1.Recent progress on study of the selection of intraocular lens in the bilateral sequential cataract surgery

Bilateral sequential cataract surgery is very common in clinical,but the ophthalmologists often ignore how to choose the timing of operation and whether the first eye can be used to improve the intraocular lens (IOL) power calculation for the second one.Despite the refinements in IOL selection with the use of third-generation IOL formulas,5% of eyes still result in a postoperative refractive surprise of ± 1.00 D or more.This paper introduced the methods of using refraction error of the first eye to improve the IOL power calculation for the second eye,and the refractive error of the second eye can be improved by modifying the IOL power to correct up to 50% of the error from the first eye in details.Because of the variation of the postoperative refractive surprise within a month,the interval of three to four weeks between two eyes cataract surgery is highly recommended.These conclusions may provide the advantageous guidance for us in the selection of IOL in bilateral cataract surgery.
2.Optimization of coding sequences and expression of antimicrobial peptide magainin II in Escherichia coli and Pichia pastoris.

The antimicrobial peptide magainin II is expressed in the skin of the African clawed frog, Xenopus laevis, and exhibits a broad spectrum of antimicrobial activity as well as tumoricidal properties at low concentrations. In addition, magaininII plays a synergistic role during antimicrobial and tumoricidal processes with another antimicrobial peptide PGLa that is also expressed in Xenopus laevis. The optimized cDNA sequence of magainin II and magainin II-PGLa hybrid peptide according to E. coli or Pichia pastoris codon usage frequency were synthesized and sub-cloned into prokaryotic expression vector pGEX and Pichia pastoris secreted expression vector pPIC9k. The resulting recombinant plasmids were named as pGEX-magainin II and pPIC9k-magainin II-PGLa. The GST-magainin II fusion protein was highly expressed in E. coli. Furthermore, magainin II was successfully purified by digestion with PreScission Protease to cleave the GST tag. Additionally, our data obtained from the ELISA revealed that magainin II -PGLa hybrid peptide was successfully expressed in Pichia pastoris. These experiments establish a useful system for further studies of these antimicrobial peptides.

3.One Yin-Yang Wu-Xing Model of TCM

Although Yin-Yang Wu-Xing (Yin-Yang and Five-Elements, subsystems of human body) has been the theoretical basis of traditional Chinese medicine (TCM) for more than 5 000 years, it has been primarily analytical or empirical in nature without a formal scientific foundation. Based on bipolar set theory, an equilibrium/non-equilibrium computational model of Yin-Yang Wu-Xing is proposed. The Yin-Yang Wu-Xing dynamical systems are formulated so that equilibrium and non-equilibrium conditions can be established and proved. Computer simulations of equilibrium and non-equilibrium processes show that this new approach can provide diagnostic decision support in TCM. Thus, this equilibrium-based approach provides a unique scientific basis for future research in TCM, Qi (vital energy), QiGong, Meridians and Collaterals (acupuncture channels) and herbal treatment. On the other hand, it provides a basic Yin-Yang cellular network architecture for modem scientific research in genomics such that regulation mechanisms of the ubiquitous YY1 protein for cell processes can be explained.
4.Effects of Lipoic Acid on Proteinuria,Serum and Urine MCP -1 Secretion in Patients with Stage IV Type 2 Diabetic Nephropathy

Objective To investigate the effect of on serum and urine MCP -1 secretion in patients with Stage IV Type 2 diabetic nephropathy treated by lipoic acid .Methods We enrolled 76 diabetic nephropathy patients ,who were randomly divided into two groups . Patients in group T (24 males and 14 females) were treated by lipoic acid 0.6 gram per day.Those in group C (22 males and 16 fe-males) were treated by routine drugs.The serum creatinine (Scr), urea nitrogen (BUN), fasting blood sugar(FBS) and 24h urinary pro-tein,serum and urine MCP -1 secretion were measured at the experiment onset and 3 weeks later.Results There was no significant difference in the serum creatinine , urea nitrogen, fasting blood sugar, HbA1c between the two groups neither at the experiment onset nor after 3 weeks.Compared to experiment onset , 24h urine protein (Tp/24h), serum and urine MCP-1 secretion were all significantly de-creased (P<0.05) in group T after 3 weeks.Compared to group C (2.41 ±0.91g/24h, 91.45 ±33.41pg/ml, 114.78 ±36.35pg/ml), all the levers in group T (1.89 ±0.72g/24h, 39.50 ±13.68pg/ml, 63.41 ±19.57pg/ml) was significantly decreased (P<0.05) after 3 weeks.There was a positive correlation between the serum , urine MCP-1 levels and Tp/24h (r=0.572, P<0.05;r=0.697,P<0.05).Conclusion Lipoic acid can reduce urine protein excretion in diabetic nephropathy patients , maybe by decreasing serum and u-rine MCP-1 secretion .
5.Advance of treatment for newly diagnosed multiple myeloma

Multiple myeloma is a plasma cell malignancy.At diagnosis,most patients are older than 65 years,In newly diagnosed myeloma patients younger than 65 years,high-dose of melphalan followed by autologous stem cell transplantation(ASCT)is regarded as the standard treatment.In elderly patients,usually older than 65 years,oral melphalan and prednisone(MP)has been considered as the standard regimen recently.However,the discovery of thalidomide,lenalidomide,and the proteasome inhibitor bortezomib will increase the effect of clinical treatment.This review will summarize literatures and focus on the treatment of newly diagnosed myeloma.
6.Advance on macrolide-resistant Mycoplasma pneumoniae infection

Macrolide(Ma) resistance has been spreading for more than 13 years worldwide accompanying Ma widely used for anti-mycoplasma pneumoniae (Mp)infection,and has been rapidly increasing.Prevalence of Ma resistance has been higher in Asia than in Europe and America.More than 10 positions of Mp mutation were found.Of them A2058G in domain V of 23S ribosomal RNA gene is frequently found in Europe and America,and A2063G is found dominant in Asia.Different position points of Mp mutation are related with different Ma resistance.Real time PCR for detecting Mp and its mutation position directly from bronchoalveolar lavage fluid is the most accurate method.Treatment with minocycline and garenoxacin for Ma-resistant Mp infection are known most effective.
7.The relationship of bronchial provocation tests and the onset of the disease

Objective To investigate the correlation between bronchial provocation tests and the onset of the disease.Methods One hundred and seven patients who hospitalized in General Hospital of China Aviation from 2013-2014 were selected as our subjects.They all were with unexplained symptoms such as cough and asthma suppress.All the patients were performed bronchial provocation tests.In accordance with the onset time,patients were divided into acute (< 3 weeks),subacute (3-8 weeks) and chronic (> 8 weeks) groups.The positive rates of bronchial provocation test of three groups were compared.Results Of all 107 patients,36 patients were positive and 71 were negative in terms of bronchial provocation test.The cases with acute,subacute and chronic disease history were 16,36 and 55 respectively.The positive rate of bronchial provocation tests in different onset were 56.3% (9/16),36.1% (13/36),23.6% (13/55) respectively,and there was statistics significant difference(x2 =6.275,P < 0.05).The positive rate of bronchial provocation tests in acute group was higher than that in chronic group (x2 =6.165,P < 0.05).There was positive correlation between the positive rate of bronchial provocation tests and the onset,and the correlation coefficient Pearson was 0.239 (P <0.05).Conclusion There is a correlation between the positive rate of bronchial provocation tests and the onset.We should strengthen the monitoring in patients with acute onset and follow-up.
8.Hypoglycemic Effect of n ̄butyl Alcohol Extraction from Anoectochilus roxburghii

Objective To study the hypoglycemic effect and mechanism of n ̄butyl alcohol extraction from Anoectochilus roxburghii. Methods The total of 12 normal rats were used as normal control group, and 48 diabetic model rats were divided into model control group, and high dose, middle dose and low dose treatment groups with 12 rats in each group,three treatment groups received Anoectochilus roxburghii at dose 600,400,200 mg.kg-1 , respectively, normal control group and model control group were administered with equal volume of 0.9% sodium chlorade solution once a day.After 21 days of drug administration, the weights of rats, the levels of blood glucose and insulin were examined. Results Compared with normal control group, the body weight in model control group decreased significantly(P<0.01), while blood glucose significantly increased (P<0.01).Compared with model control group, the body weight in Anoectochilus roxburghii groups exhibited no obvious difference,while blood glucose decreased significantly (P<0.01 or P<0.05).The level of insulin in high dose group increased remarkably (P<0.05). Conclusion The n ̄butyl alcohol extraction from Anoectochilus roxburghii shows markedly hypoglycemic effect,specifically for which at high dose increases the level of insulin significantly,its mechanism may involve with promoting β cells regeneration.
9.Endoscopic thyroidectomy for cN0 papillary thyroid carcinoma, report of 90 cases

Objective To discuss the feasibility of endoscopic thyroidectomy via breast approach for papillary thyroid carcinoma.Methods From April 2009 to December 2013,clinical data of 90 papillary thyroid carcinoma cases undergoing endoscopic thyroidectomy was analyzed retrospectively.Results There was no conversion to open surgery,the mean operation time was 97 min,the mean intraoperative blood loss was 10 ml,the mean postoperative drainage volume was 65 ml,the mean hospital stay was 4.5 d,the mean lymph node number dissection of central compartment was 5.3.No obviously chest pain and numbness occurred.No skin flap ecchymosis,necrosis,effusion,and hematoma occurred.No hypocalcemia convulsions occurred.Transient hoarseness occurred in 3 cases which recovered within 1 to 2 months.Follow-up time ranged from 1 to 5 years,the median follow-up time was 41 months,with no tumor recurrence and lymph node enlargement.All patients were satisfied with the cosmetic results.Conclusions Endoscopic thyroidectomy via breast approach is a safe and feasible procedure in treating papillary thyroid carcinoma (cN0).
10.Clinical significance of changes in peripheral lymphocyte count after surgery in early cervical cancer

Objective To evaluate the clinical significance of changes in peripheral lymphocyte count after surgery in early cervical cancer.Methods The 123 patients with stage Ⅰ bl and Ⅱ al treated by abdominal type Ⅲ radical hysterectomy from May 2008 to December 2012 were reviewed.The median age of patients was 43 years old (range:30 to 66 years).The median follow-up was 25 months with a range of 5-61 months.Peripheral blood samples were obtained on pre-operative,post-operation day 3 and 7.The log-rank test was used to compare the homogeneity of progression-free survival functions across strata defined by categories of prognostic variables.The Cox proportional hazard model was used to assess the significance of potential prognostic factors for progression-free survival.Results Univariate analyses preoperative lymphocyte count (P =0.012) and lymph nodes metastases status (P =0.001) and parametrial invasion (P =0.013) were significant risk factors for progression-free survival rate.On multivariate analyses,preoperative lymphocyte count [hazard ratio (HR) =6.087,95% CI:1.743-21.251,P =0.005] and lymph nodes metastases status (HR =5.984,95% CI:1.803-19.802,P =0.003) were independent risk factor of progression-free survival rate.Conclusion Peripheral lymphocyte counts after cervical cancer surgery may be a important prognostic factor.
