1.Prevention of childhood tuberculosis
International Journal of Pediatrics 2011;38(6):532-535
In recent years the incidence rate of tuberculosis (TB) has gone up.It was estimated that 1/3of people had asymptomatic latent tuberculosis infection globally. The current widespread use of Bacillus calmetteguerin (BCG) vaccine program against tuberculosis in children has a good preventive effect, but due to the limited duration of the immune responses induced by BCG,there also had situation of prevent failure,so researchers begin to explore new TB vaccines, such as live attenuated mutant strains, subunit vaccines and DNA vaccines, and achieved certain results. For the pure protein derivative tuberculosis skin test positive and immunosuppressed children,in addition to active immunization would also need anti-TB medicine for chemoprophylaxis.

PurposeTo evaluate the curative effect and possibility of the intraarterial chemotherapy followed by radical hysterectomy for locally advanced cervical cancer. Methods59 patients with FIGO stage lb-IV cervical cancer were enrolled and the tumor size with stage lb and Ua must be more than 4 cm. Treatment consisted of bilateral internal iliac artery infusion of cisplatin 80 rog, 5-Fu 1 500 rog, and AT1258 60 mg for 1 - 3 courses separated by 3 weeks. The tumor size was measured by vagina ultrasonic and CT before and after chemotherapy to calculate the shrunk size percent. All patients underwent radical hysterectomy. Patients with disease in the vagina, parametrium, pelvic lymphnodes or microscopic tumor emboli were followed postoperative pelvic irradiation. Results59 patients underwent 122 courses of intraarterial chemotherapy for 1 - 3 courses each. The total response rate was 95 %. Complete response (CR) was achieved in 18 of 59 patients (31%) ,while a partial response (PR) was noted in 38 (64%) and stable disease (SD) in 3 (5% ). No patient had progressive disease. 14 patients with pelvic lymphnode metastasis were all found to have PR and SD. No patient had lymphnode metastasis in CR. Of 18 patients with CR, 17 patients had not recurrence. Only one patient with stage lla recurred 20 months after operation. But he also cured. All 18 patients with CR were living free of disease. But 3 in 38 patients with PR died and 3 patients with SD were all died. There were no severe complications related to intraarterial chemotherapy and radical hysterectomy. Conclusionslntraarterial chemotherapy before operation can shrink tumor size, decrease the lymphnode metastasis. It is feasible in helping elevating cure rate of patients with locally advanced cervical cancer who are at high risk for failure with conventional treatment.

Objective To investigate the relationship between coagulation function and pediatric critical illness scores (PCIS) in children with severe pneumonia. Methods The PCIS were collected in 152 children with severe pneumonia (pneumonia group) admitted in our pediatric intensive care unit from Jan 2010 to Jul 2011,and 20 healthy children in the same period were selected as the healthy control group. The coagulation indicators of children in both groups were detected and the relationship between coagulation markers and severity of pneumonia was analyzed. Results There were significant differences in platelet count,fibrinogen,D-Dimer,soluble P-selectin between pneumonia group and healthy control goup [(185.74 ±116.26) × 109/L vs (287.10 ±90.01) × 109/L,(3.51 ±0.50) g/L vs (3.15 ±0.15) g/L,(1.39 ±2.18) μg/ml vs (0.36 ± 0.07) μ g/ml,(110.07 ± 83.47) ng/ml vs (33.74 ± 9.47) ng/ml,P < 0.05].There were positive correlation between soluble P-selectin,D-Dimer and severity of disease and negative correlation between platelet count and severity of disease in children with severe pneumonia.Regression equation:y =1.154 +0.003 × soluble P-selectin + 0.089 × D-Dimer-0.001 × platelet count (P < 0.05).As the children's critical condition getting worse, soluble P-selectin and D-Dimer levels increased (P < 0.05).Hatelet count showed no significant difference between critical group and extremely critical group, which was significantly lower than that in non-critical group (P < 0.05).Conclusion Soluble P-selectin,D-Dimer,and platelet count are associated with the severity of pneumonia. The children with severe pneumonia are easy to have coagulation disturbance.

CT laser simulation positioning system is a necessary auxiliary device for radiotherapy. Its main purpose is to position patients by simulating different kinds of treatment machine. In order to demarcate the mark of the iso-center, it is common to use the laser positioning device to indicate the iso-center. The kernel technology of the laser positioning system is the controlling of the step progress motor by using the MCS, which is to control the movement of step progress motor using the wheel of the motor. This design uses MCS-51 to control step progress motor by the way of exporting the rectangle wave form through I/O port of 8255 chip. The system configuration is simple, the operation is convenient and the positioning is precise.

Objective To study the extraintestinal lesions induced by rotavirus(RV)infection and explore the pathogenesis. Methods Simian rotavirus SA11 was cultured in MA-104 cells. After inoculation, the pathological changes in brain, lung, heart, liver, pancreas and kidney tissues were observed, the RV antigens detected, and the apoptotic cells observed.Besides,we stained the filamentous actin(F-actin)with Phalloidine-TRITC,and then quantified the F-actin amount. Results Several pathological changes, inclusive of myocardial interstitial edema, granular degeneration in cardiacytes, hepatic congestion, and hepatocellular vacuolar degeneration, were found, but not found in the brain, lung, and pancreas tissues.Meanwhile, several ultrastructural changes, inclusive of dissolved myocardial F-actin,extended smooth endoplasmic reticulum, swollen mitochondria, and widened perinuclear space, were found. No difference was found in the quantity of myocardial F-actin.Apoptosis was found in liver cells, but not in myocardial cells.RV RNA was detected in tissues such as brain, lung, heart, liver and pancreas tissues. Conclusion All the results suggest that RV may spread from the intestine to various extraintestinal organs and hence induce injury.Filamentous actin depolymerization, cytoskeleton damage and apoptosis induced by RV infection are the important mechanism not only in intestinal damages, but also in extraintestinal lesions.
