4.Strategy and Hypothesis of Molecular Targeted Therapy for Metastatic Colorectal Cancer

Colorectal cancer is one of the most commonly seen malignant tumors,and the metastatic colorectal cancer is of poor prognosis. The median overall survival for chemotherapy is only 1-1. 5 years. Recently,molecular targeted therapy targeting at vascular endothelial growth factor and epidermal growth factor receptor was used widely for metastatic colorectal cancer and becoming a hot spot of related researches. This article focused on the strategy and hypothesis of molecular targeted therapy for treatment of metastatic colorectal cancer.
5.A clinical interpretation of 2009 EASL guidelines on chronic hepatitis B treatment

With the availality of new nucleotides (acid) analogues,the experience of anti-virus treatment has been continuously enriched.However,the problem of drug resistance becomes increasingly significant and the concern of individualized strategies,duration and endpoints of therapy as well as drug-resistance prevention for nucleoside (acid) analogues treatment has become a hot issue.Thus,the European Association for the Study of the Liver updated their guideline on diagnosis and treatment of chronic hepatitis B based on the principles of evidence-based medicine,so as to make it more flexible and practical.We briefly review and compare the updated content of this new guideline,and make analysis combined with the clinical practice,so as to promote the standardization of diagnosis and treatment of chronic hepatitis B.
6.Effect of Removing Phlegm Softening and Removing Blood Stasis on Pulmonary Morphology and Hydoxyprolin of Pulmonary Fibrosis Rats

[ Objective] To observe the effect of removing phlegm,softening and removing blood stasis on pulmonary morphology and hydoxyprolin of pulmonary fibrosis rats. [Methods]The experimental pulmonary fibrosis models were produced by injecting BLMA5.All animals were randomly divided into 5 groups which were control group, model group, Chinese herb high-dose group, Chinese herb low-dose group and prednisone group respectively, the drugs were given daily starting from the second day and the rats were killed on the 7th and 28th day. The shape of rats’ lung tissues was observed, the levels of hydroxyproline in lung tissues were measured. [Results] In model group, alveolitis increases(much inflammatory cell infiltration), pulmonary interstitial fibroblast increases. There is less pulmonary tissue inflammatory cell infiltration in drug-given groups than that in model group and so is fibroblast. Compared with the model group, the content of HYP of pulmonary tissues of the drug-given groups is remarkably decreased. [Conclusion] The Chinese herb of removing phlegm, softening and removing blood stasis can effectively inhibit the formation of pulmonary fibrosis caused by BLMA5.
7.Application of nutritional risk screening in inpatients of digestive department

Objective: To investigate the prevalence of nutritional risk,malnutrition and application of nutritional support in department of digestive trad in our hospital in Ningbo.Methods: The information of 205 patients were collected in our hospital.Nutrition status was assessed according to Nutrition Risk Screening(NRS) published by ESPEN in 2002.Results: A total of 205 patients were enrolled,and 189 patients(92.2%)underwent NRS 2002.Prevalence of malnutrition was 20.0% and nutritional risk was 32.7%.Totally 18 patients(8.8%) receiving nutritional support had a NRS2002≥3.The PN∶EN ratio was 2.6 ∶1. Conclusion: A large proportion of inpatients were at nutritional risk or malnutrition in our hospital.The application of PN and EN is inappropriate in inpatients.Evidence-based guidelines are required to improve this situation.

9.Methods for Reducing Failure Rate of Monitor

10.Transcatheter closure of congenital heart defects in 16 cases using domestic-made occluders

Sixteen patients with congenital heart disease,4 males,12 females,aged 5-52 years,admitted to Department of Cardiology,Tianchang People's Hospital from October 2007 to November 2008 were retrospectively analyzed.All patients were diagnosed by echocardiography as 4 with congenital patent ductus arteriosus(PDA),6 atrial septal defect(ASD) and 6 ventricular septal defect(VSD).The diameter limitation of PDA,ASD and VSD are 5-17 mm,20-32 mm,and 4-10 mm,respectively.All patients received transcatheter-closure therapy by domestic-made occluders,and diameters of PDA,ASD and VSD were monitored using angiography and echocardiography.The treatment effect was evaluated by echocardiography 72 hours after procedure.The closure rate was 100%,without severe complications.No residual shunt was observed 72 hours after procedure.Four patients developed fever,including 2 acute respiratory infection and 2 pulmonary infection with balanced anesthesia;1 patient with ASD and 2 with VSDs developed arrhythmia.Results suggest that transcatheter closure of congenital heart defects(PDA,ASD and VSD) using domestic-made occluders is safe and effective according to strict indications.
