1.Hospital Infections among 246 Elderly Patients:Analysis and Countermeasures

90 years old patients was 13.04%.The longer the hospitalization time the incidence among inpatients longer than 60d was 43.51%,the higher was the hospital infection rate.CONCLUSIONS There are many risk factors inducing hospital infections among elderly,there fore the monitoring must be strengthened,and the disinfection isolation and asepsis must be strictly conducted,with limited invasive manipulations,reasonable use of antibiotics,and active treatment of the primary affection,all above can effectively reduce the occurrence of hospital infection among ellerly patients.

Objective To observe the therapeutic effect of therapy of dispelling wind,cooling blood and tonifying kidney for refractory idiopathic thrombocytopenic purpura(RITP).Methods Fifteen RITP patients received therapy of dispelling wind,cooling blood and tonifying kidney.After treatment for 3 courses(18 weeks in total),the changes of platelet count and megakaryocyte maturation were observed.Results Of 15 RITP patiens,8 were clinically cured,4 markedly effective,2 effective,1 ineffective and the total effective rate was 93.3%.After treatment,the platelet count was increased(P

Objective To observe the influence of wind-dispelling therapy on therapeutic effect of blood-cooling and kidney-tonifying herbs for chronic idiopathic thrombocytopenic purpura(CITP),and to investigate its immunoregulatory effect.Methods Forty-two CITP patients were randomized into 2 groups:the treatment group(N=22) received therapy of dispelling wind,cooling blood and tonifying kidney,and the control group(N=20) received therapy of cooling blood and tonifying kidney.Six weeks constituted one treatment course,and the treatment lasted 3 courses.Serum interleukin 2(IL-2),soluble IL-2 receptor(sIL-2R),Fas protein,Fas-L protein,and platelet-associated immunoglobulin G(PAIgG) levels and lymphocyte subtypes were compared before and after treatment.Results In the treatment group,14 patients were clinically cured,5 markedly effective,2 effective,1 ineffective and the total effective rate was 95.5%;in the control group,8 patients were clinically cured,4 markedly effective,3 effective,5 ineffective and the total effective rate was 75.0%.The therapeutic effect in the treatment group was superior to that in the control group(P
4.A qualitative research on medication compliance of patients with ulcerative colitis

Objective To describe the factors influencing medication compliance of patients with ulcerative colitis (UC) and provide a reference for health care workers developing effective response plans.Methods In-depth interview was conducted to 10 UC patients and the interview results were analyzed by phenomenology research method.Results There were five factors influence medication compliance of UC patients including drug affect,economic burden,living events,follow-up appointment and stigma.Conclusions The medication compliance of UC patients is mainly related to patients lacking of drugs knowledge,heavy burden on drugs cost and shortage of social support etc.The health education and medication supervision on UC patients should be strengthened and the social health insurance system on UC needed to be improved.
5.Effect of Tongsaimai Tablet on experimental atherosclerosis in rats

AIM:To investigate the effect of TSM tablet on rat's atherosclerotic model's endothelial cell and explore the mechanism of it.METHODS:Fifty Wistar rats were randomly assigned into five groups:control group,model group,Simvastatin group,TSM high group,TSM low group.All groups were fed with high fat diet and vitamin D3 except for control group to set up atherosclerosis model.After 12 weeks we detected circle endothelial cell,angiotensinⅡ and electron microscopy morphology of arteries.RESULTS:Level of circulating enthelia cell(CEC) and AngⅡin model rats were significantly higher.TSM can reduce level of the CEC and AngⅡ.Model group rat's artery endothelial cells were severely damaged under electron microscopy;rat's artery endothelial cellsin TSM group were basically intact and its internal elastic membrane was unbroken on it thickness was even,without significant lesion.CONCLUSION:TSM by reducing the number of CEC,the level of AngⅡ used for experimental atherosclerotic endothelial cell in rats has the protective effect.
6.The effects of fluvastatin on the prevention of heart muscle ischemia reperfusion injury in rabbits

Objective To investigate the effects and mechanism of different dosage of fluvastatin on the prevention of heart muscle ischemia reperfusion injury in rabbits.Method Thirty-five rabbits were randomly divided into 5 groups with 7 rabbits in each: sham group,myocardial ischemia reperfusion control group,low dosage of fluvastatin pretreatment group (2 mg/kg,Group F1),middle dosage of fluvastatin pretreatment group(5 mg/kg,Group F2) and large dosage of fluvastatin pretreatment group(20 mg/kg,Group F3).The left ventricular systolic pressure(LVSP),the max rate of rise of left ventricular pressure(?dp/dt_(-max)) and left ventricular end-diastolic pressure(LVEDP) were detected during the experiment.At the end of reperfusion,the infarct size and area at risk were defined by Evans blue and TTC staining,and the levels of myocardial nitrogen monoxidum(NO) and nitricoxide synthase(NOS) were measured.Result Compared with the ischemia reperfusion group,the indexes of heart function improved significantly,the level of myocardial NO was increased significantly and the myocardial infarct size was decreased significantly in the groups F2 and F3.There was no significant difference between the group F1 and ischemia reperfusion group.Conclusion Fluvastatin exerts a cardioprotective effect against myocardial ischemia reperfusion injury in rabbits.NO is likely involved in this protective mechanism.
7.Aspirin inhibits tumor cell metastasis mediated by HGF/c-Met

In this study, we investigated the effect of aspirin on tumor biological effects mediated by hepatocyte growth factor/cellular-mesenchymal-epithelial transition factor (HGF/c-Met) axis, and preliminarily explored the molecular mechanism of inhibiting tumor metastasis by aspirin. The binding of aspirin to c-Met was predicted by molecular docking; cellular thermal shift assay (CETSA) was used to verify the binding of aspirin to c-Met at the cellular level. The inhibitory effect of aspirin on c-Met kinase was detected by kinase activity; Western blot, cell scattering test, cell branching morphogenesis and Transwell test were used to evaluate the cell signal transduction, morphological changes and migration and invasion ability. The results showed that aspirin could effectively inhibit the kinase activity of c-Met with a half inhibitory concentration of 0.95 mmol·L-1. The results of docking showed that aspirin could bind to the ATP pocket of c-Met protein, and the main binding sites were Tyr1230, Tyr1159 and Met1229. The CETSA test also showed that aspirin could form binding complex with c-Met protein. Western blot results showed that aspirin could inhibit the up-regulation of phosphorylated Met stimulated by HGF in a concentration-dependent manner. The results of cell scattering test showed that aspirin could block HGF/c-Met promoted cell scattering in a concentration dependent manner. Aspirin could almost completely block the biological function mediated by c-Met activation at the concentration of 4 mmol·L-1, and this effect was independent of HGF. Similarly, the results of MDCK cell branching morphogenesis experiment showed that aspirin could inhibit HGF/c-Met mediated invasive growth in a concentration dependent manner. The results of Transwell test showed that aspirin could block HGF/c-Met mediated cell migration and invasion in a concentration-dependent manner. Aspirin could almost completely block the biological function mediated by c-Met activation at the concentration of 4 mmol·L-1, and this effect was independent of HGF. The above results indicate that aspirin can bind to c-Met, thereby blocking the biological effects mediated by HGF/c-Met, and inhibiting tumor metastasis. This study revealed the new biological function of aspirin, and provided a new theoretical basis for a comprehensive understanding of the anti-metastatic effect of aspirin.
8.Pathological and immunopathological changes of skeletal muscle microvessels in dermatomyositis and polymyositis

Objective To assess pathological features of muscles as well as microvascular changes between muscle fibers in patients with dermatomyositis (DM),and to analyze pathological differences in muscles between DM and polymyositis (PM).Methods Specimens were obtained from involved muscles of 16 patients with DM,5 patients with PM,and from normal muscles of 9 patients with bone trauma (controls).Routine histopathological examination and immunohistochemical staining for CD34 and CD61 were conducted.Results Of the 16 patients with DM,6 (37.5%) had perifascicular atrophy,3 ( 18.8 %) had an obvious inflammatory cell infiltration around microvessels between muscle fibers.Perifascicular atrophy was absent in muscle specimens from patients with PM or bone trauma.The number of CD34-positive microvessels between muscle fibers was reduced in patients with DM,but normal in those with PM.CD61 was positive in perifascicular area of 10 patients (5 moderately positive and 5 weakly positive) with DM,with an expression rate of 62.5%,however,only 1 case of PM was weakly positive for CD61,and all the controls were negative.Conclusions There is a decrease in the number of microvessels but an enhancement of neovascularization between muscle fibers in involved muscles of patients with DM,which may serve as a pathological marker to distinguish DM from PM.
9.Medical students' attitude toward living liver donation in China

BACKGROUND: A better understanding of medical students' attitude towards living liver donation in China would be of importance significance in the development of organ transplantation. OBJECTIVE: To investigate the medical students' attitude toward living liver donation in China and to analyze the factors that affect such an attitude.METHODS: A random sample of 250 medical students was taken and stratified by grades with 50 students in each grade. The attitude was evaluated by a modified psychosocial questionnaire from Spain. The questionnaire was completed anonymously and was self-administered. A bivariate analysis including the Student's t test and the Chi-square test was performed using SPSS software. RESULTS AND CONCLUSION: The questionnaire completion rate was 97.2% (n=243) for the medical students. Among the respondents, only 13.2% (n=32) of them were in favor of unrelated living liver donation. Another 66.3% (n=161) were only in favor if the donation was for a relative. Of the rest, 6.6% (n=16) did not agree with living liver donation, and the remaining 14.0% (n=34) were undecided. The factors related to this attitude were the decision of donating the organs of a family member (P=0.002), attitude toward deceased donation (P=0.000), a willingness to receive a donated living liver organ if one were needed (P=0.000), attitude toward living kidney donation (P=0.000). The medical students have a favorable attitude toward living liver donation in China. The students' attitude toward other types of organ donation and the willingness to accept living donated liver influence their attitude toward living liver donation. There is a great lack of education about the organ donation and transplantation for the medical students in China and more education programs should be incorporated into their traditional curriculum.
