1.Acute insular infarct increases risk of electrocardiogram changes and sudden cardiac death.

OBJECTIVETo investigate the influence of location and size of acute insular infarct on stroke-related electrocardiogram (ECG) changes and cardiovascular events.
METHODSNinety-nine cases admitted to hospital from October 2007 to June 2009, who were diagnosed as acute middle cerebral artery territory infarct within 48 h after onset and without the history of cardiac diseases, were included in the study. The patients were further divided into three groups: major insular infarct, minor insular infarct and control group, according to the infarct size on MRI diffusion-weighted image. The clinical data, ECG changes and cardiovascular events were compared between left and right insular infarct. Logistic regression was applied to determine the independent risk factors of ECG changes and cardiovascular events.
RESULTLarge artery atherosclerosis was the main cause of acute insular infarct (71.8 %), which was associated with higher NIHSS score compared to the control group (P < 0.01). Comparing the left and right insular infarct, the frequencies of sinus bradycardia and sudden cardiac death were significantly higher in left insular infarct (P < 0.01 and P < 0.05), while there was a trend that the frequency of atrial fibrillation was higher in right insular infarct (P = 0.079). With the larger size of insular infarct, the frequency of sinus bradycardia, new atrial fibrillation and sudden cardiac death (P<0.01, P<0.05 and P<0.05, respectively) became much higher. Logistic regression analysis showed that major insular infarct was related to the higher frequency of sinus bradycardia (OR = 4.660, 95% CI: 1.646 ~ 13.195; P = 0.004).
CONCLUSIONAcute insular infarct is associated with the stroke-related ECG changes and sudden cardiac death. Left insular infarct is related to sinus bradycardia, possibly due to the enhanced parasympathetic tone. It deserves clinical attention that the incidence of cardiac autonomic disturbance becomes higher with the enlarged insular infarct size.

2.Relationship between contraceptive use behavior and related knowledge and cognition among vocational high school students.

OBJECTIVETo study the relationship between contraceptive use behaviors and related knowledge and cognition among vocational high school students.
METHODSA computer-administrated anonymous cross-sectional survey was conducted among grade one students from three vocational high schools in a district of Shanghai. Based on Health Belief Model, four cognition indexes including perceived benefits, barriers, sex behavior related risks and self-efficacy were applied.
RESULTSAmong 1612 respondents, the students who had sexual experience accounted for 7.01% (113/1612). At first sex, 56.64% (64/113) of respondents reported contraceptives use; and the methods most reported were condom (58.70%, 27/46) and oral contraceptive (27.78%, 5/18) respectively for male and female. Most reasons for non-use were feeling shy to buy it (26.53%, 13/49) and the opposite sex's unwillingness to use (18.37%, 9/49). Among those who had experienced sex, 68.35% (54/79) and 68.42% (52/76) of respondents thought withdrawal and rhythm were effective methods. Only 23.26% (10/43) of subjects who had ever used condom could gain the full score of knowledge on how to use condom. A few respondents (22.86%, 24/105) who had sex experience knew that "If a person is infected with STD, he/she may not have noticeable symptoms". And 40.63% (26/64) of respondents who had not use contraceptive at first sex considered that "Infrequent intercourse cannot cause a girl pregnant even she has experienced her menstruation". The level of perceived risks was lowest among four cognition indexes. Contraceptive related knowledge and cognition may contribute to respondents' contraceptive use behaviors at first sex, and only related cognition contribute to contraceptive use behaviors at recent sex.
CONCLUSIONIt should be urgent to improve contraception knowledge and cognition among vocational high school students and provide youth-friendly contraceptive services.

Objective To understand the status of health literacy of residents in Qiandao Lake town in order to complete the surveillance system.Methods A total of 484 residents aged 15 to 69 years old from 9 communities were selected using cluster random sampling method.Then the household interview was conducted.Results The overall level of health literacy of residents was 26.03%.As for the 4 health literacy related aspects including basic health knowledge,health lifestyle and behaviors,health related skill and health beliefs,the rates were 32.54% ,4.77% ,60.30% and 92.62%,respectively. The multivariate logistic regression analysis showed that the level of health literacy of residents was related to age and education.Conclusion The health education in Qiandao Lake town has got some achievements.
4.Comparison of Therapeutic Efficacy for Neonatal ABO Hemolytic Disease Treated with Intravenous Immunoglobin G by Different Modes of Administration.

OBJECTIVETo compare the therapeutic efficacy of patients with neonatal ABO hemolytic disease treated with introvenous immunoglobin G (IVIG) by different modes of administration.
METHODSNinety-three in patients with neonatal ABO hemolytic disease treated in our hospital were divided into group A (31 cases), B(31 cases) and C (31 cases). Based on basic treatment, the patients in group A were treated by a single high dose of IVIG (1 g/kg), patients in group B were treated by multiple low-dose of IVIG (0.5 g/kg), and the patients in group C treated by placebo without IVIG used as controls. The phototherapy time, jaundice time in 3 groups were observed; the total bilirubin levels in 3 groups were compared before and after treatment; the incidence of anemia, the rate of blood transfusion and the occurrence of bilirubin encephalopathy were compared after treatment between 3 groups.
RESULTSThe phototherapy time, jaundice time in group A were statistically significantly shorter than those in the group B and C (P<0.05), but there was not statistical significantly difference between group B and C(P>0.05). Before treatment, serum TBIL level in 3 groups was not significantly different (P>0.05); and after treatment for 24 h and 48 h, the serum TBIL levels in group A were significantly lower than that in group B and C (P<0.05); after treatment for 72 h, the serum TBIL level in group A was all lower than 34.2 µmol/L; before treatment, Hb levels in 3 groups were not significantly different (P>0.05); Hb level in group A was significantly higher than that in group B and C after treatment for 24 h, 48 h and 72 h (P<0.05). The incidence of anemia in group A after treatment was significantly lower than that in group B and C, and that in group B significantly lower than that in group C(P<0.05). The rate of blood transfusion in group A was significantly lower than that in the group B and C (P<0.05); the rate of blood transfusion was not statistically significantly different between group B and C(P>0.05).
CONCLUSIONThe single high dose of IVIG infusion can effectively reduce the serum TBIL level, shorten treatment time and reduce the incidence of anemia and blood transfusion, so the therapeutic efficacy is significantly improved.
5.The fundamental role of stage control technology on the detectability for Salmonella networking laboratory.

OBJECTIVETo evaluated the fundamental role of stage control technology (SCT) on the detectability for Salmonella networking laboratories.
METHODSAppropriate Salmonella detection methods after key point control being evaluated, were establishment and optimized. Our training and evaluation networking laboratories participated in the World Health Organization-Global Salmonella Surveillance Project (WHO-GSS) and China-U.S. Collaborative Program on Emerging and Re-emerging infectious diseases Project (GFN) in Shanghai. Staff members from the Yunnan Yuxi city Center for Disease Control and Prevention were trained on Salmonella isolation from diarrhea specimens. Data on annual Salmonella positive rates was collected from the provincial-level monitoring sites to be part of the GSS and GFN projects from 2006 to 2012.
RESULTSThe methodology was designed based on the conventional detection procedure of Salmonella which involved the processes as enrichment, isolation, species identification and sero-typing. These methods were simultaneously used to satisfy the sensitivity requirements on non-typhoid Salmonella detection for networking laboratories. Public Health Laboratories in Shanghai had developed from 5 in 2006 to 9 in 2011, and Clinical laboratories from 8 to 22. Number of clinical isolates, including typhoid and non-typhoid Salmonella increased from 196 in 2006 to 1442 in 2011. The positive rate of Salmonella isolated from the clinical diarrhea cases was 2.4% in Yuxi county, in 2012. At present, three other provincial monitoring sites were using the SBG technique as selectivity enrichment broth for Salmonella isolation, with Shanghai having the most stable positive baseline.
CONCLUSIONThe method of SCT was proved the premise of the network laboratory construction. Based on this, the improvement of precise phenotypic identification and molecular typing capabilities could reach the level equivalent to the national networking laboratory.

6.Effect of neck muscular strength training at different time durations for military pilots

Objective To test and evaluate the effect of neck muscular strength training for military pilots at different time durations based on a self-developed pilot neck muscle training device. Methods The neck muscular strength training device, CME-1, was used to train the military pilots and military smithery members for 2, 3 and 6 weeks, respectively. The isometric and changeable velocity and resistant (CVR) modes were performed and the neck muscular strength at those different time durations was measured to evaluate the training effect. Results For all 60 military pilots trained at different time durations, the average neck muscular strength and the mean value of peak impulse in 10 seconds in anteflexion, retroflexion, right/left lateroflexion were significantly improved after training (P<0.05). After 3 weeks’ training, the average neck muscular strength of the four flexion was increased by 47.3%, 44.8%, 70.5% and 59.6%, respectively, while the mean value of peak impulse in 10 seconds were increased by 49.3%, 41.3%, 65.0% and 55.5%, respectively. The mean value of the peak impulse in 10 seconds of the retroflexion muscles after 3 weeks’ training was increased in comparison with 2 weeks’ training, but not significant (P>0.05). For 11 military smithery members after 6 weeks’ training, both the average muscular strength and the mean value of peak impulse in 10 seconds in all directions were significantly enhanced (P<0.001), with the average muscular strength increased by 71.1%, 83.7%, 78.6% and 75.2% respectively, while the mean value of peak impulse in 10 seconds was increased by 136.9%, 138.5%, 114.1% and 114.4%, respectively. Conclusions This study provides the physiological basis for establishing the military standards of neck muscular strength training for different fighter pilots, the proper method for training those pilots with low neck muscular strength, as well as the reference of neck muscular training duration and strength target for high performance aircrafts and carrier aircrafts.
7.Clinical application of supraclavicular fasciocutaneous island flap in the repair of tracheal defects.

Objective: To explore the clinical application of supraclavicular fasciocutaneous island flap (SIF) in the repair of tracheal defect. Methods: From May 2016 to March 2021, the clinical data of 10 patients (8 males,2 females,aged 27-73 years old) were retrospectively analyzed who underwent repair surgery with SIF for trachea defects after resection of cervical or thoracic tumors, including 2 cases of laryngotracheal adenoid cystic carcinoma, 2 cases of laryngeal carcinoma, 3 cases of esophageal carcinoma, 2 cases of thyroid carcinoma and one case of parathyroid carcinoma. All of the primary tumors were at T4. The outcomes of 10 cases with tracheal defect repaired by SIF were evaluated. Results: The areas of the SIF were (3-7) cm × (6-10) cm, the thicknesses of the flaps were 8-11 mm, and the lengths of the pedicles were 10-15 cm. The blood supply of the SIF came from the transverse carotid artery. The skin defects of the donor areas of the shoulders were directly closed. After 1-60 months of follow-up, all the flaps survived. The flaps, tracheas as well as shoulder wounds healed well. Conclusion: The SIF is suitable for the repair of tracheal defects. It has perfect thickness compatible with the trachea. The technique is simple and microsurgical technique is not needed, with a good application prospect.
