1.Meta analysis of nasal surgery alone for obstructive sleep apnea hypopnea syndrome.

OBJECTIVE:
To investigate the effect of nasal surgery alone for OSAHS using Meta-analysis.
METHOD:
The published articles were searched from PubMed, Chinese Biomedical Literature CD-ROM databases, China Academic Journals Full-text database. The quality of the searched original studies were assessed and the outcomes with the requirements were analyzed.
RESULT:
Nineteen articles and 621 cases were finally included, including 15 English literatures and 4 Chinese literatures. The results of Meta analysis showed that: (1)The weighted mean between pre-and postoperative AHI was no significantly difference [weighted mean difference (WMD) = 0. 11, 95% CI (-0. 02,0. 23), P>0. 05]. (2) The nasal surgery alone significantly reduced theEpworth sleepiness scale scores in thirteen studies [WMD=0. 94,95% CI (0. 62,1. 26), P<. 01].
CONCLUSION
The meta-analyses show that nasal surgery alone can effectively improve the subjective clinical symptoms of patients with OSAHS. However, nasal surgery alone has a limited efficacy in treating OSAHS.

2.Therapeutic observation on lung-clearing and spleen-strengthening tuina in children with exogenous cough

Objective: To observe the clinical efficacy of lung-clearing and spleen-strengthening tuina on exogenous cough in children. Methods: A total of 77 children with exogenous cough that met the inclusion criteria were randomly divided into a treatment group and a control group according to the random number table, with 39 cases in the treatment group and 38 cases in the control group. The routine lung-clearing manipulation was used in the two groups, while the spleen- strengthening manipulation was added in the treatment group. Both groups were treated once a day, 5 times as a course of treatment. After one course of treatment, the symptom score and clinical efficacy were observed. Results: There were one dropout in the treatment group and 2 dropouts in the control group. Therefore, a total of 74 cases were finally included in the analysis, with 38 cases in the treatment group and 36 cases in the control group. After treatment, there was a statistically significant difference in cough score between the two groups (P<0.05). There was a statistically significant difference in the score of poor appetite between the two groups (P<0.05). After treatment, the total effective rate was 89.5% in the treatment group versus 75.0% in the control group, showing a statistically significant difference (P<0.05). Conclusion: The therapeutic efficacy of lung-clearing spleen-strengthening manipulation is better than that of lung-clearing tuina manipulation alone for children with exogenous cough; regulating spleen and stomach can improve the curative efficacy of exogenous cough in children.
3.Patent applications in hospitals of Guangdong Province since 2000

The relation between patent applications and published papers in hospitals of Guangdong Province since 2000 on China Intellectual Property Network was analyzed , which showed that the quality and quantity of patents were lower in hospitals of Guangdong Province than in those of Shanghai and Beijing , the quality of patents was re-lated with the medical service level in hospitals, the patent applications were centralized in large class A hospitals of Guangzhou , and inventive patents were those of medical preparations .Suggestions were thus put forward for hos-pitals of Guangdong Province to improve their awareness of patent application, the quality of patents, transformation of patents, cooperation with patent service organizations, research of patent application, and balance of patent dis-tribution in different areas.
4.Effects of traditional Chinese medicine for invigorating spleen to resolve dampness and dredging collaterals on patients with rheumatoid arthritis and anemia

OBJECTIVE: To observe the clinical effects of Xinfeng Capsules (XFC), a traditional Chinese medicine for invigorating the spleen to resolve dampness and dredging collaterals, on patients with rheumatoid arthritis (RA) and anemia, and to investigate its mechanism. METHODS: Forty patients with RA and anemia were divided into three groups: XFC-treated group (n=20), Tripterygium glycosides Tablets (TGT)-treated group (n=10) and methotrexate (MTX)-treated group (n=10). The patients in each group took corresponding medicine for three months. The response rates of the three groups were evaluated after treatment. The general symptoms and specific signs and symptoms of RA were observed before and after the treatment. The indexes of blood routine examination and some other laboratory indexes such as erythrocyte sedimentation rate (ESR), and blood levels of rheumatoid factor (RF), iron, C-reaction protein (CRP), immunoglobins (Ig), complements 3 and 4, tumor necrosis factor-alpha (TNF-alpha), interleukin-10 (IL-10) and erythropoietin (EPO) were all examined and compared before and after treatment. RESULTS: The total response rate in the XFC-treated group was similar to those both in the TGT-treated and MTX-treated groups. The effects on relieving specific symptoms of RA in the three groups were also similar. The ESR and serum levels of RF, CRP, IgG, IgA and IgM were all decreased after treatment as compared with those before treatment in the three groups, and there were no significant differences among those laboratory indexes in the three groups after treatment. The XFC displayed more obvious effects on improving the general symptoms of patients with RA and anemia, increasing the blood levels of hemoglobin, iron and IL-10, while decreasing the serum level of TNF-alpha and regulating the serum EPO level, as compared with those in the TGT-treated and MTX-treated groups. CONCLUSION: The XFC for invigorating the spleen to resolve dampness and dredging collaterals was developed on the basic principle of regulating spleen. It can obviously improve the symptoms and laboratory indicators of RA. Such effects may be related to maintaining the balance of cytokines, regulating the serum level of EPO and increasing the serum iron level in patients with RA and anemia.
6.Advances in Study on Helminths in Treatment of Inflammatory Bowel Disease

Inflammatory bowel disease (IBD) is a group of intestinal chronic non-specific inflammatory disease with unknown etiology, and includes ulcerative colitis (UC) and Crohn''s disease (CD).Studies have indicated that environmental factor, genetic factor, disorder of immunity are associated with the pathogenesis of IBD.More and more basic and clinical studies focus on treatment with helminths in IBD.The accumulated evidences have shown that helminths infection has beneficial effects on the alleviation of IBD and is relatively safe.This article reviewed the advances in study on helminths in the treatment of IBD.
7.Effect of Acupuncture Combined with Herb Decoction on Nerve Cell of Focal Brain Ischemia Rats

Objective To study the mechanism of acupuncture combined with herb decoction for brain occlusion. Methods one-side middle cerebral artery occlusion (McAo) rat by ehermoregulation was used as focal cerebral ischemia experimental animal. All animals were divided into four groups as model group, acupuncture group, herb decoction group, acupuncture combined with herb decoction group. And the dynamic changes of c-fos protein in ischemia region of brain tissue after treatment were observed. Results All of acupuncture group, herb decoction group and combining group could enhance the expression of c-fos protein in all stages of ischemia, and combining group was enhanced significantly. Conclusion Compared with herb decoction or acupuncture, the method of acupuncture combined with herb decoction could improved the stress ability of nerve cells of acute brain ischemia by enhance the content of c-fos protein significantly.
8.Delayed Bile Leakage after Laparoscopic Cholecystectomy:Report of 12 Cases

Objective To Discuss the causes,prevention and treatment of delayed bile leakage following laparoscopic cholecystectomy (LC). Methods From November 1994 to December 2007,totally 11 000 cases of LC were performed in our hospital,12 of them developed delayed bile leakage after the procedure. The mechanism,characteristics,treatment,and prognosis of the disease were analyzed retrospectively. Results Among the twelve cases of delayed bile leakage,eight cases showed perforation of the bile duct caused by thermoelectric effect,two cases had leakage of the aberrant bile-duct,and two cases were injured by unknown reasons. B-ultrasonography-guided drainage was performed in four of the patients,laparoscopic or open repair combined with T-tube drainage were carried out in three,abdominal puncture combined with endoscopic nasobiliary drainage (ENBD) was done in three,abdominal puncture with endoscopic retrograde biliary drainage (ERBD) was made in one,and open repair and bilio-jejunal anastomosis was performed in one. All patients were cured and discharged in 8 to 21 days with a mean of 15 days. No intra-abdominal infection,hemorrhage,recurrent bile leakage or intestinal leakage occurred after the operations. Re-examination by B-ultrasonography or cholangiopancreatography performed ten days after the drainage showed no stricture or dilation of the bile duct,and contrast media drained out fluently. The 12 cases were followed up for one year,none of them showed biliary stenosis or infections during the period. Conclusions Post-LC injury of the bile duct is mostly caused by thermoelectric effect. Gentle operation and separation strictly following the anatomical levels are the keys in prevention of postoperative delayed bile leakage; while endoscopy and interventional treatment are main methods for the diagnosis and treatment of the disease.
9.Purification and cytokine profile of autologous conditioned serum

BACKGROUND:Autologous conditioned serum (ACS) is a new and safe biological therapy method mainly used for osteoarthritis, rheumatoid arthritis and other similar diseases. OBJECTIVE:To explore the purification method and cytokine profile of ACS. METHODS:Blood obtained equipment (containing 3 mm of glass beads) was used to collect peripheral blood samples that were mixed uniformly and then placed in a 37℃ thermostatic incubator for 24 hours, finally filtered to obtain ACS. The quantitative determination of interleukin (IL)-1Ra, IL-6, IL-8, and IL-1β was conducted, and the 36 kinds of cytokines were semi-quantitatively detected by cytokine antibody array. RESULTS AND CONCLUSION: The levels of various cytokines in ACS were obviously increased, among which, IL-1Ra level (important for alleviating osteoarthritis) was increased from 140.25 ng/L to 1125 ng/L(increased by above 8 times); IL-6 level was increased from 389.5 ng/L to 2802 ng/L; IL-8 level was not detected firstly and finally increased to 2822 ng/L. Cytokine antibody array results showed that at least 10 kinds of cytokines in ACS were changed, and most of cytokines were on a rise, and few in decline. These results indicate that the ACS contains IL-1Ra and a variety of cytokines, and some chemokines in the ACS recruit more immune cells to the inflammation sites and accelerate the inflammatory process, further exerting therapeutic effects.
10.Prognostic value of CEA and CA19-9 expression in peripheral blood of patients with advanced gastric cancer

Objective The aims of this study were to investigate the prognostic value of CEA and CA19-9 expression in advanced gastric cancer and the prognostic factors of advanced gastric cancer.Methods A total of 255 patients with advanced gastric cancer who underwent radical R0 radical surgery were enrolled in the Department of Gastrointestinal Surgery,Harbin Medical University Cancer Hospital from January 2010 to December 2010,and were divided into group A(CEA<2.19ng/mL and CA19-9<10.78U/mL),group B(CEA≥2.19ng/mL or CA19-9≥10.78U/mL)and group C(CEA≥2.19ng/mL and CA19-9≥10.78U/mL).These groups were analyzed the differences in gastric cancer patients with clinical pathology and survival,and prognostic factors of patients with advanced gastric cancer factors.Results There were statistically significant differences in clinical N staging,clinical TNM staging and pathological TNM staging in three groups of patients with advanced gastric cancer(P<0.05).There was no significant difference in the median survival time in three groups of patients with advanced gastric cancer(P>0.05).The 1-year,3-year and 5-year survival rates of group A were higher than those of group B and group C(P>0.05).The factors affecting the prognosis of gastric cancer included the degree of tumor differentiation,pathological T staging,pathological TNM staging,the total number of lymph nodes,the number of positive lymph nodes and lymph node metastasis.Conclusion Combined detection of CEA and CA19-9 expression indicates a certain reference value for prognosis of gastric cancer.
