Objective To investigate risk factors of chronic renal failure in IgA nephropathy.Methods A total of 384 cases of patients with IgA nephropathy from the First Affiliated Hospital of Guangxi University in Guangxi during 1997—2006 were divided into renal function normal group(320 cases) as the contrast and renal dysfunction group(64 cases) according to serum creatinine level whether beyond 133 ?mol/L,clinical and pathological materials were contrasted,stepwise multiple variance regression analysis was adopted to find the serum creatinine related factors.Results Significant difference was found between the two groups in rates of maleness,dropsy,hypertension,proteinuria,which were remarkably increased in the renal dysfunction group(P0.05).The scores of glomeruli index,interstitial index,blood vessel index,sclerotic index and so on were remarkably higher in the renal dysfunction group(P

Objective To analyze the relationship between atrial fibrillation (AF) and plasma mononuclear cells or cardiac func-tion .Methods From April 2010 to Jun 2013 ,298 patients with AF from department of cardiology in our hospital were selected as study group ,300 patients without AF were selected as control group in the same period .Both group and the subgroup were analy-sised by t test .Results Plasma monocyte NT-proBNP ,left atrial diameter in AF group were significantly higher than the control group(P<0 .05) ,LVEF was lower than the control group ,the statistical differences between the two groups (P<0 .05) .Compare HF combine AF with HF only ,the NT-proBNP and left atrial diameter were significantly higher in former and the LVEF is lower than the latter ,both with the difference statistical significantly (P< 0 .05) .Merge heart failure combined with atrial fibrillation (HF+AF) and heart failure combined sinus rhythm (HF+ SR) group ,the former in the NT-proBNP and left atrial diameterwere significantly higher than the latter ,LVEF lower than the latter ,the difference was statistically difference (P<0 .05) .Conclusion In the development of AF ,the monocytes may be involved in the occurrence and development of AF .
3.The influences of repeated firing cycle on the color of the glass/alumina composite -veneering porcelain structure

The maximum color differences ( ?E) in the selected firing cycles were 2.279 8 and 1.389 4 for GI Ⅱand Vita In Ceram Alumina groups respectively, the maximum ?E between the two groups was 2.295 6, which fell into the clinically acceptable threshold for color differences. Conclusion: Color stability of all ceramic restorations with GI Ⅱand Vita In Ceram Alumina as substructure can be guaranteed after repeated firing of veneering porcelain up to 10 times.

Objective The relationship among angiotensin converting enzyme(ACE) gene polymorphism,angiotensin converting enzyme activity and coronary heart disease(CHD) was studied.Method Teh polymerase chain reaction was used to detect an insertion/deletion(I/D) of ACE gene polymorphism in the sixteen intron,then genotype and allele freqencies were counted .The method of enzyme coupling ratio was used to detect ACE activity .Results In 51 case with CHD ,the DD genotype and D allele freqencies of ACE gene were 35% and 61% respectively ,ACE average activity was (350 3?91 1)su/L.In 83 normal control subject,they were 16% and 45% and (286 7?79 6)su/L respectively.The freqencies of DD genotype and D allele and ACE average activity had significant difference.Conclusion The deletion polymorphism of ACE gene might be an latent risk factor.The higher ACE activity might play an important role in the development of CHD.
5.The influence of platelet rich plasma gel applied to adipose-derived stem cells repair soft tissue wounds

Objective To explore the role of ADSCs and PRP in soft tissue defect repairing .Methods Harvest adipose tissue from inguinalis fat pad of SD rats ,isolation ,culture ,and identification the ADSCs through three differentiation method .Take 30 male SD rats about 6-7 weeks old randomly .Randomly selected 12 rats been take blood from heart .Preparation the PRP with modi-fied appel method .To count platelet of whole blood and PRP under microscope .Take the remaining rats .Divided the rat into 3 groups(n=6 in each group) randomly ,of which group A treatment with ADSCs and PRP ;Group B treatment with ADSCs ;Group C treatment whit PRP .Selected one side of skin defect wound for test randomly and the relative side skin defect is for control ,Handle all of the control wound to group D ;Observe the grow th of granulation tissue on the wound surface ;observe the inflammatory sur-rounding and the degree about epithelial .statistical analysis and record the wound size ,and calculation the shrinkage rate of wound in different periods .Record the time of completely healing time .Histologic observation of the wound healing tissue .Results Plate-let counting showed platelet of PRP is 5 .21 times than whole blood .The wound completely healed time :group A (18 .25 ± 1 .44 ) days ,group B(19 .13 ± 1 .28)days ,group C(19 .72 ± 0 .87)days ,group D(22 .31 ± 1 .65) days ,The time of each treatment group and control group was significantly obvious(P< 0 .05) .At 3 ,7 ,11 and 15 days after experimental treatment ,compared with the control group the experiment group of wound contraction rate was significantly obvious (P<0 .05) .Conclusion Application of AD-SCs with PRP can enhance the quality and shorten the wound healing time than used them alone .
6.Clinical application of high resolution melting analysis in mutation detection of phenylulanine hydroxylase gene

Objective To detect PAH gene mutations in classical PKU patients by HRM analysis. MethodsMutation scanning of PAH gene were performed in 17 classical PKU patients by HRM analysis ( LightScanner), covering the 13 exons and exon-intron boundaries. The HRM results were further confirmed by DNA sequencing, and the sensitivity and specificity of HRM method in PKU diagnosis were also evaluated. In addition, prenatal diagnosis was performed in two fetuses at risk for classical PKU. The prenatal diagnosis results of the two fetuses : one was diagnosed as normal, the other was diagnosed as a carrier. In this study, the sensitivity and specificity for mutation detection by HRM were 100%, and the HRM results were consistent with DNA sequencing results. Conclusions HRM analysis is a simple,accurate, rapid, high-throughput and low-cost genetic analysis approach. It could be applied to mutation scanning of classical PKU of PAH gene and rapid prenatal diagnosis in parents with known mutations.
7.Construction of bladder tissue-engineered grafts by urothelium-induced bone marrow mesenchymal stem cells and bladder acellular matrix

BACKGROUND:Urothelial cells are important seeding cells for urinary tissue engineering, but they are difficult to proliferate in vitro. Several studies have shown that bone marrow mesenchymal stem cells can differentiate into urothelial cells, but how these cells functions in vivo in epithelium generation after implantation, and the application of these cells in tissue engineering, are rarely studied. OBJECTIVE:To explore the isolation and proliferation of rabbit bone marrow mesenchymal stem cells that are induced into urothelial cells in combination with rabbit bladder acellular matrix to construct tissue-engineered grafts, and to assess the effect of the induced cells as seeding cells. METHODS:Twelve 8-week-old male New Zealand white rabbits were chosen to obtain bone marrow samples through tibia puncture, and to isolate bone marrow mesenchymal stem cells by density gradient centrifugation. Then the fourth or fifth generation of bone marrow mesenchymal stem cells were cultured in conditioned medium for 2 weeks, and then identified by PCR and immunofluorescence. After that, the induced cells were seeded on rabbit bladder acellular matrix to construct tissue-engineered grafts for bladder repairing. Another 12 rabbits served as control group, and urothelial cells combined with bladder acellular matrix was used for bladder repairing. RESULTS AND CONCLUSION:Bone marrow mesenchymal stem cells were successful y cultured and proliferated in vitro. After induction, PCR detection suggested that stem cellmarker (CD44) expression decreased, and epithelial cellmarker (UP1a) expression increased in the induced cells. Immunofluorescence staining demonstrated that the induced cells rather than bone marrow mesenchymal stem cells were positive for specific urothelial marker, UP1a. A stable continuous epithelial layer was observed on tissue-engineered grafts constructed by induced cells after 2 weeks, similar to the grafts built by urothelial cells. Induced bone marrow mesenchymal stem cells can differentiate into urothelial cells that can be used as seeding cells for urinary tissue engineering, which may be another choice out of urothelial cells.
8.Value of optimization of bedside Gram staining of sputum smear in the early diagnosis and treatment of ventilator-associated pneumonia

Objective To investigate the significance of optimization of bedside Gram staining of sputum smear in the early diagnosis and antimicrobial treatment for ventilator-associated pneumonia(VAP)patients. Methods The data of patients with VAP undergoing mechanical ventilation over 48 hours in the Department of Critical Care Medicine of Tianjin Fourth Central Hospital from June 2009 to June 2014 were analyzed. The patients were divided into two groups according to whether or not bedside Gram staining of sputum smear was used or not. The sputum samples from lower respiratory tract of all VAP patients were collected daily with tracheal catheter. In empirical examination group(from June 2009 to December 2011,n=43),the patients received antibiotics at the time of onset of VAP, selection of antibiotics depended on the information of bacterial epidemiology of the intensive care unit(ICU),and also existence of high risk factors of multi-drug resistant bacteria. In target treatment group(from January 2012 to June 2014,n=43),the patients received antibiotics according to the results of bedside instant sputum smear examination and empirical antibiotic regime. The correlation between the results of sputum smear examination and culture result was analyzed. The levels of body temperature,white blood cell(WBC)count,procalcitonin(PCT)level,and high sensitivity C-reactive protein(hs-CRP)were measured on the 1st day and 3rd day. The length of antibiotics treatment, duration of mechanical ventilation,and the time of ICU stay were recorded for both groups. Results There were 512 qualified sputum specimens for culture,from which 336 pathogens were found,and 358 strains of pathogenic bacteria were found from microscopic examination of 512 qualified sputum smear. The coincidence rate of results of bedside examination of sputum smear and that of sputum culture was 78.32%(401/512). The diagnostic acumen of the former was 85.42%(287/336),specificity was 64.77%(114/176),positive predictive value was 80.17%(287/358),and negative predictive value was 74.03%(114/154). On the 1st day,no statistical differences in infection index between the two groups could be found,but on the 3rd day,the results were significantly improved in both groups. Compared with the empirical treatment group,the body temperature,WBC,PCT and hs-CRP in the target treatment group were significantly lower〔body temperature(℃):36.83±0.69 vs. 37.64±0.71,WBC(×109/L):7.91±2.75 vs. 9.66±3.39,PCT(μg/L):7.14±3.89 vs. 10.14±4.32,hs-CRP(mg/L):12.24±6.28 vs. 15.54±5.94,P<0.05 or P<0.01〕. Compared with the empirical treatment group,the time of antibiotics use(days:6.00±2.55 vs. 9.20±3.46), the duration of mechanical ventilation(days:5.00±1.73 vs. 7.00±1.94),and the length of ICU stay(days:7.43±1.72 vs. 12.57±4.16)were significantly shortened(P<0.05 or P<0.01). Conclusions The results of bedside sputum examination and sputum culture showed a good correlation,and the former is helpful in early diagnosis and treatment of VAP. The result of high quality sputum smear in significant in guiding the first choice of antibiotics,reduce the time of antibiotic use,shorten the duration of mechanical ventilation and the length of ICU stay,and improve the outcome of the patients.

10.Correlation Study on Storage Time and the Content of 5-Hydroxymethylfurfural in Corni Fructus

Objective To study the correlation between storage time and the content of 5-hydroxymethylfurfural (5-HMF) in Corni Fructus because of the color change caused by storage time. Methods Corni Fructus samples of different storage time with 0, 1, 2, 3, and 4 years were collected. The contents of 5-HMF were determined by HPLC. Results The HPLC determination method of 5-HMF in Corni Fructus was established. The contents of 5-HMF varied from undetected value to 0.292 8%, with the increase of storage time of 0, 1, 2, 3, and 4 years and the color gradually deepened from red, dark red, reddish brown to brown. The contents of 5-HMF in black wine-prepared Corni Fructus were 0.954 4%-1.837%. Conclusion The browning of Corni Fructus is related to the production of 5-HMF. With the extension of storage time of Corni Fructus, the color gradually deepens and the content of 5-HMF increase significantly. The storage time of Corni Fructus can be suggested to be one year.
