1.Research progress on the molecular biomarkers for patients with hepatocellular carcinoma recurrence after surgery

Tumor recurrence is the main factor affecting the postoperative prognosis of hepatocellular carcinoma.The forecast of tumor recurrence by monitoring biomarker can effectively improve the prognosis of such disease.To improve the detection rate of early cancer recurrence can be achieved by detecting the level of the alpha-fetoprotein (AFP),Lens culinaris agglutinin-reactive fraction of AFP (AFP-L3),des-γ-carboxyprothrombin (DCP),or combined detection.In recent years,the researches on the miRNA and Long Noncoding RNAs (LncRNA),which are key regulators in the upstream of signal pathways,could provide a new way for HCC recurrence monitoring.The combined detection of biomarkers in different control levels,maybe is the effective measure to improve the tumor recurrence detection rate,and is also the focus of the further research.

The cardiovascular disease, accompanied by psychological disorder frequently, often re-quiresphysical and mental (double heart) treatment. The clinical teaching of cardiovascular graduate stu-dents is very important for cultivating qualified cardiovascular doctors. According to present situation, we improved the diagnosis rate and the doctor-patient communication skills by the strengthening of learning thedouble heart theory, the analysis ofdouble heart typical cases, and the clinical practice of double heart medical model. Accordingly, we enhanced the consciousness of double heart, which is helpful to reducing the conflicts between doctors and patients, promoted the rational use of medical resources, and fi-nally promoted the application ofdouble heartmedical model in clinical practice.
3.Anticoagulant treatment of non-valvular atrial fibrillation in cardiovascular department

Objective To investigate the present situations of anticoagulant treatment of non-valvular atri-al fibrillation in order to get better guidance of the clinical anticoagulant therapy and reduce the incidence of stroke caused by atrial fibrillation. Methods The clinical data of NVAF patients hospitalized in the first affili-ated hospital of Chongqing Medical University from January 1 , 2012 to December 31 , 2014 were retrospectively reviewed and analyzed in terms of basic characteristics , medical treatment plan and INR monitoring. Results A total of 1,390 patients were enrolled, 85.97% of whom needed anticoagulant therapy and 25.18% of whom were treated with anticoagulants. For 15.71% of the NVAF patients treated with warfarin , INR met the standard during hospitalization. In the study, such factors as age, coronary heart disease, history of stroke and history of bleed-ing might affect prescription of anticoagulant drugs. Conclusion Most NVAF patients need anticoagulant thera-py, but the rate of anticoagulant therapy is lower than we expected. Among those patients treated with warfarin during hospitalization , the rate of INR meeting the standard is at a lower level. The novel oral anticoagulants are not in wide use in the clinical practice.
4.Study on the anti coagulation therapy of non valvular atrial fibrillation elderly patients with thromboembolic complications from 2012 to 2014

Objective To investigate the present situation of anticoagulant therapy on the elderly non valvular atrial fibrilla‐tion (NVAF) hospitalized patients with thromboembolic complications [ischemic stroke ,transient ischemic(TIA)] .Methods A to‐tal of 255 hospital patients in the First Affiliated Hospital of Chongqing Medical University from 2012 to 2014 were recorded by retrospective case analysis .The basic characteristics ,the risk factors of AF ,the medical treatment plan and INR monitoring were analyzed respectively with SPSS20 .0 statistical software .Results A total of 255 patients were enrolled ,66 cases (25 .88% ) were treated with anticoagulation ,and 157 cases (61 .57% ) were treated by antiplatelet drugs .In patients who were taken anticoagulant drugs ,the rate of INR was 15 .15% .Conclusion In the elderly nonvalvular atrial fibrillation patients with thromboembolic compli‐cations ,the rate of anticoagulant therapy is lower than expected .More than half patients were treated by antiplatelet drugs .The pa‐tient who were treated anticoagulation therpy had a low rate of INR standard rate .

Objective To explore the correlation between up-regulation of cyclooxygenase-2(COX-2) (expression) and hepatocellular carcinoma(HCC) angiogenesis.Methods The expression of COX-2,vascular endothelial growth factor(VEGF)、basic fibroblast growth factor(bFGF) and angiopoientin-2(Ang-2) were examined in eighty matched sets of HCC specimens using immunohistochemistry and reverse (transcription-polymerase) chain reaction(RT-PCR).Results In HCC,the expression rate of COX-2,VEGF,bFGF and Ang-2 was 75.0%,62.5%,60.0% and 61.25%,respectively.(Immunohistochemical) staining scores of VEGF、bFGF and Ang-2 were 5.98?1.16,4.57?0.26 and(5.87)?0.12,respectively in strongly postive group of COX-2;and were 3.30?0.22,2.61?0.16 and 2.63?0.13,respectively in moderately weak postive group of COX-2.The expression rates of VEGF,(bFGF) and Ang-2 were 100.0%(95/95),94.29%(33/35) and 97.14%(34/35),respectively in strongly postive group of COX-2;and were 60.0%(15/25),60.0%(15/25) and 60.0%(15/25),respectively in moderately weak postive group of COX-2.There was significant difference in HCC(angiogenesis) between the two groups(P

Objective To investigate α-interferon (α-IFN) and cyclooxygenase-2 (COX-2)inhibitor celecoxib synergistically inhibit the growth of human liver cancer SMMC-7721 cells xenografts and tumor angiogenesis in a nude mouse model.Methods The effects of celecoxib and α-interferon on tumor volumes and weight were observed.The expressions of VEGF and Cox-2 were determined by immunohistochemistry and RT-PCR,and the effect of α-interferon on MVD also was observed by immunohisto chemistry.Results During the period of observation tumor volume increased progressively in control group,while it was suppressed obviously in other drug treatment groups.The average tumor volume was significantly smaller in celecoxib + α-IFN group than that in IFN group,celecoxib group and control group (P < 0.01,respectively),its inhibitory rate was 61.84%.Immunohistochemistry showes that the VEGF and MVD was significantly smaller in celecoxib + IFN group than that in α-IFN group,celecoxib group and control group (P < 0.01,respectively).RT-PCR shows that the COX-2mRNA and VEGF mRNA pression was lower in the celecoxib + α-IFN group than in α-IFN group,celecoxib group and control group (P < 0.01).Conclusions The COX-2 inhibitor celecoxib and α-interferon synergistically reduces xenografts growth of human liver cancer SMMC-7721 cells effectively via suppressing tumor growth and angiogenesis.
7.Effects of Tian-Shu Dripping Pills on Cerebral Blood Circulation among Anesthetized Canine

This study was aimed to observe the effects ofTian-ShuDripping Pills (TSDP) on cerebral hemodynamics among anesthetized dogs, in order to demonstrate its clinical treatment functional correlation on angioneurotic headache. A total of 25 hybrid dogs were randomly divided into the normal saline (NS) group, TS capsule (positive control: 0.4 g·kg-1 crude medicine) group, low-dose TSDP (0.2 g·kg-1 crude medicine) group, middle-dose TSDP (0.4 g·kg-1 crude medicine) group, and high-dose TSDP (0.8 g·kg-1 crude medicine) group.Intraduodenal administration was given once under anesthesia. Changes of mean arterial blood pressure, blood flow of vertebral artery and internal carotid artery blood flow were detected and the cerebral blood flow and cerebral vascular resistance were calculated. The results showed that TSDP of low-dose, middle-dose and high-dose group can significantly reduce the cerebral vascular resistance and increase the cerebral blood flow within 180 min after medication. It was concluded that TSDP can improve the cerebral blood circulation of anesthetized dogs. Its mechanism may be through the reduction on cerebral vascular resistance and increasing of cerebral blood flow.
8.Clinical application of intraoperative intraperitoneal hyperthermic chemotherapy using sustained-release .fluorouracil in radical gastrectomy for gastric cancer

ObjectiveTo investigate clinical application of intraoperative intraperitoneal hyperthermic chemotherapy using sustained-release fluorouracil in radical gastrectomy for advanced gastric cancer.MethodsThe clinical data of 280 advanced gastric cancer patients admitted from September,2002 to September,2010 were analyzed retrospectively.They were divided into three groups randomly and followed up.The postoperative morbidity,the mortality and the overall survival rates were evaluated.ResultsThere were no significant differences in these three groups with respect to postoperative morbidity ( P > 0.05 ).The incidence of recurrence in intraperitoneal chemotherapy using sustained-release fluorouracil ( treatment group) was significantly lower than those of intraperitoneal chemotherapy and operative treatment( 16.18%,37.61% and 41.28%,P <0.05).The 1,3- and 5-year overall survival rates of treatment group were 85.51%,61.28% and 53.67%,respectively,and the 1-,3- and 5-year overall survival rates were 84.11%,39.98% and 28.12%,and 81.28%,29.88% and 25.21% respectively in intrapeitoneal chemotherapy group and operative group.1-year overall survival rate had no significant differences among three groups with respect to ( P>0.05).3-and 5-year overall survival rates in treatment group were higher signfficantly than those of intraperitoneal chemotherapy and operative treatment( P<0.05).Conclusions Intraoperative intrapeitoneal hyperthermic chemotherapy using sustained-release fluorouracil is a kind of convenient,safe,and highly effective comprehensive treatment method,and it can kill isolated intraperitoneal cancer cells.It may reduce postoperative recurrence and improve survival rates.
10.Effect of chemotherapy of DC combined with cinobufacini in the treatment of advanced stomach cancer

Objective To observe the therapeutic effect of the combination of cinobufacini with DC chemo-therapy in the treatment of advanced stomach cancer .Methods 65 patients who were diagnosed advanced gastric cancer on the basis of histocytology and pathology results , were randomly divided into DC chemotherapy group (31 cases) and combined treatment group (34 cases).The clinical curative effect ,adverse reaction,quality of life ( KPS questionnaire ) and the easement rate of pain were analyzed .Results The two groups had no statistically significant differences in adverse reaction (t=-0.0047,P>0.05) and clinical curative effect (χ2 =0.077,P>0.05),and the clinical response rate of the combined treatment group (35%) was similar to the DC chemotherapy group (32%).Besides,the easement rate of pain in the combined treatment group increased to 71%,which in the DC chemotherapy group increased to 45%(χ2 =19.969,P<0.05).And the improvement rate of quality of life (QOL) in the combined treatment group was 82%,which of the DC chemotherapy group was 55%,the difference was statistically significant(χ2 =13.923,P<0.05).Conclusion Cinobufaconi combined with DC chemotherapy in the treatment of advanced stomach cancer has no significant effect on increase of short -term efficacy ,but it can improve quality of life and alleviate pain obviously .
