1.Lens-sparing vitreoretinal surgery for treatment of giant retinal tears

Objective To evaluate the effect of vitreoretinal surgery with lens sparing technique in treating the detachment with giant retinal tear(GRT) associated with proliferative vitreoretinopathy(PVR). Methods Thirty one consecutive eyes with GRT underwent vitrectomy were analysed retrospectively. Operative techniques included peeling of pre retinal membrane, injection of perfluorodecalin liquid, retinotomy and retinectomy,endolaser,and silicon oil or C 2F 6 gas tamponade. Lens sparing vitrectomy was performed in 28 phakic eyes. Follow up period ranged from 11 to 34 months. Results Anatomic retinal attachment was achieved intraoeratively in 29 eyes. In 16 eyes of 28 eyes with postoperative cataract formation,3 eyes underwent cataract surgery with or without intraocular lens implantation. The corrected final visual acuity ranged from 0.4 to 0.01. Conclusion Most phakic eyes of retinal detachment with GRT PVR can be successfully operated on with an outcome of improving the visual acuity by using techniques of lens sparing vitrectomy, perfluorodecalin liquid and silcone oil tamponade.
2.A study on Genetic Polymorphism of GPT loci by FLDAS-PCR and PCR-RFLP

Objective To establish a new allele specific PCR method for GPT typing: and study GPT polymorphism by FLDAS-PCR and PCR-RFLP. Method 248 unrelated individuals of Han were genotyped using FLDAS-PCR and PCR-RFLP. Results Three genotypes were identified with DP of 0.631 in Han population. Typing results by FLDAS-PCR were completely consistent with those by PCR-RFLP. The allele frequencies were GPT * 1 = 0.5423 and GPT * 2 = 0.4577. The population data fitted the Hardy-Weinberg law. Conclusion GPT-FLDAS-PCR and GPT-RFLP method is useful for forensic identification.
3.Analysis of Dynamic Hip Screw (DHS) for the treatment of femoral intertrochanteric fracture in 54 cases

Objective To Summarize experence of Dynamic Hip Screw for the treatment of femoral intertrochanteric fracture. Methods 54 cases with femoral intertrochanteric fracture were treated by DHS internal fixation. Results All patients were followed up for 4~36 months with an average of 7 months. Satisfactory effect was obtained. Conclusions The recovery of intact posterointernal cortex of femoral intertrochante, the standard postiton of fixation of DHS and an incarease in postoperative functional exercise are the key to obtain a satisfactory therapeutic effect.
4.Prevention and Treatment Policy of Pan-drug Resistant Bacteria for Surgical Critical Patients

OBJECTIVE To investigate the distribution scope of the pan-drug resistant bacteria in the SICU patients including their producing environment and high risk factors in these two years and to approach its prevention and treatment policy. METHODS The distribution scope and the high risk factors of the pan-drug resistant bacteria in SICU were reviewed and analyzed from Jan 2005 to Jan 2007.RESULTS There were 11 cases in the period from Jan 2005 to Jan 2006; and was only 1 case from Jan 2006 to Jan 2007. CONCLUSIONS To establish the drug resistance monitoring system, attach great importance to isolation and education, to monitor the original region of the pan-drug resistant bacteria, and to adopt the comprehensive antibiotic policy to control the drug resistant bacteria. Among them, the early effective isolation of high risk patients may be very effective to reduce the producing and developing of the pan-drug resistant bacteria.
5.Diagnostic Value of MSCT and MRI for Stasis Cirrhosis

Objective To investigate multi-slice spiral CT (MSCT) and MRI features of stasis cirrhosis and the diagnostic value of MSCT and MRI. Methods MSCT and MRI findings of 35 patients with stasis cirrhosis were studied. The size of liver and spleen, the diameter of hepatic vein (HV), enhancement pattern of liver parenchyma, contrast medium reflux in inferior vena cava (IVC) and (or) HV, ascites, number of varices and correlated abnormalities were reviewed. Results The volume index of liver and spleen of 35 patients was 4 434.95 cm3 and 621.92 cm3 respectively. The mean diameter of HV of 27 patients (77.1%) was 3.61 cm and HV of other 8 patients (22.9%) were too small to show. Number of patients showed waves of borderline, inhomogeneous pattern of parenchymal contrast enhancement, contrast medium reflux in IVC and (or) HV, varices and ascites was 5 (14.3%), 29 (82.9%), 20 (57.1%), 16 (45.7%), and 6 (17.1%), respectively. Correlated abnormalities included cardiac enlargement 4 cases (11.4%), pericardium thickening 11 cases (31.4%), and pericardial effusion 2 cases (5.7%). Conclusions Stasis cirrhosis mainly demonstrate liver enlargement, inhomogeneous pattern of parenchymal contrast enhancement, contrast medium reflux in IVC and (or) HV, and slight portal hypertension. MSCT and MRI play invaluable roles in diagnosis, differential diagnosis and etiological diagnosis of stasis cirrhosis.
6.Comparison of 20 MHz B-scan ultrasound with spectral domain optical coherence tomography in the diagnosis of maculopathy

BackgroundThe morphological examination of macular disease is very important information for the early diagnosis.Spectral domain optical coherence tomography (SD-OCT) is a gold criteria in the diagnosis of macular diseases,but its clinical application is limited in the eyes with refractive medium opacity.The resolution ratio of conventional B-scan sonography is unsatisfactory.20 MHz B-scan sonography may be a new approach to maculopathy.ObjectiveThis study was to evaluate the clinical value of 20 MHz B-scan ultrasound by comparing morphological changes of different maculopathies observed by 20 MHz ultrasound and spectral-domain OCT.MethodsA prospective study was designed.51 eyes of 51 patients with macular diseases who were determined by Tianjin Medical University Eye Center were enrolled in this study.Spectral-domain OCT was used to image the macular area,and then the 20 MHz B-scan ultrasound was used to examine the same eye to assess the value of 20 MHz B-scan ultrasound diagnosis.The macular diseases were typed according to the OCT manifestation.The characteristics of 20 MHz ultrasound for the same maculopathy were analyzed.All the patients subscribe the informed consent before the study.ResultsIn 10 eyes with macular hole determined by spectral-domain OCT,shallow dome-shaped lesion with central hollow was exhibited on the ultrasound image.The macular edema,retinal neuroepithelium layer detachment and retinoschisis were found in 16 eyes,6 eyes and 4 eyes by spectral-domain OCT,but the 20 M Hz B-Scan ultrasound presented with the focal belt-shaped elevation with non-echo zone below.In contrast,in 15 eyes of retinal pigment epithelium detachment in OCT,spectral-domain OCT showed stronger and thicker belt-shaped echo.Conclusions20 MHz B-scan ultrasound has certain value in the diagnosis and differentiation for several common maculopathies.Although it is not accurate as OCT in recognizing diseases,it could play an important role in refractive media opacity eye.
7.Diagnostic value of chest X-ray film of pulmonary emphysema signs for in-fantile bronchitis

Objective To evaluate the diagnostic value of chest X-ray film of pulmonary emphysema (PE)signs for infantile bronchitis.Methods Clinical data of 60 infants with bronchitis (case group)in a hospital between Janu-ary 2010 and December 2014 were analyzed retrospectively,and compared with data of 30 infants with non-respira-tory diseases (control group).Results Of 60 infants with bronchitis in case group,95.00%(57/60)showed mani-festations of PE on X-ray,and 18.33% (11/60)of whom were with mild PE(apical or intercostal pneumocele), 76.67%(46/60)were with typical PE (diaphragm descent);one case (3.33%)in control group showed intercostal pneumocele,the other 29 cases (96.67%)were normal X-ray findings and without signs of PE.The sensitivity, specificity and accuracy of PE signs on chest X-ray film for diagnosing infantile bronchitis were 95.00%,96.67%, and 95.56% respectively.Conclusion Signs of PE on chest X-ray film have important diagnosis value for infantile bronchitis.
8.Establishment of a transgenic zebrafish line with tdTomato expression in the ultraviolet-sensitive cone photoreceptors

Objective To better understand the mechanisms of cone opsin transport , we set to generate a trans-genic zebrafish line with red fluorescence proteins expressing in the cone photoreceptors .Methods We used sws1 promot-er to drive the expression of a chimerical protein , in which the last 44 amino acids of rhodopsin of Xenopus laevis were fused to the C-terminus of tdTomato to restrict its localization to the outer segment of photoreceptors .Results We successfully i-solated such a transgenic zebrafish line and confirmed the localization of tdTomato by immunostaining analysis .Conclu-sions This transgenic zebrafish line will help us to better understand the transport mechanisms of cone opsins , especially the transport in live photoreceptors .
9.Clinical characteristics and homology of Acinetobacter baumannii infection in patients in a respiratory intensive care unit

Objective To study transmission characteristics of Acinetobacter baumannii (A.baumannii )clone and clinical features of infected patients in a respiratory intensive care unit (RICU),so as to guide the effective pre-vention and control of A.baumannii transmission.Methods 40 A.baumannii strains isolated from RICU patients were performed homology analysis by pulsed-field gel electrophoresis (PFGE)and cluster analysis,relationship of strains was determined,antimicrobial resistance and clinical data were analyzed.Results 40 A.baumannii strains were divided into 11 genotypes(A-K),the main epidemic strains were type A,B and E.The resistant rate to imipen-em was the lowest(70.00%),the next was levofloxacin (77.50%).The average age of 40 infected patients was 67 years old,the average length of hospital stay was 41 days,12 patients died because of invalid treatment.There was overlapping hospitalization among patients infected with type A and E strains;among patients infected with type B strain,there was no overlapping hospitalization among the last 6 and first 6 infected patients.Conclusion The main epidemic strains of A.baumannii were type A,B and E,antimicrobial resistant rate is high;the infected patients are with high average age and long length of hospital stay.It is important to reduce the transmission of A.baumannii through rational use of antimicrobial agents,strict aseptic operation,and intensified disinfection and sterilization of hospital environment and medical devices.
10.Propofol inhibits the metastasis of tumor cells via activating PI3K/Akt signaling pathway

ABSTRACT:Objective To study the effects of propofol on the metastasis of tumor cells related PI3K/Akt signaling pathway.Methods The breast cancer model was established by transplanting human derived breast cancer cell lines into immunodeficient mice with naked gene.The mice,inoculated successfully,were randomly divided into 4 groups:control group (C group,n =6),propofol group (P group,n =6),propofol+PI3K inhibitor (BYL71 9)group (P+B group,n =6),and PI3K inhibitor group (BYL71 9)(B group,n =6).The expressions of PI3K,p-Akt and Akt were examined by Western blot at week 4 after administration;the gene levels of PI3KR1, Akt1 and Akt2 were detected by RT-PCR at week 4 after administration;the number of metastatic lung nodules from both lungs was also observed at week 4 after administration.Results Compared with those in C group,the expressions of PI3K and p-Akt were significantly higher in P group (P <0.05),the level of PI3KR1 mRNA but not Akt1 and Akt2 mRNA was significantly increased(P < 0.05 ),and metastatic lung nodules significantly decreased (P <0.05).In B group,the expressions of PI3K and p-Akt were significantly decreased (P <0.05 ),the levels of PI3KR1,Akt1 and Akt2 mRNA were not significantly increased (P >0.05),but metastatic lung nodules significantly increased (P < 0.05 ).Compared with those in B group,in P+ B group the expressions of PI3K and p-Akt were markedly higher (P <0.05),the level of PI3KR1 mRNA but not Akt1 and Akt2 mRNA was significantly increased (P <0.05),and metastatic lung nodules significantly decreased (P <0.05).Conclusion Propofol can inhibit the metastasis of tumor cells through the upregulated and activated PI3K/Akt signaling pathway.
