Objective To analyze the bone metabolism in hospitalized patients with Graves disease and the changes after 131I therapy. Methods The differences of bone metabolism were analyzed between 315 patients with Graves disease and 300 normal controls in a case-control study. The changes in bone turnover markers and BMD levels before and one year after 131I therapy were observed in 60 patients. Results Compared to normal control, bone turnover markers were markly higher and BMD levels were lower in patients with Graves disease. The level of thyroid hormones were positively related to bone turnover markers, while negatively related to total hip BMD (Z-score).But there was no linear relationship with lumbarand femoral neck BMD (Z-score).After one year of 131I therapy, bone turnover markers were markly lower than that before treatment, while BMD levels were partly higher than that before treatment. Conclusions In Graves disease patients, bone turnover markers were generally increased, while BMD levels decreased compared with normal people. After 131I therapy, along with the improvement of thyrotoxicosis, the high bone turnover rate can be suppressed, and BMD can partly recover.
4.Clinical characteristics of cognitive impairment in patients with Parkinson disease with or without neurogenic orthostatic hypotension
Journal of Apoplexy and Nervous Diseases 2023;40(6):530-535
Objective Orthostatic hypotension(OH),a result of autonomic dysfunction,is an important risk factor for cognitive impairment in patients with Parkinson disease(PD). However, the direct relationship between cognitive levels and neurogenic OH in patients with PD remains unclear. We aimed to investigate the relationship between cognitive dysfunction and neurogenic OH in patients with PD,and compare the scores of various cognitive domains between patients with PD with asymptomatic OH and those with symptomatic OH. Methods We retrospectively included 79 patients with PD in the Department of Neurology,Xuanwu Hospital,Capital Medical University, including 19 with symptomatic OH,20 with asymptomatic OH,and 40 without OH. All the patients were examined for the presence or absence of OH by beat-to-beat blood pressure monitoring and the active stand test. The OH questionnaire was used to distinguish between symptomatic and asymptomatic OH. Cognitive function was assessed using a global neuropsychological scale. We used one-way analysis of variance and the Kruskal-Wallis test to determine the association between OH and cognitive dysfunction as well as the characteristics of different cognitive domains in different types of patients. Results There was a positive correlation between the years of education and the levels of cognitive function in all PD groups. Patients with symptomatic OH were more likely to develop poor delayed verbal recall at an earlier stage. Patients without OH had impairment in visual space/executive function more frequently. Conclusion Our findings provide new insights into PD with cognitive impairment that patients with symptomatic OH may be more prone to memory impairment due to the underlying pathophysiological mechanism of OH.

OBJECTIVETo explore the Ilizarov mini-invasive distraction technique for the treatment of severer ankle and foot deformities secondary to ischemic contracture of the leg.
METHODSBased on the tension-stress low of Ilizarov, a serial of adjustable three dimensions external distractive instrument was developed in our department. From April 2002 to March 2004, 8 patients with ankle and foot deformities secondary to ischemic contracture of the leg induced by trauma and fracture were treated with the distractive instrument. Of them, 4 patient were male and 4 female, aged from 13 to 31 years with an average of 23 years. Five affected legs were in the left and 3 in the right. Preoperative abnormal style included talipes equines in 6 feet and equinovarus in 2 feet, with extensive scar contracture in the legs. Five patients suffered from failure of soft tissue release before, two patients with severe bony deformity of the feet were underwent limited foot triple osteotomy in this department before the distractive correction. The distraction was begun from 7 d after operation and distractive time from 29 to 60 d with an average 46 d.
RESULTSAll of the 8 patients were followed up from 10 months to 29 months, with an average of 13 months. All of deformities in the feet were corrected satisfactorily, full feet contacted with the ground in stand or walking and achieved with good function. No complication, such as pin tract infection, skin necrosis and neurovascular injury was occurred in this group.
CONCLUSIONSMini-invasive distraction technique for treatment of severe ankle and foot deformity secondary to ischemic contracture of the leg is safe and mini-injury, it is also an effective approach for the treatment of various kinds of rigid foot anomaly.

6.The Ubiquitin-Proteasome System and F-box Proteins in Pathogenic Fungi.
Mycobiology 2011;39(4):243-248
The ubiquitin-proteasome system is one of the major protein turnover mechanisms that plays important roles in the regulation of a variety of cellular functions. It is composed of E1 (ubiquitin-activating enzyme), E2 (ubiquitin-conjugating enzyme), and E3 ubiquitin ligases that transfer ubiquitin to the substrates that are subjected to degradation in the 26S proteasome. The Skp1, Cullin, F-box protein (SCF) E3 ligases are the largest E3 gene family, in which the F-box protein is the key component to determine substrate specificity. Although the SCF E3 ligase and its F-box proteins have been extensively studied in the model yeast Saccharomyces cerevisiae, only limited studies have been reported on the role of F-box proteins in other fungi. Recently, a number of studies revealed that F-box proteins are required for fungal pathogenicity. In this communication, we review the current understanding of F-box proteins in pathogenic fungi.

7.Comparison of the accuracy of RIFLE, AKIN and KDIGO criteria in predicting early death after liver transplantation
Organ Transplantation 2020;11(3):369-
Objective To compare the accuracy of three acute kidney injury (AKI) criteria of RIFLE, Acute Kidney Injury Network (AKIN) and Kidney Disease: Improving Global Outcomes (KDIGO) in predicting the early (30 d) postoperative death of liver transplant recipients. Methods Clinical data of 173 liver transplant recipients were retrospectively analyzed. The incidence of postoperative AKI was calculated according to the three criteria of RIFLE, AKIN and KDIGO. The all-cause fatality rate and cause of death at postoperative 30 d were analyzed. The risk factors of death within 30 d after operation were analyzed by binary Logistic regression. The prediction accuracy of three criteria for death within 30 d after operation was compared by the receiver operating characteristic (ROC) curve. Results According to the RIFLE, AKIN and KDIGO criteria, the incidences of postoperative AKI were 48.0%, 51.4% and 53.8%, respectively. Thirteen patients died within 30 d after operation and the fatality rate was 7.5%. RIFLE, AKIN and KDIGO stages were the independent risk factors for death within 30 d after operation (all

OBJECTIVETo study the role of the synchronized springy lengthening apparatus for the tibia and calcaneal tendon designed by the author in preventing the clubfoot of secondary to the Ilizarov tibia lengthening.
METHODSBased on the Ilizarov tibia lengthening apparatus, a special synchronized springy lengthening apparatus for the tibia and calcaneal tendon was designed. The tibial was made of distal and proximal 2 rings respectively and 4 threaded rods, and the calcaneal was made of a half ring, 2 hinges and a threaded rod with spring. The half ring was fixed to the calcaneus by 2 crossed wires. The fracture tibia and fibula, ankle joint, talocalcaneal joint were attached to the apparatus. At the same time of tibia lengthening, the soft tissue was simultaneously stretched, the ankle joint could move, and the leg could bear weight. If the clubfoot angle was larger, the percutaneous fasciotomy of calcaneal tendon was performed; if the angle was less than 20 degrees, the pes deformities were corrected only by the stretch of calcaneal tendon.
RESULTSSeventy-seven patients' tibia were lengthened averagely 4.6 cm, with an average speed of 0.7 mm/d. The healing made tibia lengthened, and the index was 1.35 months/cm. There were not the secondary varus and valgus deformities and clubfoot in all the patients. The clubfoot with 100-400 angle of the 16 patients were corrected after tibia lengthening.
CONCLUSIONSThe new apparatus coincides with the biomechanical principle and can effectively prevent the secondary deformities of foot such as clubfoot, talipes varus and valgus after tibia lengthening procedure.
