1.Echocardiography study of cardiac structure and function in Hb Bart′s fetal edema syndrome

Objective To search different weeks′ gestation atrioventricular size and cardiac function change in Hb Bart′s fetal edema syndrome. Methods Color Doppler echocardiography was adopted to study 67 cases with Hb Bart′s fetal edema syndrome in different weeks′ gestation.Atrial and ventricular diameter and cardiac function were measured, and compared with those of normal fetuses in same weeks′ gestation.Results In the cases with Hb Bart′s fetal edema syndrome,the heart was enlarged from 25 weeks′ gestation,right atrial enlargement came earliest, along with gestation weeks′ followed with right ventricle,left atrium and left ventricle aggravated enlargement.From 29 weeks′ gestation left and right cardiac diastolic function was involved with complicated pericardial,pleural,peritoneal effusion the damage was clearer.During 33 weeks′ gestation,left and right heart systolic function was involved.From 25 weeks′ gestation,fetal foramen ovale began to enlarge,and the enlarged foramen was difficult to be differentiated from secondary atrial septal defect.Conclusions Echocardiography is sensitive to Hb Bart′s fetal edema syndrome,and provides reliable basis for early clinical intervention.

Objective To investigate the changing features of microtubule associated protein 2 (MAP2) expression, which was a neural dendrite marker, of epileptic rat and the intervention results after using Diazepam. This paper introduces the hardware of data acquisition system based on LabVIEW platform, i.e. the system for data acquisition and control. The system is composed of the units for signal selection, programmable magnification, filtration, restriction, and A/D and D/A conversion circuits. With the system, the following can be available including eight-channel low-frequency signals, eight options on programmable magnification, 1000Hz low-pass filter, 50Hz power-frequency trap, and A/D and D/A conversion.

6.Effects ofⅠ,ⅡCrystal and Amorphous Forms of Lercanidipine Hydrochloride on the Preparation

OBJECTIVE:To study the effects of Ⅰ,Ⅱ crystal and amorphous forms of lercanidipine hydrochloride on the preparation,and provide theoretical basis for its development and consistency evaluation. METHODS:X-ray powder diffraction (XRD),infrared spectrophotometry(IR)and differential scanning calorimetry(DSC)were adopted to identify the 3 crystal forms of lercanidipine hydrochloride. XRD was used to compare the effects of crushing,grinding,pressing technology,wetting granula-tion,adhesive solvents(water,ethanol)and drying temperature(50,60,70℃)on stability of 3 crystal forms of lercanidipine hy-drochloride;the dissolution in vitro in water,hydrochloride,pH 4.5 acetate buffer,pH 6.8 phosphate buffer were compared among 3 crystal forms of Lercanidipine hydrochloride tablet. RESULTS:XRD showed both Ⅰ,Ⅱ crystal forms had characteristic diffrac-tion peak with inconsistent 2 θ values,amorphous had no characteristic diffraction peak;IR showed 3 crystal forms had different absorption intensity and absorption peak number;DSC showed Ⅰ,Ⅱ crystal forms had obvious endothermic peak in 194.6 ℃, 207.3 ℃,respectively,amorphous had obvious endothermic peak in 86.1 ℃ and exothermic peak in 299.8 ℃. Crushing,grinding, pressing and drying temperature had no effects on the stability of 3 crystal forms;water had no effect on the stability of crystal in wetting granulation,ethanol may cause the change of Ⅰcrystal form. Except for the comparison between Ⅰ,Ⅱ crystal forms in hydrochloride (f2=68),the dissolution f2 of 3 crystal forms in 4 kinds of medium were lower than 50. CONCLUSIONS:XRD, IR,DSC methods can identify the 3 crystal forms of Lercanidipine hydrochloride tablet. When preparing lercanidipine hydrochlo-ride by Ⅰcrystal form,wetting granulation should avoid using ethanol as a adhesive solvent,instead of water. Different crystal forms can affect the dissolution in vitro of prepared Lercanidipine hydrochloride tablet.
7.A bibliometrics study of literature on medical image processing for the past ten years.

We searched and retrieved literature on the topic of medical image processing published on SCI journals in the past 10 years. We then imported the retrieved literature into TDA for data cleanup before data analysis and pro cessing by EXCLE and UCINET to generate tables and figures that could indicate disciplinary correlation and research hotspots from the perspective of bibliometrics. The results indicated that people in Europe and USA were leading researchers on medical image processing with close international cooperation. Many disciplines contributed to the fast development of medical image processing with intense interdisciplinary researches. The papers that we found show recent research hotspots of the algorithm, system, model, image and segmentation in the field of medical image processing. Cluster analysis on key words of high frequency demonstrated complicated clustering relationship.

8.Comprehensive reform to improve the performance management of primary health care service in rural China

With the support of World Bank ( WB ) and UK Department for International Development ( DFID) , China Rural Health Project ( hereinafter referred as Health XI Project) began to introduce the idea of performance management to 40 counties in 8 provinces in 2008 . The project implemented cyclic performance manage-ment strategies, including performance planning, performance communication, performance evaluation and perform-ance improvement. With the continuous improvement of performance as the goal, the project attempted to establish incentive mechanisms based on the performance of health care services. After five years of pilots in 40 counties, it has achieved good results in the aspects of improving the quality and efficiency of health care services, motivating the enthusiasm of health care managers and workers, etc. Moreover, it has successfully built several advanced counties with exemplary performance management and accumulated some experience, which provides reference and demonstra-tion for implementing performance management in other areas. The key experiences of implementing performance management of rural primary health care include taking health care quality as the core of performance, appropriately combining economic and non-economic incentives, encouraging personnel participation in performance management in order to ensure the sustainability of performance improvement, and effectively applying the ideology of systematic per-formance management in order to effectively enhance the management level of hospitals. The paper also puts forward some policy suggestions based on emerging issues during implementation of performance management.
9.Effects of dexmedetomidine combined with subanesthetic dose of ketamine on emergence agitation in patients undergoing thoracotomy

Objective To evaluate the effects of dexmedetomidine combined with subanesthetic dose of ketamine on the emergence agitation in the patients undergoing thoracotomy.Methods Eighty ASA physical status Ⅱ or Ⅲ patients,aged 55-75 yr,weighing 50-75 kg,scheduled for elective esophageal cancer resection,were randomly divided into 4 groups (n =20 each) using a random number table:normal saline group (NS group),dexmedetomidine group (group D),subanesthetic dose of ketamine group (group K),and dexmedetomidine combined with ketamine group (group DK).In DK and K groups,ketamine 0.5 mg/kg was injected intravenously (within 1 min) at 10 min before the end of the operation.In DK and D groups,dexmedetomidine 0.5 μg/kg was infused intravenously over 10 min starting from 10 min before the end of operation.In group NS,the equal volume of normal saline was infused intravenously over 10 min starting from l0 min before the end of operation.The emergence time,extubation time,duration of ICU stay,occurrence and degree of agitation,and development of cardiovascular events and hypoxemia within 24 h after operation were recorded.Ramsay sedation scores were recorded before induction of anesthesia (T1),immediately after completion of administration at the end of surgery (T2),and at 0,5,10 and 30 min after extubation (T3-6).Results There was no significant difference in the emergence time,extubation time,and duration of ICU stay between the four groups.Compared with group NS,Ramsay sedation scores were significantly increased at T3-6,the incidence and degree of agitation were decreased,and the incidence of cardiovascular events and hyoxemia was decreased in D,K and DK groups.Compared with D or K group,Ramsay sedation scores were significantly increased at T3-6,the incidence and degree of agitation were decreased,and the incidence of cardiovascular events and hyoxemia was decreased in group DK.Conclusion Dexmedetomidine combined with subanesthetic dose of ketamine can prevent the emergence agitation in the patients undergoing thoracotomy,which provides better efficacy than either alone.
10.Reversal effect of tetrandrine on multidrug resistance in laryngeal carcinoma

Objective To study the reversal effect of tetrandrine (TET) on the multidrug resistance of Hep-2/ADM cell line and the possible mechanisms. Methods MTT method was used for the detection of the inhibitory effect of different concentrations of TET on the multidrug resistance of Hep-2/ADM cell line for selection of the non-toxicity concentration. MTT reduction assay was used to investigate the drug interaction of TET and vincristine (VCR) on HNE-1 (200) cell line. The reversal index (RI), accumulation and excretion of drug, and the apoptosis of Hep-2/ADM cell line were detected by flow cytometry. Results No toxicity to Hep-2/ADM cell line was found when TET concentration was lower than 1.5 ?g/ml. After interaction with TET (1.0 ?g/ml and 1.5 ?g/ml), RIs of VCR on Hep-2/ADM were 1.72 and 2.00 folds, respectively. TET at the concentration of 1.5 ?g/ml could increase the drug excretion and accumulation and also the apoptotic rate of VCR-induced Hep-2/ADM cell line. Conclusion TET can reverse the multidrug resistance of Hep-2/ADM cell line. The reversal effect may be related to the concentrations of TET. The possible mechanism may be that TET can increase the cell apoptosis of VCR-induced Hep-2/ADM cell line and can also increase the drug excretion and accumulation of the multidrug resistance cell line.
