Objective To evaluate the role of mierosurgery in surgical treatment to urethrocutaneous fistula after urethroplasty in hypospadias and improve surgical results.Methods From 1999 to 2006,44 urethrocutaneous fistulae (more than 3mm in diameter) after urethroplasty for hypospadias in 33 patients were repaired with different skin flaps.For example,Thiersch technique,urthroplasty,etc.Microsurgical tech- nique was employed in every case.Results The success rates of different procedure were 84.8% (28/33) for Thiersch technique,100% (11/11) for urethroplasty respectively.The total success rate was 88.6%(39/ 44).Conclusion It's just application of skin flap for repairing of big or complex urinary fistula after hypos- padias surgery.The application of microsurgical technique can increase success rate.It is necessary to excise scar and partial urethra for hypospadias fistula combined with urethral structures,cicatricial eontracture and in- curvation of penis.Rich blood-supply,low tension and atraumatic technique are all very important to improve surgery success rates of urinary,fistula after hypospadias repair.

Objective To explore the reliability and validity of the Chinese version of driver stress inventory (DSI). Methods Three hundred drivers were investigated with DSI. The structure validity,content validity and internal reliability were examined. Results (1)Except items 9,16 and 18, the others had good discrimination. (2) Six factors were extracted which could explained 45.277% of the total variance. The six factors were significantly correlated with DSI total score(r = 0.241～0.622, P<0.01). (3)The spilt-half reliability was 0.557, The six factors ' Cronbach alphas were 0. 569, 0. 631, 0. 799, 0. 675, 0. 587 and 0. 685 respectively. ConclusionDSI has good reliability and validity, and is an effective instrument to test drivers' stress.

Objective:To explore the effect of feeding via the transpyloric route on the gastrointestinal function in very low birth weight (VLBW) infants and find the best early enteral nutrition protocol.Methods:Sixty VLBW infants were randomly devided into transpyloric feeding group(TP group) (n =30) and intragastric feeding group(IG group) (n =30).The frequency of apnoea,weight gain,the time of birth weight regain,feed intolerance,time of reaching full enteral feeding,the incidence of extrauterus-growth retardation (EUGR),motilin,gastrin,the length of hospital stay,necrotizing enterocolitis and duodenal perforation were observed in two groups.Results:The intolerance and time of reaching full enteral feeding were reduced significantly during transpyloric feeding compared with intragastric feeding (P ＜ 0.05).The number of episodes of apnoea was decreased significantly during transpyloric feeding compared with intragastric feeding (P ＜ 0.01).Conclusion:Transpyloric feeding can be used in VLBW infants.

Objective To investigate the current prevalent status of endemic fluorosis in the alluvial plain area of the Yellow River in Shandong Province,in order to provide a scientific basis for developing control measures against the disease.Methods Six counties were selected as survey counties by random sampling,ten improvedwater projects were selected in each survey county,for the projects,the operating effect was investigated and water fluoride content was tested;three epidemic villages were chosen as survey villages in each county,the operating condition and the fluoride level of water were investigated in the villages with water improvement projects.Dental fluorosis of children aged 8 to 12 was surveyed,the urinary fluoride and skeletal fluorosis X-ray diagnosis were checked of adults over the age of 25.Water fluoride and urinary fluoride contents were detected via the F-ion selective electrode method,and the prevalence of dental fluorosis of all children aged 8 to 12 was diagnosed based on Clinical Diagnostic Criteria of Dental Fluorosis (WS/T 208-2011),skeletal fluorosis X-ray diagnosis was done based on the National Standard for Diagnosis of Endemic Skeletal Fluorosis (WS 192-2008).Results ①In the six counties,58 improved-water defluoridation projects were investigated,all projects were operated normally,the qualified rate of water fluoride content was 44.83％ (26/58).②A total of 19 villages that had water-improving and defluoridation projects were investigated,and all projects were operated normally,the qualified rate of water fluoride content was 42.11％ (8/19).③The overall rate of dental fluorosis among children aged 8 to 12 years old was 49.28％ (480/974),with the index of dental fluorosis as 1.01 in 19 villages,the prevalence of dental fluorosis was mild.In the qualified villages of water fluoride concentration,the positive rate of dental fluomsis among children aged 8 to 12 was 33.41％ (148/443),dental fluorosis index was 0.65,and the prevalence of dental fluorosis was extremely low;in the exceeding-standard villages of water fluoride content,the positive rate of dental fluorosis among children aged 8 to 12 was 62.52％ (332/531),dental fluorosis index was 1.36,and the prevalence of dental fluorosis was mild;the detection rate of dental fluorosis among children aged 8 to 12 of the villages with qualified fluoride content was lower than the rate of the villages with excessive fluoride (x2 =81.91,P ＜ 0.01).④X-ray detection rate of skeletal fluorosis in adults over the age of 25 was 6.14％ (53/863) in 17 villages,there was no statistically significant difference between the qualified villages and the exceeding-standard villages of water fluoride concentration (x2 =1.55,P ＞ 0.05),X-ray detection rate of skeletal fluorosis in adults was 5.04％ (20/397) and 7.08％ (33/466),respectively.In the qualified villages and the exceeding-standard villages of water fluoride,urinary fluoride geometric average and the exceeding rate of urinary fluoride were 1.50 mg/L,44.16％ (170/385) and 2.09 mg/L,62.58％ (286/466),respectively.In the qualified villages of water fluoride,the exceeding rate of adult urinary fluoride was lower than that in the exceeding-standard villages (x2 =28.58,P ＜ 0.01).Conclusions The prevention and control measures on endemic fluorosis have had some effects in the alluvial plain area of the Yellow River in Shandong Province.But the water fluoride content of water improving defluoridation projects still exceeds the national standard seriously,and endemic fluorosis has not been effectively controlled.Prevention measures should be further strengthened.

Objective To investigate the gait differences among patients with Alzheimer's disease (AD) and mild cognitive impairment(AD-MCI) and the person with normal cognitive function,and analyze the reasons for the abnormal gait.Methods Eighty patients were included and divided into 3 groups according to the cognitive status:noncognitive impairment (NCI group,n =30),patients with mild cognitive impairment (AD-MCI group,n =29),Alzheimer's disease (AD group,n =21).The mini-mental state examination (MMSE),the Montreal Cognitive Assessment (MoCA),Addenbrooke's Cognitive Examination (ACE-R),the connection test (TMT-A),Digit Span Test(DST),activities of daily living questionnaire(ADL) and associated gait and balance scale (5 times sit to stand test (FTSTT),3 m walking test (TUG),Berg balance scale) were used to evaluate all research objects in each group.Results There were significant differences in NCI group compared with AD-MCI group and AD group in terms of FTSTT,TUG,Berg balance Scale and leg speed((9.59±2.39) s vs.(13.71±4.65) s vs.(14.15±4.20) s,(7.70±1.58) s vs.(11.13±3.02) s vs.(11.35±4.43) s,(54.20±1.56) points vs.(48.17±4.93) points vs.(48.10±5.46) points,(82.12±22.79) cm/s vs.(57.49± 14.89) cm/s vs.(57.70±14.68) cm/s;P＜0.05).There were significant differences in NCI group compared with AD-MCI group and AD group in terms of MMSE,MoCA,ACE-R,TMT-A,DST((28.67±1.27) points vs.(26.76±2.59) points vs.(21.86±5.29) points,(26.03±2.58) points vs.(22.39±5.05) points vs.(16.90±5.05) points,(85.80±5.90) points vs.(78.03±7.58) points vs.(60.95± 13.99) points,(12.66±5.36) s vs.(18.99 ± 11.46) s vs.(30.49±26.98)s,(18.60±4.64) points vs.(15.94±4.76) points vs.(12.86±5.23) points;P ＜0.05).Conclusion Gait disorder might be an early sign for cognitive impairment.

Obiective To expand the functions of the original data integration platform combined business process to achieve cross-system integration in hospital, Methods On the basis of efficient data interaction for every medical information system in hospital, more related function modules for the DIP were added. Results The new function modules can satisfy DIP multiple needs in the hospital daily work and make the DIP more powerful. Conclusion The applicability and rationality of DIP are improved by extended function.

Objective To investigate the association between single nucleotide polymorphisms (SNP)and expression levels ofα-adducin(ADD1)gene in coronary artery disease (CAD)patients.Methods Extracted DNA and RNA samples of peripher-al blood white cells from 114 CAD patients and 116 healthy individuals in Jan 2011 to Oct 2013 from the General Hospital of the PLA Rocket Force.SNPs of rs3775067 and rs1263359 mutations in the ADD1 gene were analyzed with allele-specific flu-orogenic oligonucleotide probes combining hybridization.The gene expression levels were analyzed with fluorescence labeled and capillary electrophoresis technology.Results The frequencies of the genotypes and alleles of the two SNPs in the ADD1 gene were not significantly different between the two groups (χ2=0.018~1.317,all P>0.05).The ADD1 gene expression levels of CAD group (0.226±0.284)were obviously higher than that of control group (0.153±0.144,P<0.05).The gene expression levels of TC genotype of rs3775067 were obviously higher in CAD group (0.250±0.319)than that of control group (0.154±0.156,P<0.05),but the levels of the other genotypes had no significant difference between the two groups (t=0.557~1.867,all P>0.05).Conclusion The elevated ADD1 gene expression level would be risk factor for CAD.The polymorphisms of rs3775067 and rs1263359 had no relevance with CAD susceptibility.
