2.The Clinical Observation on Reflux Laryngitis

Objective To summarize the clinical diagnosis and treatments of 55 reflux laryngitis (RL) pa-tients, and to study the relationship between gastroesophageal reflux disease (GERD) and RL. Methods The pa-tients went through the following diagnostic process consisting of the collection of medical history, laryngoscopy, gastroscope, X-ray barium meal examination, vocal evaluation, therapy of anti-acid and anti-reflux. Results The laryngeal manifestations of RL mainly consisted of laryngeal discomfort or foreign body sensation (89. 1%), chronic cough (36.4 % ), hoarseness (20 % ), laryngeal pain (18. 2 %), even severe aspiration or laryngismus (10.9%) and so on. With the laryngoscopy we could see congestion in arytenoids area (67.3%), swelling or pachynsis of a mucosal fold in interarytenoid area (43.6%), edema of true or false vocal cord(40%), a little dropsy staying in piriform sinus (32.70% ), contact ulcer or granuloma on posterior extremity of a vocal cord (20 % ), artificial sulcus under vocal cords (16.4 %). RL patients might have mild voice disorder, among whom 65.5 percent displayed ten-sion dysphonia. Fundamental frequency (F0), jitter, shimmer and normalized noise energy (NNE) of RL patients increased. Compared with the controls, there were significant differences (P<0. 05) except for the F0 of female pa-tients. The manifestation and physical symptoms were improved or disappeared after the treatment with proton pump inhibitor (PPI). Conclusion RL is one of the important manifestations of GERD out of digestive tract. Typi-cal RL manifestation is mainly a pathological process in the postero-glottis. Clinically, PPI can be used as not only a treatment but also a tentative diagnosis of RL.
3.Studies on RNase H of hepatitis B virus DNA polymerase to up-regulate the expression of cellular apoptosis susceptibility gene

Objective To study the activation effect of HBV RNase H protein on the transcription of cellular apoptosis susceptibility gene (CAS). Methods The promoter of DNA sequence of CAS gene was identified in GenBank by bioinformatics and amplified from HepG2 genome by PCR using sense (5′-GGTACCCGATTACATGTTGTACATGAAGG-3′) and antisence (5′-CTCGAGGCTGAGTTCCATTGCTATAG-3′) primers. As these primers contained Kpn I and Xho I recognition sites on their respective 5′-ends, the amplified DNA fragments were tested by sequencing and then subcloned into Kpn I/Xho I sites of pCAT3-Basic reporter vector by routine molecular biological methods. The reconstructed plasmid named pCAT3-CASp was identified by enzyme digestion of Kpn I/Xho I, in which the expression of chloramphenical acetyltransferase (CAT) was under the control of the promoter of CAS. The HepG2 cells were transfected by pCAT3-CASp, and then co-transfected by pCAT3-CASp and pcDNA3.1(-)-RH plasmids. At the same time, the empty pCAT3-basic and pCAT3-TXNRD1p were transfected (self-contructed by the authors) as controls. After 24h culturing, cells were collected and the expression of CAT activity was detected by ELISA according to the manufacturer′s protocol. Results The optical density of expression of CAT of pCAT3-CASp was 0.043 by ELISA, in contrast, the optical density of expression of pCAT3-Basic was 0.024. The expression of CAT in co-transfection of pCAT3-CASp and pcDNA3.1(-)-RH(0.065) was 1.5 times as higher as pCAT3-CASp plasmid (0.043), and 2.7 times as higher as pCAT3-Basic. Conclusions The CAS gene promoter identified in present study has transcription activity and HBV RNase H protein may activate the expression of CAS gene.
4.The long-term outcome of the patients with severe coronary heart disease treated by homemad Costant coronary stent

Objective To evaluate the safety and clinical effect of homemad Costant coronary stent implantation.Methods The immediate and two-year follow-up results of Costant coronary stent implantation in severe coronary heart disease(CHD)patients were retrospectively analyzedin the General Hospital of Shenyang Command of PLA and the Guanzhou People's Hospital from Mar.2001 to Aug.2003.Results Implantation of 86 homemad Costant coronary stents was successfully performed in 71 severe CHD patients although 3 main adverse cardiac events(MACE)occurred during hospitalization(2 patients died and 1 patient had sub-acute in-stent thrombosis).Follow-up was made in 63 patients for(21.5?4.4)months on an average and 4 patients died.Repeat coronary angiogram in 21 patients showed that in-stent restenosis for Costant stent existed in 4 patients and in-stent restenosis in other types of stent was found in 3 patients and in-stent restenosis in all stents was found in 1 patient who died of shock and neo-coronary lesions was found in 5 patients.Costant stent restenosis rate was 7.9%(5/63);MACE rate was only 12.7%(8/63)during follow-up.Conclusion Homemad Costant coronary stent can be safely implanted in severe CHD patients with ideal long-term outcomes.
5.Analysis on safety of combination therapy with amlodipine plus terazosin in middle aged and old male patients with essential hypertension

Objective To evaluate the safety of combination therapy with amlodipine and terazosin in middle aged and old male patients with essential hypertension.Methods Randomized,prospective,parallel study was carried out in middle aged and old male patients with essential hypertension in Anqing between August 2005 and February 2006.In this article,508 patients were chosen to appraise safety of the combination therapy.Results ①After 4 weeks,the heart rate of combination group changed from 68.9?7.7 to 67.7?6.9.There was no significant difference before and after treatment.②The combination group didn't affect the levels of blood glucose,lipid,electrolyte and the function of kidney and liver.③As regards tolerability,adverse experiences were observed in 4.1%,12.3%,and 13.2% of patients,respectively.Orthostatic hypertension did not happen in three groups during treatment.Conclusions The combination of amlodipine and Terazosin have no influence on the levels of blood glucose,lipid and the function of kidney and liver.Moreover,the combination was tolerated as well as its components.Through the adjustment of drug dosage,orthostatic hypotension did not happen in three groups during treatment.
6.Influencing Factors on Popularization of Tap Water in Endemic Fluorosis Areas

Objective To know the influencing factors for the popularization of tap water in endemic fluorosis areas. Methods The families were randomly selected from Dongdingwang and Xinzhuang in Donghai county,Jiangsu province and were investigated with questionnaire and all members from these families were diagnosed for dental fluorosis with Dean’s method on May 2006. The influencing factors were analyzed with multifactor non-conditional Logistic regression. Results There was no significant difference on the popularization of tap water between the 2 villages (P=0.454). The relative location of the home in village was the risk factor. The size and the economic condition of the families and the householder’s knowledge on the tap water were protective factors. Conclusion The popularization of tap water should be extremely improved based on the improvement of the main influencing factors.
7.Exploration and Practice of the Reform of Pharmacochemistry Experiment Teaching for Pharmacy Major in Our School

OBJECTIVE:To improve the level of pharmaceutical experiment teaching for pharmacy major. METHODS:The ex-periment teaching of pharmacochemistry for pharmacy major in our school was taken as a pilot,and the teaching contents,teach-ing methods and appraisal system were explored. RESULTS:The teaching contents of“designing experiment”of pharmacochemis-try for pharmacy major in our school were added,multimedia teaching mode was introduced and scientific and comprehensive as-sessment mode was established. CONCLUSIONS:The reform of experiment teaching has cultivated the independent thinking and creative ability of students,improved enthusiasm and initiative,achieved good teaching quality and promoted the improvement of professional quality of teaching staff.
8.Comparative Study on the Level of Resistin,Adiponectin between Blood Stasis Syndrome and Non-Blood Stasis Syndrome in Diabetic and Primary Hypertension Patients

Objective To investigate the correlation between resistin,adiponectin and the blood stasis syndrome in diabetic and primary hypertension patients.Method The primary hypertension and diabetes patients were drawn vein blood to make plasma and serum.Enzyme linked immunosorbent assay(ELISA) method was used in checking serum adiponectin and resistin.Radioimmuoassay method was applied in checking serum insulin,leptin,ET and tumor necrosis factor(TNF).The fasting blood sugar,total cholesterol(TC),triamid-glycerin(TG),high dense lipoprotein-cholesterol(HDL) and low dense lipoprotein-cholesterol(LDL) were detected by the chemi-enzymic method.The relation between the indexes and blood stasis syndrome was analyzed by SAS9.1 for windows.Results The detectable rate of the blood stasis syndrome in the diabetic patients was 0.423,while 0.246 in the primary hypertension patients.The difference was significant in statistics.Incidence of blood stasis syndrome increased with the course of illness.Compared with the non-blood stasis syndrome group,the content of leptin,ET and TNF in the blood stasis syndrome group significantly raised,but the content of adiponectin and insulin diminished.The content of resistin did not have obviously change.Compared with the primary hypertension patients,the level of leptin,adiponectin,resistin,insulin in the diabetic patients raised,but there wasn't significant difference in the level of ET and TNF.Conclusions There is negative correlation between adiponectin and the blood stasis syndrome.The link between obesity and the blood stasis syndrome may be through fat factor regulating vasoactive factor.The blood stasis syndrome caused by obesity maybe have nothing to do with the consistency of resistin.
9.Distribution of Common Pathogens in Clinical Blood Culture and Their Antibiotic Resistance Analysis

OBJECTIVE To understand the distribution of common pathogens and their drug resistance trend and to provide the basis of the correct selection of antibiotics for clinic.METHODS Retrospective analysis was taken for blood culture specimens during 2 years in our hospital.And statistical analysis was done.All of 4028 cases of blood culture specimens were detected on automatic BacT/Alert3D rapid blood culture system,strains isolated were taken to VITEK-2 automatic microbiological analysis/sensitivity system for identification and drug susceptibility testing.Drug susceptibility results were analyzed using WHONET 5.3 software.RESULTS Totally 435 pathogen were isolated from 4028 cases of blood culture sample and the positive rate was 10.8%.195 Gram-negative bacteria strains accounted for 44.8%,mainly Escherichia coli,Klebsiella pneumoniae,Burkholderia cepacia,Acinetobacter baumannii,and Pseudomonas aeruginosa.Gram-positive bacteria accounted for 49.9%;mainly Staphylococcus epidermidis,S.haemolyticus and S.aureus dominated.Fungi were 23 strains(5.3%),mainly Candida albicans.Among them,extended-spectrum ?-lactamases(ESBLs) produced by E.coli and K.pneumoniae strains were 43.9% and 48.8%,respectively.Meticillin-resistant S.aureus and coagulase-negative staphylococci were 35.3% and 72.9%,respectively.But vancomycin-resistant S.aureus was not found.CONCLUSIONS The bacteria identification detected in blood culture class is quite complicated,and the drug resistance is high.Laboratories should increase the detection rate of bacterial culture and provide drug monitoring results for the clinics on time based on CLSI norms.Clinicians should use the antibiotics reasonably based on the drug susceptibility results in order to reduce nosocomial infections and the emergency of multiple drug-resistant strains.
10.Synergism in the cytotoxic effects of nedaplatin combined with adriamycin or mitomycin or Fluorouracil on human liver cancer cell line SMMC7721 in vitro

Objective: To investigate the suppressive effects of nedaplatin on human liver cancer cell line SMMC7721 and the interactions between nedaplatin and adriamycin or mitomycin or fluorouracil. Methods: The cytotoxic index of nedaplatin alone or combined with other chemotherapy agents on SMMC cells were detected by MTT method. Results: SMMC was sensitive to nedaplatin with a positive correlation between cytotoxic index and nedaplatin concentration. There were significant synergism in the cytotoxic effects of nedaplatin combined with adriamycin or Mitomycin or Fluorouracil on SMMC cells. Conclusion: Nedapltin is a promising agent in the treatment of human liver cancer.
