The prevalence of obesity,type 2 diabetes,and other chronic non-communicable diseases is increasing rapidly world-wide.Most recent epidemiological data showed that China is now in an era of obesity,and under a huge burden of the complications related with obesity,diabetes,as well as cerebral-and cardiovascular disorders.Strenuous efforts,comprehensive strategies,effective measures,and practical solutions must all be taken to combat against these chronic diseases.
3.Clinical effects of Sini powder combing with enema in treatment of chronic nonspecific ulcerative colitis

Objective To research clinical effects of Sini powder combing with enema in treatment of chronic nonspecific ulcerative colitis.Methods132 patients with chronic nonspecific ulcerative colitis treated in our hospital from January 2015 to October 2016 were randomly divided into two groups,with 66 cases in each group.The control group was treated with enema,and observation group was treated with Sini powder combing with enema to analyse effect of Sini powder.Results①The total effective rate was 93.94% in the observation group,higher than control group 75.76%(P<0.05).②After treatment,serum CRP,IL-8 and TNF-ɑ level of observation group were (14.92±3.04)mg/L、(0.52±0.05)μg/L、(18.65±3.18)ng/L,lower than control group(23.04±3.31)mg/L、(0.75±0.08)μg/L、(25.59±3.62)ng/L(all P<0.05).③ ESR and PLT level of observation group were(11.16±2.27)mm/h、(220.6±15.8)×10.9/L,lower than control group(15.93±2.58)mm/h、(258.3±18.9)×10.9/L(all P<0.05).④The adverse reaction rate was 15.15% in the observation group,and 21.22% in the control group, the difference was not statistically significant.⑤ The recurrence rate was 23.81% in the observation group,lower than control group 39.06%(P<0.05).ConclusionSini powder can improve the effect in treatment of chronic nonspecific ulcerative colitis and has higher security.

5.Pathogenic Bacteria Distribution and Drug Resistance of Hospital Infection in Pediatrics Unit

OBJECTIVE To investigate the major pathogenic bacteria and drug resistance of hospital infection in pediatrics unit,and to provide laboratory evidence for clinical diagnosis and treatment.METHODS Pathogenic bacteria were isolated from patients who suffered from hospital infection in pediatrics unit from 2004 to 2006.the method was taken to carry out the sensitive test and bacteria identification by VITEK32,Streptococcus pneumoniae,Moraxella catarrhalis and Haemophylus influenzae were assayed with French Bio-Merieux API System ATB STREP5 and ATB HAEMO,respectively.RESULTS Totally isolated strains were 489.Of them,108 strains of coagulase negative Staphylococcus(CONS),being 22.1%.58 strains were S.haemolyticus.The isolated rate of CONS was 53.7%.The drug test for meticillin-resistant coagulase negative Staphylococcus(MRCNS) showed multiple drug-resistance.The detection rate of the ESBLs from the Escherichia coli and the Klebsiella pneumoniae was 18.9% and 12.5%,respectively.CONCLUSIONS MRCNS are the major pathogens in pediatrics unit.The detectable rate of MRCNS is high.Glycopeptide antibiotics are the first-choice drugs for MRCNS infection.
6.Laparoscopic Biliary Reoperation

Objective To evaluate the feasibility and clinical efficacy of laparoscopic biliary reoperation. Methods From May 2000 to May 2007, 105 patients received laparoscopic biliary reoperation, including choledocholithotomy and T-tube drainage in 96 cases, Roux-en-Y anastomosis of the gallstone/hepatic-duct in 5, and liver resection in 4. Results The operations were completed in all the 105 patients without severe complications. The operation time was 60-185 min [mean, (125.3?23.2) min]. Intraoperative blood loss was 15-310 ml [mean, (21.1?8.6) ml]. The patients stayed at hospital for 5-9 days after the operation. The 5 patients, who underwent Roux-en-Y anastomosis of the gallstone/hepatic-duct, were followed up for 6 to 65 months [mean, (13.2?8.6)months]. Partial liver resection was performed in 2 cases (resection of the left hepatic lobe in 2, and the right lobe in 2); they were followed up for 8-25 months [mean, (10.2?3.6)months]. In the 96 patients, who received laparoscopic choledocholithotomy and T-tube drainage, the follow-up were lasted for 3-72 months [mean, (13.2?9.6)months]. None of the patients who were followed up had recurrence of gallbladder stone. Conclusions Laparoscopic biliary reoperation is feasible and safe with minimal trauma to patients.
7.Correlation of serum lipoprotein(a)level and coronary artery lesion degree in elderly coronary heart disease patients with type 2 diabetes mellitus

Objective To investigate the correlation of serum lipoprotein(a)[Lp(a)]level and coronary artery lesion degree in elderly coronary heart disease (CAD) patients with type 2 diabetes mellitus.Methods 163 elderly patients with CAD in accordance with the control fasting glucose(FPG)levels on admission were divided into A,B,C three groups:FPG <6.1 mmol/L for group A(50 cases);6.1 mmol/L≤FPG <7.0 mmol/L for group B (69 cases);FPG >7.1 mmol/L for group C (44 cases).The clinical data,count in patients with coronary artery lesions and lesion severity index(Gensini score),blood lipid and lipoprotein(a)[Lp(a)],etc were recorded.Results There was significant difference in serum Lp(a)among three groups(F =21.577,P <0.001),the Lp(a)of group C [(413.5 ±63.1)mmol/L]was higher than group B[(376.3 ±58.3)mmol/L],group B was higher than group C [(335.3 ±52.5 )mmol/L].The differences of serum Lp (a)between coronary lesion groups were statistically significant(F =69.447,P <0.001),three lesion group Lp(a)[(436.6 ±60.2)mmol/L]was higher than the double branch lesions[(362.5 ±55.7 )mmol/L],double branch lesion group was higher than the single lesion group [(315.4 ±49.9)mmol/L];Gensini score between different coronary lesions group had significant differences (F =179.969,P <0.001),three lesion group Gensini score[(61.9 ±15.4)points]was higher than double branch lesion group[(34.3 ±12.5 )points],double branch lesion group was higher than the single lesion group [(17.2 ± 9.1)points];Lp(a)≥372.3 mmol/L group Gensini score[(58.4 ±12.5)points]was significantly higher than the Lp(a)<372.3 mmol/L group[(19.2 ±10.3)points](t =21.836,P <0.01);Lp(a)≥372.3 mmol/L group three lesions proportion(57.3%)was significantly higher than 18.5% of Lp(a)<372.3 mmol/L group(χ2 =37.142, P <0.001);Pearson correlation analysis showed that the serum level of Lp(a)was positively related with Gensini score(r =0.439,P <0.001 );Multiple linear regression analysis showed that the Lp(a)was the independent risk factors affecting the Gensini score(β=4.210,P <0.001).Conclusion In elderly patients with CAD 2 diabetes significantly higher serum levels of Lp(a),the higher level is closely related to the severity of coronary lesions,and can be used as an independent predictor of coronary artery lesion severity.
8.Association of Polymorphism in Angiotensin Converting Enzyme Insertion/Deletion Gene with Curative Effect of Hypotensive Drugs:A Meta Analysis

Objective To assessthe association of polymorphism in angiotensin converting enzyme insertion/deletion (ACE I/D) gene with curative effect of hypotensive drugs.Methods The studies concerning association of ACE I/D with curative effect of hypotensive drugs were retrieved by searching data base,and then screened according to inclusive and exclusive criteria.Meta-analysis was performed by review manager 5.3 software.WMD and OR were used to assess difference of levels or effective rate of blood pressure degression among DD,DI and Ⅱ gcnotype.Results Totally 12 articles with 2 528 patients were included in this study.For all patients,WMD (95% CI) with DD:Ⅱ genotype reflecting degression level of systolic pressure (SP) and diastolic pressure (DP) were respectively 5.92(4.09~7.75) and 1.70(1.03~ 2.38),and OR (95%CI) reflecting effective rate of blood pressure degression was 2.49 (1.44~ 4.31),with statistical significance (P< 0.05).For subgroup analysis according to hypotensive drugs,using angiotensin Ⅱ receptor blocker (ARB),WMD (95 %CI) of SP and DP were respectively 9.16(6.51~11.82) and 1.84(0.10~3.57) in DD:Ⅱ,or 6.75(4.21~9.28) and 3.38(0.16~6.61) in DI ∶ Ⅱ genotype,all with P<0.05.There were no statistical significance for diuretic in DD ∶ Ⅱ,DD ∶ DI and DI ∶ Ⅱ.Using angiotensin converting enzyme inhibitor (ACEI),OR (95%CI) were respectively 3.79(1.80~8.01) and 2.97(1.34~6.55) in DD:Ⅱ and DD:DI,with P<0.05.Conclusion Compared with I allele in patient,ACEI and ARB were more effective for D allele,while there were no difference for curative effect of diuretic between I allele and D allele.
9.Capillary Electrophoresis of Neurotransmitters Under the Control of Radial Electric Field

Separations of neurotransmitters such as dopamine (DA), ser otonin (5-HA), norepinephrine (NE) and epinephrine (E) were performed successf ully using a homemade electric field modulated capillary electrophoretic system, which could offer both radial and axial electric fields with only one high volt age power supply. DA and 5-HT were eluted simulaneously and could not be resolv ed in 0.01 mol/L phosphate buffer at pH 2.5. Alcohol additives, such as methanol , ethanol or 1-propanol were added to the buffer to change the solvation shell of the solutes, which changed their effective sizes and electrophoretic mobiliti es of the solutes accordingly. The optimum composition was a buffer of 20% (V /V) 1-propanol, with resulted resolutions 0.74 (DA/5-HT), 0.56(5-HT/NE) and 0.77 (NE/E). If a positive radial voltage of 6.6 kV was applied, the resolut ions were improved to 1.48, 0.71 and 1.32, respectively.
10.Changes of myocardial ATPase activity and plasma TNF-α concentrations in rats with acute craniocerebral injury

Objective To investigate the changes of myocardial ATPase activity and plasma tumor necrosis factor- alpha (TNF- a) levels in rats with acute craniocerebral injury (ACI).Methods Sixtyfour male SD rats were randomly (random number) divided into sham group and model group.The observation intervals were set at 2 h,6 h,24 h and 72 h after ACI (n =8,at each interval).ACI model was established in rats by free falling objects hitting skull.Plasma levels of cardiac troponin Ⅰ (cTnⅠ) and TNF-αwere determined by using enzyme -linked immunosorbent assay (ELISA).And the myocardial Na +-K+ -ATPase and Ca2+ -ATPase activity changes were determined by colorimetric method.Then the pathological change of myocardium with HE staining were observed under light - microscopy.Results Plasma cTnⅠ and TNF-α levels increased (P < 0.05),while myocardial Na+-K+-ATPase and Ca2+- ATPase activity decreased in the rats with ACI (P < 0.05 ); There was a negative correlation between myocardial Na + -K + -ATPase、Ca2 + -ATPase activity and plasma Ievels of cTnI ( r =0.357,r =0.557,P < 0.05),and positive correlation between plasma levels of TNF-α and cTnⅠ ( r =0.920,P < 0.05 ).Vacuolar degeneration and necrosis with inflammatory cell infiltrates of myocardium were observed under light -microscopy.Conclusions ACI can lead to myocardial injury,resulting in increase in plasma cTnⅠ and TNF-αand decrease in myocardial ATPase activity.Decrease in myocardial ATPase and increase in TNF-αmay involve in the pathogenesis of myocardial injury after ACI.
