1.Effects of C-peptide on development of chronic diabetic complications and its mechanism

Recent studies have suggested that C-peptide in the nanomolar concentration range binds specifically to cell surfaces and plays an important role in the pathological mechanism of chronic complications of diabetes mellitus, via improving the microcirculation, diminishing glomerular hyperfiltration, reducing urinary albumin excretion, protecting the cardiac function and preventing nerve dysfunction. The effects of C-peptide on the development of the chromic diabetic complications and its mechanisms are reviewed.
2.Effect of Violet Flower Bidai Extracts on the Level of Blood Lipid and Leptin of Obese Rats

Objective To study the effects of violet flower Bidai extracts on body weight,blood lipid level,serum leptin level,the activity of lipoprotein lipase and hepatic lipase of obese rats,and to discuss the mechanism of violet flower Bidai extracts on weight reduction.Methods Male obese rats which induced by high energy diet were randomly divided into four groups.All rats were given high energy diet.One control group was gavaged with normal saline and one western medicine group was gavaged with sibutramine,and two violet flower Bidai extracts groups was gavaged with water solutions of violet flower Bidai extracts at doses of 6 g(crude drug)/kg BW,12 g(crude drug)/kg BW,respectively.After six weeks,body weight,obese index,weight of abdominal fat tissue,fasting lipid,leptin in serum,the activity of lipoprotein lipase and hepatic lipase were measured.Results Body weight,obese index,weight of abdominal fat tissue in the two violet flower Bidai extracts groups were significantly lower than those in obese control group(P
3.Clinical curative effect of pendant laparoscopic uterine fibroids divest and factor analysis of recurrence

Objective To compare the chnical curative effect between pendant laparoscopic and traditional laparoscopic apphed in uterine fibroids divest,and analyze the risk factors of recurrence after resection of uterine fibrnids.Methods According to the different methods,130 patients with uterine fibroids were divided into suspension group (pendant laparoscopic uterine fibroids divest) and control group (traditional laparoscopic uterine fibroids divest),and 65 cases in each group.The intraoperative blood loss,operative time,exhaust time,heating duration,laparoscopic drainage were recorded,and the complications and the factors of recurrence were evaluated.Results The operative time,intraoperative blood loss,laparoscopic drainage in suspension group was less than that in control group [(53.5 ±20.9) min vs.(100.8 ± 26.7) min,(92.7 ± 18.9) ml vs.(168.4 ± 19.6) ml,(94.3 ± 13.8) ml vs.(186.5 ± 24.7) ml],the complications rate in suspension group was lower than that in control group [3.08% (2/65) vs.33.85% (22/65)],and there was significant difference (P < 0.05).Followed up for 1-3 years,the recurrence rate was 29.23% (38/130),3 cases were recurred within 3-6 months in control group,no recent recurrence in suspension group.Among them,the menarche age ≤13 years old,multiple myoma,operation age < 35 years old were correlated with recurrence (P< 0.01 or < 0.05).Conclusions Pendant laparoscopic uterine fibroids divest is fast and safe in operation and the prognosis effect significantly,faint pain degrees,myoma resection completely,short hospitalization period,less postoperative recurrence and other advantages,is worthy of reference for clinical promotion.The menarehe age ≤ 13 years old,multiple myoma,operation age < 35 years old are correlated with recurrence.

5.Analysis of Clinical Features and Drug Sensitivity of 67 Newborn Infants with Klebsiella Pneumoniae

Objective To explore the clinical characteristics and drug sensitivity of newborn pneumonia of klebsiella pneumoniae,and investigate the positive rate of extended-spectrum ? lactamases(ESBLs)in klebsiella pneumoniae.Method Sixty-seven cases of sputum samples newborn pneumonia of klebsiella pneumoniae from Jan.2003 to Dec.2005 were collected.Bacteria identification and drug sensitivity test were performed.At the same time,ESBLs were detected.Results The clinical characteristics of newborn pneumonia of klebsiella pneumoniae were non special,and the positive rate of ESBLs in klebsiella pneumoniae was 17.91%.The klebsiella pneumoniae was 100%sensitivie to carbopenems,but resistant to commonly used ?-antibiotics such as penicillin and cephalosporin.ESBLs negative strains had higher drug sensitivity than ESBLs strains.Conclusions Partial klebsiella pneumoniae are ESBLs strains and insensitive to commonly used antibiotics.In order to decrease ESBLs strains,the antibiotics treatment shall be used appropriately and the selection of antibiotics shall be guided by the results of drug sensitivity tests.
6.Effect of methazolamide in patients with refractory uveitic macular edema

AIM:To evaluate the efficacy and safety of methazolamide in treating refractory uveitic macular edema.METHODS: Retrospective self-controlled study was designed.A total of 15 patients (20 eyes) with refractory uveitic macular edema which used methazolamide as adjuvant therapy were enrolled in Shanghai First People`s Hospital from January 2015 to June 2016.The changes of central macular thickness (CMT) and best corrected visual acuity (BCVA) were observed at baseline and 2, 4, 8wk after treatment.We also focused on the incidence of complications and relapse.RESULTS: The CMT was 445.95±154.10μm, 338.83±138.34μm, 251.50±40.20μm, 244.90±35.68μm at baseline, 2, 4 and 8wk after treatment, respectively.The differences among them were statistically significant (F=15.467, P<0.05).The BCVA (log MAR) were 0.40±0.17, 0.28±0.21, 0.19±0.20, 0.18±0.21 at baseline, 2, 4 and 8wk respectively, with a significant difference among them (F=5.208, P<0.05).When the cumulative dose reached to 700mg and 1400mg, no one had methazolamide-related complications;and when it came to 2800mg, 5 patients (33%) had methazolamide-related complication.After the withdrawal of methazolamide 1wk, 1 and 3mo, 3 patients (20%), 5 patients (33%) and 8 patients (53%) relapsed, respectively.CONCLUSION: Methazolamide is beneficial in improving macular edema and vision in 4wk.When the cumulative dose is more than 1400mg, we need pay attention to the complications.After discontinuing methazolamide for 1wk, macular edema relapsed in some patients, and more than half of patients recurred after 3mo.So the patients should be followed closely in 3mo after withdrawal of methazolamide.
7.Expression and significance of metallothionein in liver transplantation for patients with benign end-stage liver diseases

Objective To investigate the expression of metallothionein (MT) in 113 specimens of liver transplantation for patients with benign end-stage liver diseases and 10 normal liver specimens,and analyze their clinical significance and prognostic value.Method 113 specimens of liver transplantation for patients with benign end-stage liver diseases and 10 normal liver specimens were included in this study.MT in these specimens was detected by using immunohistochemistry.Cox regression model was used to analyze the multi-factors for the prognosis.Results MT levels were lower in specimens of liver transplantation for patients with benign end-stage liver diseases than in normal liver specimens.MT levels were higher in specimens of HCV-related cirrhosis than in those of autoimmune hepatic (AIH) cirrhosis,but there was no significant difference among all other groups.Conclusion MT expression level in AIH cirrhosis tissue is higher than in HCV-related cirrhosis tissue.MT is the independent influencing factor for survival following liver transplantation in patients with benign end-stage liver diseases.
8.Effects of HBO therapy on language function of aphasics and it's mechanisms

AIM: To explore the effects of hyperbaric oxygen (HBO) therapy on language function of aphasics and it's mechanisms. METHODS: Forty patients with aphasics after mild and moderate cerebral trauma and stroke were chosen and divided into therapy group (20 patients) and control group (20 patients). All the patients in both groups received routine therapy.Besides routine therapy, patients in therapy group also received HBO therapy. The HBO treatment contained three courses, each lasts for 15 days at an interval of 3 days. The language functions ( including 11 sub - items) were tested with the Apparatus ZM2.1for Diagnosis and Treatmern of Language Disorders (Language Disorders ZM2.1) before and after each course of HBO and be compared between the two groups. RESULTS: The therapeutic effects between the therapy group and the control group: the scores of 10 sub- items are significantly higher than those of the control group after the 2nd course (P<0. 05); After the 3rd course, all 11 sub - items were improved significantly ( P < 0.05). The therapeutic effects before and after each course of the therapy group:the scores of the advanced dictation, voice expression and semantic expression increased significantly after the 1st and the 2nd course ( P < 0.05). CONCLUSIONS: The HBO therapy might facilitate the recovery of the language function of the aphasics by promoting the recovery and the self- reparation of the neurocytes and alleviating the focal ischemia - reperfusion. The result of this effect would facilitate the original recovery velocity and not be focused on some language functions and much more obvious in the 1st and the 2nd course.
9.Morphologic differences of vascular buds in vertebral cartilage endplate in age-specific rabbits

Objective To observe the morphologic changes of of vascular buds in vertebral cartilage endplate in age-specific rabbits and also to investigate the correlation between the changes of vascular buds and interverbral disc degeneration. Methods There were 15 New Zealand white rabbits in our study,which include three groups, 2-week-old rabbits, 1-year-old rabbits and 3-year-old rabbits, and each groups had five rabbits. The X-ray radiograph, histology and scanning electron microscope were used to observe the changes of vertebral cartilage endplate. According to Miyamoto standard, the interverbral disc was graded 1-5, and scored 1-5 respectively. Results The changes of micro-vascular structure of vertebral cartilage endplate were observed during aging. Under the scanning electron microscope, the vascular structure degenerated gradually, and disappeared in the end. The blood vessels in the central region of the vertebral cartilage endplate reduced more obviously than those in periphery region. The severe degeneration was found in vertebral endplate, compared with intervertebral disc. The changes of vascular buds in rabbits vertebral cartilage endplate had positive correlation with the vertebral endplate calcification and the interbertebral disc degeneration. Conclusion Changes of vascular buds in vertebral endplate may accelerate intervertebral disc degeneration.
10.Detection of large deletions in mitochondrial DNA during skin aging

Objective To quantitatively observe the actual levels of 4977 bp and 3895 bp mitochon-drial DNA (mtDNA) deletion in healthy human skin of different age, and to explore their relationship with intrinsic aging and photoaging. Methods Seventy-one samples of skin tissue were obtained from healthy volunteers, including 40 samples from UV-protected areas (buttock, thigh, waist or abdomen) and 31 from UV-exposed areas (neck, back of hands, forehead or face). Nuclear and mitochundrial DNA was extracted from these samples. Conventional PCR was performed to detect the incidence of 4977 bp and 3895 bp mtDNA deletion. Then, SYBR Green real-time PCR was applied to quantitatively analyze the target mutations in posi-tive samples. Results Conventional PCR showed that the incidence of both 4977 bp deletion and 3895 bp deletion increased with age. For example, the incidence of 4977 bp deletion and 3895 bp deletion accounted for 47.5% (19/40) and 30% (12/40), respectively in samples from volunteers older than 40 years, signifi-cantly higher than that in those from volunteers younger than 40 years (X2 = 4.673, 6.118, respectively, both P < 0.05). The total incidence of 4977 bp deletion and 3895 bp deletion in UV-exposed areas was 48.4% (15/31) and 32.3% (9/31), respectively, which did not differ from those in UV-protected areas. The results from real-time PCR revealed a positive correlation of the copy number of 4977 10p deletion and 3895 bp deletion with age (rg = 0.907, 0.845, respectively, both P < 0.05). When the UV-exposed area was compared with the UV-protected area, no significant difference was found in the copy number of 4977 bp deletion ( P = 0.264), whereas a higher level of 3895 bp deletion was noticed in UV-exposed area (P = 0.014). Conclu-sions The 4977 bp mtDNA deletion is primarily associated with chronological aging, and might serve as a biomarker for the process of chronological aging of skin. Deletion of 3895 bp mtDNA seems to be more sus-ceptible to ultraviolet radiation.
