The etiology and pathogenesis of intrahepatic cholestasis are complex,and those are not still very clear in current.Studies suggest that genetic factors play an important role in the pathogenesis of the disease.Some familial cholestasis have been confirmed by gene mutation causing.Bile secretion process regulated by a number of bile relation gene at the molecular level.Farnesoid X receptor (FXR) gene is related to intrahepatic bile secretion process.Bile secretion is indirect control by FXR which formats a complex network,becoming more attention to researcher in recent years.
2.Risk factors for nosocomial sepsis in preterm infants

Objective To identify the risk factors for nosocomial sepsis in preterm infants.Methods A case-control study (1 ∶ 2) was conducted in 81 preterm infants with nosocomial sepsis and 162 preterm infants without nosocomial sepsis as age-matched controls (admission time was the most closely) hospitalized in Fujian Maternity and Children Hospital from January 1,2007 to December 31,2011.Data of preterm infants including maternal,delivery and neonatal records were collected.Risk factors for nosocomial sepsis were analyzed using t test,x2 test and multivariate Logistic regression.Results Nosocomial sepsis occurred in 81 preterm infants with an incidence rate of 1.50% (81/5 392).Univariate analysis showed that the gestational age [(31.8 ±2.4)vs(33.8 ± 1.8)weeks,t=-7.260,P<0.01] and birth weight [(1 545± 349) vs (2 174±465) g,t=-10.750,P<0.01] of neonates with nosocomial sepsis were lower than those in the controls.Compared with the controls,the neonates with nosocomial sepsis had higher incidence of small for gestational age [27.2% (22/81) vs 11.7% (19/162)],multiple birth [35.8% (29/81) vs 21.6% (35/162)],neonatal asphyxia [19.8%(16/81)vs 8.6%(14/162)],admission to neonatal intensive care unit [81.5%(66/81) vs 49.4% (80/162)],incubator usage [87.7% (71/81) vs 29.0% (47/162)],intracranial hemorrhage [27.2% (22/81)vs 14.2% (23/162)],noninvasive ventilation [35.8% (29/81)vs 14.8% (24/162)],feeding intolerance [64.2% (52/81) vs 17.9% (29/162)],using probiotics [65.4% (53/81) vs 37.0% (60/162)],duration of parenteral nutrition >7 days [77.8% (63/81) vs 16.0% (26/162)],combined administration of antibiotics [61.7%(50/81) vs 43.8%(71/162)],duration of antibiotics administration >7 days [65.4%(53/81) vs 9.3% (15/162)],intravenous immunoglobulin [76.5% (62/81) vs 46.9% (76/162)] and central vena catheterization [16.0% (13/81) vs 1.2% (2/162)] (all P<0.05).The Logistic regression analysis showed that low birth weight (OR=2.087,95%CI:1.074 4.057),duration of parenteral nutrition >7 days (OR=3.075,95%CI:1.158 8.164),feeding intolerance (OR-4.328,95%CI:1.776-10.544) and duration of antibiotic administration >7 days (OR=18.443,95%CI:5.084-66.913) were independent risk factors for nosocomial sepsis in preterm infants (all P<0.05).Conclusions Preterm infants with low birth weight,long duration of parenteral nutrition,long-term antibiotic treatment and feeding intolerance have high risk for nosocomial sepsis.
3.Clinical observation of diabetic retinopathy after transplantation of autologous stem cell in diabetic patients

To observe the effect of autologous stem cell transplantation in diabetic retinopathy.
●METHODS:Totally 58 cases (116 eyes) who underwent autologous stem cell transplantation were confirmed as no diabetic retinopathy (18 eyes), mild non-proliferative diabetic retinopathy (NPDR) (41 eyes), mid-level NPDR (51 eyes); severe NPDR (6 eyes) by ophthalmoscope directly or indirectly and fluorescence fundus angiography (FFA ). Follow - up was 6 - 12mo, the changes of retinopathy were observed.
●RESULTS: The total effective rate of vision and retinopathy was 84. 4%, 76. 7%. The results of severe NPDR was statistically worse than the other groups ( P <0. 05).
●CONCLUSlON: The stable blood glucose level and improved pancreatic function after autologous stem cell transplantation might be helpful in diabetic retinopathy, the long effects need to be researched further.
4.Ecological Significance and Processes in Research of the Viable but Nonculturable State in Bacteria

The viable but noncuturable (VBNC) state is a survival strategy when bacteria are exposed to en- vironment stress. The VBNC state is part of the life cycle of non-differentiating bacteria, and it has a far-reaching impact on traditional bacteriology. Cells in the VBNC state fail to grow on the routine bacterio- logical media, here its significance in human health and environment science are detailed. Cells entering the VBNC state exhibit dwarfing and a number of metabolic changes in respiration rates and macromolecular synthesis. This paper summarized the variations in DNA and protein comparing to the culturable cells. We also discussed the ability of cells to resuscitate from the VBNC state and return to an actively metabolizing and culturable form. Some new methods for monitoring the VBNC state were listed. Finally the future was suggested.
5.Effects of Salvianolate on Myosin Heavy Chain in Cardiomyocytes of Congestive Heart Failure Rats.

OBJECTIVETo explore the effect of Salvianolate on myosin heavy chain (MHC) in cardiomyocytes of congestive heart failure (CHF) rats.
METHODSSixty male SD rats were divided into 6 groups according to random digit table, i.e., the normal control group (NCG), the model group, the Captopril group (CAG), the low dose Salvianolate group (LSG), the high dose Salvianolate group (HSG), the Captopril and high dose Salvianolate group (CSG), 10 in each group. CHF rat model was established with peritoneal injection of adriamycin in all rats except those in the NCG. Equal volume of normal saline was peritoneally injected to rats in the NCG, once per week for 6 successive weeks. Corresponding medication was started from the 5th week of injecting adriamycin. Rats in the CAG were administered with Captopril solution at the daily dose of 10 mg/kg by gastrogavage. Rats in the LSG and the HSG were administered with Salvianolate solution at the daily dose of 24.219 mg/kg and 48.438 mg/kg respectively by gastrogavage. Salvianolate was dissolved in 2 mL 5% glucose solution and administered by peritoneal injection. Rats in the CSG were peritoneally injected with high dose Salvianolate solution and administered with Captopril solution by gastrogavage. Two mL normal saline was peritoneally injected to rats in the model group, once per day for 8 successive weeks. Eight weeks later, the cardiac function and myocardial hypertrophy indices were detected by biological signal collecting and processing system. mRNA expression levels of alpha-MHC and beta-MHC in cardiac muscle were detected by fluorescence quantitative PCR. Expressions of protein kinase C (PKC) in cardiac muscle were detected by Western blot.
RESULTSCompared with the normal control group, heart mass index (HMI) and left ventricular mass index (LVMI) obviously increased in the model group (P < 0.01). Compared with the model group, HMI and LVMI decreased in HSG, CAG, and CSG groups (P < 0.05, P < 0.01). It was more obviously lowered in the CSG group than in the CAG group (P < 0.05). Compared with the NCG, the mRNA expression level of alpha-MHC in cardiac muscle decreased, the mRNA expression level of p-MHC and the expression of PKC in cardiac muscle increased in the model group (P < 0.01). Compared with the model group, the mRNA expression level of alpha-MHC in cardiac muscle was increased, and the mRNA expression level of beta-MHC and the expression of PKC in cardiac muscle were decreased in HSG, CAG, and CSG groups (P < 0.05, P < 0.01). There was statistical difference between the CSG group and the CAG group (P < 0.05).
CONCLUSIONSSalvianolate could up-regulate the mRNA expression level of alpha-MHC, and down-regulate the mRNA expression level of beta-MHC in cardiac muscle. Its mechanism might be related to decreasing the expression of PKC.

6.Application of Network Management System of Preterm Infant

ObjectiveTo explore the role of Network Management System (NMS) in decreasing mortality and incidence of cerebral palsy in preterm infants.MethodsThe data of 356 preterm infants transported by NMS from January 2004 to December 2005 were analyzed.ResultsNo death cases occurred during the transportation of 356 preterm infants, the success rate was 100%. 292 cases (84.39%) were cured and 36 cases (10.4%) were effective. 7 case dead for compliance, the mortality was 19.6‰. 3 cases suffered from cerebral palsy , the incidence of cerebral palsy was 8.6‰.ConclusionNMS applied to preterm infants is a high-effective medical model, and plays an important role in improving the forward prognosis of preterm infants.
7.Impact of obesity and hypertension on left atrium size.

OBJECTIVETo investigate the impact of obesity and hypertension on the size of the left atrium.
METHODSA total of 383 subjects were divided into normal group (NS, n = 95), hypertension group (HT, n = 97), obesity group (OB, n = 98) and hypertension with obesity group (HT + OB, n = 93). Clinical information, echocardiographic parameters and left atrial size in the four groups were compared.
RESULTS(1) The left atrial volume index (LAVI) was arranged in the order: HT + OB > OB > HT > NS (28.07 ± 6.63 > 24.61 ± 4.89 > 22.73 ± 5.16 > 19.18 ± 4.82 ml/m, P < 0.05); Difference on LAVI was significant in four groups after adjusting for age, gender, HR, FPG, TC, TG and HDL (P < 0.05). (2) Pearson correlation analysis showed closer correlation between BMI with LAVI compared to SBP and DBP with LAVI (r values: 0.505, 0.240, 0.209, P < 0.001). After adjusting for other factors, the correlation between BMI and LAVI remains significant (β = 0.411, P < 0.001) while no significant relationship between SBP, DBP and LAVI was found. One unit BMI increase is associated with 0.052 cm enlargement of left atrial diameter (LAD), 1.053 ml increase of left atrial volume (LAV) and 0.710 ml/m increase of LAVI.
CONCLUSIONThe impact of obesity on the size of left atrium is greater than that of hypertension. Obesity and hypertension have synergistic effects on enlarging left atria.

9.Study on the Nitrite-reducing Activity of Aerobic Denitrifying Bacterial Strain N6-1

The nitrite-reducing activity of aerobic denitrifying bacterial strain N6-1 was studied. It showed that the nitrite-reducing activity reached the highest at 30℃, 120 r/min, pH 8.5 and C/N ratio 12, using CH3COONa and NaNO2 as the sole carbon source and nitrogen source, respectively. When the initial NaNO2 concentration was 2 g/L, NO2--N was reduced completely after 20 hours cultivation with the reducing rate of 20.3 mg/L?h. There would be no effect on its nitrite-reducing activity in the present of 1.5% NaCl or 1% peptone. The cell concentration could reach 1.2?1011 CFU/mL after 24 hours cultivation in 10 L fermentor.
10.Identification and Simulation Mutation of a High-productive Strain of Poly(?-glutamic acid) Independent of Glutamic Acid

A high productive poly ?-glutamic acid(?-PGA) strain PGA-N in a culture medium containing no L-glutamine was isolated from fermentation products.With the following identifications of colony mor-phology,physiological and biochemistry experiments,and genetics,the strain PGA-N was classified as a Bacillus licheniformis.According to the product environment,the base culture medium having no L-glutamine was simulated.In order to enhance the production of the strain PGA-N,the fermentation condi-tions,such as carbon source,nitrogen source,were optimized and the ?-glutamic acid production reached 5.16 g/L after getting the optimum formulation of this culture medium.PGA-N was mutagenized with com-bination of NTG and UV.A mutant PGA-N-C10 was screened which PGA production was increased from 5.16 g/L to 8.82 g/L.The study also investigated the effects of agitation speed on the cell biomass,?-PGA production and the ?-PGA molecular weight.The ?-PGA yield of PGA-N-C10 was as high as 11.00 g/L when the agitation speed was 400 r/min.
