1.Present situation of processing of traditional Mongolian medicine and its normalized suggestion.

The processing technology of traditional Mongolian medicine materials is distinctive, and it is one of the main characteristics of Mongolian pharmacy. Most of Mongolian medicines were used in the raw, but a quarter of medicinal materials need to be produced. Since ancient times, the processing of Mongolian medicine have cooperated with the Mongolian medicine clinical, which plays an important role in improving curative effect of Mongolian medicine and ensuring the safety of the drug. At present, the Mongolian medicines are processed still according to the traditional methods of the ancient literature method which has a lot of problems such as lag in technology, method of diversification, ambiguous indicators and unclear mechanism. Standardization of Mongolian medicine processing was based on traditional Mongolian medicine basic theory, which both projecting the characteristic, inheriting the traditional colleagues and reference to modern medicine, pharmacology, toxicology and other disciplines of knowledge. In this article, the processing situation, existing problem and standardization research of Mongolian medicine were described that providing a reference for the modernization and standardization of Mongolian medicine.

2.Discussion on the development of the satisfaction questionnaire for emergency patients in general hospitals

Satisfaction questionnaires for emergency patients were developed in the present study and face to face surveys were conducted. The rate of effective retrieval was 96.8% and the response rate was 96.2%. The ? coefficient for internal consistency was fairly high (0.7099~0.9217) and there was a fairly strong correlation between the various items and the subordinate factors (0.744~0.912, with the exception of B15). A factor analysis indicated that the factor load and structure were basically consistent with the contents of the questionnaire. A statistical analysis showed that the satisfaction questionnaire for emergency patients, which had both credibility and validity, could be used for evaluating emergency patient satisfaction and could provide hospitals with the basis for measuring medical quality.
3.Study on content determination of lobetyolin and gallic acid in Eighteen Flavors Dangshen Pill from different factories

Objective To develop a HPLC method for determining the contents of lobetyolin and gallic acid in Eighteen Flavors Dangshen Pill(EFDSP) produced by different factories.Methods The HPLC analysis was performed on a VP-DOS C18 column (4.6 mm×150 mm,5 μm).The mobile phase was acetonitrile and 0.4% glacial acetic acid(21∶79) in the determination of lobetyolin content.The detection wavelength was 267 nm and the flow velocity was 1 mL/min.the column temperature was25 ℃ and the sample size was10 μL.The mobile phase was methanol and 0.4% glacial acetic acid(1∶99) in the determination of gallic acid content.The detection wavelength was 280 nm.The column temperature was 25 ℃ and the sample size was 10μL.Results The contents of lobetyolin and gallic acid in EFDSP were 1.0835mg·g-1and 15.334 0 mg/g for Qinghai Gela Dandong Tibetan Pharmaceutical Factory;0.628 9 mg/g and 15.159 5 mg/g for Changdu Tibet Medicine Factory;0.306 5 mg/g and 8.762 7 mg/g for Tibetan Hospital of Tibet.Conclusion This method has the advantages of good reproducibility,good accuracy,simple and fast operation.The contents of lobetyolin and gallic acid in EFDSP produced by different manufacturers are significantly different.The gallic acid content has greater difference.It provides the reference for quality control of EFDSP
4. Improvement of liver fibrosis diagnostic models based on Youden index

Objective: By using Youden index, to improve the performance of the hepatic fibrosis diagnostic models, and to solve the problem of unbalanced diagnostic sensitivity when there is a big difference in the sample size of two groups. Methods: Two hepatitis B virus (HBV) datasets available on GitHub were selected, including 482 HBV infected subjects recruited from Shuguang Hospital in affiliation with Shanghai University of Traditional Chinese Medicine (train set) and 86 HBV infected subjects from Xiamen Hospital of Traditional Chinese Medicine (validation set). By using the two datasets, linear discriminant analysis model, random forest model, gradient boosting model and decision tree model were established, based on four clinical parameters (age, glutamic-oxaloacetic transaminase, glutamic-pyruvic transaminase, and platelet count) of patients, for the diagnosis of early and advanced hepatic fibrosis as well as the diagnosis of hepatic fibrosis and cirrhosis. Youden index was used to adjust the threshold value and the classification result of each diagnostic model. The diagnostic performances of each machine learning model and fibrosis index based on the 4 factor (FIB-4) were evaluated by accuracy, the area under the receiver operating characteristic curve (AUC) and sensitivity. Results: The intergroup sensitivity imbalance occurred in all machine learning models. After using Youden index, the difference of intergroup sensitivity was greatly reduced, and the total accuracy and AUC values of machine learning models were generally higher than those of FIB-4 index. Conclusion:The improved diagnostic models based on Youden index can reduce the difference of intergroup sensitivity and improve the comprehensive performance of the diagnostic models of hepatic fibrosis.
5.Effects of clinical nursing pathway chart in perioperative patients with thoracoscope pneumonectomy

Objective? To explore the effects of clinical nursing pathway chart in rehabilitation of perioperative patients with thoracoscope pneumonectomy. Methods? From January 2017 to December 2018, we selected 105 patients with thoracoscope pneumonectomy. All of the patients were randomly divided into observation group (n=52) and control group (n=53). Observation group carried out perioperative nursing according to the clinical nursing pathway chart, while control group implemented routine nursing. We compared the pain, respiratory recovery effect and treatment compliance of patients after surgery. Results? The scores of pain of patients in observation group 24 and 48 hours after surgery was (4.85±0.42) and (3.74±0.30) respectively lower than those in control group with statistical differences (t=5.594, 13.105;P< 0.05). Five days after surgery, the blood oxygen saturation of observation group and control group was (95.78±0.29)% and (91.73±0.33)% respectively with a statistical difference (t=4.603, P< 0.05). Patients with the high level of treatment compliance in intraoperative period accounted for 73.08% in observation group and 45.28% in control group with a statistical difference (Z=4.891,P<0.05). Conclusions? Application of clinical nursing pathway chart in rehabilitation of perioperative patients with thoracoscope pneumonectomy relieves the postoperative pain and improves rehabilitation effects as well as treatment compliance.
6.Combination of Probiotics and Salvia miltiorrhiza Polysaccharide Alleviates Hepatic Steatosis via Gut Microbiota Modulation and Insulin Resistance Improvement in High Fat-Induced NAFLD Mice

Background:
Nonalcoholic fatty liver disease (NAFLD) increases the risk of hepatocellular carcinoma, which is currently the leading cause of obesity-related cancer deaths in middle-aged men.
Methods:
Probiotics with lipid-lowering function were screened from the fecal microbiota of healthy adults. Polysaccharide from different sources was screened for improving insulin resistance. The combination of probiotics and Salvia miltiorrhiza polysaccharide (LBM) was investigated for alleviating hepatic steatosis.
Results:
First, Bifidobacterium bifidum V (BbV) and Lactobacillus plantarum X (LpX) were obtained from the fecal microbiota of healthy adults. Second, to improve insulin resistance, a Salvia miltiorrhiza Bunge polysaccharide showing good performance in reducing insulin resistance was obtained. The liver total cholesterol (TC) and total triglyceride (TG) levels and the serum levels of free fatty acid, alanine transaminase, aspartate transaminase, low density lipoprotein cholesterol, TG, and TC can be significantly reduced through supplementation with LpX-BbV (LB) in NAFLD mice. Interestingly, the function of the probiotic LB can be enhanced by S. miltiorrhiza Bunge polysaccharide. Furthermore, the gut microbiota was modulated by LpX-BbV+S. miltiorrhiza Bunge polysaccharide (LBM). The lipopolysaccharide concentration of the LBM group was decreased by 73.6% compared to the NAFLD group. Ultimately, the mRNA concentrations of the proinflammatory cytokines (tumor necrosis factor α, interleukin 1β [IL-1β], and IL-6) decreased with LB and LBM treatment.
Conclusion
The results of this this study indicate that the LBM combination can be used as a therapeutic for ameliorating NAFLD via modulating the gut microbiota and improving insulin resistance.

AIMTo study the chemical composition of Tripterygirm wilfordii Hook. f.
METHODSColunm chromatography was used to separate the chemical constituents. UV, IR, MS, HRMS, 1HNMR, 13CNMR (COM and OFR), 1H-1H COSY, 1H-13C COSY, NOESY and COLOC spectra were used to determine the structures of the isolated constituents.
RESULTSTwo sesquiterpene alkaloids were isolated and their structures were elucidated as wilforgine and wilfordlongine on the basis of spectral evidence.
CONCLUSIONWilfordclonine is a new sesquiterpene alkaloid.

8.Effect of Helicobacter pylori eradication on childhood acute idiopathic thrombocytopenic purpura.

OBJECTIVETo evaluate the effect of Helicobacter pylor (Hp) eradication in children with acute idiopathic thrombocytopenic purpura (ITP).
METHODSNinety-three children with acute ITP and Hp infection were divided into two groups and treated with prednisone and Hp eradication (group A, 51 cases) or with prednisone without Hp eradication (group B, 42 cases).
RESULTSThe Hp eradication rate was 94.1% in group A. No difference was found in the therapeutic effects on IPT between the two groups, but the recurrence rate in one year in group A was significantly lower than that in group B.
CONCLUSIONNHp eradication does not obviously enhance the therapeutic effect on childhood acute ITP, but can decrease the relapse rate in one year. HP eradication therapy is recommended in children with acute ITP and Hp infection.

9.Correlative analysis on metatarsalgia and the X-ray measurement indexes under weight-bearing and non-weight-bearing of hallux valgus.

OBJECTIVETo study changes in the radiographic appearance during weight-bearing and non-weigh-bearing in hallux valgus, and to analyse the correlation between the elasticity of plantar soft tissue of hallux valgus and the pain under the metatarsal head.
METHODSFrom May 2012 to October 2012, 240 feet of 120 patients with hallux valgus were enrolled in the study. The degrees of the pian under the metatarsal head of all the patients were observed. AP and lateral X-ray films of feet were taken on the condition of weight-bearing and non-weight-bearing. So the hallux valgus angle (HVA), the inter-metatarsal angle between the first and second metatarsals (IM1-2), the inter-metatarsal angle between the first and fifth metatarsals (IM1-5), top angle of the medial longitudinal arch (TAOTMLA),and anterior angle of the medial longitudinal arch (AAOTMLA) were measured on the X-ray films. The differences of HVA, IM1-2, IM1-5, TAOTMLA and AAOTMLA between two groups were compared, and the correlation between the changes of IM1-2, IM 1-5, TAOTMLA, AAOTMLA and the degree of the pain under the metatarsal head were analysed.
RESULTSOne hundred and forty-eight feet had the pain under the metatarsal head. The IM1-2, IM1-5 and TAOTMLA increased on weight-bearing position compared with those on non-weight-bearing position, but the HVA and AAOTMLA decreased on weight-bearing position compared with those on non-weight-bearing position. There was a moderate relationship between the changes of IM 1-2,IM1-5 and the degree of the hallux valgus deformity, as well as the relationship between the different of IM1-5 and the degree of the pian under the metatarsal head.
CONCLUSIONThe degree of the collapse of the arch of foot with hallux valgus becomes serious with its deformity increasing. The pain under the metatarsal head of hallux valgus increases with the increased changes of IM 1-2,IM 1-5 and TAOTMLA. Analysis of the X-ray observation indexes of hallux valgus on weight-bearing position and non-weight-bearing position has important significance in evaluating the degree of the collapse of the arch of foot with hallux valgus,preventing and curing the the pain under the metatarsal head.

10.A case control study on the treatment of unstable distal radius fractures with internal and external fixation.

OBJECTIVETo explore the clinical effects of diaplastic external fixator on the treatment of unstable fractures of distal radius.
METHODSA clinical data of 60 patients with unstable fractures of distal radius admitted to Wangjing Hospital from August 2005 to September 2009 were retrospectively analyzed with the paired design. According to the paired design, 60 patients were equally divided into two groups (internal fixation group and external fixation group), which were respectively treated with open reduction and steel plate internal fixation (internal fixation group) or closed reduction and external fixation (external fixation group), including 8 males and 52 females, with an average age of 60.27 years ranging from 34 to 85 years. Left side was in 33 cases and right side was in 27 cases. All fractures were closed and caused by hand supporting when falling down. According to AO classification, type A3 was in 22 cases, type B2 was in 4 cases, type B3 was in 2 cases, type C1 was in 20 cases, type C2 was in 8 cases, type C3 was in 4 cases. Palmar flexion, dorsiflexion, radial deviation, ulnar deviation, pronation, supination, grip strength, radial angle, radial length, volar tilt were observed before and after operation.
RESULTSAll the patients were followed up from 12 to 18 months with an average of 15.4 months. The radial length was (4.85 +/- 2.75) mm,volar tilt (14.66 +/- 10.77) degrees, radial angle (7.90 +/- 4.70) degrees in the external fixation group,while those were (4.29 +/- 1.53) mm, (14.39 +/- 5.01) degrees, (6.19 +/- 3.15) degrees in the internal fixation group, but with no statistical significance (P>0.05). Function comparison of internal and external fixation group, P-value of every function is more than 0.05 or equal to 0.05, with no statistical significance except for supination and radial deviation. The functions of wrist joints were evaluated according to Batra scoring system at 1 year after operation. The average radiological scores of external fixation group was 86.27,in which 15 cases were excellent, 11 good, 1 fair,3 poor,while the average functional scores was 94.93, and all the cases were excellent. The average radiological scores of internal fixation group was 91.27, 16 cases were excellent, 12 good, 2 fair, while the average functional scores was 94.23, in which all the cases were excellent.
CONCLUSIONCompared with the open reduction and steel plate fixation, the unstable fractures of distal radius treating by diaplastic external fixator can be achieved a similar result, with no statistical significance.
