2.Observations on the Treatment of 32 Cases of Insomnia by Manipulations of Digital-pressing Acupoints and Tuina along Meridians

Five kinds of manipulations of plucking, circular rubbing, pecking, pinching and patting were performed on the channels of Governor Vessel and bilateral Bladder Meridian. According to syndrome differentiation, all the 32 cases were divided into 5 syndromes of the stagnated liver-qi causing fire syndrome, the phlegm-heat attacking upper parts syndrome, the hyperactivity of fire due to yindeficiency syndrome, the deficiency of both heart and spleen syndrome and the timidity due to insufficiency of qi and deficiency of blood of the heart syndrome and were then treated by different manipulations. After 2 courses of treatment, 25 cases were cured, 7 cases got marked effectiveness,no case failed and the effective rate was 100%.
3.Isobaric tags for relative and absolute quantitation analysis on serum markers of pancreatic cancer based on mass spectrometry

Objective To determine the expression of serum proteins in pancreatic cancer patients based on mass spectrometry to screen differential proteins and to find potential molecular biomarkers to prediagnosis of pancreatic cancer. Methods The technique of isobaric tags for relative and absolute quantitation (iTRAQ) and two-dimensional liquid chromatography-tandem mass spectrometry (2D-LC-MS/MS) were used to analyze the serum proteins in 15 pancreatic cancer patients and 10 healthy controls. Screening for serum differential proteins was performed via retrieving Panther, Mascot and Scaffold databases. Ingenuity Pathway Analysis was used to detect the correlation between proteins. Results The expression level of 442 proteins was quantified, and 76 proteins were found to be differentially expressed which changed corresponding biological process. The up-regulation of DNA repair protein 50 (RAD50) and down-regulation of transforming growth factor β1 (TGFβ1) and apoptosis protease-activating factor 1 (APAF1) which were correlated with cell growth and apoptosis were found in pancreatic cancer. A closely interacted network was formed between these three differential proteins and other molecules, which could effect metabolism in pancreatic cancer patients. Conclusion Differential expression of serum proteins screened by iTRAQ in pancreatic cancer patients may be the potential markers of pancreatic cancer, which can provide molecular basis for early diagnosis of pancreatic cancer.
4.Safety of dexmedetomidine-or remifentanil-based topical anesthesia for awake intubation: a meta-analysis

Objective To compare the safety of dexmedetomidine-versus remifentanil-based topical anesthesia for awake intubation.Methods PubMed,EMBASE,Web of Science,Cochrane Library,Chinese Biomedical Database,CNKI and Wanfang Database were searched from January 1990 to April 2016 for randomized controlled trials involving the safety of dexmedetomidine-and remifentanil-based topical anesthesia for awake intubation.The following data were extracted:the rate of successful intubation at first attempt and development of hypertension,hypotension,bradycardia,tachycardia,respiratory depression,bucking,body movement,sore throat,hoarseness and postsurgical memory of intubation.Meta-analysis was conducted using the RevMan 5.3 software.Results Ten studies involving 528 patients were included in this meta-analysis,and there were 262 cases in dexmedetonidine group and 266 cases in remifentanil group.Compared with remifentanil group,the incidence of respiratory depression and postsurgical memory of intubation were significantly decreased (P<0.05),and no significant change was found in the rate of successful intubation at first attempt and incidence of hypertension,hypotension,bradycardia,tachycardia,bucking,body movement,sore throat and hoarseness in dexmedetomidine group (P>0.05).Conclusion Dexmedetomidine-based topical anesthesia provides higher safety than remifentanil-based topical anesthesia when used for awake intubation.
5.Discussion on Clean Area Environmental Monitoring Standards of PIVAS in Hospital

OBJECTIVE:To improve clean area environmental monitoring standards of Pharmacy intravenous admixture service (PIVAS)in hospital. METHODS:Referring to related national standards and specifications,combined with the work practice,the existing problems of environmental monitoring requirements in Quality Management Standard for Pharmacy Intravenous Admixture to clean area in PIVAS were explored. RESULTS:There was no provision on clean area environmental monitoring standards in Quality Management Standard for Pharmacy Intravenous Admixture;there was no specific numerical value of pressure difference, illumination and wind speed;there was no rule on airborne particles;there was no clear provision on settling microbe and airborne microbe;there was also no rules on the frequence of wind speed and airborne particles monitoring. CONCLUSIONS:Clean area environmental monitoring standards should be separately listed in Quality Management Standard for Pharmacy Intravenous Admix-ture,and clear provisions are given on detection method,monitoring project,judging standard and monitoring frequency,so that pharmacists are easy to operate and carry out.
6.Treatment of 30 Cases of Prolapse of Lumbar Intervertebral Disc by Electroacupuncture plus Tuina

Purpose: To observe the clinical therapeutic effect of electro acupuncture plus Tuina on treating prolapse of lumbar intervertebral disc. Methods: Electro -acupuncture at Jiaji(Ex-B 2) acupoints plus reduction manipulations were used to treat 30 cases of prolapse of lumbar intervertebral disc. Results: After 2 courses of treatment, among the 30 cases, 8 cases were cured and 22cases got improvements, and the effective rate was 100%.Conclusion: Electroacupuncture plus reduction manipula -tions had a satisfactory effect on treating this disease;and it can significantly improve the clinical symptoms of the patients.
7.Clinical Observation of Acupuncture Mainly on Jiaji(EX-B2) Acupoints in Treating Lumbodorsal Myofascial Pain Syndrome

8.Eosinophil could predict the prognosis of patients with bloodstream infection: a retrospective analysis of 305 cases

Objective To investigate the value of peripheral blood for the prognosis of patients withbloodstream infection. Methods A retrospective analysis of patients with bloodstream infection was conducted inthe intensive care unit (ICU) of Mianyang Central Hospital of Sichuan from January 2012 to October 2016. Accordingto the 28-day survival, the patients were divided into survival group and death group. The white blood cell (WBC),neutrophils count (NEU), lymphocyte count (LYM), neutrophil/lymphocyte ratio (NLR), monocyte count (MO), eosinophilcount (EO), basophil count (BA), hemoglobin (Hb), platelet count (PLT) and procalcitonin (PCT) in peripheral bloodwere recorded when patients were diagnosed with blood infection. Receiver operating characteristic curve (ROC),Kaplan-Meier survival analysis and Cox regression were used to evaluate the value of these risk factors for predictingthe outcome. Results 305 patients were enrolled. 182 patients survived while 123 patients died during the 28-dayperiod. ① There was no significant difference in gender, age and comorbidities between the two groups. There was nosignificant difference in infection rate between the two groups except for fungal infection rate. The fungal infection ratein the death group was significantly higher than that in the survival group (9.8% vs. 3.3%, P = 0.019). ② The LYM,MO, EO and PLT in the death group were significantly lower than those in the survival group [LYM (×109/L):0.58 (0.29, 0.93) vs. 0.76 (0.44, 1.23), MO (×109/L): 0.47 (0.19, 0.80) vs. 0.58 (0.30, 0.94), EO (×109/L):0.00 (0.00, 0.01) vs. 0.03 (0.01, 0.09), PLT (×1012/L): 89 (47, 148) vs. 126 (82, 186), all P < 0.05]. The NLR in the death group was significantly higher than that in the survival group [17.09 (7.60, 33.51) vs. 12.86 (6.51, 24.85), P < 0.05]. There was no significant difference in the WBC, NEU, BA, Hb and PCT between the two groups. ③ It was shown by ROC curve analysis that the maximum area under the ROC curve (AUC) of EO was 0.755. When the best cut-off value was 0.015×109/L as a predictor of death in 28 days, the sensitivity was 80.3%, and specificity was 64.7%. ④ It was shown by survival analysis that the 28-day survival rate in the patients with EO < 0.015×109/L was significantly lower than that of patients with EO > 0.015×109/L [38.3% (62/162) vs. 83.9% (120/143), χ 2 = 56.999, P = 0.000]. ⑤ It was shown by Cox regression that EO was the independent factor for 28-day survival (β = 1.466, χ 2 = 39.535, P = 0.000). Risk of death was 4.331 times greater in patients with EO < 0.015×109/L than in those with EO > 0.015×109/L [hazard ratio (HR) = 4.331, 95% confidence interval (95%CI) = 2.743-6.840]. Conclusions Compared to other parameters in peripheral blood, EO has the best correlation with the prognosis of bloodstream infection. EO is the independent prognostic predictor for 28-day survival.
9. Protective effect of α-tocopherol succinate against early hematopoietic injury in mice with acute radiation sickness

Objective To explore the protective effect of α-tocopherol succinate (α-TOS) against early hematopoietic injury in mice with acute radiation sickness. Methods Male C57BL/6J mice 6-8 weeks old were randomly divided into normal group, irradiation group and α-TOS group (n=10), 400mg/kg of α-TOS was subcutaneous injected into the mice of α-TOS group 24 hours before irradiation, while vehicle was given to the mice of irradiation group. The 30-day survival rate of mice having received 9.0Gy irradiation, and the mean survival time of non-survivors were recorded. Analysis of peripheral blood was performed in mice receiving 6.5Gy irradiation at days 1, 4, 7, 10, 14, 18, 22, 31 and 45, while the number of bone marrow colony-forming cells (CFCs) and hematopoietic stem/progenitor cells were counted at 2-hour and 24-hour after irradiation. Bone marrow cells collected from 6.5Gy irradiated mice and EGFP transgenic mice were mixed in a ratio of 10:1, and then transplanted into lethal dose-irradiated mice, and the proportion of EGFP positive cells in the peripheral blood was recorded after 35 days. Results α-TOS raised the survival rate and mean survival time of mice suffering from 9.0Gy irradiation, improved the recovery of the peripheral blood picture of mice suffering from 6.5Gy irradiation, and increased the number of bone marrow colony-forming cells and hematopoietic stem/progenitor cells in the mice 24 hours after 6.5Gy irradiation. The competitive transplantation experiments showed that α-TOS markedly diminished the proportion of EGFP positive cells in the peripheral blood in lethal dose-irradiated mice at day 35. Conclusion α-TOS could effectively protect the hematopoietic stem/progenitor cells from radiation injury in γ-ray irradiated mice, promote the recovery of peripheral blood cells, and improve the survival rate.
10.Evaluation of predictive performance of propofol target-controlled infusion system incorporating the Schnider pharmacokinetic parameters

Objective To evaluate the predictive performance of propofol target-controlled infusion (TCI) system incorporating the Schnider pharmacokinetic parameters in Chinese patients. Methods Forty ASA Ⅰ or Ⅱ patients, aged 25-45 yr, with body mass index 20-25 kg/m2 , scheduled for gynecological laparoscopic surgery un der general anesthesia, were enrolled in this study. Anesthesia was induced with TCI of propofol (target plasma concentration (Cp) 3 μg/ml) and remifentanil (Cp 4 ng/ml) . Propofol was infused by Orchestra TCI system incorporating the Schnider pharmacokinetic parameters. Tracheal intubation was facilitated with rocuronium 0.6 mg/kgafter the patients lost consciousness. The patients were mechanically ventilated. PETCO2 was maintained at 30-40 mm Hg. Anesthesia was maintained with TCI of remifentanil (Cp 4 ng/ml) and propofol (Cp 3-5 μg/ml) and intermittent iv boluses of atracurium 0.2 mg/kg. BIS value was maintained at 40-45. Venous blood samples were obtained at 15, 30, 45 and 60 min after pneumoperitoneum for measurement of blood propofol concentrations by high performance liquid chromatography with fluorescence detector. Performance error, median prediction performance error, median absolute performance error, wobble and divergence of propofol TCI system were calculated. Results The value for performance error was 21 % (13%), for median prediction performance error 6.7 % (37.4%),for median absolute performance error 19% (18%), for divergence - 0.65%/h (0.82%/h) and for wobble 16.3% (15.2% ) . Conclusion The accuracy of propofol TCI system incorporating the Schnider pharmacokinetic parameters is high in Chinese patients and its predictive performance is acceptable clinically.
