1.The remote effect of D ring moiety on selectivity between 4-dimethylation and 3-O-methylation on steroidal compounds

Objective To introduce dimethyl on 4-position of 4-ene-3-one steroids. Methods Seven 4-ene-3-one steroids were treated with methyl iodide (CH3I) and potassium tert-butoxide (t-BuOK) in tert-butanol (t-BuOH). Results Four substrates were smoothly converted to 4,4-dimethyl products, while other three substrates afforded 3-O-methylation products with high yields. Conclusions The different reaction results revealed a high remote effect of D ring moiety on the selectivity between 3-O-methylation and 4-dimethylation.
4.An Exploration into Aid Model and Mechanism in Connotational Development of Newly-built Undergraduate College

Objective]Not only to promote the rational allocation of resources between government, society and universities,but also to provide theoretical basis and practical path for connotational development of newly-built undergraduate college.[Methods]Focusing on“ the development situation and the demand for the connotational development of the newly-built undergraduate college”and“ the aid possibility”,five representatives of newly-built undergraduate colleges and seven aid-donors were interviewed and inquired by Likert scale .[Results]The average index of newly-built undergraduate colleges on“ the development situation” and “ the demand for the connotational development” are respectively 2.93 and 4.03,showing a good situation and strong demand for external assistance.Meanwhile, the aid-donors also show a high will on the aid possibility” ,the average index is 4.06.[Conclusion]Based on analysis of“ the development situation and the demand for the connotational development of the newly-built undergraduate college, andthe aid possibility”,our research puts forward aid model and mechanism in connotational development of newly-built undergraduate college in order to make full use of external assistance, including government,society and universities.
5.The safety factor analysis on ventilator application based on quality management tools

Objective:Along with general hospital assessment opportunity, we explore to optimize ventilator management processes to reduce security risks by using the advanced quality management tools. Methods:Based on the ventilator maintenance statistical data in year 2012, we applied Pareto chart, Pareto curves and fishbone diagrams etc. quality management tools, and conducted PDCA cycle analysis on safety factors of ventilator clinical application. At the end, we summarized the results and provide improvements recommendations. Results:The cumulative proportions fell from 23.6%to 80%range, the human operator, compressor inlet;pipeline leaks and consumables management are a major are major factors. Through fishbone diagram analysis, we categorized them into 4 aspects:personnel, operations, equipment and supplies management. We optimized existing systems and processes from three areas:the device management, supplies management and training. By implementing these improvements plans, the security risk caused by supplies management was basically solved, the effect is very obvious. Conclusion:By using quality management tools, the safety problems during clinical application of ventilator can be reflected directly and clearly, and protect ventilator safe and stability to next level, so that it helps medical equipment management be more scientific, effective and normative.

7.Ketogenic diet therapy in the epilepsy of infancy with migrating focal seizures associated with TBC1D24 gene mutation: two case reports and literature review

Objective:To investigate the efficacy and safety of ketogenic diet (KD) therapy in the epilepsy of infancy with migrating focal seizures (EIMFS) associated with TBC1D24 gene mutation.Methods:Clinical data of two children with TBC1D24 gene-related EIMFS were collected retrospectively, who were admitted to Department of Pediatrics, Fujian Medical University Union Hospital from 2019 to 2020. Their clinical characteristics and the efficacy and safety of KD therapy were analyzed, and literature review was conducted.Results:Seizures were onset before six months old in the two children with TBC1D24 gene-related EIMFS. Multifocal myoclonic seizures were manifested and happened frequently, lasting for more than 30 minutes sometimes. Developmental retardation was obvious in the two children. A small amount of focal sharp, spike, sharp-slow complex, and spike-slow complex waves were showed in the interictal electroencephalography (EEG). TBC1D24 gene mutations were found in the two children, one with a compound heterozygous mutation (c.1025C>T, p.S342L; c.229_c.240delATCGTGGGCAAG,p.I77_K80del), and the other with a homozygous mutation [c.119G>A,p.R40H(Arg40His)]. Both of those were potentially pathogenic. A variety of anti-epileptic drugs showed poor outcome for the two children. The epilepsy was drug-refractory one. After four to 17 months of KD therapy, the epilepsy in the two children was controlled effectively. There was not obvious adverse reactions. Among six children with TBC1D24 gene-related EIMFS in the literature review, four cases were effective or partially effective for KD therapy, one was discontinued due to insufficient ketogenic ratio, and one was discontinued without effect. There were no obvious adverse reactions in the six children.Conclusions:TBC1D24 gene-related EIMFS is mostly drug-refractory epilepsy. Early KD therapy may help to control seizures.
8.Effect of liraglutide on catch-up fat in high-fat diet-induced catch-up growth rat model

Objective To investigate whether liraglutide treatment can prevent catch-up fat in high-fat dietinduced catch-up growth rats,and to discuss the possible underlying mechanisms.Methods Sixty-four SpragueDawley rats were stratified into 4 groups:normal control group,catch-up growth (CUG) group,catch-up growth with liraglutide treatment (CUGL) group,and catch-up growth with liraglutide plus exendin (9-39) treatment (CUGLE) group.All the catch-up growth animals underwent a4 week high-fat diet re-feeding phase after a4 week food restriction phase.Body weight and food intake were measured every day.At the end of food restriction and re-feeding,body composition was measured via Dual energy X-ray absorptiometry.After the rats were sacrificed,body fat distribution and plasma lipid levels were determined.Results By the end of catch-up growth,body weight,energy intake,Lee' s index,fat body mass,visceral adipose tissue/body weight,subcutaneous adipose tissue/body weight,triglyceride,free fatty acid,low density lipoprotein-cholesterol levels were lower in CUGL rats compared with CUG rats (all P<0.01).There were no significant differences in energy intake,body composition,fat body mass,and blood lipid levels between CUGLE rats and CUG rats,while the weight of CUGLE rats was significantly lower than CUG ones.Conclusions Liraglutide infusion protects rats with high-fat diet induced catch-up growth from catch-up fat mainly via a glucagon-like peptide-1 receptor dependent manner.
9.Inhibitory Effect of Polypeptide P161 Combined with Cisplatin on Proliferation of Multiple Cancer Cells

Objective In vitro detection of anti-proliferative effects of P161 combined with cisplatin ( DDP) on multiple cancer cells. Methods Growth inhibition rates of HepG2, HT29, IE8, Panc-1 and MA-782 treated by different concentrations of DDP,P161 and P161 combined with DDP were determined by MTT assay. Results DDP and P161 dose-dependently inhibited proliferation of multiple tumor cells. A synergistic effect was found in DDP combined with P161 and there was a significant difference in the effect between DDP combined with P161 and DDP alone (P<0. 05). DDP dose could be decreased to reach the same inhibitory effect. In the same concentration gradient of DDP combined with P161,the inhibition rate of Panc-1 was low and that of MA-782 was high. Conclusion P161 can increase the sensitivity of tumor cells to DDP. The combination of P161 and DDP can reduce the effective therapeutic concentration of DDP.
10.Anti-cicatricial effect of tetrandrine drug delivery system in glaucoma filtration surgery in rabbit

Background Scarring of the filtering bleb is a main cause of filtering surgical failure in glaucoma.It has been reposed that tetrandrine could suppress the proliferation of cultured human fibroblast of Tenons capsule in vitro and thus has the potential effect to prevent scarring after the filtering surgery. Objective Present study was to investigate the anti-cicatricial effect of tetrandrine drug delivery system(Tet DDS)during filtration surgery. Methods Filtration surgery was performed in bilateral eyes of 18 New Zealand white rabbits.The Tet DDS with 0.3 mg Tet,0.2 mg Tet or free-Tet were implanted subcunjunctially during the surgery.The filtering blebs were scored in 1 day,4,7,10,14 days after referring to the corneal thickness and bleb range under the slit-lamp biomicroscopy.The morphology of filtering bleb was assessed by in vivo confocal microscopy in 7 and 14 days after operation.The filtering bleb specimen was prepared in 7 and 14 days for the histopathological examination. Results The filtering bleb scores in Tet DDS implantation groups were significantly higher than those in free-Tet DDS group from 4 days through 14 days after trabeculectomy(P<0.01),and the scores showed a considerably increase in 0.3 mg Tet DDS group compared with 0.2 mg Tet DDS group from 7 days through 14 days after trabeculectomy(P<0.05).The filtering blebs of Tet DDS implantation groups were found with distinct subepithelial cystic spaces under the light microscopy and in vivo confocal microscopy on the 7th day and 14th day after surgery.Compared with free-Tet DDS group,the numbers of subepithelial mierocysts were much more(P<0.01)and the area of microcysts was larger(P<0.01)in Tet DDS group.The filtering tissue presented with more subepithelial microcysts and larger microcysts range in 0.3 mg Tet DDS group than 0.2 mg Tet DDS group in 7 and 14 days after operation(P<0.05).The inflammatory cell infiltration wag milder in 0.3 mg Tet DDS group in comparison with 0.2 mg Tet DDS group and free-Ted DDS group.Conclusion Tet DDS has strong inhibitory effects on inflammatory cells activity and fibroblagt activity the early stage after filtering surgery and therefore improve the surgery success rate.
