1.Application of clip traction to endoscopic submucosal dissection for heterotopic pancreas in stomach

Objective To evaluate the efficacy and safety of clip traction in endoscopic submucosal dissection (ESD) for heterotopic pancreas in stomach.Methods Data of 62 patients with pathologically confirmed heterotopic pancreas treated by ESD between May 2013 and February 2016 were retrospectively studied in Department of Gastroenterology of Xinqiao Hospital,whose lesions were single and origins were submucosal.Thirty-six cases underwent ESD with clip traction (clip group,n=36),and 26 cases underwent ESD without clip traction (non-clip group,n =26).The procedure time,visualization of the submucosal during dissection,complications and the result of follow-up were evaluated between the two groups.Results ESD was successful in all cases.The procedure time was significantly shorter in the clip group than in the non-clip group (19.35±10.34 min VS 27.02± 14.27 min,t'=2.333,P=0.023),and good visualization was obtained by applying clip traction.The rate of bleeding in the clip group was 55.6% (20/36),and that in the non-clip group was 61.5% (16/26).There was no significant difference between the two groups(x2 =0.222,P =0.638).Moreover,perforation did not occur in the clip group,but occurred in one patient in the non-clip group (3.8%,1/26).There was no significant difference between the two groups (P =0.419).All patients were followed up,there was no recurrence.Conclusion ESD is effective and safe for heterotopic pancreas in stomach.Clip traction can shorten the operation time and reduce complications of ESD.
2.Long-term outcomes of endoscopic submucosal dissection for gastric stromal tumors

Objective To explore the long-term outcomes of endoscopic submucosal dissection (ESD) of gastric GISTs.Methods Data of 25 patients with gastric stromal tumors,who underwent ESD,were reviewed in terms of clinical characteristics,histopathologic results,complications and long-term outcomes.Results Twenty-five patients underwent one-time complete removal of lesion, with a average tumor size of 3.0 cm (range,0.6 to 7.6 cm).The average time of operation was 65 minutes (range,15 to 154 minutes).one case was observed with intraoperative hemor-rhage of 200 mL blood,which was successfully managed by hemostatic forceps.Perforation with incidence rate of 16%occurred in 4 cases and was closed well with clips.Mucosal mechanical laceration of esophagus occurred in 1 case when the big tumor was taken out.The average length of hospi-talization was 7.3 days (range,4 to 21 days).A follow-up for 30 months (range,13 to 54 months) showed no tumor recurrence or metastasis. Conclusion ESD is an effective and safe endoscopic procedure to remove gastric GISTs for long-term outcomes in patients with no metastasis.
3.Intraoperative neurophysiological monitoring during surgery for tethered cord syndrome in pediatrics: a report of 11 cases

Objective To study the value of intraoperative neurophysiological monitoring (IONM) in microsurgical detethering of patients diagnosed as having tethered cord syndrome (TCS).Methods A retrospective analysis of the clinical data,IONM results and prognosis of 11 patients with TCS,admitted to our hospital from January 2007 to December 2010,was performed to explore the protective role of IONM in microsurgical detethering of TCS.Results IONM responses could be obtained in all patients.Postoperative deterioration of symptoms was found in two patients one day after surgery.At discharge,10 patients got improvement with only one having deterioration of symptoms.Follow-up was performed for 1.5-5.5 years; long-term deterioration of symptoms was found in only one patients.Conclusion IONM can protect the spinal cord function,prevent neurological damage,improve the safty and improve life quality of the patients.
4.Incidence of osteonecrosis of the femoral head in divers: an epidemiologic analysis in Dalian

Objective To investigate the incidence of osteonecrosis of the femoral head (ONFH) in divers of Dalian.Methods From January 2010 to December 2010,all registered 855 divers in Dalian were enrolled in this study.All divers underwent a unified medical examination and a questionnaire including height,weight,blood pressure and hip inspection.Radiological examination (anteroposterior and frog position)was used for all divers.Suspicious persons with hip pain but normal X-ray performance were confirmed by MRI.Results Sixty-eight divers were confirmed as ONFH,and the incidence of ONFH in divers of Dalian was 7.95%.According to the Ficat classification,12 patients (12 hips) were in Phase 1,40 patients (47 hips)in phase Ⅱ,3 patients (3 hips) in phase Ⅲ,13 patients (15 hips) in phase Ⅳ.The mean age of divers was 32.6+5.5 years (range,18-55 years).The onset ages of most patients ranged from 30 years to 50 years,accounted for 83.82% (57/68).Among all patients,primary school education accounted for 10.58% (38/359),junior high school education 6.28% (28/446),high school education 4.26% (2/47),university education 0 (0/3); seniority less than 5 years accounted for 4.55% (20/440),6-10 years 9.69% (28/289),11-15 years 13.04% (12/92),16-20 years 21.05% (4/19),more than 20 years 26.67% (4/15); self-employed accounted for 11.88% (19/160),private enterprise 8.41% (38/452),and national enterprise 4.53% (11/243).Conclusion The incidence of ONFH is high in divers of Dalian,which is different in terms of age,seniority,level of education,enterprise nature.
5.Characteristics and influencing factors of esophageal stenosis after endoscopic submucosal dissection for early esophageal carcinoma

Objective:To investigate the characteristics and influencing factors of esophageal stenosis after endoscopic submucosal dissection (ESD) for early esophageal carcinoma.Methods:Patients who underwent ESD in the Digestive Endoscopy Center of the Second Affiliated Hospital of Army Medical University from January 2011 to December 2018 were included. The data were obtained from medical records and follow-up. The influencing factors of stenosis were determined by single factor and Cox regression analysis.Results:A total of 654 patients underwent ESD and 79 (12.1%) of them developed postoperative esophageal stenosis. The median time of stenosis development was 27 (17, 43) days. The morphology and lesion circumferential proportion were independent factors for the occurrence of stenosis after ESD. The stenosis incidence of type Ⅱa was 6.601 times (95% CI: 1.518-28.709, P=0.012) compared with that of type Ⅱc. The incidence of stenosis in lesions with 75%-<100% and 100% circumference was 17.408 times (95% CI: 8.009-37.839, P<0.001)and 52.439 times (95% CI: 23.905-115.029, P<0.001) respectively compared with that of patients <75%. Among the 79 patients, 27 had severe stenosis, and the lesion circumferential proportion was an independent factor for stenosis. Compared with the group of lesion circumferential proportion of less than 75%, the incidences of stenosis of lesion circumferential proportion of 75%-<100% and 100% were 7.775 (95% CI: 1.977-30.577, P=0.003) and 70.062 (95% CI: 19.879-246.926, P<0.001) times respectively. Conclusion:The morphology and lesion circumferential proportion are two independent factors for the occurrence of esophageal stenosis after ESD. Additionally, lesion circumferential proportion is an independent factor for the occurrence of severe esophageal stenosis after ESD.
6. Research progress on the mechanism of iron overload in the occurrence and development of osteoarthritis

Osteoarthritis (OA) is a common chronic degenerative disease, and its condition tends to worsen with age. The pathogenesis of OA is complex, and many risk factors can lead to the occurrence of OA. Iron is one of the essential trace elements in the body, and its metabolic balance is extremely important to human health. Iron overload is closely related to the occurrence and development of OA. Excessive iron deposition in joint tissue can easily lead to lesions of articular cartilage and synovium, as well as affect subchondral bone reconstruction and lead to the occurrence of OA. The author reviewed the relevant research literature in recent years, and reviewed the mechanism of iron overload in the occurrence and development of OA, in order to provide new ideas and directions for the research and diagnosis and treatment of OA.
7.Metagenomics of Fecal Gut Microbiota in Common Traditional Chinese Medicine Syndrome Types of Irritable Bowel Syndrome with Diarrhea:A Cross-sectional Study

ObjectiveTo investigate the structural and functional characteristics of gut microbiota in common traditional Chinese medicine (TCM) syndromes of irritable bowel syndrome with diarrhea (IBS-D). MethodsIBS-D patients who visited the Hospital of Chengdu University of Traditional Chinese Medicine, and healthy participants from the Physical Examination Centre of the same hospital were recruited from 1st January 2020 to 31st March 2021.The IBS-D patients were classified into syndrome of liver constraint and spleen deficiency, and syndrome of spleen deficiency and dampness exuberance; together with the recruited healthy participants, there were liver-constraint group, dampness-exuberance group, and healthy group. General information, including age, gender and body mass index (BMI), were collected, and Irritable Bowel Syndrome Symptom Severity Scale (IBS-SSS) as well as Irritable Bowel Syndrome Quality of Life Scale (IBS-QOL) scores were additionally collected from IBS-D patients. Fresh fecal samples were also collected and tested by macro-genome sequencing technology for abundance statistical display, PCoA, Anosim, LEfSe bioinformatic analysis of the annotated gut microbiota structure and function. ResultsThere was no statistically significant difference in the general information of the participants in the three groups (P>0.05); the difference in the IBS-SSS and IBS-QOL scores between liver-constraint group and dampness-exuberance group were not statistically significant (P>0.05). The study included 28 cases each in liver-constraint group, dampness-exuberance group, and healthy group. The number of specific genes to patients in liver-constraint group was 269 135, with 216 156 in dampness-exuberance group and 249 759 in healthy group, accounting of total 1 784 036 in the three groups. There were differences in the relative abundance distribution of the top ten species of gut microbiota among the three groups, with smaller differences at the phylum, class and order levels, and larger differences at the family, genus and species levels. There were differences in the relative abundance of structure and function of the gut microbiota among the three groups. Species PCoA and Anosim analyses at the species level showed significant differences in the composition of the microbiota among the three groups. Further LEfSe analyses showed that patients in liver-constraint group were screened for 14 dominant strains, of which Clostridium sp. CAG 217, Lachnospira pectinoschiza, Anaerotruncus sp. CAG 528, Paeniclostridium sordellii, Eubecterium sp. CAG 76, Bacillus cereus were affected to a greater extent in abundance differences; dampness-exuberance group screened 24 species of dominant bacteria, of which Roseburia inulinivorans, Eubacterium sp. CAG 251, Roseburia hominis, Unclassified Eubacterium rectale, Roseburia intestinalis, and Megamonas funiformis were affected to a greater extent in abundance differences; no dominant functional genes were screened for patients in liver-constraint group, and dampness-exuberance group was screened for flagellum assembly (ko02040), porphyrin metabolism ( ko00860), salmonella infection (ko05132), and benzoic acid degradation (ko00362). The differentially dominant functional genes in liver-constraint group and dampness-exuberance group may mainly focus on metabolism (including biodegradation and metabolism of exogenous substances, energy metabolism, lipid metabolism, etc.). ConclusionIBS-D with syndrome of liver constraint and spleen deficiency is characterized by the enrichment of 14 gut microbiota, such as Clostridium sp. CAG 217, while IBS-D with syndrome of spleen deficiency and dampness exuberance is characterized by the enrichment of 24 gut microbiota, such as Roseburia inulinivorans, and 4 functional enrichments, such as flagellum assembly. Clostridium sp. CAG 217 and Roseburia inulinivorans are expected to be biomarkers for IBS-D patients in the two syndromes, respectively.
8.Research progress of traditional Chinese medicine regulating related signaling pathways to promote tendon-bone healing

Tendon-bone healing is a complex biological process. Multiple signaling pathways are involved in tendon-bone healing, including transforming growth factor-β signaling pathway, bone morphogenetic protein signaling pathway, Wnt signaling pathway, fibroblast growth factor signaling pathway and nuclear transcription factor-κB signaling pathway. This paper summarizes the research status of traditional Chinese medicine regulating related signaling pathways to promote tendon-bone healing. It is found that a variety of traditional Chinese medicine monomers or herbal extracts (such as baicalein, icariin, total flavonoids of Drynaria fortunei, parthenolide, total saponins of Panax notoginseng, etc.) and traditional Chinese medicine compounds (such as Taohong siwu decoction, Liuwei dihuang pill, Xujin jiegu liquid, etc.) can promote bone formation, anti-inflammatory, anti-oxidation, by regulating the above signaling pathways, thereby effectively promoting tendon-bone healing.
9.The accuracy of tumor size measurement and clinical T staging for resected lung cancer: Retrospective analysis of 1 880 patients

Objective To elucidate the correlation between radiological tumor size (RTS) and pathological tumor size (PTS), and to evaluate the accuracy of clinical T staging. Methods Data on patients who underwent complete resection between September 2018 and June 2019 were retrospectively collected. The correlation between RTS and PTS was analyzed by and we assessed the agreement between clinical and pathologic T staging. Results Finally, 1 880 patients were included. There were 778 males and 1 102 females at average age of 57±11 years. In the entire cohort, the RTS and PTS was 19.1±13.5 mm and 17.7±14.0 mm, respectively (P<0.001). The RTS and PTS showed a strong linear correlation with the Pearson’s correlation coefficient calculated as 0.897. The mean RTS was significantly larger than PTS (P<0.001) in tumors≤3 cm, but significantly smaller in tumors>4 cm. The overall concordance rate between clinical and pathological T staging was 65.6%. Clinical staging failed to detect T4 disease in 29.4% (5/17) of patients. Male patients and the presence of cavities within nodules were independent significant factors leading to inaccurate clinical T staging. Conclusions The correlation between the tumor sizes measured on thin-section computed tomography and pathologic specimens varies with the real tumor size. Methods and techniques for improving clinical T staging accuracy is in urgent need.
10.Selective mediastinal lymph node dissection: Precision surgery in the era of precision medicine

Along with the popularity of low-dose computed tomography lung cancer screening, an increasing number of early-stage lung cancers are detected. Radical lobectomy with systematic nodal dissection (SND) remains the standard-of-care for operable lung cancer patients. However, whether SND should be performed on non-metastatic lymph nodes remains controversy. Unnecessary lymph node dissection can increase the difficulty of surgery while also causing additional surgical damage. In addition, non-metastatic lymph nodes have been recently reported to play a key role in immunotherapy. How to reduce the surgical damage of mediastinal lymph node dissection for early-stage lung cancer patients is pivotal for modern concept of "minimally invasive surgery for lung cancer 3.0". The selective mediastinal lymph node dissection strategy aims to dissect lymph nodes with tumor metastasis while preserving normal mediastinal lymph nodes. Previous studies have shown that combination of specific tumor segment site, radiology and intraoperative frozen pathology characteristics can accurately predict the pattern of mediastinal lymph node metastasis. The personalized selective mediastinal lymph node dissection strategy formed from this has been successfully validated in a recent prospective clinical trial, providing an important basis for early-stage lung cancer patients to receive more personalized selective lymph node dissection with "precision surgery" strategies.
