Objective To investigate the feasibility and value of combined administration of rapamycin and low dose of cyclosporin A in kidney transplantation recipients. Methods Twenty-five recipients who received their kidney transplantations for 2 months to 10 years were transferred to a new immunosuppression regimen of combined administration of rapamycin, low dose of cyclosporin A and prednisone. During the clinical observation, the average serum trough level of cyclosporin A was maintained between 0.042 and 0.083 μmol/L,and the average serum trough level of rapamycin was controled to 4~8 μg/L. Meanwhile, the dosage of immunosuppressive drugs was adjusted properly according to the white blood cells counting, liver function, blood lipid level and clinical settings.Twenty-five kidney transplant recipients with usual immunosuppression regimen of cyclosporin A in combination with MMF and prednisone served as controls. Results Druing a follow-up period of 2 to 8 months,no acute rejection events were found in this group. All the recipients survived well with functional renal grafts. Two patients in the experimental group quitted during the follow-up. One was incurred to serious pulmonary infection diagnosed as cytomegalovirus infection, and another was sufferred from persistent unknown fever. Other main adverse effects included leucopenia, hyperlipemia and dental ulcer, similarly as control group. No significant abnormal liver function event was found in this group. Conclusions This new immunosuppression regimen of combined administration of rapamycin, low dose cyclosporin A and prednisone in kidney transplantation recipients could avoid or diminish the adverse and toxic effects of calcineurin inhibitors, mainlain well-functional allografts and not increase the incidence of severe untoward reactions. Meanwhile, the costs for taking medicine were also significantly reduced with this regimen. Therefore,it would be an ideal and optimal immunosuppressive maintainence regimen.

Objective To observe the histopathologic and ultrastructural characteristics of cornea and adjacent conjunctiva in Mooren’s ulcer. Methods The samples of limbal and central cornea and adjacent bulbar conjunctiva taken from active Mooren’s ulcer after lamellar keratoplasty were cut into paraffin sections and ultrathin sections and observed by light and transmission electron microscopy. The samples taken from patients of a Terrien’s marginal degeneration and a bacterial corneal ulcer were used as controls. Results Chronic inflammation including lymphocytes and plasma cells infiltrating existed in bulbar conjunctiva and sclera of Mooren’s ulcer. Limbal corneal epithelium, Bowman’s membrane, anterior stroma and adjacent superficial sclera melted and the inferior stromal collagen disorganized. The epithelial basement membrane of ulcer progressive edge had been destroyed while the epithelium and stroma kept quiescent. Lymphocytes infiltrated in conjunctiva and corneal epithelium of Terrien’s marginal degeneration with normal epithelial basement membrane, while Bowman’s membrane was destroyed. The epithelial basement membrane of bacterial corneal ulcer was intact. Conclusion Bulbar conjunctiva may act as a local lymph node of Mooren’s ulcer. Epithelial basement membrane of Mooren’s ulcer may have some abnormality as it was invaded first during ulcer progressing and it’s valuable to have a further study.

Chinese Journal of Tissue Engineering Research 2007;0(44):-
Stent-assisted angioplasty of intracranial and extracranial arteries is a complex pathophysiologic process. Therefore, understanding of related factors is significant for guiding clinical treatment and reducing restenosis rate.Intravascular stent as a foreign body leads to platelet activation and aggregation, cytokine secretion, resulting in thrombosis and leukocyte aggregation at vessel injury site to produce cytokine and inflammatory reaction. In addition, the smooth muscle cells migrate to injury site and proliferate, leading to intimal hyperplasia, vessel wall reconstruction and intra-stent restenosis. Moreover, the restenosis following stenting is related to stent type and medicine supportive treatment.Drug-eluted stent with anti-inflammation and vascular smooth muscle cell proliferation can reduce body inflammation intensity and duration, and postoperative medicine supportive treatment can reduce incidence of intravascular restenosis.

BACKGROUND: The role of inflammatory reaction and inflammatory factors and the mechanism of pathophysiology of restenosisfollowing the stent implantation remains controversial. OBJECTIVE: To observe the changes of platelet activation markers and correlated factors following carotid artery stenting in rabbits. DESIGN, TIME AND SETTING: Observation experiment was performed at the Institute of Neurobiology of Henan University fromMarch to September 2008. MATERIALS: A total of 26 rabbits, male or female, weighing 2.1-2.9 kg, were included. The alloy stent was made of 90% platinumand 10% iridium with wire diameter of 0.13 mm and monofilament winding spiral structure. The alloy stent was submerged 100 g/Lgelatin solution, and made into protein-coated stent after dried. METHODS: Animals were anesthetized by sodium pentobarbital (30 mg/kg), and the right carotid artery was exposed. The stent was inserted in the proximal end after administration of aspirin and clopidogrel dual antiplatelet therapy. Penicillin was intravenously injected, 800 000 IU per day. MAIN OUTCOME MEASURES: The platelet-activating factor (PAC-1 and CD62P) of peripheral blood, platelet-monocyte aggregates and interleukin (IL-6) immediately before and 0.5 hour, 6 days, 1 month and 6 months after implantation were determined.RESULTS: Twenty-six rabbits were included in the final analysis. The level of PAC-1, CD62P and platelet-monocyte aggregates at 0.5 hour, 6 days and 1 month after inserting were significantly elevated compared with the levels immediately before inserting (P

Objective To investigate the effect of dexmedetomidine ( Dex) combined with ropivacaine in brachial plexus block ( BPB) on serum basic fibroblast growth factor ( bFGF) levels of upper limb fracture patients in the perioperative period. Methods 80 patients ( ASA gradeⅠ~Ⅱ) scheduled for selective upper extremity surgery were randomly divided into the dexmedetomidine combined with ropiva-caine group ( RD group) and the ropivacaine group ( R group) with 40 patients in each group. BPB was performed at the point of 2 cm below coracoid directed by nerve stimulator. Ropivacaine (200 mg) was diluted into 40 mL in group R and ropivacaine (200 mg) +1 μg/kg dexmedetomidine were diluted into 40 mL in group RD. Drew 5 mL blood at T0~T4 points respectively, and serum bFGF concentration were determined. Results Compared with group R,MAP and HR at T1~T3 points were significantly lower in group RD (P<0. 05). SpO2 in the two groups at each point had no statistical significance (P>0. 05). Compared with T0 points,bFGF concentration were increased in two groups at T1~T4 points (P<0. 05),and it was higher in group RD compared with group R (P<0. 05). Conclusion Dex combined bra-chial plexus block promote the secretion of bFGF more than simple brachial plexus block, which is conducive to fracture healing after opera-tion,but it should be cautious to apply for patients with bradyarrhythmia.
