2.The clinical analysis of long term treatment and management of 122 asthmatic patients

Objective To explore a brief regimen to treat and manage asthmatic patients.Methods 122 asthmatic patients(including 12 inpatients) treated and managed at respiratory clinic were followed up for 1~10 years.The data of these patients were collected and analysed Results On the base of asthma education patients were received inhaled glucocorticoid combined with ? 2 agonist and/or theophylline for two to five years Of 122 patients,20(16 4%) patients remised and remained good control without any medication,90(73 8%) patients relapsed within 1 to 2 years after drug withdrawal Conclusions Asthmatic patients must be treated and managed regularly according to GINA It was especially emphasized that inhaled glucocorticoid is the mainstay of ling term threapy Most patients can remise for quite a long time with agreeable quality of life while a few patients can be cured
3.Cluster Analysis of Syndrome Patterns of Osteoporosis in Traditional Chinese Medicine

【Objective】 The characteristics of syndrome patterns of osteoporosis(OP) in traditional Chinese medicine (TCM) were investigated by cluster analysis,thus to supply evidences for the prevention and treatment of OP.【Methods】The data of 246 OP patients collected by four diagnostic methods were classified into items by cluster analysis.Thenthe principal component analysis was usedtosummarizethe TCMsyndrome patters of OPbyanalyzing the representativeness of eachitem.【Results】Forty-sevenitems were obtained after cluster analysis,andtheitems of short of breath withreluctancetotalk,lassitude and weakness,soreness of thelower back,white coating,vertigo,pale and enlargedtongue had a higher incidences.Whenthe items of OP were classified into four patterns of deficiency of liver and kidney yin,deficiency of spleen and kidney yang,deficiency of Qi and blood,and Qi stagnation and blood stasis,the patterns distribution was clear and had a good representiveness.The patterns of deficiency of liver and kidney yin,and deficiency of spleen and kidney yang had higher incidences of 34.7 %and 30.1 %respectively.【Conclusion】OP classifiedintofour patterns of deficiency of liver and kidney yin,deficiency of spleen and kidney yang,deficiency of Qi and blood,and Qi stagnation and blood stasis is practical in clinic,and most of OPpatients have the manifestations of kidney deficiency.
4.Pathological characteristics of and human papillomavirus(HPV)genotype profile in subclinical HPV infection in patients with condyloma acuminatum

Objective To investigate the clinicopathologic characteristics of and HPV subtypes in vulvar condyloma acuminatum(CA)and subclinical HPV infection.Methods Eighty patients with a positive acetowhite test and suspected subclinical HPV infection were selected from 272 patients with typical CA lesions in perianal and external genital region.Tissue specimens were obtained from typical CA and suspected subclinical HPV-infected lesions followed by pathological examination and HPV-DNA detection.Finally,71 patients were confirmed to suffer from both CA and subclinical HPV infection.A comparative analysis was performed to assess the differences in histopathological manifestation and HPV genotypes between CA and subclinical infection lesions.Results Pathological examination revealed typical histological changes of CA in 71(88.75%)typical CA specimens and 4(5%)suspected subclinical infection specimens,as well as squamous dysplasia in 9(11.25%)CA specimens and 71(88.75%)suspected subclinical infection specimens.HPV-DNA was positive in all(80)of the CA specimens and 93.75%(75)of the suspected subclinical infection specimens,negative in 5(6.25%)suspected subclinical infection specimens.Obvious differences were observed in pathological manifestation,koilocyte number(P < 0.05),but not in the distribution of HPV subtypes(P > 0.05),between typical CA and concurrent subclinical HPV infection lesions in patients.Conclusion The diagnosis of subclinical HPV infection should be based on the result of acetowhite test,with the results of pathological examination and HPV DNA detection as an adjuvant.
5.Experimental studies of the long-term effect of La(NO3)3 on the gastric mucosa in rats

Objective:To probe into the long-term effect of La(NO3)3 fed orally on gast ric mucosa of rats.Methods:The gastric mucosa of rats was studied after La(NO3)3 was administe red orally for a long-term,by the use of ordinary histological technology and m orphometry.Results:In the groups of 20 mg·kg-1 and 10 mg·kg-1,the cytopl asma of more parietal cells lying in the top of the gastric gland was loose.Proporti on of cell types of the gastric gland was related to the doses. The acid mucus l evel of the mucous neck cells of male rats in the groups of 20 mg·kg-1 an d 10 m·kg-1 decreased. In the groups of 2 mg2kg-1, 0.2 mg·kg -1 and 0.1 mg*kg-1,the acid mucus level of the mucous neck cells of both male and female rats increased.Conclusion:La(NO3)3 fed orally for a long-term could injure gastric muc osa in higher dose (20 mg·kg-1,10 mg·kg-1) but promote the protect ive action of the gastric mucosa in lower dose (0.2 mg·kg-1,0.1 mg·kg- 1).
6.Determination of floxuridine in plasma by HPLC

Purpose The aim is to establish the HPLC method for the determination of Fluoro-deoxyuridine in plasma.Methods The Chromatography conditions include: Chromatography column: Nova-pak C18(3.9mm×150mm,4μm), mobile phase: 0.05mol/L sodium phosphate monobasic -methanol-water(0.5∶7∶92.5), UV detection at 260nm, FUDR was extracted with ethyl acetate. Results The average recoveries were 96.4%,96.5%,97.8% for concentration 0.23、1.67、20.0μg/ml (n=5).The corresponding reproducibility were RSD 1.61%, 1.98%, 3.17% respectwely for iner-day and RSD 3.56%, 1.90%, 2.63% for the intra-day(n=5). The FUDR concentration was linear with a correlation coefficient of 0.999 4 over the range of 0.099~20.0μg/ml. Conclusion The method was sensitive and accurate and suitable for pharmacokinetics and bioavailability study of FUDR.

Objective To give an empirical study on the interests claims of employees in public hospitals. Methods By questionnaire investigation, to sum up data using factor analysis and pairedsamples T test, and to compare difference between different kinds of patients using ANOVA. Results The interests claims of employees can be summed up to seven factors and there is some significant difference between different kinds of employees. Conclusion At current stage, public hospitals need pay more attention to material interests claims of employees.
8.Influence to early postoperative quadriceps strength of minimally invasive and conventional approaches in total knee arthroplasty: a prospective randomized controlled

Objective To compare the quadriceps strength and short-term outcome in patients who had undergone primary total knee arthroplasty (TKA) with minimally invasive (MIS) or conventional surgical technique.Methods Sixty-eight knees undergoing TKA were evaluated and they were randomly divided into two groups.One group included 34 knees which were undergone conventional approach,and the other group included 34 knees undergone MIS approach.The age,BMI,quadriceps strength,extension lag,American Knee Society Score (KSS),Western Ontario and McMaster Universities (WOMAC) osteoarthritis index and visual analogue scale (VAS) of pain of all patients were evaluated at the day before surgery and the 1st,2nd,3rd,6th,and 12th months after surgery.Results At the 1st,2nd,3rd,6th,and 12th months after surgery,the quadriceps strength in MIS group was 0.59±0.12,0.74±0.18,0.86±0.16,1.02±0.17,1.05±0.04 N·m/kg respectively,while in conventional group was 0.46± 0.56,0.56±0.12,0.70±0.16,0.94±0.19,1.04±0.03 N· m/kg respectively.The extension lag in MIS group was 13.11°±6.56°,8.53°± 5.12°,4.79°±3.62°,1.53°±2.64°,1.62°±1.76° respectively,while in conventional group was 22.47°±8.41°,16.23°±6.26°,10.58°± 4.4°,2.58°±2.85°,1.62°±1.76° respectively.There were significantly differences in the two groups.Meanwhile,compared with conventional patients,MIS patients demonstrated significantly higher KSS scores at the 1st and 2nd months postoperatively.WOMAC scores were significantly lower in MIS group than in conventional group at the 1st month after surgery.During the first postoperative week,MIS technique significantly decreased VAS scores,showing significance.On the other hand,no significant difference was observed with respect to deep infection incidence and radiological outcome between two groups.Conclusion MIS TKA offers a significant improvement in extensor muscle strength over conventional surgery.These results suggest that the MIS approach results in better outcomes with regard to maintaining extensor strength than the conventional surgical approach.
9.Regulatory effects of CD44 antibody-A3 D8 on IL-3 Rαand downstream PI3 K/Akt pathway in NB4 cells

Aim To investigate the effect of CD44 anti-body-A3 D8 on the expression of IL-3 Rα and down-stream PI3K/Akt in NB4 cells. Methods The ex-pression of IL-3 Rα mRNA was detected by real-time quantitative RT-PCR, the IL-3Rα protein expression and changes of PI3 K/Akt signal pathway in NB4 cells treated with A3D8 were analyzed by Western blot. An-nexin-V-FITC/PI double staining flow cytometry was u-tilized to detect the apoptotic cells. The inhibitor of PI3 K/Akt signaling LY294002 combined with A3 D8 was used to inhibit the PI3K/Akt in NB4 cells. Re-sults After treated with A3 D8 , both the transcription-al level and translational level of IL-3 Rα were remark-ably reduced, and the PI3K/Akt pathway was inhibi-ted. LY294002 improved the inhibitory and apoptotic effects of A3D8 on NB4 cells. Conclusion CD44 antibody A3 D8 can downregulate the expression of IL-3Rα and inhibit the downstream PI3K/Akt pathway.
10.Risk factors influencing short - term prognosis of acute central nervous system viral infection in children

Objective To study the risk factors for children with acute central nervous system(CNS)viral in-fection,so that pediatrician may identify children with poor prognosis at early stages of the disease,and provide them with a theoretical basis for clinical treatment. Methods The clinical data of a cohort patients of acute CNS viral infec-tion who were hospitalized at the First Affiliated Hospital of Fujian Medical University between January 2010 and June 2013 were retrospectively collected and analyzed. According to Glasgow outcome scale on discharge,children were di-vided into good prognosis group and poor prognosis group. Clinical data and outcomes were analyzed by using univariate analysis and binary Logistic regression multivariate analysis. Results Three hundred and one cases were enrolled,278 (92. 36% )patients were assigned to the good prognosis group,and 23(7. 64% )patients were assigned to the poor prognosis group. By univariate analysis,the patients in the poor prognosis group had longer duration of sickness before admission,longer time of fever,lower white blood cell count in cerebrospinal fluid,a relatively lower calcium level,con-scious disturbance at the early stage,multiple seizures,convulsive status epilepticus,meningeal irritation sign,muscle weakness,severe changes in electroencephalogram(EEG),and abnormal neuroimaging findings(computed tomography or magnetic resonance imaging,or both)had significant differences between the good prognosis group and the poor short - term outcome groups(all P < 0. 05). By binary Logistic regression multivariate analysis,factors indicating a poor prognosis during the early stage were conscious disturbance at the early stage(0R = 4. 885,95% CI:1. 523 - 15. 670, P = 0. 008),multiple seizures(0R = 6. 352,95% CI:1. 905 - 21. 178,P = 0. 003),severe changes in EEG( 0R =4. 269,95% CI:1. 708 - 10. 666,P = 0. 002),and abnormal neuroimaging findings( 0R = 9. 740,95% CI:2. 360 -40. 192,P = 0. 002). Conclusions Conscious disturbance at the early stage,multiple seizures,severe changes in EEG and abnormal neuroimaging findings are risk factors for acute viral infection of CNS in children.
