1.Effectiveness of swallowing training in patients with dysphagia after acute stroke

Sixty patients with dysphagia following the acute stroke were enrolled and divided into training group and control group with 30 cases in each group.Both groups received the routine medication,but patients in training group had additional swallowing rehabilitation training.After 4 weeks,the recovery rate of swallowing function in training group was higher than that in control group [ 93 % (28/30) vs.63 %(19/30),P < 0.05 ] ; the incidence rate of aspiration pneumonia in training group was lower than that in control group[ 3.3% ( 1/30 ) vs.26.7% ( 8/30 ),P < 0.05 ].The results showed that swallowing training applied to acute stroke patients with dysphasia may promote the recovery of swallowing function,reduce the complications and improve their quality of life.

From the characteristics of nursing ethics and be-havior dynamic analysis, suggest to establish a focus on the humanities concern nursing ethics culture of the people-oriented, fundamentally effectively motivate nurses, specific include: respecting nurses and ethical personality differences;to establish a trust mechanism for nursing staff;meet the needs of nursing staff learning;to establish a nursing staff self management team, thus improve hospital nursing ethics management work, for the nursing staff to provide a stable and efficient working and living environment.
3.Clinical thinkings on the diagnosis of jaundice

Jaundice is a common clinic presentation which can be caused by various benign or life-threating disorders.Charaterization by unconjugated(indirect)hyperbilirubinemia and conjugated(direct)hyperbilirubinemia may help clinic diagnosis and treatment.The reasons causing unconjugated hyperbilirubinemia include hemolytic anemias,ineffective hematopoiesis,malfunction of bilirubin absorption and combination by hepatocytes or hyperbilirubinemia after hepatitis.Moreover,liver excretion disfunction by inherited disease,hepatocellular jaundice,or various obstruction of the duct system can lead to conjugated hyperbilirubinemia.This article discusses the category of jaundice according to different serum bilirubin,which warrants to clarify the cause quickly so that treatment can begin as soon as possilble.
4.Drug eluting stents prevent restenosis following stent implantation

OBJECTIVE:To discuss the research and clinical application of drug eluting stents,in addition,to explore the effects of drug eluting stents on preventing and curing coronary restenosis. METHODS:The databases of Medline and Chinese CNKI were retrieved using computer from 1993 and 2009,and the literature was selected according to inclusion and exclusion criteria. The rapamycin eluting stent and paclitaxel eluting stent were widely used in preventing and curing in stent restenosis,the problems existed in this process were discussed. RESULTS:Drug eluting stents could prevent in stent restenosis following stent implantation,however,the problems such as poor adherence or stent fracture still occurred in clinic. CONCLUSION:Drug eluting stents exhibit efficiency and safety in preventing restenosis,but there still existed problems need to further explore.
5.Analysis of 1 035 fine needle aspiration cytology of breast masses

Objective To analyse the sensitivity of fineneedle aspiration cytology (FNAC) of breast masses.Methods A series of 1035 FNAC of breast masses was reviewed.502 cases acccepted operation for histopathologic diagnosis.Results The sensitivity of FNAC for breast malignant tumor was 98.9% and the specificity was 85.7%.Conclusions The FNAC of breast masses has high accuracy.
6.Clinical characteristics of elderly patients with ulcerative colitis

Objective To analyze the clinical characteristics of ulcerative colitis (UC) in elderly patients.Methods A total of 166 cases with diagnosed UC were recruited.The clinical characteristics were compared between the elderly group (n=35,≥60 years) and the young group (n =131,20 59 years).Results The number of male patients were more than female patients in both groups.The incidence of abdominal pain was significantly lower in the elderly group than in young group (P<0.05).The number of severe patients were significantly lower in the older group than in young group.Severe UC prevalence was 27.5% (36 cases) in the young versus 11.4% (4 cases) in the elderly (x2 =3.89,P<0.05).The prevalence of total colitis was lower in elderly group than in younggroup [31.4% (11 cases) v,.51.1% (61 cases),~ =4.31,P<0.05],however,the prevalence of proctitis is more common in the elderly than in young group.The extra-intestinal manifestations and the ratio of receiving immune inhibitor and operation showed no significant differences between the two groups.Conclusions Compared with young group,there are a lower incidence of abdominal pain,a higher number of severe patients,a lower incidence of total colitis,and a higher incidence of proctiits in elderly group.
7.Costimulatory molecules and adhesion molecules in experimental colitis of rats

Objective:To investigate costimulatory molecules and adhension molecules in the pathogenesis of experimental colitis in rats.Methods:Models of experimental colitis were established by enema with trinitrobenzenesulphonic acid. Prednisolone or salicylazosulfapyridine was given as treatment.Nomal rat and untreated colitis rat groups were set up as controls.Histological scoring was performed.Expression of CD11b,CD28 and CTLA-4 were detected by flow cytometry in peripheral blood,spleen and intraepithelial mononuclear cells of colon.Results:Expressions of CD11b,CD28 and CTLA-4 were high in experimental colitis. CD11b and CD28 decreseased after treatment of prednisolone or salicylazosulfapyridine,while CTLA-4 increased further,which were significant statistically.Conclusion:Adhesion molecules may have a role in the pathogenesis of experimental colitis.The dysregulation of CD28 and CTLA-4 may be one of the mechanisms in IBD.The mechanisms of prednisolone and salicylazosulfapyridine may be associated with costimulatory molecules and adhension molecules.

9.Study on the activation and related mechanism of hepatitis B virus X protein on NLRP3 inflammasome

Objective To investigate the influence of hepatitis B virus X (HBx) protein on the activity of NLRP3 inflammasome as well as the mechanism of accelerating HBV-related hepatocellular carcinoma (HCC).Methods The HepG2 cell strains were divided into the 5 groups:blank control group (without plasmid transfection),empty vector group [transfected with pE green fluorescent protein (GFP)-N1 vector plasmid],fulllength HBx protein group (transfected with pEGFP-N1-X plasmid),HBx1-127 group (transfected with pEGFP-N1-X1-127 plasmid),HBx1-101 group (transfected with pEGFP-N1-X1-101 plasmid).(1) The expressions of HBx protein and NLRP3 inflammasome were detected by Western blot [Lipopolysaccharide (LPS) + ATP intervention was performed in the blank control group].(2) The HepG2 cells in the full-length HBx protein group were respectively intervened by glibenclamide and ammonium pyrrolidinedithiocarbamate (APDC),and the expressions of IL-1β and IL-18 were detected by enzyme-linked immunosorbent assay (ELISA).(3) The expressions of reactive oxygen were detected by flow cytometry.The measurement data with normal distribution were presented by (x) ± s.The one-way ANOVA was adopted in the comparison among groups while the t test was used in the pairwise comparison.Results (1) The results of Western blot showed:① the relative expressions of HBx recombinant plasmid fusion protein inside the HepG2 cells in the blank control group,empty vector group,full-length HBx protein group,HBx1-127 group and HBx1-101 group were 0.07 ±0.03,0.92 ±0.13,0.84 ±0.11,0.30 ±0.06 and 0.29 ± 0.05,respectively.The expressions in the HBx1-127 group and the HBx1-101 group represented the expressions of HBx1-127 protein and HBx1-101 protein.There were statistically significant differences among the 5 groups (F =61.790,P < 0.05).The relative expression of full-length HBx protein group was significantly different from that of blank control group,HBx1-127 group and HBx1-101 group (t =12.070,7.465,7.801,P <0.05).There was no statistically significant difference between full-length HBx protein group and empty vector group (t =0.867,P >0.05) and between the HBx1-127group and HBx1-101 group (t =0.146,P>0.05).② The relative expression of NLRP3 inflammasome protein inside the HepG2 cells in the blank control group,full-length HBx protein group,HBx1-127 group,HBx1-101 group and LPS + ATP group were 0.29 ±0.06,0.83 ±0.14,0.27 ±0.06,0.27 ± 0.05 and 0.90 ± 0.16,respectively,with a statistically significant difference among the 5 groups (F =29.550,P < 0.05).The relative expression of NLRP3 inflammasome protein of LPS + ATP group was significantly different from that of blank control group,HBx1-127 group and HBx1-101 group,respectively (t =6.310,6.565,6.741,P <0.05).There were statistically significant differences between the full-length HBx group and the HBx1-127 group or HBx1-101 group (t =6.381,6.584,P < 0.05) and no statistically significant difference between LPS + ATP group and full-length HBx protein group (t =0.580,P > 0.05).(2) The results of ELISA showed:①) the expression of IL-1β inside the HepG2 cells in the blank control group,full-length HBx protein group,HBx1-127 group,HBx1-101 group and LPS + ATP group was (87 ± 9)pg/mL,(587 ±56)pg/mL,(125 ±12) pg/mL,(113 ± 13) pg/mL and (677 ± 74) pg/mL,respectively,with a statistically significant difference among the 5 groups (F =139.010,P < 0.05).The expression of IL-1 β of LPS + ATP group was significantly different from that of blank control group,HBx1-127 group and HBx1-101 group (t =13.691,12.752,13.001,P <0.05).The expression of IL-1β of full-length HBx group was significantly different from that of the HBx1-127 group and the HBx1-101 group (t =14.051,14.283,P < 0.05).There was no statistically significant difference between the LPS + ATP group and the full-length HBx protein group (t =1.691,P >0.05).The expression of IL-18 in the blank control group,full-length HBx protein group,HBx1-127 group,HBx1-101 group and LPS + ATP group was (43 ±8)pg/mL,(252 ±38)pg/mL,(70 ± 13)pg/mL,(63 ± 10)pg/mL and (263 ±48)pg/mL,respectively,with a statistically significant difference among the 5 groups (F =44.010,P <0.05).The expression of IL-18 of LPS + ATP group was significantly different from that of blank control group,HBx1-127 group and HBx1-101 group,respectively (t =7.848,6.722,7.065,P < 0.05).The expression of IL-18 of full-length HBx group was significantly different from that of HBx1-127 group and HBx1-101 group (t =7.882,8.331,P < 0.05).There was no statistically significant difference between LPS + ATP group and full-length HBx group (t =0.326,P > 0.05).②The expressions of IL-1β and IL-18 in the HepG2 cells of the full-length HBx protein were (587 ± 91)pg/mL and (243 ± 22) pg/mL before the addition of glibenclamide,(115 ± 17) pg/mL and (90 ± 12) pg/mL after the addition of glibenclamide,respectively,with statistically significant differences before and after the addition of glibenclamide (t =8.800,10.566,P < 0.05).The expressions of IL-1β and IL-18 in the HepG2 cells of the full-length HBx protein were (573 ± 89) pg/mL and (252 ± 24) pg/mL before the addition of APDC,(124 ±21)pg/mL and (116 ± 15)pg/mL after the addition of APDC,respectively,with statistically significant differences before and after the addition of APDC (t =8.516,8.269,P < 0.05).(3) The results of flow cytometry showed that the relative expression of reactive oxygen in the HepG2 cells in blank control group,fulllength HBx protein group and LPS + ATP group were 66 ± 14,275 ± 54 and 388 ± 88,with statistically significant differences among the 3 groups (F =22.130,P < 0.05) and between the full-length HBx protein group or LPS +ATP group and blank control group (t =6.489,6.256,P < 0.05).There was no statistically significant difference between full-length HBx protein group and LPS + ATP group (t =1.887,P > 0.05).Conclusion HBx protein may play an important role in the occurrence and development of HBV-related HCC by activating NLRP3 inflammasome through inducing reactive oxygen generation in the HepG2 cells.
10.Clinical features of 82 patients with brucellosis in Liaoning Province

Objective To identify the clinical features of patients with brucellosis in Liaoning Province in recent 5 years, and to improve the diagnostic level of the disease.Methods The clinical data including epidemiology, clinical features, laboratory data and diagnosis of 82 hospitalized patients who were diagnosed with brucellosis in the First Affiliated Hospital of China Medical University from 2011 to 2015 were collected and reviewed retrospectively.Results The majority of the 82 patients were middle-aged (45-59 years old) (47.6%) and male gender (63.4%).Fever (90.2%), muscle and joint pain (61.0%), hepatosplenomegaly (36.6%), lymphadenopathy (24.4%) and weight lose (32.9%) were the main complaints.The peripheral leucocyte counts were usually normal (70.7%).C-reactive protein (81.3%), procalcitonin (81.8%) and erythrocyte sedimentation rate (64.2%) increased in most cases.The results of the serum agglutination test in 67 patients were all positive (100.0%).The pathogen isolation was conducted in 60 patients and 49 patients were positive for Brucella species.Among them, 35(71.3%) isolates were identified as Brucella melitensis.A total of 90.2%(74/82) cases were firstly diagnosed with fever of unknown origin and were hospitalized.Misdiagnosis accounted for 52.4(43/82)%.Conclusions Contact history with cow and sheep should be inquired thoroughly in patients with clinical manifestations of fever, muscle and joint pain and hepatosplenomegaly.Pathogen isolation, agglutination test and other specific tests should be performed as soon as possible to diagnose the disease early.
