2.Clinical significance of preserving the intercostobrachial nerve in modified radical mastectomy for breast cancer

Objective To analyze and discuss the effects of preserving the intercostobrachial nerve(ICBN)in modified radical mastectomy for breast cancer. Methods A total of 80 patients with breast cancer were randomly divided into the observation group and control group(n = 40 in each). The ICBN in the observation group were reserved and it was removed in the control group. Operation time,the number of lymph nodes,blood loss and complication rates were compared between groups. Two groups of patients were followed up for abnormal sensory function and relapse and metastasis. Results There were no differences in the operation time,the number of dissected lymph nodes and blood loss(P > 0. 05). There was no difference in complication rate(12. 50% vs 15. 00% )between the observation group and control group(P > 0. 05). There were significant differences in the incidence of abnormal sensory function at the first week(10. 00% vs 52. 50% ),the first month(5. 00% vs 47. 50% ),the third month(2. 50%vs 45. 50% ),the sixth month(2. 50% vs 37. 50% ),and the twelfth month(0. 00% vs 27. 50% )after op-eration between the observation group and control group(P < 0. 05). During the one-year follow-up peri-od,there was no relapse or metastasis. Conclusion Preserving ICBN in modified radical mastectomy can effectively reduce the incidence of abnormal sensory function of the upper limb.
5.HPLC Fingerprint Analysis of Disporum Cantoniense

Objective To establish the HPLC fingerprint of Disporum cantoniense. Methods HPLC analysis was performed on Agilent Zorbax SB-C18 chromatographic column ( 250 mm × 4. 6 mm, 5 μm) with the mobile phase consisting of methanol-0.05% phosphoric acid in gradient mode.The flow rate was 1.0 mL·min-1, the detection wavelength was 256 nm and the column temperature was 30 ℃. Results The HPLC fingerprint of 15 batches of Disporum cantoniense was established. Thirteen common peaks in the fingerprint were demarcated, four of which were identified by reference substances. Chemical pattern recognition of fingerprint was performed by hierarchical cluster analysis and principal component analysis. Conclusion The method is simple, accurate and has a good repeatability, and can be used for quality control of Disporum cantoniense.
6.Prognosis and therapy of patient with renal cell carcinoma accompanied with venous tumor thrombus

Objective To investigate the clinical features and prognosis of the patients with renal cell carcinoma and venous tumor thrombus.Methods Fifty-six patients with renal cell carcinoma and venous tumor thrombus were selected as our subjects,who underwent radical nephrectomy and thrombectomy in the Second Hospital of Tianjin Medical University from June 2002 to May 2014.There were 28 patients with stage Ⅰ tumor thrombus,l5with stage Ⅱtumor thrombus,9 with stage Ⅲ tumor thrombus and 4 with stage Ⅳtumor thrombus.All patients underwent renal tumor resection and embolectomy,and were follow-up.Results Of all the patients,46 were male and 10 were female with a mean age of 61.7(age ranging from 42 to 83).Twenty-four were presented on the left kidney and 32 were right.The clinical features were as follows:The tumor size was 2.5 to 14 cm in diameter(mean:6.2 cm),there were 21 cases with low back pain,18 cases with hematuria,lcases with abdominal.Pathological results showed that 39 with clear cell carcinoma,9 with papillary cell carcinoma,3 with chromophobe cell carcinoma,1 with low-differentiated cell carcinoma and 1 with undifferentiated cell carcinoma,3 with sarcomatoid differentiation.Forty-three patients were followed up,and the median follow up period was 20.4 months (2-90 months).The median survival time for the patients was 47 months and the 5 year overall survival was 45.2%.The survival time of the patients with early tumor thrombus(below hepaticvein) was (55.3 ± 4.9) month,significantly longer than that of the patients with advanced tumor thrombus (above hepaticvein) ((24.8 ± 5.3) months,P =0.047).Conclusion Patients with renal cell carcinoma and venous tumor thrombus are characterized with high malignancy and poor prognosis.Surgical operations are effective therapies for the patients.Long term outcome of the early tumor thrombus patients is significantly better than that of the advanced tumor.

8.Effects of subcutaneous and intraperitoneal administration of insulin to KK mice's oxygen free radicals metaholism

ObjectiveTo observe the subcutaneous and intraperitoneal insulin injection's effect of the level of oxygen free radicals of type 2 diabetes model.MethodsC57BL/6J mice were chosen as normal control group (C group,n=9).KK mice were randomly divided into intraperitoneal injection of insulin group (i.p.group,n=9),the subcutaneous insulin group (s.c.group,n=9) and untreated group (U group,n =9).The i.p.group and the s.c.groups were given certain amount of insulin (insulin injecta and protamine insulin injecta by volume ratio of 2:1 mixture)for one month,maintained the GLU at normal levels (6±1.5) mmol/L.SOD,GSH-PX activity and MDA content of serum,liver,kidney and heart in each group were detected.Results The liver,kidney,heart and serum's SOD and GSH-PX activity significantly reduced and MDA content significantly increased in the U group.Both kinds of delivery methods could increase serum SOD and GSH-PX activity and reduce the content of MDA to the normal control group level,but the intraperitoneal injection had stronger effect.Two kinds of delivery methods could both reduce the MDA content of liver,and had almost the same effect; but the subcutaneous injection group had better effect on increasing the liver's SOD activity,and the intraperitoneal injection had better effect on increasing liver's GSH-PX activity.Intraperitoneal injection had better effect on reducing kidney' s MDA content and increased SOD activity.Two kinds of delivery methods had the same effect on reducing the heart's MDA content.Conclusion The two delivery methods can both make the MDA levels of KK mice in serum,heart,liver and kidney fall to as normal as that of control group,but the two delivery methods have different ways of improving the antioxidant capacity in different organs.Intraperitoneal injection can reduce MDA content in serum and kidney better.
9.Human embryonic olfactory ensheathing cell transplantation for treating the sequel of myelitis:A self-control study of 32 cases using American Spinal Injury Association Scoring Standard

BACKGROUND:Animal experimental studies have confirmed that cell transplantation,neurotrophic factor infusion or transplantation as well as other methods can alter the local environment of injured spinal cord and promote its partial function recovery. OBJECTIVE:This study aimed to assess the clinical efficacy of olfactory ensheathing cell transplantation for the treatment of the sequel of myelitis,and to explore whether it would promote the recovery of the spinal cord function. DESIGN:A non-randomized self-control study. SETTING:Ward of Second Department of Surgery of Taian Disabled Soldiers Hospital of Shandong Province. PARTICIPANTS:Thirty-two patients with obsolete myelitis,who come from all over China and suffered from disease for 0.5 to 7 years,admitted to our hospital between June 2004 and July 2007 were recruited in this study.The involved patients,including 21 males and 11 females,were aged 5-48 years.Their neurological functions were not obviously improved after various conventional treatments and limb function exercise.Meanwhile,various sensorimotors and autonomic nerve functional impairments were left.Among the patients,18 suffered from acute viral myelitis,8 from acute purulent myelitis and 6 from tuberculous myelitis.After onset,they were all given large doses of radiosonde, dexamethasone,anti-inflammatory and immunomodulatory drugs and various neurotrophic drugs.Twenty-six patients presented complete injury and six patients incomplete injury.Informed consent of treatment was obtained from each patient.The therapeutic protocol was approved by the Ethics Committee of the hospital.Embryonic olfactory bulbs were harvested from aborted embryo,which was donated voluntarily by the patients or their relatives. METHODS:Cells were isolated from embryonic olfactory bulbs,cultured and purified for 7 to 14 days,and finally they were digested into single-cell suspension.Under the surgical miscroscope,the cells were transplanted onto the regions which were above or below the spinal cord injury site.Two weeks to 2 months postoperatively,neurological function of spinal cord was assessed by using the American Spinal Injury Association(ASIA)Scoring Standard formulated in 2000,and was compared to pre-operation function.
