Objective To evaluate the treatment of ureteral stones with pneumatic lithotripter under ureterscopy and the prevention of complications. Methords 43 cases of ureteral stones that had been treated with pneumatic lithotripter under ureteroscopy was reviewed. Results 93.0%(40/43)of the stones were fragmented on one session,93.0%(40/43)of the stones were expelled within 7 days afer the procedure.No complications occurred. Conclusions Treatment of ureteral stones with pneumatic lithotripter under ureterscopy is safe and effecive.
2.Treatment of ureteral stones by pneumatic lithotripsy combined with ESWL

Objective To explore the therapeutic efficacy of pneumatic lithotripsy combined with extracorporeal shock wave lithotripsy (ESWL) in patients with ureteral stones. Methods Pneumatic lithotripsy combined with ESWL was employed in the treatment of 365 cases of ureteral stones. Results Of the 365 cases,the operation succeeded in 360 cases and failed in 2 cases,and ureteral perforation occurred in 3 cases.ESWL was required in 80 cases after pneumatic lithotripsy. Conclusions Pneumatic lithotripsy combined with ESWL offers advantages of high efficacy,minimal invasion and broad indications.

Aim To investigate the effect of zacopride ( Zac) on cardiac arrhythmia in isoproterenol ( ISO)-in-duced myocardial hypertrophic rats and the underlying electrophysiological mechanisms .Methods ① Fifty-one rats were randomly divided into control group ( n=17 ) , ISO group ( n=17 ) and ISO +Zac group ( n =17 ) .Rat model with cardiac arrhythmia and hypertro-phy was established by intraperitoneal ISO ( 5 mg?kg -1 ) injection.②ECGs were recorded to observe the effects of Zac on arrhythmia in model rats .③ Whole-cell patch clamp was applied to record inwardly rectifi-er potassium current(IK1), resting membrane potential ( RMP ) and amplicated delayed afterdepolarizations (DADs).Results ① Echocardiographic examination showed that , left ventricular end-diastolic dimension (LVEDD) and left ventricular end-systolic dimension (LVESD) significantly decreased in rats in ISO group compared with control group , whereas left ventricular posterior wall end-diastolic thickness ( LVPWd) and in-terventricular septum end-diastolic thickness ( IVSd ) increased ( P<0.05 ) , suggesting rat model of isoprot-erenol-induced myocardial hypertrophy was successfully established .② ECGs showed that 88.89% of rats in ISO group had ventricular premature beats ( VPBs ) , which significantly decreased to 11.11% after the ap-plication of Zac ( P <0.05 ) .③ Values of RMP de-creased from ( -71.05 ±1.27 ) mV in control group to (-69.38 ±1.21 ) mV in ISO group ( P<0.05 ) . After Zac administration , RMP significantly increased to ( -73.86 ±1.33 ) mV compared with control and ISO group(P<0.05).④DADs and TA incidence sig-nificantly decreased from 88.24% in ISO group to 11.76%in ISO+Zac group ( P<0.05 ) .⑤ Compared with control group , IK1 density was markedly reduced in ISO group, whereas Zac could effectively rescue IK1 suppression to normal level .Conclusions Zac, as a selective IK1 channel agonist , can significantly inhibit cardiac arrhythmia in isoproterenol-induced myocardial hypertrophic rats , which is mainly attributed to in-creased RMP by enhancing IK1 and subsequent suppres-sion of DADs.

BACKGROUND:The effect of advanced glycation end products (AGEs) on bone resorption is controversial. Our previous study has shown that bone resorption is significantly inhibited when AGEs present with pre-osteoclast cells RAW 264.7, while the effect of AGEs on osteoclastic acidification remains unknown. OBJECTIVE:To investigate the effect of AGEs on osteoclastic acidification and the underlying mechanism. METHODS:RAW 264.7 cells were induced by RANKL (15μg/L;normal group) to generate osteoclasts, and AGEs (50-400 mg/L;experimental group) or bovine serum albumin (100 mg/L;control group) were added at the beginning of the induction. The effect of AGEs on bone resorption was evaIuated by anaIyzing the area of bone resorption on the Osteo Assay Surface plates, and the effect of AGEs on osteoclastic acidification was evaluated by acridine orange staining. Furthermore, the expression levels of V-ATPase a3 and CIC-7 were detected to investigate the underlying mechanism. RESULTS AND CONCLUSION:The bone resorption area in the AGEs group was significantly decreased compared with the normal group (P<0.05). Acridine orange staining reveaIed that the red fluorescence (620 nm) intensity in the AGEs group was significantly decreased compared with the normal group (P<0.05), and this inhibitory effect became obvious with the increase of AGEs concentration. Immunocytochemistry, western blot assay and PCR findings showed that the expression levels of V-ATPase a3 and CIC-7 in the AGEs group were decreased significantly compared with the normal group (P<0.05). To conclude, AGEs exert inhibitory effect on osteoclastic acidification, probably by inhibiting the expression of V-ATPase a3 and CIC-7.

BACKGROUND:The effects of advanced glycation end products (AGEs) on osteoclast-induced bone resorption is controversial and the underlying mechanisms remain unclear. Most of the studies indicate that AGEs can enhance bone resorption, while some othersshowthe opposite effects.
OBJECTIVE:To investigate the effects of AGEs on osteoclast-induced inorganicmatrixdissolution and organic componentdegradation and the underlying mechanisms.
METHODS:RAW 264.7 cels were induced to generate osteoclasts,and AGEs (50-400 μg/mL) or control-bovine serum albumin (100 μg/mL) was added since the beginning of the induction. The effect of AGEs on bone resorption was evaluated by analyzing the area of resorption pits on the Osteo Assay Surface plates and the expression of cathepsin K. Furthermore, the number of tartrate-resistant acid phosphatase (TRAP)-positive multinucleated cels, nuclei per osteoclasts and the expression of integrinανβ3were detected.
RESULTS AND CONCLUSION:The area of resorption pits and expression of cathepsin K in AGEs groups were significantly decreased compared withthecontrol group, and this inhibiting effect became more obvious with the increase of AGEs concentration. TRAP staining also showed that number of TRAP-positivemultinucleated celsand nuclei per osteoclast were significantly reduced in an AGE dose-dependent manner. Quantitative PCR revealed that the expression of integrin ανβ3decreased significantly with the extension of AGEs incubation time. These data indicate that AGEs can exert inhibitory effects on organic and inorganicmatrixdegradation induced by osteoclasts. The underlying mechanism may be involved in the inhibitory effects of AGEs on directed differentiation and cel fusion of osteoclast precursor cels, and migration and adhension of osteoclasts.
7.Practice and Effectiveness of New Culture Construction in Public Hospitals in the Context of High-Quality Development

Objective Using the indicator system to objectively evaluate the content and effectiveness of the new culture construction in typical public hospitals,so as to identify problems and put forward suggestions for improve-ment.Methods Applying the new culture index system for the high-quality development of public hospitals con-structed by the research group,it quantitatively evaluated the practice of new culture construction in a tertiary hospi-tal in Shanghai.Results It found that the main body of the hospital's new culture construction scored 98.9 points.The 2 dimensions of hospital culture system construction and carrying out effectiveness(additional indicators)were all full scores.In addition,the dimension of caring for medical staff was 24.8 points,the dimension of strengthening pa-tient demand orientation was 24.6 points,and the dimension of distinctive hospital culture was 24.5 points.There are still deficiencies in the dimensions of the post-diagnosis experience of patients,the cultivation of health habits of staff,and the transformation of scientific research.Conclusion The hospital has taken advantage of the top-level de-sign of cultural construction under the leadership of the Party Committee and has improved the relevant systems.The hospital also focuses on several special cultural projects such as cultural heritage,employee care,optimization of ac-cess to medical care,and popularisation of scientific brands.It propose to promote the high-quality development of the hospital's new culture by carrying out Internet medical care,organizing special staff activities,strengthening the guidance of scientific research policies,and increasing the financial support for projects.
8.Analysis of the Current Status and Influencing Factors of Medical Staff's Perception and Experience of Hospital Culture

Objective It investigates medical staff's perception and experience sense of hospital culture,and analy-ses its influencing factors.This can further play the role of hospital culture construction on medical staff's motiva-tion,and improve medical staff's work motivation to provide reference basis.Methods The Medical Staff's Percep-tion and Experience of Hospital Culture scale developed by the research team was used to conduct a questionnaire survey with on medical staff in the sample hospital.Descriptive analysis,single factor analysis and multiple linear re-gression analysis were used to study the medical staff's perception and experience of hospital culture and its influ-encing factors.Results A total of 629 valid questionnaires were collected.The overall score rate of medical staff's per-ception and experience of hospital culture was over 90%.Among them,the highest scoring rate of the mission bearing subscale,and the dimension with higher scores was vision and goals.However,the experience of work environ-ment,promotion system and research environment is relatively low.The results of the one-way test showed that the differences in the scores of hospital culture perception and experience among medical staff with different educa-tion levels,titles and job positions were statistically significant(P<0.05).Conclusion Medical staff generally support the construction of hospital culture.It is suggested that the hospital culture development strategy should be formulated from the perspective of the staff and according to their goals,problems and needs.It is also recommended to con-tinuously improve the medical staff's perception and experience of the hospital culture according to their needs,so as to play a positive role in the high-quality development of the hospital by culture construction.
9.Qualitative research on occupational identification of full-time postgraduate nursing students

Objective To describe the real feelings about occupational identification of full-time postgraduate nursing students, in order to provide reference for hospital administrators and nursing educators.Methods in-depth ( five) interview were conducted among 18 full-time postgraduate nursing students individually by transcripts and recordings to collect the interview contents; and data were analyzed by Colaizzi thematic analysis method. Results The results of this study were divided into 3 themes and 6 aspects. The 3 themes included the motivation to pursue a postgraduate nursing degree, nursing career awareness and feeling, the personal evaluation of nursing job and the 6 aspects contained personal career development and academic pursuits, getting rid of the clinical environment, academic achievement and complex, learning postgraduate nursing course follow the trend, positive feedback (easy to find a job, higher education, fast development), and negative feedback (low social status, frequent night duty, harsh recruitment and little attention from hospital). Conclusions Generally postgraduate nursing students have high occupational expectation, but low occupational acknowledgment in society. Nursing managers should pay increasing attention on the value of the postgraduate nursing student, give full play to the advantages of talent. Postgraduate nursing education should take into account both research and practice ability to guide their reasonable employment, and improve occupational identity. Postgraduate nursing students should properly adjusted self-role, and establish the scientific concept of employment.
