1.Efficacy of Iron Dextran Combined with EPO in the Treatment of Renal Anemia

Objective:To compare the intravenous (Ⅳ) administration with an oral iron agent administration in their efficacy and side effects in hemodialysis patients with renal anemia. Method:50 renal anemic patients were randomly grouped as the treatment group (TG) and controlled group (25 cases for each). 100 mg iron dextran injection was intravenously dropped twice weekly for the TG patients in the dialysis course, totally 10 times. 150 mg iron polysaccharide complex capsules was orally used qd in the controlled group. All the patients were treated in combination with 8000u of erythropoietin (EPO) weekly and observed for 8 weeks. The changes in Hb, serum ferritin level and agent side effect were monitored. Objective To study the consistency in the diagnosis of preoperative TNM rectal cancer staging using high resolution magnetic resonance imaging (MRI) and transrectal ultrasound (TRUS) combined with carcinoembryonic antigen (CEA) and postoperative pathological TNM.Methods 156 cases pathologically proven were retrospectively analyzed and divided into 4 groups including preoperative MRI group (39 cases),TRUS group (39 cases),MRI and TRUS group (39 cases),MRI and TRUS combined with CEA group (39 cases).The differences between preoperative T,N staging and postoperative pathologic T,N staging were analyzed.Results There were statistically significant differences in the diagnosis of preoperative T and postoperative pathological T in 4 groups (T: Kappa =0.685,P =0.000; N: Kappa =0.544,P =0.000),but there were no significant differences in preoperative N and postoperative pathological N staging in preoperative MRI group,TRUS group,MRI and TRUS group (Kappa =0.142,P =0.329; Kappa =0.154,P =0.645; Kappa =0.154,P=0.229),and significant difference was observed in MRI and TRUS combined with CEA group (Kappa =0.544,P =0.000).There were no significant differences in the accuracy of T staging among the 4 groups (x2 =0.326,P =0.574; x2 =0.562,P =0.719; x2 =0.287,P =0.986),but significant difference in the accuracy of N staging were showed among the 4 groups (x2 =4.643,P =0.026; x2 =6.643,P =0.026; x2 =5.243,P =0.019).Conclusion Preoperative evaluation by the MRI add TRUS combined with CEA can improve the accuracy of preoperative staging,which can provide more reliable basis for decision-making and improve the coincidence rate of operative procedures in line with the estimate.It also provides the basis fur the accurate preoperative diagnosis and individualized treatment.
3.Effects of Embolization and Clipping Therapy on Cognitive Function in Patients with Intracranial Aneurysms

Objective:To evaluate the effects of endovascular embolization and microsurgical clipping on cognitive function in patients with intracranial aneurysm.Methods:A total of 80 patients with intracranial aneurysms,who were treated in Chongqing Hospital of Traditional Chinese Medicine from March 2014 to September 2015,were selected and randomly divided into control group(n=40) and experimental group (n=40).The control group was treated with microsurgical clipping,and the experimental group,with endovascular embolization.The postoperative cognitive function was assessed by the mini mental state examination scale (MMSE),and length of stay,in-hospital mortality rate,mortality rate in 1 year and the incidence of postoperative adverse reactions were compared between the two groups.Results:The total score (26.78± 0.85) of MMSE in the experimental group was higher than that (22.25± 0.63) of the control group (P<0.05).The cognitive dysfunction rate (37.50%) of the experimental group was lower than that (55.00%) of the control group (P<0.05).The length of stay [(6.7± 3.9) days] of the experimental group was lower than that [(9.6± 4.5) days] of the control group(P<0.05).There were no significant differences in in-hospital mortality rate and mortality rate in 1 year,and the incidence of adverse reactions between the two groups (P>0.05).Conclusion:Endovascular interventional therapy can improve the cognitive function of the patients with intracranial aneurysms and shorten the length of hospital stay.

Cerebral apoplexy is one of the main causes of death in the middle-aged and elderly population,which has higher mortality and disability rate.The incidence of the disease is increasing year by year and it is a serious threat to human life and health.Therefore,it is of great significance to find an effective target for the diagnosis and treatment of stroke.Thioredoxin (Trx) is the major thiol reducing agent in the cells,it is involved in many signal transduction pathways in the cells by regulating the redox state of the cell.It has disulphide reductase activity,which can reduce the oxidative stress injury in the rats after the stroke.Thioredoxin interacting protein (TXNIP) is an endogenous inhibitor of Trx,it can destroy the redox balance and promote the oxidative stress by binding/inhibiting the activity of Trx,while the inhibition or knockdown of TXNIP has obvious neuroprotective effects.Recent studies suggest that Trx/TXNIP may be involved in the pathophysiology of cerebral apoplexy by a variety of pathways.This article analyses the research status of Trx/TXNIP and studies the localization of Trx system in the central nervous system and the progress of Trx system in ischemic cerebral apoplexy.It reviews the mechanism of Trx/TXNIP in cerebral apoplexy and prospectes the signaling pathways involved in the pathophysiological process of Trx/TXNIP to provide new ideas for the treatment of cerebral apoplexy.
5.Quality Monitoring and Nosocomial Infection Prevention in Newborn Spa Room

OBJECTIVE To understand disinfection quality in newborn Spa room,to give prevention messures against nosocomial infection.METHODS The air,UV intensity,the object surface,staff′s hands,and the using of disinfectant were detected.RESULTS The total qualified rate was 98.61% and that of the object surface was 97.22%,of staff′s hands was 98.61%,of UV intensity and indoor air was 100% and of the using disinfectant was 97.22%.CONCLUSIONS Strengthening the disinfection,finding the weak timely and taking the effective measures can prevent the occurrence of hospital infection.
6.Therapeutic Evaluation of ~(18)F-FDG Coincidence SPECT Imaging in Malignant Lymphoma

Objective To study the clinical value of ~(18)F-fluorodeoxyglucose(~(18)F-FDG) coincidence SPECT imaging in the therapy of malignant lymphoma. Methods Serial ~(18)F-FDG SPECT imaging were performed on 42 patients with malignant lymphoma before and after treatment.The results were compared with other conventional imaging.Patients were divided into two groups. Twenty-seven new-diagnosed patients(group Ⅰ) and 15 operated patients(group Ⅱ) had ~(18)F-FDG imaging pre-and post-chemotherapy. Results(In group Ⅰ,) 15 cases(achieved) clinical remission,five cases relapsed and one case progressed.In group Ⅱ,tumor residues were detected in four patients,and one patient relapsed. Conclusion Serial ~(18)F-FDG imaging during treatment is very useful for therapeutic evaluation in malignant lymphoma.

This study has investigated the effect of a potent bioflavonoid, troxerutin,on diabetes-induced changes in pro-inflammatory mediators and expression ofmicroRNA-146a and nuclear factor-kappa-B (NF-κB) signaling pathway in aortic tissueof type-I diabetic rats. Male Wistar rats were randomly divided into four groups(n = 6/each): healthy, healthy-troxerutin, diabetic, and diabetic-troxerutin. Diabeteswas induced by streptozotocin injection (60 mg/kg; intraperitoneally) and lasted 10weeks. Troxerutin (150 mg/kg/day) was administered orally for last month of experiment.Inflammatory cytokines IL-1, IL-6, and TNF-, as well as intercellular adhesionmolecule-1 (ICAM-1), vascular cell adhesion molecule (VCAM), cyclooxygenase-II(COX-II), and inducible-nitric oxide synthase (iNOS) were measured on aortic samplesby enzyme-linked immunosorbent assay. Gene expressions for transcription factorNF-κB, interleukin-1 receptor-associated kinase-1 (IRAK-1), TNF receptor-associatedfactor-6 (TRAF-6), and microRNA-146a were determined using real-time polymerasechain reaction. Ten-week diabetes significantly increased mRNA levels of IRAK-1,TRAF-6, NF-κB, and protein levels of cytokines IL-1, IL-6, TNF-, adhesion moleculesICAM-1, VCAM, and iNOS, COX-II, and decreased expression of microRNA-146a ascompared with healthy rats (p < 0.05 to p < 0.01). 9.The role of 18F-FDG SPECT-CT in detecting recurrence and metastases in breast cancer patients with elevated tumor markers.

OBJECTIVETo compare retrospectively the role of (18)F-FDG SPECT-CT and conventional imaging in the detection of recurrence and metastases in postoperative breast cancer patients with elevated level of tumor markers, and to evaluate the impact of (18)F-FDG SPECT-CT on the management of breast cancer patients.
METHODS(18)F-FDG SPECT-CT was performed in 35 breast cancer patients with suspected recurrence based on elevated level of serum tumor markers. Chest, abdomen and pelvic CT were performed in all patients and whole-body bone scan was performed in only 21 patients. The final diagnosis of recurrent breast cancer was confirmed by either pathology or observation by imaging during the follow-up for more than 1 year.
RESULTSAmong the 35 patients, the final diagnosis of recurrence or metastasis was established in 19 patients. Of the 114 sites of increased FDG uptake, 93 were interpreted as malignant and 21 as benign. On site-based analysis, the sensitivity, specificity, accuracy, positive and negative predictive values were 93.1%, 55.6%, 84.2%, 87.1% and 71.4%, respectively, for (18)F-FDG SPECT-CT, and 80.5%, 60.5%, 75.6%, 80.2% and 65.1%, respectively, for conventional imaging. On the patient-based analysis, the sensitivity, specificity, accuracy, positive and negative predictive values were 84.2%, 62.5%, 74.3%, 72.7% and 76.9%, respectively, for (18)F-FDG SPECT-CT, and 74.1%, 67.6%, 70.6%, 68.3% and 73.9%, respectively, for conventional imaging. The results of (18)F-FDG SPECT-CT led to changes in the subsequent clinical management of 40.0% of these patients.
CONCLUSIONIn postoperative breast cancer patients with elevated level of tumor markers during the follow-up, (18)F-FDG SPECT-CT is more sensitive for detecting recurrence and metastases than conventional imaging.

Objective To analyze the feasibility and curative effects of one-stage reconstructing the anterior and posterior cruciate ligaments injuries with autologous hamstring tendon using TightRope devices all inside.Methods Thirty-four patients who suffered from the anterior and posterior cruciate ligaments injuries referred to our center from July 2013 to July 2015.Thirty-two of them with the average age 37.4±9.2 years old (range:18-61 years old),including 23 male and 9 female,were followed up over 2 years.According to the anatomy classification for knee dislocation,14 patients were KD Ⅱ type,11 patients KD Ⅲ M type,7 patients KD Ⅲ L type.The interval from injury to surgery was 4-6 weeks.One-stage reconstruction of the anterior and posterior cruciate ligaments was conducted by using autologous hamstring tendon with TightRope device all inside under arthroscopy.Ipsilateral hamstring tendon was taken in the patients of KD Ⅱ type,while opposite hamstring tendon was used in the patients of KD Ⅲ.The semitendinosus graft acted as posterior cruciate ligament,and gracilis graft acted as anterior cruciate ligament.Fold the tendon graft in half into four strands,with the length 6-7 cm,diameter 7-9 mm,and link together with two TightRope devices.The anterior and posterior cruciate ligaments injuries were reconstructed in one-stage with autologous hamstring tendon using TightRope all inside.The medial or lateral collateral ligament in patients of KD Ⅲ type was repaired.The anterior and posterior drawer test,Lachman test,valgus stress test,IKDC classification,Lysholm scores were recorded before and after surgery to evaluate the efficacy of reconstruction.Results Thirty-two patients who were treated with surgery were followed up 2-4 years with average 2.6 years.Preoperative symptoms relieved apparently in the last follow-up in all patients.The anterior and posterior drawer test and Lachman test were negative.The IKDC score of 27 patients at two years after surgery were rated as normal (84%),while that of 4 patients were rated as near normal (13%),1 patients as abnormal (3%).The rates of the normal and near normal were up to 97%.The overall score was improved from preoperation (36.4±4.6) to postoperation (90.1± 10.7,t=27.96,P=0.000).The knee function Lysholm scores was significantly increased from preoperation (52.7±5.3) to postoperation (91.5±9.2,t=23.26,P=0.000).Conclusion The one-stage reconstructing the anterior and posterior cruciate ligaments injuries with autologous hamstring tendon using TightRope devices all inside can recover the stability of knee joint with satisfied joint function postoperatively.
