1.Surgical management of gastric cancer with liver cirrhosis and portal hypertension

Objective To review the experience in the management of gastric cancer with liver cirrhosis and portal hypertension.Method A retrospective analysis was made in 35 gastric cancer cases with liver cirrhosis and portal hypertension admitted into our hospital from January 2000 through June 2009.Result There were no intraoperative death in this group.Postoperative ascites occurred in 19 patients,anastomotic leakage in one case,anastomotic bleeding in 2 cases,wound bleeding in 2 cases,intraabdominal infection in 17 cases (of which combined fungal infection in 4 patients),incision infection in one patient and chylous leakage in one case.The morbidity rate was 71%.Four patients died during hospitalization including multiple organ dysfunction syndrome in 2 cases,with mortality rate of 11%. Conclusion In order to reduce the incidence of mortality and complications,we should complete preoperative evaluation,correct perioperative management,guide surgery with damage control concept,follow individualized principles and emphasize on preventive devascularization.
2.THE LIPOGENIC AND LIPOLYTIC METABOLISM AT DYNAMIC PHASE OF OBESITY IN VENTROMEDIAL HYPOTHALAMUS-LESIONED OBESE RATS

Objective: To investigte the lipogenic and lipolytic metabolism at dynamic phase of obesity in ventromedial hypothalamus-lesioned obese rats. Methods: Female SD rats were divided into two groups, one group received bilateral electrolytic lesions of ventromedial hypothalamus(VMH),and the other one was used as sham control. Samples of blood, livers and subcutaneous, parametric and mesenteric adipose tissues were collected one week after VMH lesions and sham operations. Results: Activities of microsomal triglyceride transfer protein(MTP) in hepatocytes, and phosphatidate phosphohydrolase (PAP), malic enzyme(ME), glucose-6-phosphate dehydrogenase(G6PDH)in liver, parametric and mesenteric adipose tissue in VMH-lesioned rats were increased as compared to their sham counterparts. Activity of hormone sensitive enzyme(HSL) in parametric and mesenteric adipose tissue in VMH-lesioned rats was not changed when compared to sham group. Activity of HSL in subcutaneons adipose tissue was increased, while activity of HSL in gastrocnemius was decreased. Activity of lipoprotein lipase(LPL) in parametric and mesenteric adipose tissue and gastrocnemius were enhanced significantly compared to sham group. Conclusion: In dynamic phase of obesity of VMH-lesioned rats, hepatic production and transportation of triglyceride in these rats were increased significantly ,and lipogenic metabolism and storage of triglyceride in adipose tissues such as parametric and mesenteric adipose tissues were also enhanced.Meanwhile, lipolytic metabolism in subcutaneous adipose tissue and gastrocnemius was also increased.
3.Three-dimensional image reconstruction and anthropometry of nasal cartilages in Chinese

Objective To conduct a morphologic and anatomic study on nasal cartilages (including nasal alar cartilage, nasal septal cartilage and upper lateral cartilage) in Chinese, and to compare measured parameters with the other races. Methods 20 adult embalmed cadavers were dissected, 40 nasal alar cartilages, 20 nasal septal cartilages and 40 upper lateral cartilages were collected and scanned by CT, and three-dimensional images were reconstructed. As Zelnik' and Ofodile's work, the data of the images were collected. Results The distances from the nostril rim to the caudal boarder of the lateral crus in the series were (4. 54 ±0. 59) mm anteriorly, (5. 54±0. 69) mm in the middle, and (10. 45± 1. 15) mm posteriorly. The length of the lateral crus was (18. 51 ±1. 63) mm, the width was (7. 99±1. 66) mm, and the thickness was (0. 49±1. 28) mm. Conclusion The Caucasian nasal cartilages are the biggest in all races, and the other's were approximately same.
4.STUDY ON MECHANISM OF FATTY LIVER IN VENTROMEDIAL HYPOTHALAMUS(VMH)-LESIONED OBESE RATS

Objective: To investigate how fatty liver was developed in ventromedial hypothalamus(VMH)-lesioned obese rats. Methods: Two groups of rats were prepared: (1)VMH-lesioned obese rats, and (2)sham VMH-lesioned rats. One week after VMH lesions, livers of all rats were isolated for morphological observation and for determination of microsomal triglyceride transfer protein(MTP), phosphatidate phyosphohydrolase (PAP), malic enzyme (ME), and glucose-6-phosphate dehydrogenase(G6PDH). Results: Triglyceride contents in livers of VMH-lesoned obese rats increased significantly, and were about 1.8-fold of control group. Activities of ME, G6PDH and PAP in the livers were also enhanced markedly compared to their controls. Many lipid droplets in cytoplasm of hepatocytes from VMH-lesioned obese rats were observed, while there was no similar finding in hepatocytes of control rats. MTP activity in livers of VMH-lesioned obese rats was higher than that in livers of sham-operated non-obese rats [0.201?0.013 vs. 0.175?0.014 ?g/(mg protein?h),[WTBX]P0.05). Conclusion: Hepatic triglyceride production and activity of MTP were increased in VMH-lesioned obese rats, but magnitude of the latter did not exceed the former. This resulted in hepatic triglyceride accumulation in spite of increase in transport of triglyceride out of liver by MTP. This may contribute to the development of fatty liver in VMH-lesioned obese rats.
5.RELATIONSHIP BETWEEN BMI, PERCENTAGE OF BODY FAT AND BODY FAT DISTRIBUTION IN HEALTHY ADULTS

Objective: To investigate the prevalence of overweight and obesity and relation of body mass index (BMI) to body fat percentage and waist circumference in Chinese healthy adults. Methods: 1005 healthy adults were divided into five groups by BMI,which was made by WHO's recent classification of BMI for Asian adults. Anthropometric measurements (waist and hip circumference , skinfold thickness and ratio of waist and hip circumference) and bioelectrical impedance analysis (BIA) were done in subjects. Results: The prevalence of overweight and obesity classⅠin males were significantly higher than in females. The prevalence of obesity classⅡwas lower both in males and in females. Females had higher percent body fat than males measured by BIA and by skinfold thickness measurements. BMI was significantly correlated with waist circumference in all age groups. Waist circumference was more than 80 cm in overweight females, and more than 90 cm in obesity classⅠin males. Conclusion: The age of overweight and obesity in males was younger than in females. The body fat in females was higher than in males. BMI cut off points for obesity diagnosis are over estimated much more than BIA and skinfold thickness measurements for percent body fat in adults. It should be noted that age, sex and physical activity need to be considered besides BMI when obesity is diagnosed.
6.Repair of sheep metatarsus defects by using tissue-engineering technique.

Tissue-engineering bone with porous ,betatricalcium phosphate (3-TCP) ceramic and autologous bone marrow mesenchymal stem cells (MSC) was constructed and the effect of this composite on healing of segmental bone defects was investigated. 10-15 ml bone marrow aspirates were harvested from the iliac crest of sheep, and enriched for MSC by density gradient centrifugation over a Percoll cushion (1. 073 g/ml). After cultured and proliferated, tissue-engineering bones were constructed with these,cellS seeded onto porous f-TCP, and then the constructs were implanted in 8 sheep left metatarsus defect (25 mm in length) as experimental group. Porous ,-TCP only were implanted to bridge same size and position defects in 8 sheep as control group, and 25 mm segmental bone defects of left metatarsus were left empty in 4 sheep as blank group. Sheep were sacrificed on the 6th, 12th, and 24th week postoperatively and the implants samples were examined by radiograph, histology, and biomechanical test. The 4 sheep in blank group were sacrificed on the 24th week postoperatively. The results showed that new bone tissues were observed either radiographic or histologically at the defects of experimental group as early as 6th week postoperatively, but not in control group, and osteoid tissue, woven bone and lamellar bone occurred earlier than in control group in which the bone defects were repaired in "creep substitution" way, because of the new bone formed in direct manner without progression through a cartilaginous intermediate. At the 24th week, radiographs and biomechanical test revealed an almost complete repair of the defect of experimental group, only partly in control group. The bone defects in blank group were non-healing at the 24th week. It was concluded that engineering bones constructed with porous -TCP and autologous MSC were capable of repairing segmental bone defects in sheep metatarsus beyond "creep substitution" way and making it healed earlier. Porous ,-TCP being constituted with autologous MSC may be a good option in healing critical segmental bone defects in clinical practice and provide insight for future clinical repair of segmental defect.

7.Cartilage engineering utilizing a porous bioceramic (beta-TCP)-cell complex.

OBJECTIVETo evaluate the feasibility of growing tissue-engineered cartilage using chondrocytes seeded onto a biodegradable porous bioceramic, the beta-tricalcium phosphate (beta-TCP).
METHODSA porous bioceramic template of beta-TCP was created in the shape of a disc. Chondrocytes isolated from rabbit articular cartilage were seeded on the beta-TCP template and then kept in rotatory cell culture system (RCCS) for 1 week prior to subcutaneous transplantation into athymic mice. The three-dimensional structure was well-maintained 16 weeks after implantation. After 4, 8, 16 weeks, the specimens were harvested and examined macroscopically, histologically and immunohistochemically.
RESULTSGross morphological and histological analysis of the specimens from the chondrocyte-beta-TCP complex demonstrated new cartilage construction. The overall configuration of the experimental specimens closely resembled the structure of beta-TCP template.
CONCLUSIONThese findings suggest that porous bioceramic (beta-TCP) is a good "matrix" for chondrocyte, and can be used for cartilage engineering.

8.Development of single base extension-tags microarray for the detection of food-borne pathogens.

We developed single base extension-tags (SBE-tags) microarray to detect eight common food-borne pathogens, including Staphylococcus aureus, Vibrio parahaemolyticus, Listeria monocytogenes, Salmonella, Enterobacter sakazaki, Shigella, Escherichia coli O157:H7 and Campylobacter jejuni. With specific PCR primers identified and integrated for eight food-borne pathogens, target sequences were amplified and purified as template DNA of single base extension-tags reaction. The products were hybridized to microarrays and scanned for fluorescence intensity. The experiment showed a specific and simultaneous detection of eight food-borne pathogens. The system limits is 0.1 pg for a genomic DNA and 5x10(2) CFU/mL for Salmonella typhimurium cultures. The single base extension-tags assay can be used to detect food-borne pathogens rapidly and accurately with a high sensitivity, and provide an efficient way for diagnosis and control of disease caused by food-borne pathogens.
