1.Establishment and Application of "Information System for the Introduction and Examination and Approval of New Drugs" in Our Hospital

OBJECTIVE: To strengthen the management on procedures for the introduction,examination and approval of new drugs in hospital.METHODS: The experience on the current procedures for the introduction,examination and approval of new drugs in our hospital was summarized,and the Microsoft.NET technology was used to develop Web-based "information system of introduction,examination and approval of new drugs".RESULTS & CONCLUSIONS: The new system adopted three-level "browser / server" structural pattern and it is composed of external network new drug data collection system and internal network new drug examination and approval system.The system is advanced,stabile,practical,intelligent and safe,and it has simplified the new drug examination and approval procedure,enhanced the working efficiency.Its application is characterized by justice,publicity and transparency.
2.Efficacy and safety of glucocorticoids in community-acquired pneumonia: A systematic review and meta-analysis of randomized controlled trials

Objective To evaluate efficacy and safety of glucocorticoids for community-acquired pneumonia (CAP).Methods We searched relevant randomized controlled trials (RCT) from Pubmed,EMBASE,Cochrane Library,Chinese Journal Full-text Database,Chinese Biomedical Literature Database and traced the related reference to obtain the information that had not been found by using key words of steroids,glucocorticoids,cortisol,corticosteroids,community acquired pneumonia and CAP.The quality of RCT included was evaluated.Meta-analysis was performed with RevMan 5.0 provided by the Cochrance Collaboration.Results Seven RCTs involving 944 patients were included in the meta-analysis.Mean length of hospital stay in glucocorticoids treatment group was significant shorter than that in conventional treatment group (WMD =-1.70,95% CI:-2.01-1.39,Z =10.81,P <0.01).No significant differences were found regarding mortality (RR =0.77,95% CI 0.46-1.27,Z =1.03,P =0.30),mean length of ICU stay (WMD =1.17,95% CI:-1.68-4.02,Z =0.81,P =0.42),the risk of double infection (RR =1.32,95% CI 0.66-2.63,Z=0.79,P=0.43),the incidence of hyperglycemia (RR=1.84,95% CI0.76-4.41,,Z=1.36,P=0.17),the incidence of upper gastrointestinal bleeding (RR =1.98,95% CI O.37-10.59,Z =0.80,P =0.42) between conventional treatment group and glucocorticoids treatment group.Conclusions Glucocorticoids treatment used in patients with community-acquired pneumonia may shorten the course of illness and possess better safety,but could not reduce the mortality.

4.Promotion of Function of Endothelial Progenitor Cells with Shexiang Baoxin Pill Treatment under Shear Stress.

The aim of this study was to investigate whether shear stress could promote function of endothelial progenitor cells (EPCs) with Shexiang Baoxin Pill (SBP) treatment in vitro, and to study whether shear stress contributed to vascular injury repair by EPCs. EPCs were isolated and characterized; EPCs' proliferation, migration, adhesion, tube formation and eNOS protein level in vitro were investigated by culturing confluent EPCs in 4 mg/mL SBP under physiological shear stress (15 dyne/cm2) for up to 24 hours. Afterwards, EPCs were transfused into rats after wire-induced carotid artery injury augmented re-endothelialization. The results showed that, compared to the SBP group, the shear stress+SBP group obviously enhanced EPCs proliferation, migration, adhesion, tube formation and eNOS protein expression in vitro (P<0.01). After one week, immunofluorescence staining showed that endothelial regeneration rate obviously enhanced in shear stress+SBP group (P<0.01). The present study demonstrates that shear stress can promote function of endothelial progenitor cells treated with SBP, which improves the vascular injury repair potentials of EPCs.

5.Application of bronchoscopy in the therapy of pediatric status asthmaticus

Objective To explore the therapeutic value of bronchoscopy in pediatric status asthmaticus.Methods A total of 16 children with status asthmaticus received standard medical therapies (therapy group) from January 2008 to August 2010 in pediatric intensive care unit (PICU) at First Hospital,Xiamen University.However,l0 of them underwent fiberoptic bronchoscopy and bronchoalveolar lavage (BAL) (bronchoscopy group) along with standard medical therapies from September 2010 to July 2012.The values of oxygenation index (PaO2/FiO2),arterial partial pressure of carbon dioxide (PaCO2),blood oxygen saturation (SaO2),heart rate (HR),respiratory rate (RR) within 1 hour before bronchoscopy procedure,within 6 hour post procedure and within 24 hour post procedure were observed.The extinction time of wheezing sound,PICU length of stay and admission length of stay were observed between therapy and bronchoscopy groups.Results Bronchoscopies revealed a lot of thick mucus plugs and secretions.Large airways were lavaged for clearance of obstructive secretions with normal saline.Within BAL fluid counts of the bronchoscopy group,there were neutrophilia (n =8),eosinophilia (n =4) and neutrophilia/ eosinophilia (n =3).Within BAL fluid cultivation of the bronchoscopy group,4 patients had positive results for Streptococcus pneumoniae (n =2),Pseudomonas aeruginosa (n =1) and Staphylococcus aureus (n =1).In bronchoscopy group,the values of PaO/FiO2,PaCO2,SaO2,HR and RR were 319 ± 19,(40 ±4) mm Hg(1 mm Hg =0.133 kPa),(92.6 ± 1.5) %,(128 ± 12) rates/min and (35 ± 4) breaths/min within 6 hour post procedure versus 255 ± 24,(54 ± 5) mm Hg,(89.2 ± 2.6) %,(148 ± 10) rates/min and(50 ± 6)breaths/min within I hour before procedure (P < 0.01).At 24 hour post procedure,the values of PaO2/FiO2,PaCO2,SaO2,HR,RR for bronchoscopy group were 354 ± 21,(40 ± 3) mm Hg,(93.4 ±1.1)%,(125±9)rates/min and(34 ±3)breaths/min versus 317 ±21,(46 ±4)mm Hg,(90.1±2.5) %,(138 ± 8) rates/min and (43 ± 3) breaths/min respectively for therapy group (P < 0.01).The extinction time of wheezing sound was (67 ± 22) hours for bronchoscopy group vs.(98 ± 23) hours for therapy group(P < 0.01).The mean PICU length of stay was (1.6 ± 0.7) days for bronchoscopy group vs.(2.6 ± 0.7) days for therapy group (P < 0.01).The mean admission length of stay was (5.0 ± 0.7) days for bronchoscopy group vs.(6.6 ± 1.2) days for therapy group(P < 0.01).All patients for bronchoscopy group tolerated the procedure without any complications.Conclusion As a safe adjunctive therapy in pediatric status asthmaticus,flexible bronchoscopy with bronchial lavage may reduce the admission and PICU length of stay and alleviate clinical symptoms.
6.Effect of baicalin on the expression of receptor activator of nuclear factor-?B ligand and osteoprotegerin in human periodontal ligament cells

Objective To study the effect of baicalin on the expression of receptor activator of nuclear factor-?B ligand (RANKL) and osteoprotegerin (OPG) in cultured human periodontal ligament cells (HPDL cells) as well as its action mechanism. Methods We first constructed small interferring RNA (siRNA) eukaryotic expression vector targeted transforming growth factor ?Ⅱ receptor (TGF-?RⅡ),and then transfected it into T cells. Then HPDL cells together with T cells transfected with siRNA or not were placed in medium that had been added with lipopolysaccharide (LPS) and baicalin. They were divided into six groups and cultured for 48 hours. Reverse transcriptase-polymerase chain reaction (RT-PCR) was used to observe the effect of baicalin on OPG-RANKL expression in HPDL cells. Results The clone sequence correctly identified by RT-PCR was consistent with the designed target sequence. Objective:In order to obtain safer HBV DNA vaccine,recombinant kanamycin resistance eukaryotic expression plasmid containing the gene of HBV surface antigen was constructed and used to study humoral immune response in BALB/c mice.Methods:HBV antigen middle protein(preS2+S) encoding gene fragment was isolated from the recombinant eukaryotic expression plasmid pcDNA-S 2S;subcloned into eukaryotic expression vector pVAX1.After confirmed the presence of the gene by restriction endonuclease analysis,it was used to immunize the healthy BALB/c mice at different doses,and the levels of anti-HBs in serum was determined by ELISA.Results:Restriction endonuclease analysis confirmed recombinant plasmid pVAX-S 2S was what was expected.Serum anti-HBs was detected at the 2nd week after DNA vaccination at all three doses(100 ?g,50 ?g,10 ?g per mice)and their titers were increased with time.Quantitative comparison of the serum anti-HBs levels revealed significant(P

9.Preparation and Properties of Oxymatrine Pellets

OBJECTIVE:To prepare oxymatrine pellets and study the pellets' properties.METHODS:Oxymatrine pellets were prepared using an experimental low-temperature extrusion-spheronization granulator.L9(34)orthogonal design was used to obtain optimal formulation.The micromeritic properties and in vitro dissolution of the pellets at different dosage were determined.RESULTS:Oxymatrine pellets prepared by extrusion-spheronization were round and smooth and well-distributed.The optimal technical conditions were as follows:water∶MCC=0.90∶1;spheronization velocity=35 Hz;spheroniza-tion time=5 min;extrusion velocity=40 Hz.The in vitro dissolution was more than 75% within 30 minutes.CONCLUSION:The process of preparing pellets by extrusion-spheronization was simple and feasible and the quality of pellets was excellent.
10.Clinical significance analysis of radical operation for gastric cancer in elderly patients

Objective To analyze the therapeutic effect of the radical operation for gastric cancer in elderly patients and to investigate relevant factors affecting the efficacy Methods 216 cases of elderly patients with gastric cancer were divided into two groups:radical distal-and proximal-gastrectomy group (n=108,each).The therapeutic effect in the near future,intraoperative and postoperative recovery,numbers of lymph nodes resection as well as factors affecting the long-term survival rate were compared between two groups.Results The radical distal-versus proximal-gastrectomy group showed that the mean operation time was (241.8 ± 40.2) min vs.(244.7 ± 39.7)min,blood loss was(299.5 ± 114.9) ml vs.(273.8-± 121.4) ml,the number of lymph node dissection was(28.4± 9.4) vs.(27.7 ± 9.1) particles,the complication rate was[14/108 (13.0%)] vs.[15/108 (14.0 %)],3-year survival rate was (46.9 %) vs.(47.5 %),without statistically significant differences (all P > 0.05).Multiple regression analysis showed that TNM stage (OR =1.536),lymph node metastasis(OR =0.739),cancer infiltration depth (OR =1.534) and chemotherapy regimens (OR =1.337)are risk factors for the postoperative 3-year survival rate.Conclusions Surgical treatment of elderly patients with gastric cancer is significantly effective,TNM stage,lymph node metastasis,depth of invasion,tumor size and the chemotherapy regimens are closely associated with the life quality of patients.
