1.The comparative study of different nerve reconstruction for curing the brachial plexus root injury with wrist and finger drop

ObjectiveTo evaluate the influence of different surgical technique on the pup and adult rats after the rupture of upper trunk and middle trunk so as to find the best way of surgical therapy.MethodsFrom January 2010 to June of 2010, forty-eight Sprague-Dawley male rats were divided into two groups of 24 each, i.e., pup groups at the age of 7 days postnatal (P7)and one adult (3 months old)group.After C5-7 injured,each group was subdivided into three equally:group A (repair C5-7),B(repair C5,6),and C (no reinnervation performed).In the 3rd month postoperatively,the following tests were performed to estimate the recovery of each group:(1)Compared the rate of carpoptosis of each group.(2)Compared the regenerated nerve traversing rate of the radial nerve in each group.(3)HE stain to compared the cross-sectional area recovery rate.(4)The MyoD1 and Myogenin of the affected extensor of forearm were measured by Western blot.Results(1) The ethology test:there were no carpoptosis in group of adult A and pup A; the carpoptosis rate of pup B and pup C were higher than that of adult A(P < 0.05) ;the carpoptosis rate of pup A was lower than that of B and C(P < 0.05); there was no difference between carpoptosis rate of adult A and B,but they were both lower than C(P < 0.05).(2) Histological test of radial nerves:The percentage of myelinated both higher than C (P < 0.05).(3) Western blot:The protein level of MyoD1 and Myogenin in radial nerve fiber in experimental side of pup A,B and C descended progressively (P < 0.05).There was no difference between adult A and B,but they were both higher than C (P < 0.05).(4) Histological test of the extensor digitorium:There was no difference between cross-sectional area recovery rate of adult A and B,but they were pup A,B and C increased progressively (P < 0.05).There was no difference between protein level of MyoD1 and Myogenin in adult A and B,but they were both lower than C (P < 0.05),ConclusionIn adult rats, there is no difference between the recovery of wrist extensor of the two surgical procedure after the C5-7 injured. The restoration of C5-7 but not C5,6 can do more good to the recovery of wrist extensor in pup rats.It's important to repaire the C7 by nerve graft after injury of C5-7 in newborn.
2.Clinical efficacy and safety of total thyroidectomy combined with 131Ⅰradioiodine therapy for patients with hurthle cell thyroid cancer

Objective To investigate the clinical efficacy and safety of total thyroidectomy combined with 131Ⅰradioiodine therapy for patients with hurthle cell thyroid cancer(HCT). Methods From January 2001 to January 2013,20 HCT patients in our hospital,who under-went total thyroidectomy combined with 131Ⅰradioiodine therapy. The complications, the thyroid dysfunction and tumor recurrence were ob-served. Results The total efficiency of patients received total thyroidectomy combined with 131Ⅰradioiodine therapy was 100%, with no hoarseness,infection and other complications. And there was no tumor recurrence or death during follow-up. Total thyroidectomy combined with central lymphadenectomy had increased the chance of injury of parathyroid. There was no significant differences of PTH levels between simple thyroidectomy and total thyroidectomy combined with central lymphadenectomy in patients with different stages of treatment in 131Ⅰtreatment (P>0. 05). Conclusion Simple thyroidectomy combined with 131Ⅰradioiodine therapy causes less chance of injury of parathy-roid that restored within a month, which is high safety.
3.Suggestions to strengthen quality management of herbal decoction pieces--based on production chain of herbal decoction pieces.

With the development of society and the improvement of people's living standards, the effect of Chinese medicine in treatment and health care is more and more prominent. The herbal decoction pieces are the important part of Chinese medicine,it can be applied directly to clinical treatment and it's also the raw material of Chinese patent medicine. Therefore, the quality of herbal decoction pieces is quite important. The parts of the production of herbal decoction pieces are numerous, and there are possibilities of adverse effects on the quality of the herbal decoction pieces in every part. In this paper, we based on the production chain of herbal decoction pieces, analyzed the main problem that affect the quality of herbal decoction pieces in the part of selection of Chinese herbal medicines, planting, purchasing, processing, packaging, storage and transport, such as the poor quality of seed and seedlings of plant-based Chinese medicines, some plants left their place of origin and have been introduced in the place that is not suitable for this kind of plant, the insufficient growth time and the excessive harmful substances. The purchasers and the accepters lack of professional knowledge and professional ethics. The mechanism of processing is not clear, the standards can not be uniformed, and lack of qualified person in processing, etc. So we suggest: intensify the basic research of key scientific issues. Improve the quality of persons who work in herbal decoction pieces; Establish an "integration" mode of operation in herbal decoction pieces enterprise; Breeding high quality plant resources, establish the large-scale planting basement; Make the packing of herbal decoction pieces standard; Establish the modernization traditional Chinese medicine logistics enterprise.

5.Syndrome distribution among patients with chronic hepatitis C and interventions of integrated traditional Chinese and Western medicine: study protocol.

Background: Chronic hepatitis C is one of the major causes of end-stage liver disease with a high incidence rate, amounting to a grave and serious problem of public health. Currently, interferon-based (with or without ribavirin) antiviral therapy has limited use due to its stringent indications, possible contraindications and side effects. Traditional Chinese medicine (TCM) may have advantages in the prevention and treatment of chronic hepatitis C and it is of significant value to discover the advantages. Through this research, a safe and effective treatment protocol of TCM or integrated TCM and Western medicine for chronic hepatitis C can be formed. To this end, during China's Eleventh Five-Year Plan, special research projects on acquired immune deficiency syndrome (AIDS), viral hepatitis and the other major infectious diseases were established. Our studies on chronic hepatitis C constitute one of the major special research topics. Methods and design: Clinical information of patients with chronic hepatitis C will be first collected in a large, multicenter epidemiological survey. Positive symptoms will be analyzed by rapid cluster analysis, principal constituent analysis and factor analysis, and syndrome types will be diagnosed based on expert advice. Concurrently, a large, multicenter, randomized, parallel-group prospective study will be launched based on evidence-based medical principles to evaluate the effects and safety of the treatment protocol for chronic hepatitis C. The evaluated indexes will include the normalization rate of liver function, virological improvement and quality of life improvement for the short-term efficacy and the incidence of liver cirrhosis and (or) primary liver cancer and mortality for the long-term efficacy. Discussion: This study will investigate the TCM syndrome differentiation norms and the syndrome distribution rules of chronic hepatitis C and evaluate the efficacy and safety of a treatment protocol for chronic hepatitis C based on TCM theory or combined treatment of TCM and Western medicine. The study results will be helpful to developing a TCM treatment program for chronic hepatitis C. 6.Effect of lifestyle intervention on high-risk adults for hypertension

ObjectiveTo evaluate the effect of lifestyle intervention on high-risk individuals for hypertension. MethodsOne hundred and four prehypertensive adults who received physical examination during October 2005 and October 2007 and showed at least 2 hypertension risk factors were randomly assigned to the experimental or control group.A muhicomponent behavioral intervention,including health education(e.g.awareness of disease),metary gmdance,physical.exercise,and psychologtcat improvement,Was conducted for 2 years.The awareness of disease,blood pressure,bedy weight,and the change of risk factors were compared.ResultsAt 2 years,the awareness of disease in the experimental group wag significantly improved,and the blood pressure,body weight,and risk factors were obviously reduced.Although the awareness of disease and the risk factors in the control group were improved,the blood pressure and body weight were increased.The difference in average systolic blood pressure,average diastolic blood pressure,body weight,and the awareness of disease between the two groups were 13 mm Hg(1 mm Hg=0.133 kPa),7.9 nlnl Hg,3.7 kg,and 37.6%,respectively(all P<0.05).ConclusionThe lifestyle intervention among high-risk individuals for hypertension could effectively reduce blood pressure and body weight. Improved lifestyle would result in less risk factors.
7.Investigation on and analysis of the current situation of disinfection and sterilization in some basic medical institutions in Longzihu District,Bengbu City in 2007

Objective To learn about the current situation of nosocomial infections and disinfection and sterilization in some basic medical institutions,then to investigate strategy in regulating the management of nosocomial infections,and to propose the improving measure.Methods With unified questionnaire,the following were investigated:(1)the management,organization and rules and regulations of nosocomial infections and disinfection and sterilization,(2)installation and usage of disinfection armamentaria,and(3)usage and disposal after using of disposable medical products in some basic medical institutions.The data were analyzed statistically by SPSS 13.0 statistics software.Results Some basic medical institutions were deficient in organization,rules and regulations about the management of nosocomial infections,lacked disinfection armamentaria,with disinfectants often used incorrectly.The qualified ratio of the effect of disinfection monitored was low(66.9%).Medical wastes and disposable medical products were disposed of inappropriately.Conclusions To normalize the management of nosocomial infections,training should be reinforced,investment increased and awareness and health supervision enhanced.Meanwhile,air disinfection must be fulfilled more intensively.
8.Anesthesia and perioperative management for simultaneous liver-kidney-pancreas transplantation: a report of one case

Objective To evaluate the anesthesia and perioperative treatment for simultaneous liver-kidney-pancreas transplantation. Methods The preoperative preparation included improvement of the hepatic function, alleviation of uremia and control of blood glucose level. General anesthesia was employed and maintained with isoflurane combined with intermittent intravenous administration of midazolam, fentanyl and vecuronium. Dopamine and low dose of epinephrine were used to maintain arterial blood pressure if necessary. The parameters of blood coagulation, the indexes of hepatic and renal function, blood glucose and amylase levels in blood and urine were surveyed regularly. The treatment was adjusted according to the results of tests mentioned above. Results The circulation was stable and blood gas was normal in the course of surgery. The concentration of blood glucose was higher at the end of the operation than that of pre-operation. Normal hepatic and pancreatic function was achieved about a week after operation, while the renal function showed no satisfactory improvement. The patient was given hemodialysis until the second transplantation of kidney. Three days later, the renal function recovered to normal. Up to the present, all the grafted organs showed good function. Conclusion Appropriate preoperative preparation, optimal anesthetic procedure and management, perfect protection of function of multiple organs, and maintenance of stable hemodynamics and homeostasis were the key points of successful anesthesia and management for simultaneous liver-kidney-pancreas transplantation.
9.Effects of iodine on the thyroid sodium iodide symporter mRNA expression and thyroid function in rats

The effects of different iodide intakes on the thyroid sodium iodide symporter (NIS) mRNA expression, thyroid hormones, urinary iodine and tissue iodine in thyroid were observed. NIS mRNA expression was elevated; and urinary iodine, thyroid tissue iodine and thyroid hormones were significantly lowered in low iodine group. In high iodine group, NIS mRNA expression was inhibited and thyroid hormones decreased. The results show that NIS is the important component of this autoregulatory mechanism. Both low and high iodine intakes can lead to hypothyroidism.
10.Diagnostic value of string beads sign in peripheral small cell lung cancer

Objective To investigate the formation mechanism of string beads sign in peripheral small cell lung cancer (SCLC) and evaluate the significance of it in differential diagnosis among SCLC,peripheral lung adenocarcinoma and peripheral lung squa-mous cell carcinoma.Methods 78 cases of SCLC,69 cases of peripheral lung adenocarcinoma and 33 cases of peripheral lung squa-mous cell carcinoma,confirmed pathologically were included in this study.The positive rates of string beads sign,mediastinal lymph node metastasis and mediastinal lymph nodes larger than primary lung lesions were calculated and analyzed in these three groups.Results 10 out of SCLC cases (12.8%)have string beads sign,in which all mediastinal lymph nodes were larger than lung lesions.Mediasti-nal lymph node metastases were observed in 63(80.8%)of 78 cases,and 42 (53.8%)cases had larger mediastinal lymph nodes than lung lesions.No string beads sign was observed in patients with peripheral solid lung adenocarcinomas,but 25 of 69 cases (36.2%) have mediastinal lymph node metastasis and 2 cases (2.9%)had larger mediastinal lymph nodes than lung lesions.13 cases(39.4%) of 33 patients with peripheral lung squamous cell carcinomas had mediastinal lymph node metastasis,and 6 cases (16.7%)had larger mediastinal lymph nodes than lung lesions.The statistical results showed the positive rate of string beads sign was not significantly different between peripheral SCLC group and peripheral lung squamous cell carcinoma group,but that of mediastinal lymph node and larger mediastinal lymph nodes than lung lesions were statistically different among these three groups.Conclusion To some extent, string beads sign on CT could reflect the biologic character of SCLC.It played an important role in differential diagnosis of peripheral SCLC,peripheral lung adenocarcinoma and periph-eral lung squamous cell carcinoma,but it should be combined with mediastinal lymph node size.
