2.Current status of laparoscopic therapy of severe acute pancreatitis

The surgical intervention and criteria of severe acute pancreatitis has significantly changed in the last several years with the advent of enhanced imaging techniques and minimally invasive surgery.Many kinds of minimally invasive technologies which take laparoscopy as a representative are constantly applied to the treatment of severe acute pancreatitis and make its mortality and postoperative complication rate decrease obviously.However,there are many different ways using laparoscopic techniques to treat severe acute pancreatitis.In this review,the author will discuss the current status of the different approaches.
3.Effect of budesonide nebulization in treatment of patients with chronic obstructive pulmonary disease and ventilator-associated pneumonia

ObjectiveTo explore the effect of budesonide nebulization in treatment of patients with chronic obstructive pulmonary disease and ventilator-associated pneumonia. MethodsA total of 49 cases with COPD and VAP was randomly divided into two groups:the control group consisted of 24 patients received salbutamol nebulization on the routine treatment.The observe group consisted of 25 patients received budesonide plus salbutamol bulization on the routine treatment.Variables such as TNF-αt and IL-8 in the bronchial alveolar lavage fluid(BALF),and efficacy of pneumonia and clinical healing rate were observed. ResultsThe levels of TNF-α and IL-8 in BALF of the observe group were significantly lower than those in the control group after 72h(all P<0.05).The days of symptoms,signs and bedside chest X-ray improved,off-machine time,extubation time and the days of inpatient were statistically shorter in observe group compared with the control group(all P<0.05).But the clinical cure rate of the observe group was no significant improvement. ConclusionThe nebulization of budesonide could suppress the local pulmonary inflammation and reduce the time of machine ventilation and improve prognosis in COPD patients with VAP.

For the past few years,lumbar sagittal curvature is increasingly recognized as a critical parameter in spine surgery.It has become evident that good clinical outcome in the spine surgery requires proper lumbar sagittal curvature.With the development of recognization of lumbar sagittal curvature,many new ways and views have been found in measure and recovery sagittal alignments.The purpose of this manuscript is to provide a review of the past year's published literature involving lumbar sagittal curvature.
6.Efficiency of preoperative TACE treatment to HCC patients after hepatectomy

Objective: To study the efficiency and prognosis of preoperative transcatheter arterial chemoembolization(TACE)treatment in patients with hepatocellular carcinoma(HCC)after hepatectomy. Methods: The clinical pathological data of 479 patients with resectable HCC from 1998 to 2007 were analyzed retrospectively. Fifty patients had preoperative TACE, 379 cases had not and others were lost of follow up or dead. Clinical pathological data, tumor-free survival rate and overall survival rate after hepatectomy were analyzed. Results: There was a significantly difference in the age and the tumor grade between the groups, other clinical pathological factors were no difference. The preoperative TACE group was younger than non-TACE group (?2=6.228, P=0.012), And had lower tu-mor grade, more tumor necrosis (?2=53.874, P=0.000). The disease tumor-free survival rate and overall survival rate were no difference between the groups (?2=0.736, P=0.391;?2=0.000, P=0. 995). Conclusions: For the unresectable HCC patients by one-stage operation, the preoperative TACE treatment has no adverse effect. However, tumor-free survival rate and overall survival rate are similar to non-TACE group.

10.Advances in antitumor immune mechanisms of radiotherapy in combination with immunotherapy

As a significant treatment for tumors, radiotherapy is often considered a local therapy.However, an increasing number of researches indicate that reasonable radiation dose schedules can improve the tumor environment, induce immunogenic tumor cell death, provide in situ vaccine, and stimulate systemic antitumor effect.Meanwhile, immunotherapy can enhance the effect induced by radiotherapy, which has been proved in preclinical studies and is widely investigated in clinical studies.This review focuses on the immune response and mechanisms of radiotherapy, and advances in the application of radiotherapy in combination with immunotherapy.
