1.The Stability of Tinidazole Mixed with Cefradin in Glucose In jection

Objective: To observe the stability of tinidaz ole mixed with cefradin in glucose injection under room temperature or 37 ℃ for 8 hours. Methods:Concentrations of tinidazole and cefrad in were determined by ultraviolet spectrophotometry and pH by a pH meter. Results: The concentrations of tinidazole and cefradin and the pH of the solution did not change significantly under room temperature and 37 ℃ for 8 hours. Conclusions: Tinidazole can be mixed with c efradin in glucose injection for clinical use under room temperature or 37 ℃.

Japan’s long-term care insurance takes grassroots governments as its insurer and the elderly and part of the non-elderly as its insured;The costs is shared by government、society和individual, and is, payed through isti-tutions, communities and hothes. Since its establishment, its security level has been increasing, the role of preven-tion has been rising, the status of community-based and home-based has been highlighting, and the cooperation a-mong relevant industry has been strengthening. But at the same time, its costs was soaring during the past decade, and the continued development of the system is facing transformation pressure now. Take this as a warning, the devel-opment of long-term care in China should learn its strong points, but also try to avoid its shortcomings.
3.In Vitro Cultivation Model of Cryptosporidium parvum in MDCK Cells and its Development

Objective To develop an in vitro culture system for Cryptosporidium parvum in Madin-Darby canine kidney(MDCK) cell and observe its life cycle(from desquamate to oocyst).Methods Oocysts of C.parvum were co-cultured with MDCK cells in vitro.Culture condition was optimized and the life cycle of C.parvum investigated.Results The optimal culture conditions for C.parvum in MDCK cells were 2.0?105 cells cultured for 12 h, and infected by 1.0?105 oocysts in the Dulbecco′s Modified Eagle Medium with 5% FBS.Following 72 h co-culture, desquamate, sporozoites, trophozoites, meronts, microgametocytes, macrogametocytes, zygote, thin-wall oocyst, and thick-wall oocyst appeared orderly.Between the 60th and 72th hour, many oocysts emerged.Inoculated by the C.parvum-infected cell culture supernatant at the 48th hour, the immunosuppressed mice became infected.Conclusion The culture system provides a model for propagation of the parasites and demonstrates a complete in vitro life cycle of C.parvum.

Chemotherapeutic drugs conventionally used for their cytotoxic activity can also exert their influence on the immune system by grooming the tumor microenvironment,abrogating immune tolerance and inciting immune reconstitution.Due to their immunomodulatory features,chemotherapeutics,used as a preconditioning regimen and combined with subsequent immunotherapy,can mobilize the immune system to generate antitumor immune response.The synergy between chemotherapy and immunotherapy has brought some enlightenment to the development of new protocols for cancer treatment.
5.Effect of salvia miltiorrhiza and ligustrazine on acute episode of chronic pulmonary heart disease

Objective: To observe effect of salvia miltiorrhiza and ligustrazine injection on patients with acute episode of chronic pulmonary heart disease (CPHD) complicated pulmonary artery hypertension. Methods: A total of 40 CPHD patients with pulmonary artery hypertension were randomly divided into routine treatment group (n=20, received routine treatment) and observation group (n=20, received salvia miltiorrhiza and ligustrazine based on routine treatment). Another 20 healthy subjects were regard as healthy control group. The pulmonary artery pressure (PAP), partial pressure of oxygen in artery (PaO2) , left ventricular ejection fraction (LVEF) and levels of C-reactive protein (CRP) and endothelin-1 (ET-1) were examined and analyzed in all groups before and after treatment. Results: After treatment, compared with routine treatment group, there were significant increase in total effective rate (75% vs. 90%) and PaO2 [(83.87±14.53)mmHg vs.(92.95±13.54)mmHg], and significant decrease in mean PAP [mPAP, (55.43±9.65)mmHg vs.(45.52±8.89)mmHg], CRP [(6.37±2.12) mg/L vs. (3.29±0.84) mg/L] and ET-1 [(52.37±20.79) ng/L vs. (40.29±16.04) ng/L] in observation group (P<0.05~0.01). There was no significant difference in LVEF between the two groups after treatment (P>0.05). Conclusion: For patients with acute episode of chronic pulmonary heart disease complicated pulmonary artery hypertension salvia miltiorrhiza and ligustrazine injection can significantly improve clinical effects,right heart function, decrease mean pulmonary artery pressure, improve endothelial function and inhibit inflammatory reactions.
6.Comparison of subjective refraction based on power vectors and conventional subjective refraction in different degree of astigmatism

Objective To compare subjective refraction based on power vectors and conventional subjective refraction in different degrees of astigmatism.Methods A total of 143 subjects (143 right eyes) were divided into two groups according to astigmatism degree:low astigmatism group (0-1.50 D) and high astigmatism group (1.50 -3.00 D).Subjects were then examined by both methods to fetch refraction data,and then the equivalent spherical M component,J0 component,J45 component and the duration of refraction procedures were compared and analyzed.Results There was no statistically difference in age,gender,M component,J45 component and time consuming of subjective refraction between the high and low astigmatism groups (all P > 0.05),but intergroup comparison of J0 component from subjective refraction based on power vectors and conventional subjective refraction was significantly different (all P =0.00).In the low astigmatism group,the agreement of M,J0 and J45 measured by the two methods was tmified,and the mean absolute value of M,J0 and J45 difference was 0.04 D,0.02 D and 0.02 D,respectively;moreover,within the limits of consistency,the maximum absolute value of the difference was 0.37 D,0.13 D and 0.17 D,respectively,and there was no statistically difference in time consuming of the two methods (P =0.72).In the high astigmatism group,the agreement of M,J0 and J45 measured by the two methods was poor,and the mean absolute value of M,J0 and J45 difference was 0.43 D,0.21 D and 0.00 D,respectively;moreover,within the limits of consistency,the maximum absolute value of the difference was 0.75 D,0.50 D and 0.30 D,respectively,and the difference in time consuming of the two methods approached statistical significance (P =0.00),suggesting time consuming of subjective refraction based on power vectors was longer.Conclusion Subjective refraction based on power vectors has good effect on low astigmatism,and it needs to be further improved for clinical practices.
7.Application of fibrinogen and D-dimer in the diagnosis of acute pulmonary embolism

Objective To evaluate the clinical value of fibrinogen and D-dimer(D-D)in the diagnosis of patients with acute pulmonary embolism.Methods A retrospective study was conducted.74 patients with acute pulmonary embolism were retrospectively analyzed.The general clinical data were gathered,and the patients were divided into high-risk group(n=20),moderate-risk group(n=32),and low-risk group(n=22)according to the 2008 ESC Guidelines on the diagnosis and management of acute pulmonary embolism.25 patients with physical examination were randomly recruited as control group.The plasma levels of fibrinogen and D-D were detected and compared between these groups.Receiver operating characteristic curve(ROC)was used to evaluate diagnostic biomarker performance.Results In acute pulmonary embolism patients,with the risk degree increased,the level of fibrinogen decreased[(4.20±0.82)g/L,(4.16±0.83)g/L,(3.62±0.74)g/L,(2.83±0.62)g/L](compared with control group,P=0.183,moderate-risk group,P=0.046,high-risk group,P=0.033;compared with low-risk group,moderate-risk group,P=0.041,high-risk group,P=0.037;compared with moderate-risk group,P=0.044),and the level of D-D increased[(1 845.20±3 939.56)μg/L,(4 405.27±2 356.68)μg/L,(4 360.63±2 675.40)μg/L,(16 817.00±6 878.66)μg/L](compared with the control group,low-risk group,P=0.392,moderate-risk group,P=0.042,high-risk group,P=0.027;compared with low-risk group,P=0.136,P=0.016;compared with moderate-risk group,P=0.035).The ROC curve showed that the AUC of D-D in each group were 0.865,0.834 and 0.974,respectively.AUC of FIB were 0.459,0.253 and 0.277,respectively,which were below or even significantly lower than predicted line area.Conclusion The performance of fibrinogen in the diagnosis of acute pulmonary embolism and the classification of the risk degree is very low.
8.Curative effects of microendoscopic discectomy for lumbar disc herniation

Objective To analyze clinical effects of microendoscopic discectomy in the treatment of lumbar disc herniation with or without lumbar spinal stenosis.Methods A total of 216 patients were included in the study.The patient was maintained in the prone position on an arch-like bed.Under C-arm fluoroscopic guidance,a thin pin was inserted down to the facet of the target level.A longitudinal incision was made approximately 1.7 cm at the level of the disease to introduce a series of dilators,over which a working channel was then established.Once extraneous soft tissues were removed,a curette was used to puncture the ligamentum flavum.And the local ligamentum flavum and part of inferior border of the adjacent lamina were removed with a gun-shaped rongeur.The disc space was incised and a discectomy was performed.The lateral recess was cleared of any bone stenosis.Results conversions to open traditional discectomy were conducted in 2 patients because of bleeding.The 216 patients were followed for 3 months ~ 4 years with an average of 2.5 years.According to the Nakai criteria,excellent outcomes were achieved in 210 patients,good in 2 patients,fair in 1,and poor in 3,the rate of excellent or good outcomes being 98.1%(212/216).Of the 3 patients with poor outcomes,2 patients were given a re-operation of traditional surgery 3 months after the microendoscopic discectomy and 1 patient had a missed diagnosis of spinal tuberculosis.Conclusions Microendoscopic discectomy in the treatment of lumbar disc herniation gives little influence to spinal stability and satisfactory clinical effects.
9.Study on Methods for Isolation and Purification of Cryptosporidium parvum Oocysts from Mouse Feces

Objective To explore an applicable method for isolation and purification of Cryptosporidium parvum oocysts with high purity, recovery and vigor from mouse feces. Methods Four techniques were used for isolating and purifying C.parvum oocysts from mouse feces: modified saturated saline flotation, percoll gradient centrifugation, CsCl gradient centrifugation and the classical discontinuous sucrose gradient centrifugation. Oocysts received from the methods were used respectively to infect in vitro bovine fallopian tube epithelial cells (BFTE) and the development of the oocysts was examined under microscope after 48 h and 72 h cultivation. Results The number of oocysts received by the classical discontinuous sucrose gradient centrifugation [(2.86?0.08)?107] was significantly higher than that of percoll gradient centrifugation [(1.52?0.08)?107] (P0.05). Oocysts received from CsCl gradient centrifugation showed higher purity than those by discontinuous sucrose gradient centrifugation. Conclusion In comparison to the classical discontinuous sucrose gradient centrifugation, operation of the modified saturated saline flotation is easier and faster, and the purity of oocysts isolated by CsCl gradient centrifugation is higher.
10.Laparoscopic versus open radical hysterectomy for endometrial cancer: A prospective,randomized,controlled study

Objective To investigate the clinical effect of laparoscopic radical hysterectomy in the treatment of early-stage endometrial cancer.Methods A total of 60 consecutive patients preoperatively diagnosed as having stage Ⅰ or Ⅱ endometrial cancer from August 1998 to December 2004 was divided into two groups,receiving either laparoscopic(Laparoscopic Group) or open radical hysterectomy(Open Group).According to the involvement of the muscular layer,whether or not a pelvic lymphadenectomy was required was determined.Perioperative parameters and survival time were prospectively studied.Results As compared with the Open Group,the Laparoscopic Group presented a longer operation time(247.8?77.8 min vs 196.6?63.7 min;t=2.789,P=0.007),similar blood loss(265.0?187.6 ml vs 350.0?210.9 ml;t=-1.649,P=0.104),similar number of resected lymph nodes(22.0?5.0 vs 22.8?5.2;t=-0.607,P=0.546),insignificant difference in complication rate (10/30 vs 15/30;?2=1.714,P=0.190),earlier time to first passing flatus(44.3?11.1 min vs 55.2?12.8 min;t=-3.524,P=0.000),and shorter hospital stay(8.7?3.2 d vs 10.2?2.0 d;t=-2.177,P=0.034).The patients in the Laparoscopic Group were followed for 28.6?17.4 months with tumor-free survival.In the Open Group,a follow-up for 28.9?16.6 months found 3 deaths form metastasis or recurrence.Conclusions Laparoscopic radical hysterectomy and pelvic lymphadenectomy may be an alternative for early-stage endometrial carcinoma.
