1.Retrospective study of chemoradiotherapy based on cisplatin compared with radiotherapy alone for cervical cancer

Objective To compare the treatment effects and side events between chemoradiotherapy and radiotherapy alone for patients with primary cervical cancer.Methods Totally 197 patients with primary cervical cancer in our hospital from 2000 to 2006 were studied.They were divided into two groups: radiotherapy alone(RT)or cisplatin-based chemoradiotherapy(CRT).Group of RT included 100 patients who received external irradiation by ~(60)Co and intracavitary irradiation by ~(192)Ir.Group of CRT included 97 patients who received cisplatin-based chemoradiotherapy.The RT regimen was the same as group RT.Results The local control(CR+PR)in two groups had no statistic difference(P=0.500).The total 5 year survival rate in the two groups were 82% and 79%,with no significant difference(P=0.177);however,there was a significant difference for stage Ⅲ squamous cell carcinoma of the cervix,the 5 year survival rate being 56% and 84%,respectively(P
2.Various methods of preparing acellular tissue-engineered xenogeneic valve stents

BACKGROUND: Excellent Iow-antigenicity xenogeneic biological valve scaffold is the premise of constructing tissue-engineered valve by using which kind of acellular methods.OBJECTIVE: To explore the optimal preparation method of making tissue engineered heart valves by meesuring efficiency of different acellular methods and ability to preserve the matrix.DESIGN, TIME AND SETTING: The prospective randomly controlled study was performed at the Central Laboratory of Taian Central Hospital from January 2007 to June 2008.MATERIALS: Sixteen specimens of porcine aortic valves were randomly divided into control, NaCI-sodium-dodecyl-sulfate (SDS),trypsin and triton-X100 groups.METHODS: Specimens in the control group were left intact. Three test groups were decelluladzed with NaCI, trypsin andTriton-X100 respectively.MAIN OUTCOME MEASURES: The gross structure, optical and electron microscope ultrastructure of the decelluarated porcineheart valve matrix was compared. The expression of vascular endothelial cell major histocompatibility complex (MHC)- Ⅰ antigenwas detected by immunohistochemical method.RESULTS: Treatment with NaCL-SDS achieved only incomplete decellularization. The main components of extracellular matdxwere reserved completely, but the fibrous components became unclear and swelling. Treatment with trypsin removed cellscompletely, but caused serious structural alterations, with the presence of swollen collagen fiber, crude edge, widen and irregularfiber interspace. Treatment with Triton-X100 achieved both complete decelluarization and preservation of the matrix structure.Valves following treatment of NaCI-SDS, trypsin and Triton-X100 had certain immunogenicity. However, the immunogenicity ofvalves following treatment of trypsin and Triton-X100 was significantly lower compared with the treatment of NaCL-SDS.CONCLUSION: The decellularization method by Triton-X100 is effective and complete. The Triton-X100 method does not changematrix structure and has low immunogenicity.
3.To effects of Puerarin Injection associated with Irbesartan on Cardiac Insufficiency at Decompensated Stage of chronic corpulmonale heart failure

ObjectiveTo explore the curative effects of Puerarin Injection associated with Irbesartan on decompensated chronic cor pulmonale heart failure. MethodsA total of 168 cases conforming to the diagnostic standards were randomized into control group (92 cases) and therapy group (76 cases). In the control group: Based on the results of sputum and X-ray, broad-spectrum antibiotics were firstly selected and used for 3 days after admission. Then sensitive antibiotics were selected according to the results of sputum culture and drug sensitivity test. Medicine for relieving cough and asthma, and apophlegmatisant were used routinely. Patients with heart failure were treated with Digoxin or diuresis. In addition to the above treatments, the therapeutic group were treated with Puerarin Injection 600mg and Irbesartan 75mg once a day. The two groups were both treated for 20 days. Results①The incipient slope, 0.8 slope, 0.5 slope and the minimal surplus were all significantly changed (P<0.01) in therapeutic group after the treatment. There were significant difference in the incipient slope, 0.8 slope, 0.5 slope and the minimal surplus before and after treatment both in the control group and therapeutic group. ②The plasma viscosity and blood viscosity decreased observably (P<0.01) after the treatment in the therapeutic group, and more obviously than that in the control group (P<0.01). ③The arterial oxygen pressure (PaO2) and arterial oxygen saturation (SaO2) in the therapeutic group were enhanced more prominently than that of the control group (P<0.01). ④The mean pulmonary arterial pressure (mPAP) and pulmonary vascular resistance (PVR) decreased obviously in the therapeutic group after the treatment, and more observably than those in the control group (P<0.01). ⑤The cardiac output and cardiac index were improved prominently in the therapeutic group after the treatment, and more obviously than that of the control group (P<0.01). ⑥The heart functional capacity (NYHA) improved significantly after the treatment both in the control group and the therapeutic group (P<0.05). ConclusionsPuerarin Injection and Irbesartan could remarkablely change the erytlu'ocyte deformabifity, improve the hemorheology, lower pulmonary hypertension, and improve the cardiac function and the clinical efficacy in decompensation stage.
4.Effect of protein kinase C on signal transduction in antigen activated mast cells

Objective: To investigate the effect of protein ki nase C on signal transduction such as tyrosine phosphorylation, c-fos and c-ju n mRNA expression in antigen activated mast cells. Methods: RBL-2H3 cells either untreated or treated with phorbol 12-myristate 13 -acetate (PMA) were sensitized with anti-DNP IgE, and activated with DNP-BSA, histamine release and tyrosine phosphorylation were quantitatively measured by ELISA and flow cytometry, respectively. The effect of PKC on the ex pression of c-fos and c-jun in serum-deprived RBL-2H3 cells activated by DNP-BSA detected by ethidium staining of PCR-amplified cDNA, the amplified cDNA products were subjected to Southern blot hybridization using specific prob es to determine the veracity of amplification. Results: Tyr osine phosphorylation and histamine release were significantly reduced from (4.4 7±0.03)% to (2.79±0.07)% and (104.47±1.31) nmol/L to (60.75±1.38) nm ol/L, respectively, 45 min after DNP-BSA stimulation in sensitized cells pre treated with PMA for 48 h. Bands of the size predicted for the amplified cDNA we re obtained: 299 bp for c-fos, and 651 bp for c-jun, a decrease of 91% and 82% , respectively, for c-fos and c-jun mRNAs was observed in antigen stimulated c ells pretreated with PMA for 48 h. Conclusion: PKC plays an impo rtant role in modulating the tyrosine phosphorylation and histamine release resp onses and may upregulate the expression of c-fos and c-jun in antigen activate d mast cell.
5.Inhibitory effect of RNA interference of MSTN gene expression on the downstream genes in Schizopygopsis pylzovi

Objective To investigate the silencing effect of RNA interference on MSTN gene ( myostatin, MSTN) expression, and detect the effects on the downstream genes in Schizopygopsis pylzovi. Methods To construct the recombi?nant adenovirus vector 1P3 (DSP MSTN 273+250+1737) and 1P2 (DSP MSTN 195+ 1670) for RNA interference of the MSTN gene in Schizopygopsis pylzovi, and to conduct the RNA interference in vivo experiment by injecting the vector in?to the muscle tissue of Schizopygopsis pylzovi. Real?time PCR and Western blotting were used to evaluate the silencing effects on MSTN gene expression, and to detect the regulatory function of M?CK at gene transcription level after RNA inter?ference of the MSTN gene. Results The result of real?time PCR showed that compared with the HK team ( Virus general negative control group) and N team (blank control group), the 1P3 had significant interference effect on the MSTN gene transcription in Schizopygopsis pylzovi (P<0?05), with an inhibition rate of 53?5%, but the 1P2 had no significant inter?ference effect on the MSTN gene transcription. The result of Western blotting was consistent with the results of real?time PCR. At the same time, after the 1P3 interference, the level of MSTN gene transcription was declined, and the level of M? CK gene expression was significantly increased. Conclusions Our results demonstrate that the expression of MSTN gene can be effectively suppressed, and the expression of M?CK gene can be up?regulated through the RNA interference. There?fore, it proves that MSTN gene can inhibit the transcription of M?CK gene in Schizopygopsis pylzovi, and reveals the regula?tory role of MSTN gene in the muscle growth and development in the plateau fish Schizopygopsis pylzovi.

Gastric cancer is one of the most common malignant tumors in China, and its pathogenesis is complex. At present, there is no good method for the diagnosis and treatment of gastric cancer. However, studies have shown that microRNA (miRNA) has abnormal expression in gastric cancer, which participates in the regulation of gastric cancer related genes, and has an impact on the occurrence, development, diagnosis and treatment of gastric cancer. This paper aims to review the relationship between miRNA and the diagnosis and treatment of gastric cancer, as well as the drug resistance in the treatment, so as to lay the foundation for the follow-up research and clinical diagnosis and treatment.

Adolescent idiopathic scoliosis (AIS) is a three-dimensional deformity of the spine,which not only affects spinal anatomy,mobility and trunk symmetry,but also leads to the changes of human locomotion through pelvic.During the past 30 years,though great progress has been made in the treatment of AIS,the pathogeny of AIS is still uncertain.Most scholars believe that many factors contribute to the pathogeny of AIS,however,some studies have shown that poor posture during walking and poor manage of balance could lead to the progress of scoliosis.Gait is a behaviour characteristic during walking,and normal gait is the result of nervous system,musculoskeletal system,proprioception and vision.Any disease of the system above may cause abnormal gait.Gait analysis can spot the key links and impact factors of abnormal gait by observing and measuring the testers' walking,reflect the abnormal gait objectively and quantitatively,and provide reference opinions for the recovery and treatment of patients.At present,gait analysis has been mostly applied in hemiplegia,cerebral palsy and knee osteoarthritis.In recent years,gait analysis has been increasingly used in AIS patients.There have been many researches about the kinematics and kinetics of gait analysis in adolsecent idiopathic scoliosis abroad and some positive results have been reported..But in China,it's still in the initial stage.We reviewed the published papers about gait analysis in AIS patients,and summarized the kinematics,kinetics and different means of intervention on gait in AIS patients.We hope it will provide references for our researches.
8.Estimation of the indoor external γ-dose rate from decorative stones

Objective To establish an accurate method for estimating the indoor gamma dose rate from decorative stones.Methods Combining a room model with decorating conditions,the gamma dose rates and dose rate conversion factors (DCF) at 1 m above the floor in the room center were calculated with the Monte Carlo simulations,and the calculation results were verified through experiments.Based on the results,the limit of radionuclide contents in stone materials was further discussed.Results The DCF increases with the increase of area or thickness of decorative stones in the same room.The increase of DCF with the thickness of decorative stone is approximately linear.The DCF also increases with the area of decorative stones,but the increasing trend slows down.For the same decorative stones,the smaller the room,the larger the increase of gamma dose rate.Experimental results were consistent with the simulation results within ± 20%.Conclusions The increase of indoor gamma dose rate depends not only on the radionuclide contents,but also on the area and thickness of the decorative stones as well as the room size.The method used in this study can be used to estimate,more accurately than ever,the additional external exposure to residents due to decorative stones,and it provides a theoretical basis for revising the limit standard on radionuclide contents in decorative materials.
9.Application of recombinant human growth hormone in improving difficult weaning

Objective To observe the applicated value of recombinant human growth hormone ( r-hGH) ther-apy in improving difficult weaning .Methods 40 patients with difficult weaning were randomly divided into the treat-ment group (20 cases) and control group (20 cases).The control group received routine ICU treatment .The treat-ment group was given the conventional treatment and subcutaneous injection of r -hGH 8IU,1 time a day,a total of 14 days.The serum total protein,albumin,oxygenation index,peak of blood glucose,independent tidal volume changes were observed before and after treatment , the success rate and mortality rate were compared .Results After treat-ment,the total protein,albumin,tidal volume of the treatment group were (68.1 ±13.4) g/L,(40.8 ±6.0) g/L, (432 ±57)mL,which were significantly higher than those before treatment [(52.3 ±12.5)g/L,(28.4 ±4.5)g/L, (278 ±54)mL](t=3.856,7.394,8.771,all P<0.05).While after treatment,the total protein,albumin,tidal vol-ume in the control group were (58.4 ±14.2)g/L,(31.0 ±5.8)g/L,(398 ±48)mL,compared with before treatment (53.2 ±13.3)g/L,(28.1 ±4.2)g/L,(271 ±49)mL,the differences were not statistically significant (all P>0.05).The success rate was 85% in the treatment group,which was higher than 50% in the control group(χ2 =4.103,P<0.05).In the treatment group,3 cases died,in the control group 9 cases died,there was statistically signif-icant difference between the two groups (χ2 =4.286,P<0.05).Conclusion r-hGH can improve the nutritional sta-tus of critically ill patients ,increase the strength of the respiratory muscles ,improve lung function ,help to improve the weaning rate .
10.Effectiveness of manual therapy and traction for lumbar disc herniation: a meta-analysis.

OBJECTIVETo evaluate the effectiveness of manual therapy and traction for lumbar disc herniation and analyze the current status of this kind of randomized clinical trial (RCT).
METHODSDatabase of CNKI, VIP, WANFANG, PubMed and OVID were searched. Some relevant journals were manually retrieved. A total of 2 874 literatures on manual therapy and traction for lumbar disc herniation were collected, of which 17 articles met the inclusion criteria. The Jadad score scale was used to evaluate the quality,and RevMan5.0 was used for meta-analysis of literatures.
RESULTSThe results of the meta-analysis of all trials involved were as followed:the combined effect of the effective rate was RR = 1.10, 95% CI [1.06, 1.14], the combined effect of the cure rate was RR = 1.36, 95% CI [1.21,1.52], the combined effect of the VAS was RR = 1.37, 95% CI [1.28, 1.45], the combined effect of the JOA was RR = 4.75, 95% CI [4.40, 5.09].
CONCLUSIONThe overall quality of the current RCT researches about manual therapy for lumbar disc herniation was lower,and did not support the conclusion that manual therapy was more effective than traction for lumbar disc herniation.
