.002), and there was significant relationship between the expressions of NF-?B and IL-8 and TNM staging( P

In order to evaluate the applied value of soluble intracellular adhesion molecule-1 (sICAM-1) in acute rejection (AR) following liver transplantation, the expression of sICAM-1 protein was sequentially detected by using ELISA in serum and bile of 43 patients receiving liver transplantation. In AR group, the expression levels of sICAM-1 protein were increased 3 days before and immediately on the establishment of AR diagnosis, and there was significant difference in the expression of bile between AR group and control group (P<0.01). After reversion of AR with hormone intensive therapy, there was significant difference in the sICAM-1 protein expression of serum and bile between AR group and control group. It was concluded that the sequential detection of sICAM-1 protein level in serum and bile was a reliable and noninvasive method for the early diagnosis of AR after liver transplantation and was valuable to observe the curative effects of anti-AR therapy.

5.Application of continuous intracranial pressure monitoring in patients with severe traumatic craniocerebral injury

Objective To discuss the meaning of continuous intracranial pressure (ICP) monitoring in patients with severe traumatic craniocerebral injury. Methods One hundred and twenty four patients with severe craniocerebral injury treated from August 2004 to February 2011 in our hospital, were enrolled and divided randomly into ICP monitoring group (n = 62) and routine treatment group (n = 62). The patients of ICP monitoring group had adjusted treatment plan according to the changes of ICP at any time, whereas the patients in routine treatment group underwent routine neurosurgical treatment according to the doctors' experience. Results There were 7 cases of acute kidney function failure,and 11 cases of electrolyte disturbances in the ICP group.There were 15 cases of acute kidney function failure, and 25 cases of electrolyte disturbances in the routine group. The complication rate in the ICP group was lower than that in the routine group (x2 =3. 54 and 7.67 for acute kidney function failure and electrolyte disturbances respectively, Ps <0. 01). The days of mannite using were (6±2)dand (15 ±3)d, respectively; the dosage of mannite using were (749 ± 125) g and (1545 ±250) g,respectively. The good recovery and slight disability were 28 and 16 cases in the ICP group, and 13 and 9 cases in the routine group,respectively. The severe disability,vegetative state and death were 9,4 and 8 cases in the ICP group,and 17,7 and 13 cases in the routine group. The days and dosage of mannite using in the ICP group were much less than those in the routine group (t = 8. 32 and 7.41, Ps < 0. 01). The proportion of good recovery and slight disability in the ICP group were higher than those in the routine group(x2 =5. 07 and 3. 55,Ps <0.01). However, the proportion of severe disability, vegetative state and death in the ICP group were lower than those in the routine group (x2 =0.84,0.89 and 1.43, Ps < 0. 01) . Conclusion Continuous ICP monitoring in severe craniocerebral injury shows benefits in directing treatment plan adjustment, reducing complications and improving the prognosis.
6.Study on microwave-assisted extraction technique of flavonoid from Ophiopogon japonicus

Objective:To study microwave-assisted extraction technique of flavonoids from Ophiopogon japonicus. Methods:The effect of the parameters,i.e.ratio of solid to liquid,microwave treatment time,microwave power and volume ratio of ethanol on extraction amount of flavonoid were analyzed by single factor experiment.Results:Optimum processing data were determined as follows:90.0% ethanol as extracting solvent,stock ratio 1:14(g/ml) ,microwave extracting for 4 minutes at the power of MED. The extraction rate was 39.7% higher than that of non-microwave's.Conclusion:Extraction of flavonoids by microwave from Ophiopogon japonicus is an eficient method.
7.Functional study of progesterone receptor isoforms in endometrial cancer cell lines

Objective:To study the functional differences between the two progestrone receptor isoforms(PR-A and PR-B) in human endometrial cancer,using antisense oligodeoxynucleotide(AS-ODN) to downregulate isoform B of progestrone receptor in endometrial carcinoma cell lines, After transfection of the oligodeoxynucleotide, several kinds of hormones were added in the cells to observe the different response,whereh to study the functional differences between the two isoforms. Methods: The well-differentiated endometrial cancer cell line Ishikawa and moderate-differentiated endometrial cancer cell line Hec-1B were cultrued in vitro. The cells were transfected with antisense, sense, and scramble-ODN. After 48 hours, the expressions of two progesterone receptor isoforms were detected by Western blot using specific antibody. Then the cells were planted in 96-well plates, transfected with antisense, sense, scramble-ODN and added in several hormones to search for the response in distinct hormones and oligodeoxynucleotides. Results: After transfecting antisense-ODN, two cell lines were down-regulated in progesterone receptor isoform B,but progesterone receptor isoform A was not down-regulated,and the progesterone receptor isoform B of cells transfected with sense and scramble-ODN was not changed. When stimulated by 17?-estradiol(E2)for 72 hours,the growth of Ishikawa cells was significantly higher than that of the control, Hec-1B cells only grew higher than control,but it was to significant in statistics.R5020 inhibited Ishikawa cells significantly after stimulating for 72 hours.There was the same effect in Hec-1B cells after stimulating for 96 hours.On the bases of E2 and R5020, we added mifepristone(RU486) .The cells developed after 96 hours in Ishikawa cells and developed after 48 hours in Hec-1B cells. When PR-B was down-regulated,the stimulating effect of E2 was enhanced, but the inhibitory effect of R5020 was de-creased, RU486 antagonized R5020 weaklier than the control. Conclusion: AS-ODN directed against the human PR-B can inhibit the expression of PR-B effectively,through which the PR-A expresses predominantly. E2 can cause endometrial carcinoma cell growth, PR-B is associated with the stimulating effect of E2 in endometrial carcinoma cells. Progestin (R5020) inhibits the hyperplasia induced by E2,PR-B is involved in the inhibitory effect of R5020. RU486 antagonizes the effect of R5020,inhibiting cell growth, PR-B is involved in the antagonizing effect of RU486.
8.Effects of estrogen, progestin and mifepristone on endometrial carcinoma cell lines

Objective: To study the effects of estrogen, progestogen and mifepristone on endometrial carcinoma cell lines in vitro. Methods: The well-differentiated endometrial cancer cell line Ishikawa and moderate-differentiated endometrial cancer cell line Hec-1B were cultured in vitro. The cells were divided into four groups:control, estrogen , estrogen and progestogen , estrogen and progestogen and mifepristone, then we implanted cells in 96-well plates to search the response in distinct hormones. Results: When stimulated by estrogen for 72 hours, the growth of Ishikawa cells was significantly higher than the control, Hec-1B cells only grew higher than the control, but it was no significance in statistics. Progestogen inhibited Ishikawa cells significantly after being stimulating for 72 hours, there was the same effect on Hec-1B cells after being stimulating for 96 hours. On the base of estrogen and progestogen, we added mifepristone,cells developed after 96 hours in Ishikawa cells and cells developed after 48 hours in Hec-1B cells. Conclusion: Estrogen can cause endometrial carcinoma cell growth; progestogen inhibits the hyperplasia induced by estrogen; mifepristone antagonizes the effect of progestogen on cell growth.
9.Effects of astragalus polysaccharides on metabolism of collagen in diabetic cardiomyopathy

Objective To investigate the effects of astragalus polysaccharides(APS)on the metabolism of collagen in hamsters with diabetes mellitus(DM) and explore the possible mechanism of APS on diabetic cardiomyopathy. Methods The levels of insulin,C-peptide,myocardial enzymes,glycosylated serum protein(GSP),plasma and myocardial angiotensin(AngⅡ) were tested in APS group(n=15) and DM control group(n=15).Immunohistochemistry was used to measure the expression levels of typeⅠand typeⅢ collagen,activities of matrix metalloproteinases(MMPs) were examined by using zymography,and TNF-? and TGF-? mRNA were detected by RT-PCR. Results Compared with DM control group,levels of GSP,myocardial enzymes and myocardial AngⅡ were much lower in APS group(P0.05).The level of expression of collagen Ⅰ,the ratio of collagen Ⅰ/collagen Ⅲ,and the activity of pro-MMP-2 and MMP-2 in APS group were significantly lower than those in the DM group(P
10.The effects of interleukin-28B gene polymorphisms on antiviral treatment response in patients with chronic hepatitis C

Objective To investigate the relationship between interleukin (IL)-28B gene polymorphisms (rs12979860 and rs8099917) and treatment response in patients with chronic hepatitis C in China.Methods Taqman probes single nucleotide polymorphism genotyping methods were used to detect the genotypes of rs12979860 (C/T) and rs8099917 (T/G) located at IL-28B gene in 105 included patients.The patients were treated with standard doses of pegylated interferon plus ribavirin and were followed up regularly for therapeutic response and adverse reaction.The relationship between IL-28B gene polymorphism and antiviral treatment response of patients were analyzed.Categorical data were analyzed using Pearson chi-square test or Fisher exact test.Results Totally 105 cases were included in our study and 2 cases lost to follow-up because of moving away.Eight-one cases (78.6%) of the remaining 103 patients were CC/TT genotype (CC/TT group) at rs12979860 and rs8099917, 19 cases (18.4%) were CT/TG (CT/TG group) and 3 cases (2.9%)were TT/TG (TT/TG group).No other genotypes were detected and linkage disequilibrium was discovered at the two polymorphism loci (r2=0.11).After 4 weeks of treatment, 35 cases (43.2%) in CC/TT group, 3 cases (15.8%) in CT/TG group and non in TT/TG group achieved rapid virological response (RVR).There were statistically significant differences among three groups (P=0.033).After 12 weeks of treatment, 45 cases (55.6%) in CC/TT group, 6 cases (31.6%) in CT/TG group and none in TT/TG group achieved early virological response (EVR).There were statistically significant differences among three groups (P=0.025).At the end of the treatment, 68 cases (83.9%) in CC/TT group, 10 cases (52.6%) in CT/TG group and only 1 case (33.3%) in TT/TG group achieved end-of-treatment response (ETR).There were significant statistical differences among the three groups (P=0.003).After 24 weeks of follow-up, 62 cases (76.5%) in CC/TT group, 9 cases (47.4%) in CT/TG group and 1 case (33.3%) in TT/TG group achieved sustained virological response (SVR).There were statistically significant differences among the three groups (P=0.014).One hundred and one cases in CC/TT group developed adverse events, among them 19 cases needed clinical treatment.There were 43 cases in CT/TG group developed adverse events and 9 cases needed treatment.Seven cases in TT/TG group developed adverse events and only 1 case needed treatment.There were no statistically significant difference among three groups (χ2=0.139,P>0.05).Conclusions The genotype of rs12979860 (C/T) and rs8099917 (T/G) at IL-28B gene could affect the treatment response in patients with chronic hepatitis C.RVR and SVR are higher in patients with genotype CC/TT, which might help to guide HCV treatment.
