1.Changes and clinical significance of myocardial injury markers in preterm infants with patent ductus arteriosus

Objective To investigate the effect of patent ductus arteriosus(PDA)on myocardial injury of premature infants.Methods From May 1,2010 to January 31,2011,110 preterm infants with gestational age from 28 to 36 weeks accepted echocardiography examination,and their blood samples were collected to determine cardiac troponin T(cTnT)and creatine kinase MB isoenzyme (CK-MB)levels 72 h and 3 d after deliveries.All subjects were divided into two groups according to the echocardiogram results:PDA group(n=44)and control group(n=66).The infants with PDA were treated with ibuprofen,and then echocardiography was taken again.cTnT and CK-MB were re-measured in both groups.Chi-square test,t-test,multi-variate linear regression and Spearman rank correlation test were perfomed for statistical analysis.Results Before treatment,cTnT[(0.259±0.134)μg/L]and CK-MB[(7.31± 2.69)μg/L]level of PDA group were significantly higher than those[(0.083±0.054)μg/L and(5.71±1.88)μg/L]of control group(t=9.557 and 2.588,P<0.01 and <0.05,respectively).For 34 infants with successful treatment,cTnT and CK-MB levels decreased markedly to(0.062 ± 0.039)μg/L and(5.34 ± 1.50)μg/L,respectively(t =9.268 and 5.974,all P<0.05),compared with those levels before treatment.For the ten infants failed to close ductus,the cTnT and CK-MB levels[(0.193±0.049)μg/L and(6.93±1.63)μg/L,respectively],were lower than those before treatment(t=1.525 and 0.766,all P>0.05),while higher than those of the control group(t=9.068 and 4.055,P<0.05).Level of cTnT positively related to the duration of ventilation,PDA and respiratory distress syndrome,while did not relate to gender,gestational age and birth weight.CK-MB level was associated to gender,gestational age,birth weight,duration of ventilatory support and PDA.In PDA group,the cTnT level was positively related to the diameter of the ductus,but not related to any indicators in echocardiography.Conclusions Symptomatic PDA could cause myocardial injury in preterm infants.The changes of blood cTnT and CK-MB were consistent with the severity of PDA.Serial measurements of blood cTnT and CK-MB might help to make early diagnosis and treatment for premature infants with PDA and myocardial injury.
2.A prospective cohort study on blood pressure control and risk of ischemic stroke in patients with hypertension

Objective:To explore the association between blood pressure control and risk of ischemic stroke (IS) in patients with hypertension.Methods:A total of 5 488 patients with hypertension from 60 communities were randomly selected from 101 communities in 8 streets of Nanshan District in Shenzhen City by using two-stage sampling method. The social demographic characteristics, behavior and life style, coronary heart disease and diabetes were collected and the physical condition, blood pressure and blood biochemical indexes were measured. From April 1, 2010 to August 31, 2017 as the follow-up period, the incidence of IS was annually collected by using telephone survey. Cox proportional hazard regression model was used to analyze the relationship between blood pressure control, systolic blood pressure (SBP), diastolic blood pressure (DBP) and the risk of IS.Results:The age of all patients was (58.50±12.14) years old, including 2 712 males (49.42%) and 3 112 patients with well-controlled blood pressure (56.71%). During the follow-up period, 358 new cases of IS were confirmed, and the incidence density was 1 346.27/100 000 person-years. Cox proportional hazard regression model analysis showed after adjusting for confounding factors, unstable blood pressure control, SBP≥150 mmHg (1 mmHg=0.133 kPa; compared with SBP<120 mmHg), and DBP≥95 mmHg (compared with DBP<80 mmHg) were associated with risk of IS. The HR (95% CI) was 1.29 (1.04, 1.59), 2.00 (1.26, 3.17) and 1.52 (1.01, 2.64), respectively. Subgroup analyses showed these associations only existed in female patients with hypertension. The HR (95% CI) was 1.39 (1.05, 1.85), 2.53 (1.41, 4.56) and 1.73 (1.00, 3.36), respectively. Conclusion:Unstable blood pressure control increases the risk of IS in female patients with hypertension.
3.Construction of genomic instability⁃associated LncRNA models to predict prognosis and cisplatin sensitivity in colon cancer patients

Objective :
To explore whether the long⁃chain noncoding RNA associated with genomic instability in colon cancer can predict clinical prognosis and therapeutic.
Methods :
The R package " limma" was used for differential analysis , and the prognostic risk model was constructed by univariate Cox analysis and multivariate Cox proportional risk regression analysis. The difference in prognosis was evaluated by Kaplan⁃Meier method , and the difference was significant by log⁃rank test. The efficiency of the prognostic model was evaluated using a time⁃dependent area under the subject operating characteristic curve (AUC) . The R package“ pRRophetic ”was used to predict the sensitivity of patients to anticancer drugs. R software package rms was used to build a line graph , and the consistency index of the line graph was calculated. Real⁃time quantitative PCR was used to detect the difference in the expression levels of prognostic protective factors.
Results :
A total of 22 LncRNAs associated with genomic instabili⁃y in patients with prognosis were obtained , 2 were protective factors for prognosis in patients with colon cancer, and 20 were risk factors for prognosis. A prognosis model composed of LncRNAs associated with genomic instability was constructed , and patients with high risk scores had lower AUCs and shorter median survival. The five⁃year survival AUC predicted by the model was 0. 823 in the training set , 0. 722 in the validation set , and 0. 759 in the overall TCGA colon cancer patient population. Patients with low risk scores had lower half inhibitory concentration (IC50 ) of cisplatin and higher sensitivity (P < 0. 000 1) . The expression of prognostic protective factors in colon cancer tissues was significantly lower than that in adjacent colon cancer tissues.
Conclusion
A prognostic risk model composed of 8 LncRNAs associated with genomic instability was constructed and verified. In addition , the model can also predict cisplatin drug sensitivity. A histogram was constructed combining the tumor stage and the prognosis model. The predictive ability of this graph for five⁃year survival of colon cancer patients is better than that of traditional histopathological features and prognostic models constructed by predecessors.

5.Early experience with mechanical hemodynamic support for catheter ablation of malignant ventricular tachycardia

Objective:To explore the role of mechanical hemodynamic support (MHS) in mapping and catheter ablation of patients with hemodynamically unstable ventricular tachycardia (VT), report single-center experience in a cohort of consecutive patients receiving VT ablation during MHS therapy, and provide evidence-based medical evidence for clinical practice.Methods:This was a retrospective cohort study. Patients with hemodynamically unstable VT who underwent catheter ablation with MHS at Beijing Anzhen Hospital, Capital Medical University between August 2021 and December 2023 were included. Patients were divided into rescue group and preventive group according to the purpose of treatment. Their demographic data, periprocedural details, and clinical outcomes were collected and analyzed.Results:A total of 15 patients with hemodynamically unstable VT were included (8 patients in the rescue group and 7 patients in the preventive group). The acute procedure was successful in all patients. One patient in the rescue group had surgical left ventricular assist device (LVAD) implantation, remaining 14 patients received extracorporeal membrane oxygenation (ECMO) for circulation support. ECMO decannulation was performed in 12 patients due to clinical and hemodynamic stability, of which 6 patients were decannulation immediately after surgery and the remaining patients were decannulation at 2.0 (2.5) d after surgery. Two patients in the rescue group died during the index admission due to refractory heart failure and cerebral hemorrhage. During a median follow-up of 30 d (1 d to 12 months), one patient with LVAD had one episode of ventricular fibrillation at 6 months after discharge, and no further episodes of ventricular fibrillation and/or VT occurred after treatment with antiarrhythmic drugs. No malignant ventricular arrhythmia occurred in the remaining 12 patients who were followed up.Conclusions:MHS contributes to the successful completion of mapping and catheter ablation in patients with hemodynamically unstable VT, providing desirable hemodynamic status for emergency and elective conditions.
6.Effect of RORγ gene on proliferation and migration of colon cancer cells

Objective:
To investigate the effects of retinoid-related orphan receptor γ (RORγ) gene on proliferation and metastasis of human colon cancer cells.
Methods:
RORγ knockdown cell lines were constructed and the knockdown efficiency was detected by RT-qPCR and Western blot assays ; MTT,colony formation,Transwell and wound healing assays were used to detect cell proliferation and metastasis ; the expression of epithelial-mesenchymal transition (EMT) related proteins was detected by Western blot.The relationship between RORγ gene expression and immune cell infiltration in tumor microenvironment was analyzed using TIMER 2. 0 database.
Results :
The knockdown of RORγ enhanced the viability (F = 157. 40,P<0. 01) ,clonogenesis (F = 61. 35,P<0. 01) ,migration (F = 13. 00,P<0. 01) ,invasion (F = 21. 26,P<0. 01) and wound healing ability (F = 877. 2,P<0. 01) of colon cancer cells,inhibited the expression of E-Cadherin,and promoted the expression of vimentin and N-Cadherin.TIMER 2. 0 database analysis showed that RORγ expression in colon adenocarcinoma ( COAD) tissues was associated with multiple immune cell infiltrates.
Conclusion
Downregulation of RORγ expression promoted the proliferation and metastasis of colon cancer cells.
