1.Evaluation of Regional Wall Motion in Patients with Suspected Coronary Artery Disease by Real-Time Three-Dimensional Echocardiography and Two-Dimensional Speckle Tracking Imaging

Objective To evaluate the clinical value of the regional wall motion in patients with suspected coronary artery disease by real-time three-dimensional echocardiography (RT-3DE) and two-dimensional speckle tracking imaging (2D-STI). Methods A total of 143 hospitaized patients with suspected coronary artery disease and normal wall motion de?tected by two-dimensional echocardiography (2DE) were enrolled and devided into group A (coronary stenosis ≥75%) 73 cases and group B (coronary stenosis<75%) that include 70 cases according to coronary angiography results. Left ventricu?lar segmental ejection fraction (sEF) was measured by RT-3DE and peak systolic longitudial strain (SL) was measured by 2D-STI. ROC curves of the two measurements were generated and compared. Differences in sEF and SL between group A and group B were respectively analyzed. Results (1)Both sEF of RT-3DE and SL of 2D-STI in group A were significantly low?er than those in group B (P<0.05);(2)sEF was positively correlated with SL(r=0.689,P<0.05);(3)Compared with SL, sEF had larger area under ROC curve in some segments of left ventricle where both parameters are abnormal for the diagnosis of the coronary stenosis ≥75% (0.922 vs 0.874). Conclusion Regional wall motion of left ventricular can be measured by both RT-3DE and 2D-STI which can be both used to estimate the extend of stenosis of coronary artery. And RT-3DE is su?perior over 2D-STI.
2.Serum C-reactive protein levels in subjects with varying glucose tolerances and effect of acarbose intervention

Objective To investigate the difference of serum C-reactive protein (CRP) concentrations in subjects with various glucose tolerances and the change of serum CRP concentrations in cases of impaired glucose tolerance (IGT) treated with acarbose. Methods A total of 154 subjects participated in the study and according to oral 75 g glucose tolerance test (OGTT), among them there were 44 subjects with normal glucose tolerance (NGT), 89 cases with IGT and 21 cases with type 2 diabetes (DM). Sixty IGT subjects were recruited into randomized, double-blind, placebo-controlled study. The IGT subjects received either 150 mg acarbose daily or placebo for 16 weeks. The serum CRP concentration was detected with immunoscattering assay. Results (1) The serumCRPconcentration〔median(25%, 75%)〕 was 4.60(4.10-5.25)mg/L in NGT subjects, 5.25(4.50-6.00)mg/L in IGT subjects, 5.70(5.20-7.50)mg/L in type 2 DM patients (F=157.00, P0.05). Conclusion (1) The serum CRP concentration is gradually increased with the worsening of glucose tolerence; (2) The serum CRP concentration is positively correlated with OGTT 2 h blood glucose, HbA 1C , TC, TG; (3) The serum CRP concentration is decreased in IGT subjects treated with acarbose following the improvement of hyperglycemia.
3.Analyse the relation of the stage and type with the lab diagnosis guideine in the patients with PM and DM

Objective To analyse the lab diagnosis guideline in the patients with PM and DM,and guide the clinical diagnosis. Methods Stage the controlling condition and examine the serologic zymogram,electromyography and pathology.Results In the and Hospital of Hebei medical Vnirersity,from Jan.2003 to Jan.2005 CK,CK-MB,AST,LDH and HBDH was higher in 82.9%patients of aetive stage than the stable stage,especially changed obviously in CK.The occurance ritio(75.9%,48.3%)of the inside electricity and multiphase wave were much higher than the stable stage(46.7%,20.0%).Lots of organic fibre shrank in PM,so did the inflammation of lymohocyte soakage.In DM,the fascicle began to shrink in different degrees.Conclusion CK could become the guideline to watch the activity of illness.The number of the electromyography inside electricity and multiphase wave are related to the illness stage of patients with PM and DM.In PM pathology,muscle fibre had some infected cell soakage,with the differential recessive change.The pathology character in DM was the atrophy around the facicle and the change of tiny blood vessel.
4.Relationship between the expression of thymidylate synthase, thymidine phosphorylase and dihydropyrimidine dehydrogenase and survival in epithelial ovarian cancer.

The mRNA and protein expression of thymidylate synthase (TS), thymidine phosphorylase (TP) and dihydropyrimidine dehydrogenase (DPD) and their relationship with prognosis were investigated. Real-time quantitative RT-PCR (Taqman) was used to detect the mRNA expression of TS, TP and DPD in formalin-fixed and paraffin-embedded 106 samples of epithelial ovarian cancer and 29 normal ovaries. A TATA box-binding protein (TBP) was used as an endogenous reference gene. A relationship between TS, TP, DPD expression and clinicopathologic features was investigated. The protein location and expression of TS, TP and DPD was examined in the same patients by an avidin-biotin-peroxidase immunohistochemistry. TS and TP mRNA expression levels were significantly higher in tumor group than in normal controls, with the average value of TS and TP mRNA being 6.14+/-0.62 and 0.59+/-0.06 in tumor tissue, and 0.71+/-0.14 and 0.16+/-0.04 in normal tissue, respectively. DPD mRNA expression levels were significantly lower in tumor group (0.11+/-0.02) than in normal controls (0.38+/-0.05). There was statistically significant difference in TS and TP mRNA expression levels among different pathological grades and clinical stages (P<0.05), but histological subtype was not significantly associated with TS and TP mRNA expression. DPD gene expression was not significantly associated with any clinicopathological parameters. Immunohistochemistry revealed that TP protein was mainly distributed in nucleus, and TS and DPD mainly in cytoplasm. The protein expression intensity of TS, TP and DPD was coincided with the mRNA expression levels. It was concluded that TS, TP mRNA and protein expression levels were significantly higher in epithelial ovarian cancer, and DPD mRNA and protein expression levels were significantly lower. The expression levels of TS and DPD were related to the patients' prognosis and survival. Combined gene expression levels of TS, TP and DPD represent a new variable to predict the clinical outcome in ovarian cancer. The association of TS, TP and DPD expression levels with survival suggests an importance of these genes for tumor occurrence and progression.
5.Treatment of deformities secondary to gluteal muscles and fascia contracture with Z-plasty

Objective To describe an appreciate operation method in the treatment of deformi-ties secondary to gluteal muscles and fascia contracture. Methods 25 cases of serious secondary de-formity of gluteal muscles and fascia contracture were treated by Z-plasty. The contracted fascia and scar were loosed and the skin around the wound was rearranged. Results The following-up time was from 6 months to 3 years with an average time of 1.6 years. All cases rehabilitated to normal or nearly normal in gait. Aesthetic results were also satisfactory. No recurrence was observed. Conclusion Re-arranging the contracted fascia with Z-plasty can extend the fascia and improve the movement of the hip joint. Besides, Z-plasty treatment of skin around the scar is benefit for the aesthetic result of the deformities, Z-plasty of contracted fascia and skin is an appreciate operation method in the treatment of deformities secondary to gluteal muscles and fascia contracture.
6.The prognostic value of white blood cell and hemoglobin in patients with acute aortic dissection

Objective To evaluate the prognostic value of white blood cell and hemoglobin in patients with acute aortic dissection(AAD).Methods White blood cell.hemoglobin and fibrinogen were measured in 42 AAD patients.The mortality during hospitalization was observed and the short-term prognosis in AAD patients was as-seased.Results There was 13 death during hospitalization.with the mortality of 30.95%.The value of white blood cell was higher in death group than in survival group[(13.73±6.91)×109>/L vs.(9.43±4.97)×109>/L.P< 0.05).The value of hemoglobin was lower in death group of than in survival group[(118.54±22.38)g/L vs. (131.72±18.17) g/L,P<0.05].There were no differences in the value of fibrinogen between the groups [(3.15±1.15)g/L vs.(3.48±1.24)g/L,P>0.05).The mortality in the group of elevated white blood cell(≥10.0×109>/L)was higher than that in the group of normal white blood cell(10.0×109/L)(41.18%vs.24%.P<0.05).The mortality in the group of decreased hemoglobin(≤110g/L)was higher than that in the group of nor-mal hemoglobin(>110 g/L)(50.00%vs.27.78%,P<0.05).Conclusion The value of white blood cell and he-moglobin could help to assess the short-term prognosis of patients with AAD.

Objective: To discuss the relation between dampness-heat syndrome of GBS and neuroendocrine-immunoregulatory network.Methods: 34 patients with GBS were divided into dampness-heat syndrome group and nondampness-heat syndrome group by syndromes differentiation of TCM.The autonomic nerve function,the level of immunoglobulin,cortisol(COR) and adrenocorticotropin(ACTH) of these two groups were detected.The datas were compared with that of normal control group.Results:① There were significant difference in function of autonomic nerve between the two groups(damp-heat syndrome group and nondamp-heat syndrome group) and normal control group.There is difference between dampness-heat syndrome group and nondampness-heat syndrome group,too.Dampness-heat syndrome group mainly showed sympathetic nerve excitement(60.0%).In nondampness-heat syndrome group,the incidence of sympathetic nerve excitement is 35.7%;parasympathetic nerve excitement is 28.6%.② In dampness-heat syndrome group the level of IgG was higher than that in nondampness-heat syndrome group and normal control group.The level of IgA was higher than that in normal control group(P

Inflammation and infection play an important role in the pathogenesis of many cancers. Toll-like receptors (TLRs) are a class of pattern recognition receptors that recognize conserved components of microbes and trigger the immune response against invading microorganisms. Toll-like receptor 9 (TLR9) recognizes non-methylated cytosine-phosphateguanosine (CpG) DNA sequences which are the surrogate for viral DNA. TLR9 may react to tumor development and progression during chronic inflammation that involves the tumor microenvironment. In order to study the role of TLR9 in cervical cancer, we analyzed the TLR9 expression in different types of HPV infection cervical cancer cells. Then we detected if CpG sequences influenced the TLR9 expression and the sensitivity to cisplatin (DDP) of these cervical cancer cells in vitro. The expression of TLR9 mRNA and protein in SiHa, Hela and C33A cells was detected by RT-PCR and Western blotting. Real-time PCR was used to examine the TLR9 expression changes induced by CpG. Chemosensitivity of the cervical cancer cells to cisplatin (DDP) was measured by MTT. It was observed that the expression of TLR9 mRNA and protein was increased gradually in SiHa (HPV16+), Hela (HPV18+) and C33A (HPV-) cells. Low doses of CpG increased the TLR9 expression only in C33A (HPV-) cells, but not in SiHa (HPV16+) and Hela (HPV18+) cells. Furthermore, low dose of CpG significantly increased the sensitivity of C33A (HPV-) cells, but not that of SiHa (HPV16+) and Hela (HPV18+) cells. These results indicated that TLR9 may serve as a protective agent in HPV negative cervical cancer cells. It was concluded that TLR9 could improve the sensitivity to DDP in HPV negative cervical cancer cells and might represent a potential therapeutic option in clinical practice.
10.Craniocerebral injury promotes sciatic nerve regeneration

BACKGROUND:Studies have shown that craniocerebral injury can promote the repair of sciatic nerve injury in rats, but its precise mechanism remains unclear.
OBJECTIVE:To further explore the action mechanism of craniocerebral injury on the repair of sciatic nerve injury using morphology and histology.
METHODS:Sixty specific-pathogen-free healthy male Sprague-Dawley rats were randomly divided into two groups. Rats with craniocerebral injury and sciatic nerve injury were considered as the experimental group. Rats with simple sciatic nerve injury were considered as the control group. Classical Feeney method was used in models of craniocerebral injury and SunderlandV sciatic nerve injury. At 8 and 12 weeks after modeling, sciatic nerve index was detected. Masson staining and NF200 immunofluorescence staining were used to observethe nerve regeneration atthe anstomotic site. Transmission electron microscope was used to observe the number of regenerative axons.
RESULTS AND CONCLUSION:At 8 and 12 weeks after modeling, compared with the control group, gait and sciatic nerve index recovered better in the experimental group. In the experimental group, Masson staining showed fewer nerve membrane colagen fibers, and the axon arranged neatly.NF200 immunohistochemistry showed that in the experimental group, the density of regenerated nerves was high, and nerveswere regularly distributed. Transmission electron microscopy showed that in the experimental group, regenerative axons were regularly arranged, colagen scar was less, and myelin layer arranged regularly. Results suggested that the craniocerebral injury in rats may promote the repair of peripheral nerve injury by reducing scar colagen in nerve endings.
