1.Sertraline plus Cognitive Behavior Therapy in treatment of Panic Disorder

Objective:To study the effect of sertraline plus cognitive behavior therapy in treatment of panic disorder Method:14 outpatients with panic disorder according to crteria of CCMD-2-R were collected as our sample All of them were given sertraline 50 mg/d for 6 months in addition to cognitive behavior therapy The clinical effect was assessed at the end of 1 month,3 months and 6 months Results:At the end of 1 month,3 patients were assessed as markedly improved,9 as improved At the time of 3 months,10 patients were assessed as markedly improved,2 as improved At the end of 6 months,12 markedly improved,2 improved There were 3 patients had mild side effects,which faded with time Conclusion:Sertraline plus cognitive behavior therapy has good effect in treatment of panic disorder with good compliance and little side effect
2.Clinical analysis of 388 cases of atlanto-axial instability

Objective To study the clinical characteristics of atlanto-axial dislocation and instability caused by different reasons. Methods 388 cases of atlanto-axial dislocation and instability admitted between Jan 1975 and Apr 2000 were reviewed retrospectively. Results 262 cases were caused by anomaly, 71 by trauma and 55 by other reasons. 238/262 had bony deformity, including anomaly of odontoid process, atlanto-occipital assimilation, skull basilar invagination and some others. Simple anomaly of odontoid was the most commonly seen etiology. Myelopathy occurred at similar morbidity among each kind of patients. But patients who presented symptoms for longer time were more likely to have myelopathy. It is difficult to reduce chronic fractures of the odontoid process compared with the fresh fractures. 19 cases with chronic fracture suffered from severe myelopathy. Patients with chronic atlanto-axial fracture over 1 year were more likely to have secondary damage of spinal cord than those within 1 year. Conclusion Atlanto-axial dislocation and instability is more commonly caused by anomaly than trauma. Once the patients with atlanto-axial anomaly present clinical signs, they should be treated as soon as possible. Traumatic atlanto-axial instability should be treated at the early stage to avoid myelopathy.
3.Isobaric tags for relative and absolute quantitation analysis on serum markers of pancreatic cancer based on mass spectrometry

Objective To determine the expression of serum proteins in pancreatic cancer patients based on mass spectrometry to screen differential proteins and to find potential molecular biomarkers to prediagnosis of pancreatic cancer. Methods The technique of isobaric tags for relative and absolute quantitation (iTRAQ) and two-dimensional liquid chromatography-tandem mass spectrometry (2D-LC-MS/MS) were used to analyze the serum proteins in 15 pancreatic cancer patients and 10 healthy controls. Screening for serum differential proteins was performed via retrieving Panther, Mascot and Scaffold databases. Ingenuity Pathway Analysis was used to detect the correlation between proteins. Results The expression level of 442 proteins was quantified, and 76 proteins were found to be differentially expressed which changed corresponding biological process. The up-regulation of DNA repair protein 50 (RAD50) and down-regulation of transforming growth factor β1 (TGFβ1) and apoptosis protease-activating factor 1 (APAF1) which were correlated with cell growth and apoptosis were found in pancreatic cancer. A closely interacted network was formed between these three differential proteins and other molecules, which could effect metabolism in pancreatic cancer patients. Conclusion Differential expression of serum proteins screened by iTRAQ in pancreatic cancer patients may be the potential markers of pancreatic cancer, which can provide molecular basis for early diagnosis of pancreatic cancer.
4.Effects of HAVCR2 gene silence on apoptosis and cell cycle in the radiation-caused genomic instable liver cells

Objective To explore the effects of HAVCR2 siRNA on apoptosis and cell cycle in the radiation-caused genomic instable liver cells.Methods RNAi was used to inhibit HAVCR2 gene transcription.Cellular apoptosis and cell cycle distribution were detected by flow cytometry (FCM).Expression of p53 gene was assayed by real time fluorescence quantitative PCR.Results HAVCR2 gene was effectively inhibited by RNAi (t =19.21,P < 0.05).After siRNA transferring,cell apoptosis (t =3.65,P < 0.05) and p53 gene expression (t =4.82,P < 0.05) were decreased,and G2-phase arrest was induced(t =-3.41,P < 0.05).Conclusion HAVCR2 siRNA can decrease the generation of apoptosis in the genomic instable liver cells and blocks the cells at G2 phase.
5.The attentional networks in patients with temporal lobe epilepsy

Objective To investigate the attentional network in patients with temporal lobe epilepsy(TLE) and the disease-related factors. Methods Fity-four patients with TLE and 40 controls were enrolled in our hospital from January 2007 to December 2008.The computerized ANT software was used for evaluating the attentional network efficiency and the clinical date of the patients with TLE were recorded. Logistic regression was used to identify the risk factors of attentional network efficiency. Results The mean reaction time(TLE:688.2±138.1 ms;Control:625.1±100.1 ms, t=2.06, P <0.05)and executive control network efficiency (TLE:155.7±57.0 ms;Control:108.0±33.8 ms, t=4.62, P <0.01) of the TLE group were lower than the healthy control group. The efficiency of alerting network and orienting network between the two groups were no significant difference. Logistic regression analysis showed that the positive epileptiform activity was an independent risk factor of the attentional impairment(95% CI : 1.03~42.33, OR =6.603, P =0.043). Conclusions The ANT demonstrate that patients with TLE may accompany with attentional executive control network efficiency impairment. Epileptiform discharge may cause attentional executive network efficiency impairment.
6.Study on serum proteomics in rats after accumulated irradiation with 137Cs γ-rays

Objective To investigate the changes of proteomics in serum of Sprague-Dawley(SD) rats after accumulated irradiation with 137Cs γ-rays.Methods A total of thirty mature SD rats were randomly divided into three groups:0.2 Gy group,2 Gy group and healthy control group.Rats were irradiated at a dose rate of 0.336 mGy/min for 10 d and 20 d continuously.Isobaric tags for relative and absolute quantitation (iTRAQ) was used to analyze the different protein expression in serum of irradiated rats.Gene Ontology,KEGG pathway and protein-protein interaction network analysis were conducted using softwares.Results In total,363 protein spots were identified.Twenty nine proteins were differentially expressed in both groups compared with control,of which 10 proteins were up-regulated and 19 proteins were down-regulated.Based on the information of GO categories,these differentially expressed proteins were mainly located in the cytoplasm and membrane concerning the function of binding and catalytic activity.Analysis with the PAJEK software demonstrated that 16 differentially expressed proteins could form a complicated interaction network where glutathione S-transferase P1 (GSTP1),phosphoglycerate kinase 1 (PGK1) and protein disulfide-isomerase (PDI) might be key nodes.Conclusions Accumulated irradiation can induce differentially expressed proteins in serum of irradiated rats.Analysis on functional roles of the screened proteins GSTP1,PGK1 and PDI may provide insight into further mechanistic investigations and underlying molecular biomarkers induced by accumulated irradiation.

Objective To find direct effect of Chinese herbs Bu Gu Zhi (Psoralea corylifolia L) and Bai Zhi (Dahurian angelica root) Extracts on melanocyte adhesion and migration in vitro. Methods Ethanol extracts obtained from two kind of Chinese medicinable herbs were tested. Human melanocytes were obtained from neonatal foreskins and 48-well culture dish covered with fibronectin were used to perform melanocyte adhesion assay; Motility was assessed using the micropore filter method. Results: The extracts of Bu Gu Zhi(Psoralea corylifolia L), Bai Zhi(Dahurian angelica root) obviously showed an effect in increasing of human melanocyte adhesion and migration on fibronectin. Conclusion It is suggested that Buguzhi(Fructus Psoraleae) and Baizhi(Radix Angelicae Dahuricae) might induce melanocyte adhesion and/or migration in the treatment of vitiligo.

9.Analysis of veracity of the C1 lateral mass screw insertion in the atlantoaxial fixation.

OBJECTIVETo evaluate the veracity of the C1 lateral mass screw insertion in the atlantoaxial fixation using plate and screw method without A-P fluoroscopic guiding.
METHODSIn the atlantoaxial fixation, without A-P fluoroscopic guiding, we probe lateral mass edge and identify the entrance point of C1 lateral mass screw indirectly. 159 patients treated with atlantoaxial fixation using plate and screw method were studied. Postoperative CT was used for analysis, and the position of the lateral mass screws in atlas was identified by coronary and axial CT scan. Three areas were delimited in and around the lateral mass of atlas in the axial CT scan, so as to analyze the location of screws: area A (inside the joint face), area B (outside the joint face but still in lateral mass), and area C (outside the lateral mass).
RESULTSAmong the 318 screws, 308 (96.9%) were located in area A, 5 in area B (1.6%), and 5 in area C (1.6%). All cases got atlantoaxial union at 4 months after operation.
CONCLUSIONProbing lateral mass edge and identifying the lateral mass indirectly is reliable for identifying the path of screw in atlas.

10.Posterior occipitocervical fixation using C2 pedicle screws and occipitocervical plate systems.

OBJECTIVETo observe the clinical effect of a occipitocervical fixation systems using C(2) pedicle screws and plates.
METHODSAn occipitocervical fixation system was designed. Since June 2001 to March 2003, 38 patients with instability of atlantoaxial joint underwent reconstructive surgery using this systems. Twenty-four patients were associated with congenital occipitalization. The pedicle screws were inserted into C(2) pedicles in the direction as its axis. The occipitocervical plate was slightly bent to fit the occipital contour and fixed onto the occiput. Hyperflexion alignment of the occipitoatlantoaxial complex was corrected by application of extensional force created by tightening of the nut on the pedicle screws. The autogenous cancellous bones were grafted between the occiput and the axis.
RESULTSIn this series, neither vertebral artery nor spinal cord was injured. 36 of 38 cases were followed up for an average of 18 months, all cases achieved solid bony fusion. No implant failure was found.
CONCLUSIONSOccipitocervical reconstruction by the combination of C(2) pedicle screws and occipitocervical plate systems can provide sufficient correction of malalignment in the occipitoatlantoaxial region and achieve high fusion rate.
