ObjectiveTo investigate the changes in stroke volume variation (SVV) monitored by FloTrac/Vigileo system during mechanical ventilation in patients undergoing pulmonary lobectomy.MethodsForty-four ASA Ⅰ or Ⅱ patients aged 44-64 yr weighing 47-86 kg undergoing elective pulmonary lobectomy performed under general anesthesia were studied.Blood volume was maintained by fluid (crystalloid∶ colloid 1∶1) infusion at a rate of 6-8 ml·kg-1 ·h-1.Cardiac output index (CI),stroke volume index (SVI) and SVV were measured based on arterial pressure wave form analysis by FloTrac/Vigileo System (Edwards Co.,USA) and recorded at following time points:at 5 min of two-lung ventilation (TLV) in supine position,2 min TLV in lateral position,during one-lung ventilation (OLV) before thoracotomy,at 5 and 30 min of OLV after thoracotomy,1 and 15 min OLV + PEEP of 5cm H2 O,before and immediately and 1 min after reflation of the remaining lobes.The normal value for SVV is less than 13%.ResultsThe hemodynamic parameters were stable during lobectomy.CI and SVI were within normal range.SVV was less than 13% at all time points except that at immediately after reflation of the remaining lobes.ConclusionSVV obtained with FloTrac/Vigileo system can be used to guide fluid therapy during OLV in mechanically ventilated patients undergoing pulmonary lobectomy.
3.Clinical Application Value of Magnetic Susceptibility-weighted Imaging in Cerebral Vascular Malformations

Objective To assess the clinical application value of magnetic susceptibility-weighted imaging in cerebral vascular malformations. Methods Thirty-six patients with cerebral vascular malformations were enrolled in the study. All the patients received examination of T1WI, T2WI and SWI. SWI sequence was compared with T1WI, T2WI sequence in detecting and diagnosing cerebral vascular malformations. Results Cerebral vascular malformations were low signal in SWI sequence. Cavernous angioma, developmental venous anomalies and AVM had been found in 73,8,10 cases, respectively. The bounder of lesions was clearer, more multiple small lesions, more hair-thin pulp veins and draining veins were detected in SWI sequence. Conclusion SWI sequence has more sensitivity than T1WI and T2WI sequence in detecting the cerebral-vascular malformations. SWI sequence may become routine examination se- quence in diagnosis cerebral-vascular malformations.
4.Cleaning Quality of Reusable Puncture Needles:Experimental Studies

OBJECTIVE To probe cleaning methods of reusable puncture needles and improve the quality of cleaning. METHODS The 200 sets of puncture needles with blood were divided into two groups (groups A and B). Ultrasonic cleaning was used in two groups,but in group A was with 3% hydrogen peroxide solution and in group B with 1∶150 low foam multi-enzyme cleaning liquid for 20 minutes,after rinsing,the group A used 95% ethanol 2 ml/piece for dry,but the Group B used 90 ℃ 30 min,and then the bacterial endo toxins and 3% hydrogen peroxide tests,the cost of cleaning was compared. 5.Implementation of "Five Regular Methods" Management in Hospital′s Sterilization Supply Center

OBJECTIVE To improve the management quality by discussing the usage of "Five Regular Methods" in hospital′s Sterilization Supply Center.METHODS The "Five Regular Methods" managment model into the following aspects of quality,service,safety,efficiency,people′s competency and environment was introduced.RESULTS After 6 months implementation of the model the efficiency and quality of service improved significantly.The environment became clean and shiny.Objects were put on their right place. And we realized the right integration in time and space for people,objects and places.CONCLUSIONS The "Five Regular Methods" is a set of perfect guidance for quality management.It is a management system by providing guidance from inside to outside,people to objects,soft ware to hard ware,theory to practice and rules to process.It is a good method for enhancing the service quality from all aspects.
6.Difficulties of caregivers on nursing for elderly patients

Objective To investigate the caregivers' difficulty in taking care of the elderly and the related factors. Method A self-designed questionnaire was used to investigate the difficulties in care giving among 1,517 caregivers. Results The total score on the difficulties was (16.16 ±5.79). The preventive factors were good care-related knowledge and family income and the risk factors were advanced age, poor health condition of the patients ( all P<0.01). Conclusions The difficulty in caring the elderly are varied. We should attach importance to those caregivers of advanced age, with lower income and lack of care-related knowledge. Meanwhile, we should provide consultation, education and training for caregivers according to their different situation, and carry out targeted and multi-level support services so as to solve their problems in care to the patients.

8.Simultaneous Determination of Baicalin,Baicalein and Wogonin in Huatan Pingchuan Tablet by HPLC

OBJECTIVE:To establish a method for the simultaneous determination of baicalin,baicalein and wogonin in Hua-tan pingchuan tablet. METHODS:HPLC was performed on the column was Diamonsil C18 with mobile phase of methanol-0.1%phosphoric acid(gradient elution)at a flow rate of 1.0 ml/min,detection wavelength was 277 nm,column temperature was 30 ℃, and the injection volume was 10 μl. RESULTS:The linear range was 0.069 04-0.690 4 μg for baicalin(r=0.999 4),0.054 56-0.5456 μg for baicalein(r=0.999 6)and 0.030 36-0.303 6 μg for wogonin(r=0.999 1);RSDs of precision,stability and reproduc-ibility tests were lower than 2%;recoveries were 97.64%-99.06%(RSD=0.57%,n=6),98.31%-100.64%(RSD=0.85%,n=6) and 97.53%-99.48%(RSD=0.76%,n=6). CONCLUSIONS:The method is simple,stable and reproducible,and can be used for the contents determination of baicalin,baicalein and wogonin in Huatan pingchuan tablet.
9.Value of superselective intraarterial infusion chemotherapy and embolization in advanced cervical carcinoma patients before surgery

Objective To investigate the clinical effect of superselective arterial infusion chemotherapy(SACT) and embolization in advanced cervical carcinoma patients before surgery. Methods 26 previously untreated patients with stage ⅡA-Ⅳ cervical carcinoma admitted from January 2004 to March 2008 were enrolled into the study, patients underwent SACT of the internal iliac artery(dose of the drug: CBP 80 rag, VCR 2 rag, MMC 16 mg) by Seldinger technique in the surveillance of DSA, the obvious branches of the tumor blood supply were embolized by gelfoam particles. Size of tumor before and after chemotherapy was measured by gynecological examination combined with B ultrasonic examination and CT, then calculated the percentage of tumor regression. Radical hysterectomy was administered one month after chemotherapy. Results 24 patients were evaluated as PR, the reaction rate of chemotherapy was 92.3 %. Conclusion SACT before surgery can effectively reduce tumor volume, decrease lymphatic metastasis and subclinical dissemination and enhance the opportunity of radical hysterectomy, and to improve the quality of life and OS of the advanced cervical carcinoma patients.
10.A porous tantalum uncemented acetabular cup in acetabular revision arthroplasty

BACKGROUND: Biological fixation refers to the treatment of coarse or porous prosthetic surface. It is favorable to "bone ingrowth" prosthesis to achieve long-term stability. Porous tantalum is the latest scientific product that appears suitable to prosthetic surface owing to its porous feature and has been attracting a great deal attention.OBJECTIVE: To investigate the therapeutic efficacy of a porous tantalum uncemented acetabular cup in acetabular revision arthroplasty.DESIGN, TIME AND SETTING: A retrospective case analysis was performed at the Department of Orthopedics, Changhai Hospital, Second Military Medical University of Chinese PLA between April and November 2006.PARTICIPANTS/MATERIALS: Sixteen patients (16 hips), 7 males and 9 females, aged 54-81 years old, who received treatment at the Department of Orthopedics, Changhai Hospital, Second Military Medical University of Chinese PLA were included in this study. Of these patients, 2 suffered from femoral neck fracture, 6 from aseptic femoral head necrosis, and 8 from osteoarthritis. Trabecular metal cup (Zimmer, Warsaw, Indiana) provided a titanium alloy bottom layer and porous tantalum-coated surface, which was realized by technical combination of bone trabecular tantalum and titanium alloy. The prosthesis contained an ultrahigh modulus polyethylene lining.METHODS: All patients underwent acetabular revision with modular porous tantalum uncemented acetabular cup. Prior to replacement, acetabular defects and femoral prosthesis were evaluated. A posterolateral approach of hip joint was made. Following acatabular prosthesis removal, bone cement was cleared with osteotome, simultaneously scar tissue, granulation tissue, and fibrous limiting membrane in the acetabular fossa were erased with curette to avoid further damage to sclerotin. Bacteriological examination and susceptibility test with respect to intraarticular tissue fluid confirmed aseptic loosening. Antibiotics were used in each patient to prevent infection. Antibiotics and low molecular heparin were used afterwards to prevent infection and thromboembolism, respectively. Patients could not load within 4 weeks but could walk with the aid of two canes within subsequent 6 weeks. One week after surgery, plain X-ray films were taken in the lateral plane.MAIN OUTCOME MEASURES: Prior to and 6, 12, and 24 months after surgery, Harris hip score and University of California Los Angeles (UCLA) activity score were assessed. The position of acetabular prosthesis was observed through an anteropostedor pelvic radiograph.RESULTS: The average follow-up time was 24 months (range 18-25 months). Harris hip score significantly increased from 32.6 points (range 21-54 points) prior to revision to 87.5 points (range 56-90 points) after revision (P < 0.05). UCLA activity score significantly increased from 3.8 points (2-5 points) prior to revision to 8.2 points (range 6-9 points) after revision (P<0.05). The anteropostedor pelvic radiographs taken in the last follow-up displayed no phenomena regarding non-stability including prosthesis aversion and subsidence and revealed that the in-growth of sclerotin surrounding the bone trabecular tantalum acetabular cup. Postoperative complications were found in none of 16 patients. By the last follow-up, there had been 14 cases who did not complain of pains while walking and 2 cases that had slight pain and slight limp while walking. Deep venous thrombosis or nerve injury did not appear afterwards. No cases needed another acetabular revision arthroplasty.CONCLUSION: For cases of failed primary fixation of artificial acetabular cup, application of porous tantalum uncemented acetabular cup can produces favorable results in acetabular revision if no bone defects exist.
