Objective To investigate the effect of different loading doses of atorvastatin in patients with stable plaques.Methods Consecutive 174 patients with stable plaque who underwent coronary arteriongraphy (CAG) and intravascular unltrasound(IVUS) were randomly assigned to receive 10 mg atorvastatin treatment (group 10 mg,n =47),20 mg atorvastatin treatment(group 20 mag,n =45),40 mg atorvastatin treatment (group 40 mg,n =43) and 80 mg atorvastatin treatment (group 80 mg,n =39).The endpoints including low density lipoprotein-cholesterol(LDL-C),high density lipoprotein-cholesterol (HDL-C),high-sensitivity C-reactive protein (hs-CRP) levels,necrotic and plaque volumes,were assessed after 3-6 months' follow-up.Results Mean LDL-C,HDL-C,hs-CRP,the percentage of necrotic,plaques volumes were similar at baseline (P ＞ 0.05).During 3-6 months of follow up:(1)LDL-C levels in group 10 mg,20 mg,40 mg,80 mg were lower than at baseline (t =3.12,4.23,3.26 and 5.21 respectively,P ＜ 0.01).There was significant difference between the 20 mg group and the 40 mg group(P ＜ 0.05) ; (2) There was no significant difference of HDL-C levels in 10 mg,20 mg and 40 mg groups after atorvastatin treatment.However,its level was significantly higher in the 80 mg group after atorvastatin treatment than other dose groups(P ＜ 0.05) and were higher than at baseline(t =2.35,P ＜ 0.01) ;(3)the 80 mg group's hs-CRP levels decreased significantly after treatment than at baseline((3.59 + 1.07)mg/L vs (6.10 + 2.12) mg/L,t =2.37,P ＜ 0.01);(4)According to the VH of IVUS,the percentage of necrotic in 10 mg group became higher than at baseline ((16.54 + 1.76) ％ vs.(7.83 + 1.03) ％,t =2.38,P ＜0.01) and conformed to unstable plaques diagnostic criteria(＞ 10％).There was no significant difference in group 20 mg,40 mg and 80 mg with at baseline (t =1.24,0.21,0.69 respectively,P =0.069,0.846,0.643respectively) ; (5)Plaques volumes in group 10 mg,20 mg were not larger than at baseline.However,in group 40mg and 80 mg.Plaques volumes were smaller than at baseline ((30.69 ± 8.12) mm3 vs (37.09 + 12.01)mm3,t=l.29,P=0.019;(24.99±l.01) mm3 vs (36.47+14.68) mm3,t =2.62,P＜0.01).Conclusion The effects of atorvastatin on stable plaques vary with doses.(1) For LDL-C,the use of atorvastation 20 mg/d can make LDL-C reaching standard,and atorvastation 40 mg/d was superior to 20 mg/d and similar to 80 mg/d.(2)For HDL-C,atorvastation 80 mg/d can make it higher.(3)Atorvastation 80 mg/d can make hs-CRP lower.(4)Atorvastation≥20 mg/d can make plaques stable and atorvastation 80 mg/d was superior to 20 mg/d and 40mg/d.Atorvastation 40-80 mg/d can make plaques dwindled.

Objective To investigate the association between intima-media thickness (IMT) and free fatty acids (FFA) in hypertensive patients with metabolic syndrome (MS). Methods IMT, FFA and body mass index (BMI) were measured in 69 hypertensive patients with metabolic syndrome (MS group) ,57 hypertensive patients without metabolic syndrome ( Non-MS group) ,and 50 healthy controls ( NC group). Results In the MS group,The BMI, serum levels of triglyceride (TG), fasting blood glucose (FPG) and FFA were ( 26.0 ± 2.1 ) kg/m2, (24.0 ±0.81 ) mmol/L and (5.64 ± 0.82) mmol/L respectively, which were significantly higher than those in the Non-MS group ( (24.9 ± 2.3 ) kg/m2, ( 1.93 ± 0. 55 ) mmol/L and ( 5.10 ± 1.08 ) mmol/L respectively ) and the NC group ( (23.6 ± 1.6), ( 1.49 ± 0.36) mmol/L and (4.70 ± 0.90) mmol/L respectively) ( F = 20.06, 30.96 and 15.17,P ＜0.05 or 0.01 ). FFA in the MS group ((562.11 ± 55.12)μmol/L) were significantly higher than that in the Non-MS group (402.65 ± 49.53 ) μmol/L) and the NC group ( ( 356.23 ± 45.93 ) μmol/L) ( F = 277.28, P ＜0. 01 ). IMT in the MS group, Non-MS group and NC group were (1. 10 ± 0. 13 )mm, (0. 82 ± 0. 12 )mm and (0.70 ± 0.11 ) mm, respectively, with significantly difference ( P ＜ 0. 01 ). In addition, the incidence of thickening was 28.99% (20/69), 17.54% ( 10/57 ) and 2.00% ( 1/50 ), respectively, with significantly difference ( P ＜0.01 ). Conclusions Serum level of FFA is associated with IMT in hypertensive patients with metabolic syndrome.

Objective To observe the clinical efficacy of Shiwei Longdanhua Capsule combined with western medicine on patients with mild acute exacerbation of chronic obstructive pulmonary disease (COPD) with phlegm-heat obstructing lung syndrome. Methods Totally 192 cases of COPD with mild acute exacerbation (phlegm-heat obstructing lung syndrome) were recruited. They were randomly divided into the treatment group (96 cases) and the control group (96 cases). Patients in the control group were treated in line with the regular Western treatment, while patients in the treatment group were additionally treated by Shiwei Longdanhua Capsule. After 10-day treatment, the changes of TCM syndrome score, symptom integral and the lung function before and after treatment were compared. Results After treatment, 14 patients withdrew from the treatment group and 15 patients withdrew from the control group. The total efficiency rate of TCM syndrome was 80.49% (66/82) in the treatment group, while it was 56.79% (46/81) in the control group, with statistical difference between the two groups (P<0.05). The severity and frequency of cough and expectoration of the treatment group were better than the control group (P<0.05). The breathing symptom and pulmonary function of patients in the two groups improved, with similar efficacy. Conclusion The efficiency of Shiwei Longdanhua Capsule combined with Western medicine in the treatment of acute exacerbation of COPD is superior to that of simple Western medicine treatment, especially in terms of relieving cough and expectoration.

OBJECTIVETo investigate the differentiation induction effect of 2-methoxyestradiol (2ME2), an estrogen derivative on myeloma cell line CZ-1.

METHODSThe changes of CZ-1 cells in morphology, expression of surface CD49e and quantity of light chain secretion in the supernatant were observed when treated with 0.1 approximately 0.5 micromol/L 2ME2 for 48 h.

RESULTS2ME2 could induce differentiation of CZ-1 cells. The cells appeared decreased in size of nucleus, increased in cytoplasma, decreased in the ratio of nucleus to plasma, decreased in number or disappearance of nucleolus, and thickness and pyknosis of chromatin. The expression of CD49e was increased from (12.20 +/- 1.57)% to (24.80 +/- 1.26)% (P < 0.05). Light chain secretion in the supernatant was increased from (35.97 +/- 2.60) microg/ml to (79.67 +/- 1.88) microg/ml (P < 0.05).

CONCLUSIONLow concentrations of 2ME2 could induce differentiation of myeloma cell line CZ-1.

The objective was to explore the in vitro effects of growth inhibition and apoptosis induction of 2-methoxyestradiol (2ME2), an estrogen derivative, on seven myeloma cell lines NCI-H929, HS-sultan, KM3, SKO-007, CZ-1, U266 and LP-1and to observe its synergistic effects in combination with some other drugs, such as dexamethasone, As(2)O(3), thalidomide and zoledronic acid. Seven myeloma cell lines NCI-H929, HS-sultan, KM3, SKO-007, CZ-1, U266 and LP-1 were cultured at different concentrations with or without dexamethasone, As(2)O(3), thalidomide and zoledronic acid. Cell viability was assessed by trypan blue assay, plasma cell labeling index (PCLI) was detected by BrdU assay, terminal-deoxynucleotidyl transferase mediated dUTP nick end labeling (TUNEL) assay were used to determine apoptosis cells in situ. Synergistic effects of 2ME2 in combination with other drugs were judged by King's formula. The results showed that after treatment with 1, 4, 8, 12, 16 micromol/L 2ME2 at 12, 24, 36 and 48 hours respectively, 2ME2 caused a dose- and time-dependent inhibition of the cell viability. The concentration of 50% growth inhibition (IC(50)) was between (20.8 +/- 0.27) and (34.1 +/- 0.57) micromol/L. After treatment with 12 micromol/L 2ME2 within 24 hours, 2ME2 led to a progressive decline in the fraction of S-phase cells by BrdU assay, plasma cell labeling index (PCLI) declined from (30.14 +/- 4.28)% to (14.71 +/- 6.27)% (P < 0.05). After treatment with 1, 4, 8, 12, 16 micromol/L 2ME2 at 12, 24, 36 and 48 hours respectively, 2ME2 can induce a dose- and time-dependent apoptosis of myeloma cell lines. The percentage of apoptosis was between 9% - 33% (P < 0.05). Q value of synergistic effects was between 1.13 to 1.43. It is concluded that 2ME2 can inhibit proliferation and induce apoptosis of myeloma cell lines and has synergistic effects with dexamethasone, As(2)O(3), thalidomide and zoledronic acid.

【Objective】 To study the location effect of the Mc Ab G9 to human esophageal carcinoma in tumor bearing nude mice. 【Methods】 125　I-G9 was prepared by Chloramine-T method. The distribution of 125 　 I-G9 was detected at different time (24, 48, and 72 h) after peritoneal injecti on. The percentage of the injected dose per gram of tissue(%ID/g) and the ratio of Tumor/non-Tumor were calculated. 【Results】 The distribution of 125　 I -G9 in tumor at the third day was showed by autoradiography obviously higher th an in other organ/tissue (except blood) and the highest is at the 48 h. The T/NT values are 2-7. The autoradiography indicated that radioactivity concentrated in tumor tissue. The concentration in tumor edge is more obvious than in the ce nter. 【Conclusion】 125　I-G9 has a considerable targeting activity and can well locate in esophageal carcinoma tissue in tumor bearing nude mice.

Objective To evaluate the intraocular pressure (IOP)-lowering effects achieved by nonpenetrating glaucoma surgery (NPGS) in patientswith open angle glaucoma. Methods Randomized controlled trials evaluating patientswith primary and secondary open angle glaucoma treated with NPGS were identified and were subjected to systematic review analysis. The main outcome measurements included the percentage of IOP reduction and the complete success rate. The pooled estimates were calculated using the random effect model by comprehensive meta-analysis software version 2. 0. Results Both deep sclerectomy (DS) and viscocanalostomy (VCO) were less effective than trabeculectomy (TE) in lowering IOP, with the of IOP reduction percent at 2 years being 35. 2% for DS, 30. 2% for VCO, and 45. 6% for TE. Intraoperative use of implants and mitomycin C (MMC) improved IOP-lowering effects of DS, with IOP reduction at percent 2 years being 41. 1% and 41. 7%, respectively. The complete success rates at 4 years were 35. 4% for DS and 22. 7% for VCO, lower than that of TE (47. 6%). The complete success rates of DS with implants and MMC were 64. 6% and 52. 1 % at 4 years, respectively, both being higher than that of primary DS. NPGS caused less complications than TE. Conclusion Primary deep sclerectomy and primary viscocanalostomy can significantly lower IOP and have less complication than TE. However, the IOP-lowering effect of NPGS is slighter than primary TE. The efficacy of DS can be improved by intraoperative use of implants and MMC.

Objective To explore the technique of single port laparoscopy assisted by the third generation SI class Da Vinci robot in treatment of gynecological malignant tumors. Methods The clinical data were retrospectively analyzed of 8 patients with robot-assisted single port laparoscopy of gynecological malignant tumor from June to August 2018 in Changzheng Hospital. Of the 8 patients [aged 34–51 (43±6) years and BMI of 20.1–26.3 (23.2±3.1) kg/m2], 4 were early endometrial cancer and another 4 were early cervical cancer. With the help of porous instruments of ordinary robot system and Lagiport single-site multi-channel platform, the third generation SI class Da Vinci robotic surgical system was used to complete the robot assisted transumbilical single port laparoscopic surgery. Four patients with early endometrial cancer received total hysterectomy + double-adnexy resection + sentinel lymph node biopsy, and another 4 with early cervical cancer received radical resection of cervical carcinoma. Results Robot assisted single port laparoscopic surgery was successfully performed in all 8 patients with no additional auxiliary puncture port or laparotomy. The operation time was (208±68) min, and the intraoperative blood loss was (155±71) ml. No injury of ureter, bladder, rectum and obturator nerve and blood transfusion occurred during operation. No postoperative complications occurred such as blood transfusion, persistent fever, wound exudation, pelvic and abdominal infection and deep venous thrombosis. Four patients with endometrial cancer were discharged 4 days after operation, and another 4 with cervical cancer were discharged 7 days after operation. All patients were satisfied with the wound after operation. Conclusion The third generation SI-class Da Vinci robot can complete gynecological malignant tumor surgery with the help of ordinary robot instruments and Lagiport single-site multi-channel platform, which is one of the effective surgical methods for gynecological malignant tumor.

Ankylosing spondylitis (AS) is an immune-mediated chronic inflammatory disease characterized by involvement of sacroiliac and spinal joints. The main pathological features of AS are enthesitis and new bone formationcaused joint fusion, but its pathogenesis is still unclear. Wnt signaling pathway exerts an important effect on the normal bone homeostasis, especially on the formation of osteoblasts, and may contribute to the pathogenesis of AS. Studies have shown that chronic inflammation and genetic regulation participate in the new bone formation of AS through Wnt signaling pathway. This review sums up the effect of Wnt/Β-catenin signaling pathway on bone metabolism of AS in recent years.
