Objective To compare the efficacy of single-port laparoscopic unilateral salpingectomy and adnexectomy(ovary+fallopian tube)via different approaches,and to explore the safety and feasibility of single-port single-handed laparoscopy without access platform.Methods A total of 94 patients who underwent unilateral salpingectomy and 78 patients who underwent unilateral adnexectomy in our hospital from August 2021 to November 2022 were selected and randomly divided into the single-port single-handed without access platform group,the single-port double-handed without access platform group,and the single-port with access platform group,respectively.The operation time,intraoperative blood loss,postoperative morbidity,postoperative anal exhaust time,postoperative hospitalization time,total hospitalization cost,incision length,postoperative pain visual analogue scale(VAS)score,surgical complications,postoperative body image scale(BIS),and cosmetic score(CS)of patients in the three groups were observed and compared.Results There was no statistically significant difference in operative time,intraoperative blood loss,postoperative morbidity,postoperative anal exhaust time,postoperative hospitalization time,and surgical complications among the three groups(P>0.05).There were statistically significant differences in total hospitalization cost,incision length,postoperative VAS score,postoperative BIS score,and postoperative CS among the three groups(P<0.05).Conclusion Single-port single-handed laparoscopy salpingectomy and adnexectomy without access platform is a safe and feasible surgical method,which has the advantages of smaller incision,less hospitalization cost,and higher incision satisfaction than single-port surgery with access platform and single-port double-handed laparoscopic surgery without access platform.
3.Overexpression of Hsp70 Promoted the Expression of Glycolysis-related Genes in C2C12 Cells

The aim of this study was to investigate the impact of overexpressing 70-kD heat shock pro-teins(Hsp70)on glycolysis in C2C12 cells during myogenesis and adipogenesis.Using C2C12 cells as the research material,adenovirus was used to overexpress the Hsp70 gene,and changes in the expression of glycolytic genes were detected using fluorescence quantitative PCR and Western blotting techniques.The study indicated that during C2C12 cell myogenic differentiation,the expression trend of the Hsp70 gene was consistent with that of Gsk3β,Pkm,Prkag3,Pfkm,and Hk-2 genes,suggesting a relationship between Hsp70 and the glycolytic pathway during myogenic differentiation.Overexpression of Hsp70 in the later stages of myogenic differentiation significantly upregulated the expression of Gsk3β,Pkm,Prk-ag3,and Pfkm genes(P<0.05),with no significant impact on Hk-2 gene expression(P>0.05).Dur-ing C2C12 cell adipogenic induction,the expression trend of the Hsp70 gene was similar to that of Gsk3β,Pkm,Prkag3,Pfkm,and Hk-2 genes,indicating a relationship between Hsp70 and the glycolytic path-way during adipogenic induction.Following Hsp70 overexpression,in the later stages of adipogenic in-duction,the number of lipid droplets was significantly higher compared to the control group,with a sig-nificant upregulation of Gsk3β,Pkm,Prkag3,and Pfkm gene expression(P<0.05),while Hk-2 gene expression was not significantly affected(P>0.05).In conclusion,Hsp70 in C2C12 cells in myogenic and adipogenic states promoted the breakdown of glycogen into 6-phospho-glucose,thereby enhancing the glycolytic pathway,providing insights into the functional role of the Hsp70 gene in glycolysis in C2C12 cells.
4.Consistency of chemical constituents between formula granules and standard decoction of Coptidis Rhizoma

AIM To investigate the consistency of chemical constituents between formula granules and standard decoction of Coptidis Rhizoma.METHODS Eighteen batches of standard decoctions were prepared,after which the extraction rate and contents,transfer rates of magnolflorine,jatrorrhizine,columbamine,epiberberine,coptisine,palmatine,berberin were determined,HPLC characteristic chromatograms were established.RESULTS There were 11 common peaks in the characteristic chromatograms of 18 batches of standard decoctions and 24 batches of formula granules with the similarities of 0.861-1.000,which were clusterd into two categories.The formula granules and standard decoction demonstrated approximated extraction rate and contents,transfer rates of index constituents.CONCLUSION The chemical constituents between formula granules and standard decoction of Coptidis Rhizoma display good consistency,which can provide references for the quality control,process research and clinical application of the former.

Through collecting the relevant records of "brain" in ancient traditional Chinese medicine literature, the potential structure and function of the "motor system of brain-meridian-muscle region" and "perception system of brain-meridian-cutaneous region" of the human body are analyzed preliminarily. In view of the characteristics of the symptoms of stroke sequela, focusing on the differentiation of disease location and associated with the identification of disease property, the pathogenesis of stroke is determined, "malnutrition of the sea of marrow, dysfunction of meridian muscle region and disharmony of the nutrient and the defensive in the meridian cutaneous region", while, the basic principle of treatment is formulated, "filling up the marrow, replenishing the brain, regulating meridian muscle regions, tonifying qi and facilitating blood circulation in meridians and collaterals". Based on the above, the acupoints selected in treatment and the techniques of acupuncture and moxibustion are clarified. This article presents the references for the clinical practice of acupuncture and moxibustion and provides the individual therapeutic regimen for stroke patients.

7.Clinical application effect of pedunculated rectus abdominis muscle combined with bilateral ureters for repairing refractory bladder-vaginal stump fistula through external vesical drainage.

Objective: To investigate the efficacy and safety of pedunculated rectus abdominis combined with bilateral ureteral extravestheter drainage in the treatment of refractory bladder-vaginal stump fistula. Methods: The clinical data of 8 cases of the refractory bladder-vaginal stump fistula were admitted to the Second Hospital of Hebei Medical University and Henan Cancer Hospital and underwent the clinical treatment of bladder-vaginal stump from December 2019 to December 2022 were collected. The reason of refractory bladder-vaginal stump fistula was analyzed, the operation manner of pedunculated rectus abdominis combined with peduncle and bilateral ureter for the treatment of bladder-vaginal stump through extrabladder drainage was explored. The operation time, bleeding volume and clinical effect were record. Results: The median operation time of 8 patients was 150 minutes(120~180 min), and the median blood loss was 400 ml(200~600 ml). During the perioperative period, there were 2 cases of incision infection, delayed healing by debridement and dressing, 2 cases of incision rupture and suture wound healing after reoperation, and 2 cases of urinary tract infection were cured by anti-infection. When followed up for 6 months, 8 cases of vesicovaginal stump fistula were cured. Conclusion: Bilateral ureteral external drainage of the rectus abdominis muscle, has a practical effect in the treatment of refractory bladder-vaginal stump fistula, which can be one of the clinical repairing treatment.

8. Correlation between sarcopenia,sarcopenic obesity and osteoporosis in adults of Han nationality in Liaoning province

Objective To understand the prevalence of sarcopenia,sarcopenic obesity and osteoporosis, and to analyze the influence of sarcopenia and sarcopenic obesity on osteoporosis. Methods After all the people signed the informed research agreement, the experimental subjects were selected from Han people over 20 years old in Liaoning region, and a total of 1266 cases were included. The distribution of muscle mass, fat mass and bone mineral density in different parts of adults were measured by bioelectrical impedance analyzer (BIA) and bone densitometer, and the correlation between sarcopenia and sarcopenic obesity and osteoporosis in adults was studied. Results With the increase of age, the muscle mass of limbs, trunk, total muscle mass and body weight of the Han adults in Liaoning showed a trend of increasing at first and then decreasing. There was no significant difference in the prevalence of sarcopenia and sarcopenic obesity between men and women, but there was a significant difference in the prevalence of osteoporosis between men and women. The prevalence of all three groups reached the peak in the age group above 60, and the difference was statistically significant.The risk factor for osteoporosis was sarcopenia and sarcopenic obesity. Conclusion Among adults of Han nationality in Liaoning, the prevalence of sarcopenic obesity,sarcopenia and osteoporosis is significantly different in age. Bone condition is affected by fat mass and muscle mass.

10.Clinical Characteristics and Prognosis of Patients with Hematological Malignancies Superimposed with Solid Tumors.

OBJECTIVE:
To investigate the clinical characteristics and prognosis of hematological malignancies superimposed patients with solid tumors.
METHODS:
The clinical data of 30 patients with more than two kinds of malignancy (the second is hematological malignancy) from October 2011 to October 2020 in Department of Hematology, Jiangning Hospital Affiliated to Nanjing Medical University were collected and analyzed retrospectively. The overall survival time was used as the prognostic evaluation standard, and the survival of patients were analyzed by KaplanMeier method. Logrank test and Cox regression model were used to carry out univariate and multivariate retrospective analysis on clinical and laboratory parameters of 30 patients.
RESULTS:
Among 30 cases, 20 were male, 10 were female, the median age of onset of the second tumor was 70 years old. The common types of the secondary hematological malignancies to solid tumors are myelodysplastic syndrome, acute myeloid leukemia, multiple myeloma. Univariate analysis showed that patients' gender, age, type of solid tumors, the onset of interval between two kinds of tumor, chromosome karyotype were not related to do with the patients' overall survival time. Type of hematologic disease, ECOG score were associated with patients' overall survival time, and the multivariate analysis showed that the type of hematologic disease and ECOG score were independent risk factors for patients with poor prognosis.
CONCLUSION
Patients superimposed with solid tumors complicated with myelodysplastic syndrome or acute leukemia and ECOG score ≥3 have poor prognosis and shorter overall survival time, which are independent risk factors influencing the prognosis. Bone marrow injury, immune dysfunction and genetic susceptibility after chemoradiotherapy may be the main causes of these diseases.
