1.Diagnosis and treatment of traumatic chyloperitoneum

Objective:To investigate the proper method for treating traumatic chyloperitoneum. Methods: A retrospective analysis was performed on clinical data of 4 cases of traumatic chyloperi- toneum admitted to our hospital in recent 26 years. Results: All 4 cases were cured after conserva- tive treatment. Conclusion: Perdominantly integrated therapy including absolute diet,total parrenter- al nutrition(TPN) and abdominal cavity drainage can be the effective method for the treatment of trau- matic chyloperitoneum.
2.Biological effects of nonthermal plasma combined with X-ray irradiation on human hepatocellular carcinoma in nude mice

Objective To investigate the biological effects of nonthermal plasma (NTP) combined with X-ray irradiation on human hepatocellular carcinoma in nude mice.Methods By subcutaneous inoculation, tumor models of human hepatocellular carcinoma HepG2 in nude mice were established, and these mice were divided into control group, NTP treatment group, X-ray irradiation group, combined treatment group.The changes of tumor microenvironment were observed using hematoxylin-eosin (HE) staining.The terminal deoxynucleotidyl transferase-mediated nick end labeling (TUNEL) was used to determine the level of neuronal apoptosis in tumor tissues.The expression of metal matrix proteinase-2 (MMP-2) was detected by immunohistochemical assay.Transmission electron microscope was used to observe cell structure changes.Results The final tumor volumes of control group, NTP treatment group, X-ray irradiation group, combined treatment group were (543.96±108.45), (436.54±65.49), (351.66±56.68), (281.97±35.60)mm3, with a statistically significant difference (F=9.63, P=0.01), and the difference between X-ray radiation group and the combination treatment group was significant (P=0.05).HE staining showed that there was a larger area of necrosis in the combined treatment group compared with the other groups.TUNEL showed that the apoptotic indexes were (0.95±0.13)%, (5.82±0.26)%, (7.53±0.43)%, (11.07±0.35)% respectively, with a statistically significant difference (F=547.76, P=0.00), and the difference between X-ray radiation group and the combination treatment group was statistically significant (P=0.00).The immune scores of each group were 12, 9, 9, 2.Electron microscopic observation showed that there were more apoptotic bodies in the combined treatment group than those in the other groups, accompanied by mitochondrial edema.Conclusion NTP and X-ray irradiation therapy in the treatment of human hepatocellular carcinoma in nude mice model has a synergistic effect.Probably, it can be a new type of treatment in curing cancers.
3.Expression of aquaporin-1 in human peritoneum and its effect on peritoneal dialysis patients with the different peritoneal transport characteristics

Objective To investigate the expression of aquaporin-1 (AQP1) in human peritoneum and its effect on peritoneal dialysis (PD) patients with the different peritoneal transport characteristics.Methods Peritoneal biopsies were obtained from the PD patients (n =30) at catheter insertion.Western blot and immunohistochemical staining were used to investigate AQP1 expression in peritoneal tissues.After catheter insertion,4 week,peritoneal equilibrium test (PET) was used to decide peritoneal transport characteristics.Results Western blot revealed a major band at 28 kDa band.The expression of AQP1 index was 1.49 ±0.67 in the normal subjects,and 1.61 ±0.76 in the PD patients,and there was no significant difference.Positive deposition was distributed in mesothelial cells,endothelial cells of capillaries,and small veins.The index of AQP1 expression in endothelial cells of capillaries in normal subjects and PD patients were 2.12 ±0.18,and 2.50 ±0.81,respectively,and no significant difference was found.The index of AQP1 expression in mesothelial cells of capillaries in normal subjects and PD patients were 1.95 ± 0.67,and 2.23 ± 1.07,respectively,and no significant difference was found.Among PD patients,the level of AQP1 was significantly different.Conclusions The expression of AQP1 was different in the PD patients with different peritoneal transport characteristics.In the high and high average transport groups,the level of AQP1 was lower; the low transport group was higher.It reveals that AQP1 plays a role in the pathogenesis of transcellular water transport.
4.Expression of CD38 and HLA -DR on CD +8 T cells in patients with chronic hepatitis B and HBV carriers

Objective To determine the expression of CD38 and HLA -DR on CD +8 T cells in patients with chronic hepatitis B and HBV carriers,and to disscuss the relation between immune activation and disease progression of HBV infection.Methods Thirty -two chronic hepatitis B patients receiving adefovir dipivoxil treatment,31 HBV carriers and 28 normal controls were collected.The counts of CD +4 and CD +8 T cells and the percentage of CD +8 CD +38 and CD +8 HLA -DR +T cells were tested by flow cytometry.HBV DNA and liver function were tested in the central laboratory of our hospital.Results The percentage of CD +8 CD +38 T cells in CHB patients was (58.4 ±12.7)%,and was higher than that in HBV carriers (46.8 ±8.5)% and normal controls (46.8 ±8.5)%,and decreased after adefovir dipivoxil treatment (34.2 ±9.4%)(F =8.27,P =0.000).The percentage of CD +8 CD +38 T cells in HBV carriers was (43.3 ±12.5)%,and was much higher than that in normal controls (9.8 ±5.7)%(F =13.48,P =0.000).The percentage of CD +8 HLA -DR + T cells in CHB patients was higher than that in normal controls,but similar to that in HBV carriers (37.1 ±11.3)%.CD +8 HLA -DR + T cells in CHB patients also decreased after adefovir dipivoxil treatment (P <0.05 ).Conclusion Our study demonstrates that activation of CD +8 T cells is increased in HBV infection but decreased by adefovir dipivoxil treatment.The percentages of CD +8 CD +38 and CD8 +HLA -DR +T are good markers for disease progression of HBV infection.
5.Current situation and development trend of scientific research in health-care-associated infection management

Objective To understand the current situation and development trend of scientific research in health-care-associated infection (HAI)management in China since 1986,and provide the basis for improving the research ability of HAI management.Methods A questionnaire survey was conducted in 170 hospitals of 13 provinces and military systems in China through multi-stage stratified sampling method,surveyed content included publication of papers written by HAI management professionals,publication of HAI-related books,application of scientific re-search projects and awards of scientific research.Results Of 170 hospitals,63 were secondary hospitals,and 107 were tertiary hospitals.During the period of 1986-2015,the professionals of HAI management have published 2 596 papers,2 555 were in Chinese,41 were in English.105 and 2 491 papers were written by professionals in sec-ondary hospitals and tertiary hospitals respectively.The number of published papers written by professionals in ter-tiary hospitals increased significantly after 2011.In 1986 - 2015,a total of 68 books on HAI management were written by investigated hospitals,35 (51.47%)of which were written in 2011 -2015.A total of 217 HAI-related research projects were applied in 1986-2015,213 (98.16%)were applied by tertiary hospitals.Most research pro-jects were applied after 2011,accounting for 57.60%(n= 125).Conclusion Scientific research on HAI manage-ment in China has entered a stage of rapid development,more and more HAI professionals begin to engage in re-search work.
6.Behavioral characteristics of accident proneness and its influencing factors among children and juveniles

Objective To investigate the behavior characteristics of children or juveniles with accident proneness and to explore the influencing factors of children and juveniles accident proneness in behavior.Methods The study was carried out bv case-control method,and 1 601 children and juveniles from a rural area of hunan province were investigated with homemade general injury questionnaire.Accident proneness children or juveniles screened out from cross-sectional studv were selected into research group,while uninjured children or juveniles whose gender,age and grade were the same with the research group were included into control group with 1:1 matched.Achenbach Child Behavior Checklist was used to evaluate and compare these two groups of children and juveniles' behavior.Results 147 accident proneness children and juveniles were screened out from the study and the accident proneness incidence of children and juveniles in rural areas was 9.42%.The total score and each factor score of behavior problem and behavioral problems detection rate iu research group were higher than that of the control group and there were statistically significant (P<0.01).Multivariate analysis showed that attention problem (OR=1.110,95% CI =1.041-1.185) and aggressive behavior (OR=1.123,95% CI =1.068-1.181) were risk factors of accident proneness.Conclusion Accident proneness children and juveniles' behavior is different from the uninjured,which is a risk factors of accident proneness.
7.Clinical efficacy of oxaliplatin-based systemic chemotherapy in treating advanced hepatocellular carcinoma:a com-parative study of two regimens

Objective To evaluate the therapeutic efficacy and toxicity of oxaliplatin (OXA)-based systemic chemotherapy in patients with advanced hepatocellular carcinoma (HCC).Methods Thirty patients with advanced HCC admitted to our hospital from June 2009 to June 2013 were randomly divided into CapeOX group (15 patients,treated with OXA and capecitabine or CAP)and FOLFOX group (15 pa-tients,treated with OXA and calcium folinate or CF,followed by fluorouracil or 5 -FU).The therapeutic effects were evaluated after two cycles of treatment using the Response Evaluation Criteria in Solid Tumors (RECIST 1 .1 ).Assessment criteria included overall response rate (OR),time to tumor progression (TTP),and overall survival (OS)analyzed by Kaplan-Meier survival curves,as well as the toxicity profile of the combination chemotherapy.Comparison of OR was made by chi-square test;assessment of tumor response and toxicity profile was performed by Mann-Whitney U test;OS and TTP were analyzed by log -rank test.Results In the CapeOX group,the OR was 13.3%,and the mean OS and TTP were 10.4 months and 5.0 months,respectively.In the FOLFOX group,the OR was 6.7%,and the mean OS and TTP were 9.0 months and 4.7 months,respectively.The differences between the two groups had no statistical significance in all three parameters (P=0.543,0.606,and 0.769,respectively).Compared with the FOLFOX group,the CapeOX group had significant-ly lower toxicity rates in gastrointestinal tract and myelosuppression (P=0.006 and 0.002,respectively).Conclusions The OXA-based systemic chemotherapy shows anti-tumor effects for advanced HCC,and there is no significant difference in efficacy between the two regi-mens.Decisions regarding the choice of specific treatment should be based on the patients'clinical conditions.
8.The regulation of TLR3 on NF-κB signaling pathway and its significance in the pathogenesis of tumor

NF-κBs is a member of Rel protein family of transcription factors ,which is expressed in all cells and is involved in important physiological and pathological regulatory mechanisms ,including the regulation of TLR3 on NF-κB signaling pathway ,which has been attracted widespread attention .This paper reviews the mo-lecular mechanisms of the regulation .We summarizes the role of host immune responses in tumor development , which is mediated by the related signal molecules .It reveals that some key proteins in the regulation have a great significance to the treatment of diseases ,as used as the therapeutic targets ,and is conducive to the understanding of function and diversity in tumor tissue .
9.The progression on tyrosine kinase receptor EphA7 in tumor

EphA7 is one of the most important members of the Eph family .It is a key regulatory factor of cell adhesion and repulsion ,it also contributes to establish men ,maintain ance and tissue remodeling in cell mor-phology .In recent years ,its function in tumors has been revealed gradually .It plays a variety of roles in different tumors.The overexpression of EphA7 can be detected in acute leukemia ,glioblastomamultiforme,lung cancer and hepatic carcinoma ,which suggests EphA 7 may be involved in the development of these tumors .While in colon cancer,prostate cancer,follicular lymphoma and gastric cancer ,a significant reduction of EphA7 expression with promoter hypermethylation is discovered ,which suggests EphA 7 play a tumor suppressor role in these tumors .E-ven though the function of EphA 7 is still under exploration , EphA7 and tumor development are inextricably linked,it may have roles in the pathogenesis and development of these carcinomas .
10.Comparison and enlightenment on the general practice education between Britain and Aus-tralia

By taking British and Australian medical colleges as examples, the paper introduced the current situation of British and Australian general practice education for undergraduate medical stu-dents and elaborated their theory learning content, their practice learning content ,their education re-form and research progress and their education target and effect. General practice education in Britain and Australia sheds light on how to set theory course, arrange practice part and improve educational research, which general practice education in China can draw lessons from.
