2.General considerations on adaptive designs for clinical trail and drug evaluation

Adaptive design is a trial design that allows modifications to some aspects of the trial after its initiation without undermining the validity and integrity of the trial. Adaptive design makes it possible to discover and rectify inappropriate assumptions in trial designs, reduce development costs and the time to market. It has been very attractive to the pharmaceutical industries. In this paper, it is not focused on adaptive designs for clinical trials with multiple endpoints studied using computer simulations, but is to clarify our evolution, which is taken into consideration in a trial adaptive design at present, if need modifications.
3.Discussion on TCM strategies and skills of the treatment of refractory jaundice

Based on clinical features of refractory diseases and clinical practice experience for many years,the author expounded the therapeutic strategies from establishing individualized system of differentiation treatment,treatment strategies of refractory diseases,and the key for medication,and emphasized individual concerned therapy,separating primary and secondary treatment and grasping the physiological state to treat pathological changes.Thus complicated and changeable diseases can be treated clinically.
4.A Clinical Study of Electroacupuncture plus Exercise for Rehabilitation After Surgery for Olecranon Comminuted Fracture

Objective To investigate the effect of electroacupuncture plus functional exercise in patients with olecranon comminuted fracture.Method Seventy-sevenpatients with olecranon comminuted fracture were enrolled as the subjects and allocated, using a random numbertable, to a control group (38cases) and a treatment group (39 cases).The control group received routine rehabilitation such as postoperative functional exercise and the treatment group, electroacupuncture in addition.Inthe two groups of patients, the pain score was recorded at one,sevenand fourteen days after the surgery; elbow joint activity was examined at two days and two months after the surgery; the elbow function score was recorded at six months after the surgery.Result There was no statistically significant difference in the pain score between the twogroupsat one day after the surgery (P>0.05).Thepain score was lower in the treatment group than in the control group atsevenand fourteen days after the surgery (P<0.05).There was no statistically significant difference in elbow jointactivity between the two groupsat two days after the surgery (P>0.05). Elbow joint activity was larger in the treatment group than in the control group at two months after the surgery (P<0.05).The elbow HSS score was higher in the treatment group than in the control group at six months after the surgery (P<0.05).Conclusion Electroacupuncture plus functional exercise can effectively decrease the pain score and increase elbow joint activity and the elbow function score in patients with olecranon comminuted fracture.It has greater value in clinical application.
5.Content Determination of Spironolactone Tablets by HPLC

OBJECTIVE: To establish the HPLC method for the determination of spironolactone tablets. METHODS: The determination was performed on Agilent C18 column with mobile phase of acetonitrile-water (50 ∶ 50) and detection wavelength of 238 nm. RESULTS: The linear range of spironolactone were within 5.023 1 ?g?mL-1~75.347 1 ?g?mL-1 (r=1.000 0) with an average recovery of 100.23% (RSD=0.53%). CONCLUSION: The method is reproducible, simple and convenient for the quality control of spironolactone tablets.
6.Treatment for the relapsed or refractory severe aplastic anaemia

Severe aplastic anaemia is a marrow failure syndrome characterized by pancytopenia and hypocellular bone marrow.Immunosuppressive therapy(IST)consisting of antithymocyte or antilymphocyte globulin (ATG) and cyclosporine has been recommended for children lacking HLA-matched sibling donor.For children unresponsive to initial immunosuppressive therapy or those who relapsed after first immunosuppressive therapy,they can choose repeat course of immunosuppressive therapy,allogeneic hernatopoietic stem cell transplantation,new drugs or clinical trials.And the prognosis has been greatly improved in recent years,dueing to the development of allogeneic hematopoietic stem cell transplantation and the application of new drugs such as eltrombopag.This article reviews researches advance in different therapies for children with relapsed or refractory severe aplastic anaemia,aimed at guaiding treatment methods.
7.Protective roles of fibroblast growth factor-21 on cardiovascular diseases

Fibroblast growth factor 21 (FGF21) is regarded as an important metabolic regulator playing a therapeutic role in diabetes and its complications.Recently, growing evidence showed that heart is a key target as well as a source of FGF21 which is involved in heart development and also induces beneficial effects in cardiovascular diseases.Strong evidences showed that the development of cardiovascular diseases including atherosclerosis, atherosclerosis, coronary heart disease, myocardial ischemia, cardiac hypertrophy, and diabetic cardiomyopathy is associated with increased serum FGF21 levels which was regarded as a compensatory response to induced cardiac protection.This review tries to illuminate the underlying relationship between FGF21 and cardiovascular diseases and the possible mechanisms.
8.Determination of Related Substance in Indometacin Enteric-coated Tablet by Improving HPLC

OBJECTIVE:To improve HPLC for the contents determination of 2-methyl-5-methoxy-indole-3-acetic acid(impuri-ty Ⅰ) and 4-chlorobenzoic (impurity Ⅱ) in Indometacin enteric-coated tablet. METHODS:The column was Phenomenex Phe-nyl-Hexyl with mobile phase of 10 g/L acetic acid solution- acetonitrile(gradient elution)at a flow rate of 1.0 ml/min,the detec-tion wavelength was 254 nm,column temperature was 40℃,injection volume was 20 μl. RESULTS:The linear range was 0.262 2-5.243 0 μg/ml for impurity Ⅰ(r=0.999 8)and 0.252 5-5.050 0 μg/ml for impurity Ⅱ(r=0.999 8);the limits of quanti-tation were 1.12 ng and 0.48 ng,limits of detection were 0.340 ng and 0.146 ng,respectively;RSDs of precision,stability and re-producibility tests were lower than 2%;recoveries were 99.71%-100.52%(RSD=0.28%,n=9) and 100.84%-102.14%(RSD=0.47%,n=9). CONCLUSIONS:The method is accurate and rapid,and can be used for the contents determination of impurity Ⅰand impurityⅡin Indometacin enteric-coated tablet.

10.Treatment of fracture of elbow joint with internal fixation

Objective To describe the methods of treatment of fresh and old fractures of the elbow joint with internal fixation.Methods Ten cases of fracture of the elbow with complete records were reviewed; 9 of them were male and 1 female; 6 of them were Han people and 4 Tibetans. The average age was 31 years. Four cases had open fractures and 6 had closed fractures. Among the cases of open fractures, 3 were treated with routine debridement and internal fixation of the fragments or internal fixation plus advanced skin flaps. One was treated as closed fractures after wound healing. The 6 cases of closed fractures were treated with a single tension band wiring, tension band and screw fixation, or screw fixation. Results The mean duration of the follow up was 8 months. Fracture healing was seen at 3- 4 months. The range of motion of fresh fractures of the elbow and forearm was 4.2° - 144.2° . The old fracture cases had the range of motion of 31.3° - 115° without restriction in pronation and supination. Conclusion The intra articular fracture of elbow joint must be treated with early surgery to attain anatomical reduction with firm internal fixation. Early functional exercise is also important for the recovery.
