Objective To observe the effect ofQizhi Weitong Granules combined with rabeprazole on gastroesophageal dynamics and inflammatory factors in reflux esophagitis (RE) and explore its clinical significance.Methods Nighty-six RE patients from Changping Chinese Medicine Hospital were selected in this study,which randomly divided into control group (n =48) and observation group (n =48).The control group was given rabeprazole 10 mg/time (2 times/d) for 4 weeks.The observation group was given rabeprazole 10 mg/time (2 times/d) and Qizhi Weitong Granule 5 g/time (3 times/d) for 4 weeks.The gastrin and motilin expression was detected by radioimmunity analysis.The IL-17 and IL-23 was detected by ELISA analysis.The rate of clinic effects and adverse reaction was compared.Results There was no significant difference ofgastrin and motilin between the two groups before treatment.After treatment,gastrin and motilin was increased in the two groups (P ＜ 0.05,0.01).However,gastrin and motilin was higher in observation group than that in control group after treatment (P ＜ 0.05).There was no significant difference ofIL-17 and IL-23 between two groups before treatment.After treatment,IL-17 and IL-23 was decreased in the two groups (P ＜ 0.05,0.01).However,IL-17 and IL-23 was lower in observation group than that in control group after treatment (P ＜ 0.05).The rate of clinic effects was 93.8％ in observation group,which was better than 81.3％ in control group (P ＜ 0.05).The rate of adverse reaction was 10.4％ in control group and 8.3％ in observation group.There was no significant difference between two groups.Conclusion Qizhi Weitong Granules combined with rabeprazole can improve gastroesophageal dynamics and inhibit inflammatory response in RE patients,with significantly clinical effect and safety.

Objective To explore the characteristics of clinic and epidemiology of neonatal respiratory syncytial virus(RSV) pneumonia in newborn infants.Methods Three hundred and nine neonates(male 196,female 113) who were admitted to children's hospital of Fudan University and identified as having RSV pneumonia from Jan.2004 to Dec.2008 were enrolled.RSV antigen was detected in exfoliated respiratory cells by direct immunofluorescence.All the clinical data were collected and subjects and they were divided into different groups based on path of infection,gestational age and RSV status.Results From case notes over the 5 years,309 eligible neonates with RSV infection were identified.Male term neonates were more likely to acquire infection,as compared to female,1.71.0.The incidence of RSV infection peaked in winter and spring.Compared with community acquired infection group,hospital acquired infection group had more premature infants(23.2% vs 8.7%,P=0.002),lower birth weight[(3 010.8?852.8) g vs(3 153.2?943.4) g,P=0.026],much longer mean length of hospital stay[(19.0?8.0) d vs(12.2?4.5) d,P=0],more expensive cost[(5 646.4?3632.2) RMB vs(4 175.8?2 879.2) RMB,P=0] and later occurrence day[(21.3?8.6) d vs(15.8?6.0) d,P=0].Compared with simple RSV infection group,mixed infection group had more expensive cost[(6 063.1?3 085.4) RMB vs(4 513.2?3 860.8) RMB,P=0.047] and more oxygen use(40.0% vs 25.7%,P=0.006).The clinical characteristics of patients with RSV pneumonia varied.Preterm group compared with term group more frequently exhibited apnea and cyanosis(20.0% vs 0.7%,42.9% vs 22.7%),respectively.Forty-two percent of them had bacterial infection,while 13.3% of them had infection of other part of body.The prognosis of most RSV(99.0%) infections was good.Conclusions RSV was an important cause of respiratory tract infections during the last five years.Clinical characteristics of RSV infections were atypical.Hospital acquired RSV infection was more severe than the community acquired infection in neonates.There are no specific,effective interventions for treating RSV infections,so preventive measures are most important.

Authentications of Chinese herbal medicine have a critical effect in Chinese clinical medicine. DNA molecular marker, as an important component for true or false authentication, is more and more widely used in iden-tification of Chinese medicinal materials. At the same time, many new methods for authentication of Chinese medici-nal materials are continuously emerging. But the systematically comparative analysis of these new methods is lack. The present study taking Lonicera japonica as an example, systematically compared principles, characteristics, ex-periment methods, detection time and the application scope of express sequence tag-simple sequence repeat (EST-SSR), polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP), allele-specific PCR (AS-PCR), DNA barcoding and loop-mediated isothermal amplification (LAMP), and put forward corresponding improve-ment opinions. This study can help to screen appropriate approach for rapid authentication of L. japonica and offer demonstrating to other Chinese herbal medicines.

Objective To investigate the current situation of the use of standard medical terminologies in journal, and analysis the reasons and then find the solutions.Methods Through searching the WANFANG database, we selected the nonstandard and standard terms of 100 common medical terminologies as Search Words, all the fields as Search Options to trace articles in periodicals from Jan.2011 to Dec.2013.After getting the top ten ranked words, the rate of use of the standard terms was analyzed.Results Some journals still use nonstandard terms, standard terms were mixed up with nonstandard terms in all of the 100 medical terminologies.Conclusions More attention should be paid to the use of the standard medical terms in periodicals to unify and standardize the use of medical terms.

ObjectiveTo study the dissecting necessity of lymph node around normal and abnormal hepatic artery in distal gastric cancer undergoing D2 lymphadenectomy.MethodsSixty gastric cancer patients receiving distal D2 lymphadenectomy by the same surgeon between June 2008 to June 2010 at the Department of Gastrointestinal Surgery, First Affiliated Hospital of the Guangxi Medical University were included in this study. The lymph adipose tissue around the anatomically normal and aberrant hepatic artery was carefully dissected, and the lymph nodes sent for recombinant human cytokeratin 20 (CK20) and carcino-embryonicantigen( CEA )micrometastasisimmunohistochemistry.ResultsWiththe micrometastasis immunohistochemistry of CK20 and CEA, we found the metastasise rate of lymph node around the normal hepatic artery was 27％.Patient age, tumor size, Borrmann type, TNM staging were correlated with the lymph node metastase. There were 7 cases with abnormal hepatic artery originating from the superior mesenteric artery. The hepatic artery ran in front of the pancreas in 1 case and behind the pancreas in 6 cases. We found there are no metastases in the lymph adipose tissue surrounding the abnormal artery.ConclusionsCK20,CEA are suitable immunohistochemical targets for estimating the lymph node micrometastasis. In distal gastric cancer age at 60 or older years, tumor larger than 3 cm and Borrmann Ⅲ-Ⅳ type were risk factors for metastasis of lymph nodes around normal hepatic artery, while aberrant hepatic arteries originating from the superior mesenteric artery are much less likely to have positive lymph nodes in D2 lymphadenectomy.

Objective To investigate the clinical outcomes of the medial displacement calcaneal osteotomy with reconstruction of posterior tibial tendon insertion on navicular for the flatfoot related with accessory navicular.Methods From March 2009 to October 2011,13 patients (16 feet) with flatfoot related with accessory navicular received treatment by the medial displacement calcaneal osteotomy with reconstruction of posterior tibial tendon insertion on navicular.There were 4 males and 9 females,with an average age of 41.3 years (range,18-64 years).All patients had obvious valgus calcaneus,the angle of which was 11.3°± 1.4°.According to AOFAS ankle-hindfoot scores,the arch height,calcaneus inclination angle (CI),talocalcaneal angle (TC),talar first metatarsal angle (TMT) on the lateral weight-bearing radiograph of foot,and the talocalcaneal angle (TC),talar first metatarsal angle (TMT) on the AP view of the weight-bearing radiograph of foot,and the heel valgus alignment on axial radiographs of the hindfoot were measured on the X-ray film.Results All patients were followed up for 12 to 31 months,with the average of 16.8 months.Eleven patients (13 feet) felt no pain 6 months after operation,while 2(3 feet) felt pain after long walking.There was no complication,including infection,nerve injury,un-union,and so on.The average AOFAS ankle-hindfoot score improved from 56.4-±6.4 preoperatively to 88.1±2.8 at the last follow-up.Radio graphically,all parameters were statistically significant between pre-operation and the last follow-up,including the arch height,CI,TC,TMT modifying from 3.8±0.3 mm,9.5°±1.1°,47.3°±2.5°,17.6°±1.6° to 12.0±1.1 mm,20.1°±1.5°,32.3°±2.5°,6.8°±1.0° respectively on the lateral weight-bearing view; TC improving from 39.5°±2.3° to 26.2°±2.0°and TMT improving from 15.2°±1.7° to 6.3°±1.0° on the AP weight-bearing view.Conclusion The medial displacement calcaneal osteotomy with reconstruction of posterior tibial tendon insertion on navicular is a good choice for the treatment of flatfoot related with accessory navicular with excellent clinical outcomes.
