This paper made an intensive study on existing problems of the current public hospital cost accounting system and proposed amendments by sorting out cost accounting′s content and related concepts.The application value of this paper is,on one hand,to provide a reference for the formulation of cost and expense standards in government accounting standards,and on the other hand,to better its cost accounting method,and to lay system foundation for increasing accuracy efficiency and timeliness of cost accounting.And corresponding proposals were offered to make public hospitals′cost accounting results better applied in hospital management,government pricing and social insurance payment.

Objective: To explore influence of dragon dancing sport on cardiac function and its mechanism. Methods: A total of 20 male students learning in physical education department of Physical Education College of Jishou University were enrolled, randomly and equally divided into dragon dancing sport group (dragon dancing group, received dragon dancing training) and control group (performed daily activity). Indexes of cardiac function were measured by color-code Doppler echocardiography before and 16 weeks after training. Results: Compared with control group after 16-week training, there was significant decrease in heart rate [(66.00±3.39) times/min vs. (60.50±6.02) times/min], significant increase in left ventricular early diastolic peak filling velocity（E）[(82.4±10.17)cm/s vs. (93.01±10.89)cm/s], rate of E/ A（left ventricular late diastolic peak filling velocity）[(1.89±0.24) vs. (2.29±0.47)], stroke volume [(65.5±10.61)ml vs. (75.59±13.35)ml], stroke index [(36.19±6.15)ml/m2 vs. (40.43±8.35) ml/m2], cardiac index [(2.39±0.47)L•min-1•m-2 vs. (2.46±0.65) L•min-1•m-2] in dragon dancing group, P<0.05 all. Conclusion: Dragon dancing sport can significantly improve cardiac function.

Inflammatory bowel disease (IBD) could lead to the complexity of intra-abdominal pathology and patients' malnutrition,which might greatly increase the risk of operation.Laparoscopic minimally invasive surgery is playing an important role in the treatment of IBD,with the advantages of faster postoperative recovery,shorter duration of hospital stay and lower incidence of postoperative complications.And some new techniques for IBD have been reported in the western countries,such as single-incision laparoscopic surgery,natural orifice transluminal endoscopic surgery,robot-assisted laparoscopic surgery and so on.However,the new techniques are not widely used in China,and it is necessary to carry out a series of clinical practices,which should be carefully selected for IBD patients undergoing minimally invasive surgery.

At present,the technology to predict the response to neoadjuvant therapy with biomakers has been widely used in clinical practice.The approaches of biomarkers detection are various,including immunohistochemistry,detection of serum biomarkers conventional blood tests,gene expression profile analysis,single nucleotide polymorphisms,miRNAs,proteomics analysis.With the development of biotechnology,the technology of biomarkers detection is expected to become effective means in assessment of adjuvant therapy,risk,prognosis and individualization in esophageal cancer treatment.

ObjectiveTo study the feasibility of laparoscopic assisted splenectomy for massive splenomegaly and devascularization in the treatment of patients with cirrhosis and portal hypertension.MethodA retrospective study was carried out on 12 patients who underwent laparoscopic assisted splenectomy and devascularization from October 2003 to January 2010.ResultsThe operation was successfully carried out in eight patients.The remaining four patients were converted to open surgery due to intra-operative bleeding.The mean operation times for the 12 patients and the 8 patients who underwent laparoscopic surgery were 350 min (range,190-470 min) and 313 min (range,190-410 min),respectively.The corresponding mean intra-operative blood losses were 1300 ml (range,200-2200 ml) and 700 ml (range,200-1700 ml),respectively.For the 8 patients who received laparoscopic surgery,flatus were passed 3-5 days after operation,and the postoperative hospitalization was 10-20 days.ConclusionsLaparoscopic assisted splenectomy for massive splenomegaly and devascularization is feasible.It is a treatment option for patients with cirrhosis and portal hypertension.

Objective To evaluate the efficacy of supportive psychotherapy on patients with cardiac neurosis. Method 32 patients with cardiac neurosis were treated with a comprehensive measure consists of adequate drug therapy and supportive psychotherapy. Symptom Checklist 90 (SCL - 90) was rated. Result Among the 9 factors of SCL - 90, scores of somatization, obsessive - compulsive, interpersonal relationship, depression, anxiety and phobic anxiety factors were significanly higher than that of normal controls. After treatment, the scores of somatization, depression, anxiety and phobic anxiety were statistically reduced. Patient- reported improvement efficacy was 84. 38%.Conclusion Adequate drug therapy together with supportive psychotherapy has reliable efficacy on patients with cardiac neurosis.

Objective To study the new nursing method of patients complicated with urinary fistula after gynecological malignancy operation. Methods Nursing measures were provided with sufficient nutritional support and prevention of urinary tract infection by perineal nursing, vaginal plugging by OB sliver,and using endovaginal catheter to 23 patients complicated with urinary fistula after gynecological malignancy operation. Results 18 patients recovered by treatment and nursing, 4 mild cases cured after discharge from hospital and 1 patient died of MODS and multiple metastasis. Conclusions Vaginal plugging by OB sliver, perineal nursing and sufficient nutritional support have positive effect for patients complicated with urinary fistula after gynecological malignancy operation.

Objective To study the role of ERK signal pathway in the endochondral ossification of bone mesenchymal stem cells ,and to explore the mechanism of ERK signal pathway in persistent enhanced FGFR2 function on development of mice BMSCs by a knock‐in mouse model with the FGFR2S252W/+ .Methods Mice with neo‐FGFR2 gain‐of‐function mutation were mated with EII‐Cre mice .The genotype of generation mice were identified by PCR and divided into wild type group and mutant type group ac‐cording to their genotype .6 week‐old mice were sacrificed to receive bone mesenchymal stem cells .The western blot was used to compare the level of P‐ERK and ERK and the RT‐PCR was applied to detect the genes of Col2 ,Col10 ,OC ,OP in chondrogenic dif‐ferentiation medium of BMSCs .Then ,treatment of cultured BMSCs with PD98059 ,compare the changes of genes and utilize the in vitro culture of long bones detect the role of ERK signal pathway in the endochondral ossification by FGFR2 mutant .Results We successfully derive BMSCs from FGFR2S252W/+ mutant mice and found the activity of ERK signal pathway of FGFR2S252W/+ was en‐hanced .After been cultured in chondrogenic differentiation medium ,the expressions of the BMSCs mRNA of Col2 ,Col10 from mu‐tant group were decreased ,while the expressions of OC ,OP were increased .Those OC ,OP genes levels showed an increased treated by PD98059 .Using in vitro culture of long bones ,we found the retardation of total length growth of long bones has been rescued by PD98059 treatment ,suggesting that ERK signal pathways was responsible for the retarded long bone development in FGFRS252W/+mice .Conclusion The results indicate these effects are mediated by the ERK signal pathway .Furthermore ,the retardation of long bones has been recued by PD98059 treatment ,suggesting that ERK signal pathway is responsible for the retarded long bone devel‐opment in FGFR2S252W/+ mice .

Objective To explore the relationship between fibroblast growth factor 21(FGF21) and islet β cell function in pregnant women with different glucose tolerance status.Methods A total of 441 pregnant women were selected in this study from our hospital.Their 50 g GCT at 24~28 gestational weeks were all positive.One week later,all the subjects were treated with 75 g OGTT,and divided into three groups according to their test results:GDM group (n=228),GIGT group (n=112) and GNGT group (n=91).Serum FGF21 level was tested by ELISA.Islet β cell function was evaluated by HOMA-IR,ISI-Matsuda,HOMA-IS,Stumvoll first,second phase secretion and ISSI.The correlation between FGF21 and islet β cell function was evaluated by Pearson correlation analysis.Results (1) BMI,0 h,1 h,2 h,3 hPG and 1 h,2 h,3 hIns were higher in GDM group and GIGT group than in GNGT group,and highest in GDM group (P<0.05 or P<0.01).SBP,DBP,0 hIns and HbA1c were higher in GDM group than in GNGT group and GIGT group(P<0.05 or P<0.01);(2)From GNGT group,GIGT group to DGM group,the levels of FGF21[(101.74±20.40) vs (137.93±25.52) vs (185.69±31.61) ng/L]and HOMA-IR[1.74(0.91,2.85) vs 2.39(1.31,4.87) vs 3.38(2.19,6.75)]were increased,while ISI-Matsuda[(58.74±15.68) vs (41.62±15.65) vs (39.73±18.98)],HOMA-IS[(157.69±88.41) vs (144.35±78.98) vs (107.30±87.23)]and ISSI[(72253.55±15167.53) vs (42313.91±7112.47) vs (30032.50±11500.24)]were decreased (P<0.05 or P<0.01).The stumvoll first,second phase secretion were lower in GDM group than in GNGT group and GIGT group(P<0.01),but there was no statistical significance between GNGT and GIGT group(P>0.05).(3)Pearson correlation analysis showed that FGF21 was positively correlated with HOMA-IR(r=0.255,P=0.030) and was negatively correlated with ISI-Matsuda,HOMA-β,Stumvoll first,second phase secretion and ISSI(r=-0.289,-0.256,-0.224,-0.230,-0.277,P=0.019,0.037,0.045,0.040,0.023).Conclusion Along with the worsening of glucose metabolic damage,the FGF21 level is increased gradually.FGF21 is related to islet β cell function,and may enroll in the occurrence and development of GDM.
