1.The alterations of chemokines related to dendritic cells in rats with hepatic carcinoma treated by radiofrequency ablation

Objective To investigate the changes of chemokines related to dendritic cells in liver and spleen in rats with hepatic carcinoma treated by radiofrequency ablation (RFA),and to explore the mechanism of anti-tumor responses to RFA.Methods Forty healthy SD rats with established animal model of hepatic carcinoma were randomly divided into control group (n=10),RFA 7d group (n=16) and RFA 14d group (n=14).The rats of control group were killed without treatment.The other rats were killed in 7d and 14d after RFA treatment respectively.Spleen and liver tissue around the ablation area or around the tumor were taken out.The expressions of macrophage inflammatory protein (MIP)-1a in liver tissue and MIP-3β in spleen were analyzed by ELISA.Results The expression of MIP-1a in liver tissue was (232.92±54.5B)ng/L in control group,which enhanced to (499.38±15.14)ng/L and (495.90±9.94)ng/L in RFA 7d and 14d groups respectively.There were significant differences between control and RFA 7d group,control and RFA 14d group(P<0.05).The expression of MIP-3βin spleen was (70.08±2.67) ng/L in control group,which enhanced to (151.57±48.48)ng/L and (154.57±18.25)ng/L in RFA 7 d and 14 d groups respectively.There were significant differences between control and RFA 7 d group,control and RFA 14 d group (P<0.05).Conclusions The expressions of MIP-1a in liver tissue and MIP-3β in spleen increase significantly after RFA.These changes will promote recruitment and migration of dendritic cells and may contribute to the anti-tumor responses after RFA.
2.Influence of radiofrequency ablation on dendritic cells in rats with liver carcinoma

Objective To investigate the change of dendritic cells (DCs) in rats with hepatic carcinoma treated by radiofrequency ablation (RFA),and to explore the mechanisms of anti-tumor immune response to RFA. Methods Forty healthy SD rats with established animal model of hepatic carcinoma were randomly divided into control group (n = 10) ,RFA 7 d group (n = 16) and RFA 14 d group (n = 14). The rats of control group were killed without treatment. The other rats were killed in 7 d and 14 d after RFA treatment respectively. Peripheral blood, liver tissue around the ablation area and spleen were taken out. The OX62,OX6,CD86 of DCs were analyzed with flowcytometry. Results ①OX62 cells accounted for (0.45 ± 0.19)% of mononuelear cells in peripheral blood in control group. The account of OX62 cells increased to (0.78 ± 0.30)% and (1.53 % 0.80)% in RFA 7 d and 14 d groups respectively. There were significant differences between control and RFA 7 d group, control and RFA 14 d group (P<0.05). ②OX62 cells accounted for (18.91 ± 4.58)% of mononuclear cells in liver tissue around the tumor in control group. The account of OX62 cells increased to (24.49 ± 4.59)% in RFA 7 d group (P<0.05). The expression of OX6 on DCs in liver tissue was (15.29 ± 4.59)% and increased to (34.2 ± 11.62)% and (39.18 ± 9.14)% in RFA 7 d and RFA 14 d group respectively (P<0.05). ③OX62 cells accounted for (11.69 ± 4.39)% of mononuclear cells in spleen in control group which increased to (15.10±2.37)% in RFA 14 d group (P<0.05). Conclusions The precursor DCs in peripheral blood and DCs in liver and spleen increased significantly after RFA. The expressions of OX6 on DCs in liver and spleen increased after RFA. RFA can promote the differentiation and maturation of DC. The increased function of antigen presenting may contribute to the anti-tumor responses after RFA.
4.Application of satellite ganglion block via posterior edge of sternocleidomastoid approach

Objective To observe the outcomes of satellite ganglion block(SGB)via posterior edge of stcmoclcidomastoid approach.Methods SGB was performed in 2 400 patients,who wererandomly divided into two groups with l 200 cases each.The puncture of SGB in group SPA was via posterior edge of sternocleidomastoid approach and that in group TPA via traditional paratracheal approac}L The same local anesthetics was used in two groups.The success rate(Horner'S syndrome appearance)and complications were compared.Results The success rate was higher in group SPA than that in group TPA(97% VS.92%)(P<0.05).Repeated puncture was needed in 16 cases ingroup SPA,which were less than 98 cases in group TPA(P<0.05).Laryngeal nerve block wasoccurred in 24 cases in group SPA.which were less than 37 cases in group TPA(P<0.05).Conclusion Compared to the traditional paratracheal approach,SGB via posterior edge ofsternocleidomastoid approach has the advantages of clear positioning,easy puncture,higher success rate and less complications.
5.Ultrasonography on semi-quatitative diagnosis of liver fibrosis and cirrhosis: observation of surface configuration and ligaments of liver

Objective To observe the degree of liver su rface abnormality using high frequency probes and additional methods and to investigate the diagnostic value of semi-quatitative grading in liver fibrosis and cirrhosis. Methods One hundred and twenty-seven cases of cirrhosis (including 83 cases of hepatocelular carcinoma) confirmed by operation and pathology were enrolled in the group while 56 cases of non-cirrhotic patients with no clinical signs and negative surgical gross findings (operation for other kinds of tumors) served as the control. Five additional methods such as high frequency probes, harmonic technique, local zooming out, regulating focus and decreasing gain were used to observe the configuration and thickness of liver surface. Five-grade classification of liver surface was adopted as follows: thin and regular-grade 0; slightly thickened with increased echogenicity-grade Ⅰ; thickened like fine ripples-grade Ⅱ; significantly thickened with segmental incontinuence-grade Ⅲ; significantly thickened like waves-grade Ⅳ. Observation on configuration of the round ligament: thin, regular with the thickness ≤2 mm was considered as normal whereas thickened, irregular with increased echogenicity as abnormal. Results Of the 127 cases in the study group, 11 cases were grade 0, and grade Ⅰ 21 cases, grade Ⅱ 27 cases, grade Ⅲ 32 cases, grade Ⅳ 36 cases. In the control group there were 48 cases of grade 0 and 8 cases of grade Ⅰ-Ⅱ. The sensitivity, specificity and accuracy of liver surface detection with this method were 91.3-%, 85.7-%, 89.6-%, respectively. 75% (9/12 cases) patients with liver fibrosis and 95.5%- patients ( 64/67- cases) with cirrhosis were found with abnormality of hepatic ligaments in the study group. While only 4 cases ( 8.9-%) with abnormality appeared in the control group.Conclusions Grading of changes in liver surface might sensitively reflect their pathological changes. In addition, it would be a reliable method in the diagnosis of liver fibrosis and early stage cirrhosis as well as a semi-quatitative diagnostic tool for advancement of diffuse liver diseases.
6.Evaluation on invasion depth of distal esophageal carcinoma with transabdominal ultrasound

Objective To summarize ultrasonographic manifestations of distal esophageal carcinoma compared with surgical and pathological findings,and investigate the diagnostic role of transabdominal ultrasound in evaluation of invasion depth of distal esophageal carcinoma. Methods Criteria on invasion depth of distal esophageal carcinoma by transabdominal ultrasound were as follows: intramural hypoechoic tumor with continuous smooth hyperechoic line of adventitia and free movement of esophagus with breath were considered as S1;incontinuous or interrupted hyperechoic line of adventitia and relatively poor movement of esophagus with breath were considered as S2;unclear boundaries of tumor with adjacent structures and poor or no relative movement with breath,S3. S1,S2,S3 referred to those without or with adventitia invasion or invasion to adjacent structures,respectively. Eighty-nine esophageal carcinoma and esophageal-cardiac junction carcinoma were enrolled in our study and retrospectively studied compared with surgical and pathological findings. Results For tumors of S1,S2,S3,the thickness of the esophageal wall were 0.4 - 4.3 cm, 0.5 - 2.1 cm, 0.9 - 3.2 cm,respectively. 7.Gene diagnosis in 3 family members of Duchenne muscular dystrophy

Objective To perform gene diagnosis of Duchenne muscular dystrophy (DMD) in 3 family members who were negative for DMD gene detected by multiplex PCR and to provide genetic counseling for their family members accordingly .Methods The clinical data and genomic DNA of patients and their family members were collected ,DMD gene mutation were detected by multiplex ligation dependent probe amplification (MLPA) or the 2nd generation of high‐throughput sequencing .Results In the first family ,3 male patients were detected deletion of Exon 7 and 2 female were heterozygous carriers .In the second family ,it was found in the proband that point mutation of c .3127C> T in the Exon 23 of chrX‐32486626 and c .3127C> T heterozygous mutations was confirmed in his mother ,the mother was pregnant with a girl .In the third family ,point mutation of c .2411G>A was detected in the Exon 20 of chrX‐32509581 in the proband and his mother had c .2411G> A heterozygous mutation .Conclusion MLPA or combining with the 2nd generation of high‐throughput sequencing can offer effective gene diagnosis for the patients of DMD and their family members ,and provide the basis for genetic counseling and prenatal diagnosis .

9.Experimental study on the protective effect of anisodamine on flaps with ischemia reperfusion injury.

OBJECTIVETo observe the effect of anisodamine (Ani) injection on the survival rate and histologic result of flaps with ischemia-reperfusion injury, so as to demonstrate the protective effect of Ani on the flap survival.
METHODSA total of 48 healthy male Wistar rats were randomly divided into model control, normal saline(NS) and anisodamine groups, with 16 rats in each group. An 3 cm x 6 cm axial flap was formed at the right lower abdomen with abdominal superficial blood vessel as the pedicle. 0.5 cm x 0.5 cm skin tissue was taken from the middle part of flaps in each group, immediately after operation, 12, 18, 24 h after operation. The superoxide dismutase (SOD), nitric oxide (NO), nuclear factor-kappaB contents in the specimens were detected. The histologic study was also performed. The flap survival rate was recorded 7 days after operation.
RESULTSFlap survival rate was (78.6 +/- 7.3) % in Anisodamine group. 12, 18, 24 h after reperfusion injury, the SOD was (103.3 +/- 3.9), (82.6 +/- 3.8), (67.5 +/- 4.6) U/mg; the NO was (5.33 +/- 2.05), (4.75 +/- 1.68), (4.15 +/-1.59) nmol/mg; the NF-kappaB was 0.211 +/- 0.039, 0.313 +/- 0.033, 0.096 +/- 0.028. The contents of SOD, NO and NF-kappaB had the statistical difference of at different time. The skin pathological changes in Anisodamine group was obviously better than those in NS group. Flap survival rate in Anisodamine group was significantly higher than that in NS group.
CONCLUSIONSIn the flap with ischemia-reperfusion injury, Anisodamine can reduce the damage of free radical, increase the blood flow, reduce the production of NF-KB, decrease inflammatory reaction. So Anisodamine can increase the survival rate of flaps with ischemia reperfusion injury.

10.Experimental study of the change of early regional myocardial function with diabetic using omni-directional Mmode echocardiography

ObjectiveTo evaluate the role of detecting early change of left ventricular regional myocardial function in the rabbit with type Ⅰ diabetes using omni-directional M-mode echocardiography (OME).MethodsEighty New Zealand rabbits were selected in this study..40 health rabbits were included in the control group,the other 40 with type Ⅰ diabetes were included in the experiment group.The OME was used to measure the peak time of myocardial motion during systole (Ts) and diastole (Td),myocardial velocity gradient during systole (MVGs) and diastole (MVGd) on the left ventricular short axis.These parameters were measured during the first,second,fourth,sixth and eighth week.ResultsOME was able to detect significant changes of Td during the second week,Ts and MVGd during the fourth week,and MVGs during sixth week after modeling ( P <0.05 ).ConclusionsOME is an important method to detect early changes of regional myocardial function in the patients with diabetes.
