Diabetes is seriously hazards to human health and its pathogeneses are not clear. Recent studies show that the imbalance of intestinal flora and the development of diabetes are closely related. Appropriate bacteria can improve blood sugar disorder. Treating diabetes from the theory of Pi-Wei is effective. Regulating intestinal flora has become a new pathway for treating diabetes from the theory of Pi-Wei. On the basis of intestinal flora, authors discussed the treatment of diabetes from Pi and Wei.

OBJECTIVETo study clinical value of shear wave elastography (SWE) in the evaluation of neck-shoulder myofascial pain syndrome.
METHODSFrom December 2013 to July 2014,30 patients diagnosed as neck-shoulder myofascial pain syndrome were in the treatment group, including 17 males and 13 females, with an average age of (44 ± 3) years old. Thirty healthy people were in the control group, including 22 males and 8 females, with a mean age of (37 ± 5) years old. The patients in the treatment group were treated with manipulation, once every other day, total 7 times. The SWE was used to detect tension part of trapezius muscle of patients in the treatment group before and after treatment, as well as to detect muscle belly at the descending part of trapezius muscle in the control group. The tissue elasticity and Yang's modulus value were recorded and compared.
RESULTSThe tissue elasticity chart of patients in the treatment group before treatment was mainly greenish blue with the score of 3.70 ± 1.53, and the Yang's modulus was (43.4 ± 15.6) kPa. The tissue elasticity figure after treatment was mainly blue with the score of 2.40 ± 0.87, and the Yang's modulus was (29.0 ± 5.9) kPa. Whereas in the control group, the tissue elasticity figure was mainly blue with the score of 1.60 ± 0.72, and the Yang's modulus was (24.0 ± 7.6) kPa. These were statistical differences between the two groups (P = 0.000).
CONCLUSIONSWE can be used as an evaluation method of manipulation treatment for neck-shoulder myofascial pain syndrome, which is an objective and sensitive detection method.

Objective:To investigate and evaluate the effect of antibacterial agents clinical use and management training for medi-cal institutions in Hubei province in order to promote rational antibacterial drug use. Methods:A questionnaire was designed, inclu-ding basic conception, medication principle, mechanism and medication feature etc. The data were studied by SPSS 13. 0 software to analyze the knowledge level of antibacterial agents in medical staffs. Results: Medical staffs had good acquisition in the knowledge points involved in choice questions. The correct percentage of each scoring in essay questions differed considerably within the range of 7. 88%-99. 01%. Management rules could be well understood, while the definition of antibacterial drugs and the difference between antibacterial drugs and antibiotics were paid little attention. Conclusion: The overall knowledge level of rational antibacterial agents use in the trainees is promising with comprehensive understand of the management rules. It's necessary to distinguish antibacterial a-gents from antibiotics more clearly.

Objective Retrospectively analyze primary experences of surgical correction of complicated aortic arch anomaly with autologous palmonary artery. Methods Between July 2010 and December 2012,13 cases of complicated aortic arch anomaly underwent reconstruction of aortic arch with autologous pulmonary artery.Classifications of aortic arch anomaly were interrupted aortic arch with ventricular septal defect in 4 patients, ventricular septal defect associated with coarctation in nine patients. There were 7 males and 6 females.Their age at surgeries ranged from 1 month to 16 years, and the body weight were from 3.5 kg to 52.0 kg with median weight of 12.6 kg.Cardiopulmonary bapass was estabished with dual arterial cannulations in patients with interrupted aortic arch.During cooling to deep hypothermia(rectal temperature was 18 ℃),intracardiac defects were totally corrected.Arch anomaly was reconstructed under deep hypothermia, including deep hypothermic cardiac arrest(DHCA) in 9 patients, deep hypothermia with regional perfusion in 4 patients.Anterior wall of pulmonary artery was excised in all of 13 patients. In 4 cases, the excised wall of anterior wall of pulmonary artery was sutured to form a conduit with different diameters according to the patient's bady surface area. Two ends of the conduit were anastomosed the aortic arch and desceding aorta respectively. In another 9 patients, aortic arch was augmented with tailored pulmonary artery patch in oval shape. The defect of pulmonary artery was repaired with autologous pericardial patch. Results There was only 1 death due tomutiple organ failure postoperatively.Another 12 patients survived without neurologic complications.Differences of arterial pressures between upper and lower extremeties were not monitored in all cases.During follow-up,routine echocardiogram showed satisfactory results with unobstructive blood flow at the aortic arch. Conclusion Autologous pulmonary artery can be used to relieved complicated aortic arch anomaly completely without any tension of anastomosis site and compression of left main bronchea postoperatively.More patients with long-term follow-up are necessary to draw an accurate conclusion of this technique.

Trypanosoma is a human parasite severely affecting poor tropical areas. However, current frontline drugs for Trypanosoma treatment have severe side-effects with decreased effectiveness.Based on the fact that aminoacyl-tRNA synthetase is a bonafide drug target for several microorganisms, including bacteria and fungi,it is plausible that it may also be effective target of Trypanosoma. The Trypanosoma brucei leucyl-tRNA synthetase(tbLeuRS)was cloned, expressed and purified to develop an in vitro enzymatic assay system. The assay conditions were further optimized for the effective screening of tbLeuRS inhibitors thus establishing an anti-Trypanosoma drug screening system targeting tbLeuRS.The results indicated that this system can be employed for the effective screening of anti-Trypanosoma drugs with satisfactory specificity. In addition, this system can also be used for compound optimization,as well as IC50 testing.Using this system a series of compounds are identified that are effective Trypanosoma inhibitors without toxicity to human cells. Therefore, targeting tbLeuRS may represent a new venue for the development of anti-Trypanosoma drugs.

Objective:To investigate and analyze the influencing factors in the implementation of essential drugs in the medical in-stitutions in Hubei in order to provide reference for promoting the implementation of national essential drugs. Methods: The medical staff participating in the training class of clinical application guidelines and formulary of national essential drugs in Hubei (2014) was surveyed by a questionnaire to analyze and evaluate the statistical influencing factors in essential drugs implementation. Results: A-mong the factors affecting the implementation of essential drugs, the score of rational use was the highest (3. 86) followed by that of medicine quality, rational varieties of essential drugs, affordability and management system, and the factors with score below 3. 0 were pharmacy trusteeship and income of essential drugs in medical institutions. Conclusion: Based on the investigation and evaluation of influencing factors, the corresponding measures should be adopted, which can further improve the basic drug system and enhance the use effects of essential drugs.
