Objective To investigate the effects of continuous end-eiastoloc volume index(CEDVI) adjusted by right venteicular ejection fraction(RVEF) in using for volume resuscitation of critically ill patients. Methods 51 critically ill patients who Swan-Ganz volume pulmonary artery catheters were inserted in via right internal jugular or subclavian vein underwent fluid therapy.Fluid was infused according to the relationship of RVEF and CEDVI.And the relationship of RVEF and CEDVI was that when RVEF was 0.2,0.3,0.35,0.4 and 0.5,respectively,CEDVI is 200~400ml/m~2,150~180ml/m~2,125~150ml/m~2,100~120ml/m~2,50~60ml/m~2 respectively. When RVEF is

Objective To evaluate the therapeutic effect and safety of ureteroscopy pneumatic lithotripsy(PL) and extracorporeal shock wave lithotripsy(ESWL) in the treatment of distal ureteral calculi. Methods 368 cases of distal ureteral calculi were divided into the PL treatment group(178 cases) and the ESWL treatment groups(190 cases). The clinical datas were compared between the two groups. Results PL treatment group 97.19% patients became stone free in 4 weeks, and in ESWL treatment group the stone free rate was 73.16%(P

A case of a 3-year-old child suffered with severe crush injury to the right forearm and right hand on June, 2017. A comprehensive treatment was conducted with limb salvage, free flap repair and the repair of nerve, vessel and tendon for functional reconstructions followed by rehabilitation therapies. The function and appearance of the injured limb and hand recovered well 3 years after surgery.

Objective To study the surgical indications and effectiveness of short segment pedicle screw fixation plus pecutaneous vertebroplasty (PVP) for single thoracolumbar fracture combined with Kümmell disease. Methods Between June 2012 and June 2014,a prospective case series analysis was made on the clinical data of 15 selected cases of single thoracolumbar fracture patients who were combined with Kümmell disease. There were three males and 12 females, with an average age of 62.5 years (range,48-80 years). The iujured vertebrae were located at T11 in 3 cases,at T12in 4,at L1 in 5 and at L2 in 3.All the patients were treated by short segment fixation plus PVP.Operative time,surgery-related complications and adjacent vertebral fracture during follow-up were recorded. The effectiveness of the treatment was appraised by visual analogue scale (VAS),Oswestry Disability Index (ODI),Cobb angle of operative segment, which were recorded preoperatively, one week post-operatively and at final follow-up.One-way ANOVA (Analysis of Variance) was employed for statistical analysis. Results The operative time was 0.8-2 hours (mean,1.35 hour). There was no observation of incision infection, nerve injury, cement leakage or other related operative complications.A total of 15 patients were followed up for 16-36 months (mean,22.4 months),which showed solid fusion and bone bridges in all patients, with no fracture of adjacent vertebra observed.VAS was improved from preoperative (5.93 ± 0.62) points to (1.80±0.64) points one week postoperatively and (2.60 ± 0.53) points at final follow-up.ODI was improved from preoperative 61.53 ± 4.30 to 19.93 ±3.26 one week postoperatively and 23.07 ± 4.06 at final follow-up.Cobb angle of operative segment was improved from preoperative (17.40 ± 6.73) ° to (9.53 ± 3.12) o one week postoperatively and (11.00 ± 3.20) ° at final follow-up.There was significant difference of all the indexes namely,VAS,ODI and Cobb angle, between preoperative and one week postoperative status as well as between preoperative and final follow-up (P < 0.05 or 0.01).However,there was no significant difference between ODI and Cobb angle in one week postoperatively and at last follow-up (P > 0.05).Conclusion For single thoracolumbar fracture with Kümmell disease, short segment fixation plus PVP can shorten the surgery time, reduce the rate of complications, relieve pain, recover the function and avoid loss of kyphosis correction,as is worthy of clinical recommendation.

OBJECTIVE:To systematically review the efficacy of tamsul osin a lone or combined with antimuscarinic drugs in the treatment of ureteral stent-related symptom (SRSs),and to provide evidence-based reference for the clinical treatment. METHODS:Retrieved from PubMed ,Embase,Cochrane Library ,CJFD,CBM,VIP and Wanfang database ,from the inception to Oct. 2019,RCTs about tamsulosin alone (monotherapy group )or combined with antimuscarinic drugs (combination group ) versus placebo or blank control (control group )in the treatment of SRSs were collected. After data extraction of included clinical studies met inclusion criteria ,their quality was evaluated with Cochrane collaborative network system evaluation manual 5.1.0,and Meta-analysis was conducted by using Rev Man 5.3 statistical software. RESULTS :A total of 9 RCTs were included ,involving 932 patients. Meta-analysis results showed that after stent placement ,the urinary symptoms score [MD =-4.23,95%CI(-5.96, -2.51),P<0.000 01],body pain score [MD =-4.20,95%CI(-5.30,-3.10),P<0.000 01],general health score [MD = - 1.36, 95% CI( - 1.75, - 0.98), P<0.000 01] in monotherapy group were significantly lower than control group with statistical significance ; there was no significantly 2805097532@qq.com difference in the work performance score [MD =0.19,95%CI(-0.99,1.37),P=0.76],sexual matters score [MD =-0.31 95%CI(-0.68,0.05),P=0.09] in 2 groups. 4 weeks after the stent was removed ,the body pain score [MD =-0.64,95%CI(-0.84,-0.43),P<0.000 01],general health score [MD = -0.28,95%CI(-0.41,-0.15),P<0.000 01] ,urinary symptoms score [MD =-0.64,95%CI(-0.84,-0.43),P<0.000 01] in monotherapy group were significantly lower than control group after stent placement ,with statistical significance ;there was no significantly difference in the work performance score [MD =0.31,95%CI(-0.22,0.84),P=0.25] or sexual matters score [MD = -0.31,95%CI=(-0.68,0.05),P=0.09] in 2 groups. The urinary symptoms score [MD =5.93,95%CI(2.83,9.02),P=0.000 2], body pain score [MD =3.49,95%CI(1.39,5.60),P=0.001] and general health score [MD =2.96,95%CI(1.44,4.49),P=0.000 1] in the monotherapy group were higher than the combination group ;there was no significantly difference in the work performance score [MD =1.53,95%CI(-0.12,3.18),P=0.07] or sexual matters score [MD =0.46,95%CI(-1.03,1.96),P=0.54] in 2 groups. CONCLUSIONS:Tamsulosin has a good therapeutic effect on SRSs ;its combination with antimuscarinic drugs can achieve better therapeutic efficacty.
