Objective To know basic characteristics of MSM infected with HIV treated by highly active antiretroviral thera‐py(HAART) ,and evaluate therapeutic effect based on the level of CD4+ T cell counts .Methods Basic database of patients were collected from the national AIDS comprehensive data information system ,patients were observed by the retrospective cohort study , and effect indicators were analyzed based on the level of CD4+ T cell counts .The data was analyzed by SPSS18 .0 software .Results A total of 1 758 cases were recruited in this study and the average age was (33 .5 ± 11 .1)years .The component ratio of the CD4+T cell counts <50/μL ,50 to 199/μL ,200 to 350/μL ,> 350/μL were 7 .7% ,27 .6% ,54 .9% ,9 .7% ,respectively .The CD4+ T counts increased and kept among 150 to 250/μL ,250 to 350/μL ,350 to 450/μL ,450 to 550/μL ,respectively after 12 months ,treat‐ment .The rates of viral inhibition were 70 .2% ,81 .7% ,84 .2% ,83 .3% ,respectively after 6 months′treatment .And the cumulative survival rates were 0 .81 ,0 .93 ,0 .98 ,1 .0 after 36 ,months treatment ,respectively .Conclusion HAART on MSM achieves remarka‐ble results .And the early treatment was more beneficial to improve immune function ,reduce AIDS mortality and control the HIV/AIDS epidemic of MSM .

Objective To analyze the effect of different postoperative nutrition:enteral nutrition( EN) and parenteral nutrition( PN) in patients with colorectal cancer. Methods 84 patients with colorectal cancer were collected in this study from Jan. 2012 to Jan. 2014, and they were divided into the EN group (43 cases) and the PN group (41 cases). They were given EN or PN after operation. The nutritional parameters such as ALB, TF and PA were detected before surgery and 7 days after operation, Observe the adverse reaction, time of gastroin-testinal function recovery, infection and costs after operation. Results There was no statistical difference (P>0. 05) in the nutritional pa-rameters and rate of adverse reaction between the two groups. The rate of infection in EN group was lower than that in PN group(P<0. 01), and the time of gastrointestinal function recovery was shorter in EN group compared with that in PN group (P<0. 01). Conclusion EN is better in recovery of gastrointestinal function, and there is less complication compared to PN. EN have clinical application value for patients with gastrointestinal cancer.

Objective To study the incidence rate and correlation of anomalous pancreaticobiliary ductal junction( APBDJ) for patients with biliogenic chronic pancreatitis (BCP). Methods From January 2000 to January 2012,the data of 1 373 patients who underwent ER-CP,MRCP or T-Tube cholangiography in which both biliary and pancreatic ducts in our hospital were analyzed retrospectively. Results Bil-iogenic chronic pancreatitis was found in 47 patients,of which 17 patients with APBDJ. The mean (SD) common channel was (19. 3±7. 2) mm in length ( range 11~40 mm) . Conclusion APBDJ may be the important reason of BCP among Chinese patients.

Objective To express P38 of Schistosoma japonicum in Escherichia coli BL21 and develop the monoclonal antibodies (McAb) against rSjP38. Methods The recombinant plasmid pET32(a)-P38 was transformed into E.coli BL21(DE3). 1 mmol/L IPTG (isopropyl-beta D-thiogalactopyranoside) was used to induce the expression of the recombinant rSjP38. The rSjP38 was soluble in supernatant after sonication and further purified by His-Ni chromatography. BALB/c mice were immunized with the purified rSjP38 and hybridomas were generated with traditional technique. McAbs were screened by ELISA with limited dilution. The subtype and specificity of McAb were identified by kit and Western blot respectively. Results Eight hybridoma cell lines secreting monoclonal antibodies to rSjP38 were obtained. The subtype of all the 8 McAbs are IgG1. Western blotting showed that the 8 McAbs reacted strongly and specifically with native antigen (P38) of Schistosoma japonicum. Conclusions Eight hybridoma cell lines secreting highly specific McAbs against P38 have been established.

Objective To assess the efficacy of Xuezhikang capsule on hyperuricacidemia and its safety. Methods The study was designed as a random, double-blind placebo controlled clinical trial. 80 hyperuricacidemia patients were divided randomly into test group and control group (40 in test group and 40 in control group). The course of treatment was 28 days. Results 75 patients finished the trial (38 in test group and 37 in control group). After 28 days of treatment, the differences in reduction of UA, XOD, FINs, Homa-IR, TC, TG, Lp, CRP, ET, ?2-MG and elevation of HDL of the test group were statistic significance. No side effect was found in the trial. Conclusion Xuezhikang capsule has good clinical effect and safety in the treatment of hyperuricacidemia.

Objective Interferon regulatory factors 3 (IRF-3) is a key transcription factor to regulate gene expression of interferon after virus infection. This study aims to look for new spliced isoforms of IRF-3 and to investigate their structures and functions. Methods RNA extracts from human embryonic kidney 293 cells were amplified by RACE and RT-PCR. New sequences were compared with published sequences of IRF-3 and murine EST database using bioinformatics method. A new sequence, IRF-3c, was subcloned into pcDNA3.1-flag. The IRF-3c/pcDNA3.1-flag plasmid was transfected in HEK 293 cells. Whole cell extract was analysed by Western blot and then probed with monoclonal Flag antibody. Luciferase assay was carried out by co-transfection with reporter plasmid, pGL2B with interferon ? promoter, and IRF-3c cDNA expression plasmid. At 16 hours posttransfection, cells were infected with Sendai virus for 8 hours. Cells were collected and assayed for luciferase activity. Results A novel spliced isoform of IRF-3, named IRF-3c was discovered. The new isoform is almost the same as IRF-3, except for the utilization of the 180 bp bases in intron 6 adjacent to exon 6. The first 2,3 and 4 bases are a stop codon, which may produce a protein with a truncated C-terminal stoped at amino acids 327. Western blot analysis confirmed an expected 44 kDa strong band. The new inserted bases can be found in murine EST database, suggesting a conservative function in evolution. The functional luciferase assay showed that IRF-3c inhibited the IFN? promoter activity to (around) 40%～50% as that of control after Sendai virus infection. Conclusions The discovery of a new isoform of IRF-3 provides a new insight into the functional regulation of IRF-3 family. It is a dominant-negative inhibitor for interferon ? promoter activity in the virus infection pathway, provides a mechanism for the fine-tuning of the virus-induced activation of the interferon response, and prevents interferon ? from its overexpression and its toxic effects. It is worthwhile to explore the role of IRF-3c in the pathogenesis of human diseases using IRF-3c’s specific sequence.

Objective To explore the inhibitory effect of mutant influenza A viruses to the activation of interferon regulatory factor 3 (IRF-3). Methods HEK293 cells were infected with A/FM/1/47,A/HK/1/68, A/HK/1/68-MA20, A/HK/1/68-MA20C and positive control Sendai virus (SV). Whether the slowly moved phosphorylation form Ⅲ and Ⅳ of IRF-3 appeared or not was compared by Western blot in cells infected with these viruses. Wild type of NS1 from A/HK/1/68 and mutant NS1 from A/HK/1/68-MA20 were subcloned into pcDNA3.1-flag respectively. They were transfected in HEK 293 cells respectively. At 16 hours posttransfection, cells were infected with Sendai virus for 8 hours. Whole cell extracts were analyzed by Western blot and then probed with monoclonal flag antibody to check the expression of NS1, or with anti-IRF-3 to observe the inhibitory effects of the wild and mutant NS1 to the activated IRF-3. Luciferase assay was carried out by co-transfection with reporter plasmid, pGL2B with interferon ? promoter, and wild or mutant NS1 cDNA expression plasmid. SV was used to infect these cells after the co-transfection. Results Only less virulent A/HK/1/68-MA20 and positive control SV can activate IRF-3. Activated form Ⅲ and Ⅳ of IRF-3 began to appear 9 hours post infection (h.p.i), and most significant activated IRF-3 appeared 23 and 26 h.p.i. Sequence analysis of NS1 of MA20 revealed that nucleotide position number 94 is mutated from T to C, and amino acid at position number 23 is changed from valine to alanine. Co-transfected with wild type NS1 made form Ⅲ and Ⅳ of IRF-3 almost disappear, but not mutant NS1. In the luciferase functional analysis, wild type NS1 can inhibit the luciferase activity of IFN-? promoter, which was induced by SV, to around 1/10. Again no inhibitory effects was observed of mutant NS1 in the luciferase assay. Conclusion The mechanism that A/HK/1/68-MA20 can activate IRF-3 is that point mutant NS1 abolished the inhibitory function of NS1.

Objective To know the status of disinfection conducted in the hospitals at province level in Hunan province aimed to make the intervention strategy for prevention and control of diseases transmission. Methods The related items of hospitals at province level were examined and evaluated according to the national standards such as GB15982-1995 Hygienic Standard for Disinfection in Hospital, GB15981-1995 Evaluating Method and Standard for the Efficacy of Disinfection and Sterilization, GB8987-1996 Requirements for Hospital Sewage Discharge, GB18466-2001 Requirements for Medical Organization Sewage Discharge, GB15980-1995 Hygienic Standard of Disinfection for Single Use Medical Products and Technical Standard for Disinfection (2002). Results As for the disinfection effect in the present investigation, the total qualified rate was 90.10%, the annual qualified rate was increasing. Conclusion The surveillance of hospital disinfection is a long-term task, to supervise and improve the hospital disinfection is necessary to ensure patient and society safety.

Objective To assess the diagnostic value of chronotropic index in identifying coronary artery disease (CAD). Methods One hundred and sixty two patients with suspected CAD were referred for a routine treadmill exercise electrocardiogram and subsequently underwent selective coronary angiography within 3 weeks. The magnitude of ST segment depression and chronotropic index were measured by a computerized ECG system. CAD was defined by coronary angiography. Results In detecting CAD, the sensitivity was 70. 1% versus 60.9% (P< 0.05) ,the specificity was 84.0% versus 73.3% (P<0.05), and the accuracy was 76.5% versus 66.7% (P< 0.05) by chronotropic index and standard electrocardiographic test criteria. The value of Kappa statistic for chrono-tropic index consistency in our experiment was 0.534(>0.4). The number of diseased vessels was correlated with the mean value of chronotropic index. The chronotropic index decreased from 0.88±0.09 in patients without coro-nary disease to 0.68±0.06 in those with 3 vessels involved (P<0.01). A more closed relationship was found be-tween chronotropic index and lesion in the fight coronary artery. Conclusions Chronotropic index is possibly a valu-able positive index for the diagnosis of CAD. The sensitivity,specificity and accuracy of chronotropic index are signif-icandy higher than those of standard electrocardiographic test criteria in identifying CAD.
