1.CT characteristics of acinar cell carcinoma of the pancreas

Background and purpose:Acinar cell carcinoma of the pancreas (ACCP) is a rare malignant tumor and a few radiologic reports have been published. This study aimed to evaluate the CT characteristics of ACCP. Methods:CT signs of 9 cases of pathologically conifrmed ACCP were analyzed retrospectively.Results:The mean value of longest diameter of the 9 cases of ACCP was 52 mm. Among the 9 cases, 6 cases (66.7%) had ill-deifned bor-der, 6 cases (66.7%) showed exophytic type, 8 cases (88.9%) showed enhancement degree less than normal pancreatic tissue, 6 cases (66.7%) represented heterogeneous enhancement, 7 cases (77.8%) showed invaded vessel, 5 cases (55.6%) had lymph node metastasis, and none had hepatic metastasis. Dilated pancreatic duct was observed in only 1 case. Conclusion:When pancreatic mass is large, heterogeneous, exophytic and without dilated pancreatic duct, ACCP is suggested.
2.Paroxysmal hemicrania:clinical analysis of 8 cases

Objective To clinically analyze the feature of paroxysmal hemicrania in order to improve our cognition toward it.Methods Eight patients,3 men,5 women,aging 17 to 74 years old,were prospectively analyzed over the past 2 years in our hospital.Results Their age of onset was from 9 to 60years old(mean 42.5±16.3).Seven of the 8 cases were treated with indomethacin,out of whom 5 got an immediate and complete response and one of them remitted partially.Another stopped taking indomethacin because of gastroenteric side effects.She was treated with verapamil and prednisone and partial relief was gained.Conclusions Paroxysmal hemicranial is a rare benign disorder.which needs our improved understanding.The patient who is diagnosed with paroxysmal hemicranial should firsfly receive indomethacin.and standard anti-cluster headache medications or other non-steroid anti-inflammatory drugs is used if she/he can not get relief and (or)tolerate the adverse effects.
3.Analysis of risk factors associated with postoperative lower respiratory infections in the elderly patients with lung carcinoma

Objective To explore the possible risk factors associated with postoperative lower respiratory infections in the elderly patients with lung carcinoma,and to formulate the appropriate preventive strategies according to the results of the study.Methods From January 2014 to April 2015,182 elderly patients with lung carcinoma who were undergoing surgical treatment in Renji Hospital of Shanghai Jiaotong University were collected.All patients were divided into infection group (n =29) and control group (n =153) according to whether occuring postoperative lower respiratory infections or not.The possible risk factors associated with postoperative lower respiratory infections were analyzed by single factor analysis and multivariable Logistic regression model.Results Single factor analysis showed that the postoperative lower respiratory infections of elderly patients with lung carcinoma were associated with age (t =-3.03,P < 0.05),smoking (x2 =5.69,P =0.02),albumin (t =2.40,P =0.02),forced expiratory volume (FEV) 1.0% (t =5.85,P < 0.05),lung carcinoma stage (x2 =7.06,P =0.01),small cell lung cancer (x2 =5.09,P =0.02),complication with chronic obstructive pulmonary disease (COPD) (x2 =16.28,P <0.01) and complication with coronary heart disease (x2 =10.13,P < 0.01).Multivariable Logistic regression analysis showed that age (OR =1.34,95%CI:1.13-1.59,x2 =11.17,P<0.01),smoking (OR=12.68,95% CI:2.06-78.04,x2 =7.50,P=0.01),FEV1.0% (OR =0.90,95% CI:0.83-0.97,x2 =7.76,P =0.01),squamous-cell carcinoma (OR =6.88,95% CI:1.41-33.56,x2 =5.70,P =0.02),complication with COPD (OR =12.44,95% CI:1.41-109.90,x2 =5.15,P =0.02) and complication with coronary heart disease (OR =10.61,95% CI:2.25-90.50,x2 =7.18,P =0.01) were independent risk factors for postoperative lower respiratory infections in the elderly patients with lung carcinoma.Conclusion Age,smoking,squamous-cell carcinoma,low value of FEV1.0%,complication with COPD or coronary heart disease can increase the risk of postoperative lower respiratory infections in the elderly patients with lung carcinoma.Comprehensive and effective preventive strategies should be performed to reduce the risk of postoperative respiratory infections.
4.The expression of c-fos gene in retina of flickering light-induced and form deprivation myopia

Background Flickering light is different from the normal light environment.Animal experiment proved that flickering light can induce myopia.But its mechanism remains unclear.Objective This study was to investigate the expression of c-fos gene in retina of myopic C57BL/6J mice induced by flickering light and monocular form deprivation.Methods Ninety clean C57BL/6J mice aged 28-day-old with the similar refraction in both eyes were randomly assigned to five groups.Fifteen mice in the control group were exposed to continuous white light environment.The white flickering light with the frequency of 10,5,2 Hz were used to irradiate the mice respectively in high frequency flickering group (15 mice),moderate frequency flickering group (15 mice) and low frequency flickering group (15 mice),respectively.The right eyes of other 30 mice were monocularly occluded with a semitransparent hemispherical thin plastic shell to establish the form deprivation models and then were exposed to white light environment.The diopter and ocular axial length were measured by murine-specific eccentric infrared photorefraction and A-scan ultrasonography before experiment and two weeks after the treatments.At the end of experiment,the mice were sacrificed by neck dislocation.Mice eyes were enucleated and retinal samples were prepared for the detect of c-fos protein and its mRNA by immunohistochemistry,Western blot and reverse transcription polymerase chain reaction (RT-PCR),respectively.Results Immunohistochemistry showed that the expressing rate ofc-fos protein in retina was (68.000±10.368)%,(51.000±6.519)%,(46.000±6.519)%,(31.000±7.416)% and (25.000 ± 7.071)% in the control group,high frequency flickering group,moderate frequency flickering group,low frequency flickering group and form deprivation group respectively 2 weeks after experiment.The expression rates of c-fos protein in retina in different frequencies of flickering light groups and form deprivation group were significantly lower than that in the control group (t =3.104,4.017,6.490,7.661,all P<0.05),with the lowest rate in the form deprivation group (P<0.05).The expression of c-fos detected by Western blot assay exhibited that the relative values of c-fos protein in retina (c-fos/GAPDH) was 0.804±0.050,0.687±0.047,0.667±0.036,0.558±0.036 and 0.532 ±0.056,respectively in the control group,high frequency flickering group,moderate frequency flickering group,low frequency flickering group and form deprivation group,illustrating significantly lowing in different frequencies of flickering light groups and form deprivation group compared with control group (t =2.961,3.184,6.971,6.276,all P<0.05),whereas the c-fos in the low frequency group and form deprivation group,c-fos protein was less expressed in comparison with the higher frequency flicking group (P<0.05).The expression level of c-fos mRNA (c-fos mRNA/GAPDH mRNA) in retina was 0.820±0.056,0.663±0.061,0.627±0.034,0.521±0.041 and 0.474 ±0.045 in the control group,high frequency flickering group,moderate frequency flickering group,low frequency flickering group and form deprivation group,respectively.These results demonstrated a significant decline in the expression of c-fos mRNA in different frequencies of flickering group and form deprivation group compared with the control group(t=3.262,5.070,7.173,8.305,all P<0.05),and the inhibition ability of low frequency of flickering group and form deprivation group was much stronger.Conclusions The c-fos gene level in the retina has a negative relationship with the severity of myopia induced by flickering light and form deprivation.

6.Observation on the Therapeutic Effect of the Comprehensive Therapy in the Treatment of Peripheral Facial Palsy

Thirty cases of peripheral facial palsy were treated with the comprehensive therapy, and 30 cases of peripheral facial palsy were treated with shallow electro-acupuncture as control. The results showed that the cure rate in treatment group was higher than that in the control group(P< 0.05), and no significant difference was found in effective rate between two groups.
7.Effects of electroacupuncture plus drug anesthesia on pain and stress response in patients after radical surgery for stomach cancer

Objective: To observe the effect of electroacupuncture plus drug anesthesia on pain and stress response in patients after radical surgery for stomach cancer.Methods: A total of 60 patients were randomized into a control group and an observation group by the random number table, with 30 cases in each group. The control group was given conventional drug anesthesia. The observation group was given additional electroacupuncture intervention. Before anesthesia and 2 h, 12 h and 24 h after surgery, the visual analog scale (VAS) was scored, the heart rate, the mean arterial pressure, and the levels of serum β-endorphin (β-EP) and adrenocorticotrophic hormone (ACTH) were measured. Results: Two hours, 12 h and 24 h after surgery, the VAS scores of both groups were higher than those before anesthesia (all P<0.05), and the VAS scores in the observation group were lower than those in the control group at the same time points (all P<0.05). Two hours, 12 h and 24 h after surgery, the heart rates and mean arterial pressures in the control group were significantly higher than those before anesthesia (all P<0.05), while there were no significant intra-group differences in the observation group (all P>0.05), and the indicators were lower than those in the control group at the same time points (all P<0.05). Two hours, 12 h and 24 h after surgery, the serum β-EP levels in the observation group were significantly higher than those before anesthesia (all P<0.05), and significantly higher than those in the control group at the same time points (all P<0.05). Two hours, 12 h and 24 h after surgery, the serum ACTH levels in the control group were significantly higher than those before anesthesia (all P<0.05), and were significantly higher than those in the observation group at the same time points (all P<0.05). Conclusion: Electroacupuncture plus drug anesthesia can significantly relieve pain and stress response in patients after radical surgery for stomach cancer.
8.Study on a combined antepartum and postpartum to interrupt the transmission of hepatitis B virus from mother with both positive HBsAg and HBeAg to infant

Objective The infants born to hepatitis B virus (HBV) carrier mothers are the high risk children who are susceptible to HBV infection. About ten years of studies show that hepatitis B virus vaccine plus hepatitis B immunoglobulin (HBIG) inoculation can interrupt mother to infant transmission, which has been generally accepted. About 70%~90% children can be protected by this way. Intrauterine HBV infection is responsible for most of the immunoprophylaxis failure, so we study on a combined antepartum with postpartum to interrupt transmission of HBV from HBsAg positive and HBeAg positive mothers to their infants. Methods HBV carrier mothers, whose HBsAg and HBeAg were both positive, and later their infants were randomly divided into two groups. The gravidas in group 1 received 400 IU HBIG every four week (from pregnancy for 28 week to delivery); the gravidas in group 2 didn't receive any HBIG (the control group). After delivery, all the infants in both groups were given 2 dose of 200 IU HBIG at birth and at 15 days, then accepted 3 dose of HBV vaccine at 1, 2 and 7 months. These infants were periodically followed up until 1 year of age and their sera were obtained to test HBV markers. 104 mothers and their 105 infants were followed for 12 months. Results There were 51 infants in group 1 and 54 infants in group 2. In group 1, the rate of intrauterine HBV infection was 5.9%, and at 12 month old, the rate of chronic HBV infection was 5.9% and the rate of infants with the protective level against HBV infection was 94.1%. In group 2, the rate of intrauterine HBV infection was 18.5%, and at 12 month old, the rate of chronic HBV infection was 16.7% and the rate of infants with the protective level against HBV infection was 83.3%. ? 2 test was used to compare the different rates of intrauterine HBV infection between the two groups (? 2=3.86, P
