Primary central nervous system lymphoma (PCNSL) is an extranodal Non-Hodgkin's lymphoma that is confined to the brain, eyes, and/or leptomeninges without evidence of a systemic primary tumor. Although the tumor can affect all age groups, it is rare in childhood; thus, its incidence and prognosis in children have not been well defined and the best treatment strategy remains unclear. A nine-year old presented at our department with complaints of diplopia, dizziness, dysarthria, and right side hemiparesis. Magnetic resonance image suggested a diffuse brain stem glioma with infiltration into the right cerebellar peduncle. The patient was surgically treated by craniotomy and frameless stereotactic-guided biopsy, and unexpectedly, the histopathology of the mass was consistent with diffuse large B cell lymphoma, and immunohistochemical staining revealed positivity for CD20 and CD79a. Accordingly, we performed a staging work-up for systemic lymphoma, but no evidence of lymphoma elsewhere in the body was obtained. In addition, she had a negative serologic finding for human immunodeficient virus, which confirmed the histopathological diagnosis of PCNSL. She was treated by radiosurgery at 12 Gy and subsequent adjuvant combination chemotherapy based on high dose methotrexate. Unfortunately, 10 months after the tissue-based diagnosis, she succumbed due to an acute hydrocephalic crisis.

Pharyngitis is a prevalent disease of the upper respiratory tract that requires treatment with an antibiotic. Group A streptococci (GAS) are the most frequent etiologic agents of bacterial pharyngitis. Because GAS are susceptible to penicillin, routine antibiotic susceptibility testing is not needed. Generally, patients with bacterial pharyngitis have high fever, cervical lymphadenopathy and tenderness, and tonsillar exudative discharge without symptoms of the common cold (e.g., cough, rhinorrhea, and sneezing). However, differentiating bacterial pharyngitis from viral pharyngitis based only on their clinical manifestations is problematic. Therefore, a bacterial culture or a rapid antigen detection test (RADT) is required for the diagnosis of bacterial pharyngitis. Although bacterial culture is the gold standard for diagnosis of bacterial pharyngitis, its accuracy is affected by the technical expertise of the technician, and there is a delay of 1–2 days before the results become available. In contrast, the sensitivity of RADT has increased to over 90%, making them suitable for screening purposes. The result of a RADT is available within 5–10 minutes, obviating the need for a second visit to obtain the results of culture. Use of a RADT would enable the optimal antibiotic to be administered earlier, reducing the overuse of antibiotics.

PURPOSE: The purpose of this study was to examine the moderating mediation effect of self-esteem on the relations among adolescents' abuse experiences, depression and anxiety, and suicidal ideation. METHODS: The participants were selected using secondary data from a population in the 2012 Korea Welfare Panel Survey (KOWEPS). Data were analyzed using SPSS 15.0 and SPSS Macro, and bootstrapping and hierarchical regression analysis were performed to analyze multilevel models. RESULTS: First, analysis of the mediating effect of the adolescents' abuse showed that there was significant mediating influence between suicidal ideation and depression and anxiety. Second, hierarchical regression analysis showed that self-esteem had significant mediation effect on depression and anxiety in adolescents' suicidal ideation. Third, SPSS Macro showed that self-esteem also significantly moderated the mediating effect of adolescents' abuse experiences on suicidal ideation through depression and anxiety. CONCLUSION: The study results suggest that in future research on adolescent's abuse experience, the risk of suicide in depression and anxiety scores should be selected through evaluation of each individual's self-esteem scale. Coping strategies with immediate early intervention should be suggested.

PURPOSE: This survey aimed to identify cough etiquette knowledge and practice level among university students. METHODS: 190 study subject were recruited from a university located in C city of Korea and data were collected utilizing self-reported questionnaires to evaluate cough etiquette knowledge and practice levels. RESULTS: The score of cough etiquette knowledge was 7.38 out of 12 and the correct answer rate was 61.5%. The respondents showed the highest right answer rate (85.3%) on 'cover with a paper tissue or a handkerchief while coughing' and lowest (20.7%) on 'cover with hand while coughing'. Practice level score was 27.28 out of 48. Among those items of practice, 'wearing a mask while coughing' was the lowest (1.40/48) level of practice. And the correlation between knowledge and practice was not significant. CONCLUSION: Cough etiquette knowledge and practice level was revealed to be rather low among university students. Education and social marketing are needed to be developed to encourage the practice level of cough etiquette followed by better droplet infection control and health promotion.

This study was carried out on 178 bus drivers in the Masan area to investigate their health and nutritional status. THe nutritional status of the subjects was surveyed using a self-administered questionnaire, and blood analyses were conducted through annual health examinations in June, 1999. The results were as follows: About 51% were classified as normal, 33% as health-concerned, and 17% as disease-suspected. The levels of total serum cholesterol were significantly higher in the over 20 year group (194.64 +/- 41.42 mg/dl) than in the under 10 year group (175.00 +/- 31.33 mg/dl) with respect to the length of their driving careers. Systolic blood pressure (SBP) was significantly higher in the 51 to 60 age group than in the other groups (p < 0.05). The total serum cholesterol increased with increasing age (p < 0.001), and SBP increased with increasing Body Mass Index (BMI)(p < 0.001). Diastolic blood pressure (DBP), total cholesterol (p < 0.01), hemoglobin (p < 0.05)and glutamate-pyruvate-aminotransferase (GPT)(p < 0.05) were significantly higher in the BMI of over 20, than in the BMI under 20. The GPT was significantly higher in the BMI over 25 than in the BMI under 25. In The mean daily energy intake was 1912.7 kcal (76.5% of the RDA) and the protein intake was 74.3 g (106.2% of the RDA). Compared with the recommended allowance, the energy, calcium, thiamin, and riboflavin intakes were below, and the Fe, vitamin A, vitamin C intakes were above the Korean RDA. The average carbohydrate, protein, fat ratio of energy intake was 66 : 15 : 19, respectively. The length of their driving careers had a positive correlation with their ages and total serum cholesterol levels, and their ages had a positive correlation with their blood pressures, total serum cholesterol and hemoglobin levels and BMI. Their BMI had a positive correlation with their ages, blood pressures, total serum cholesterol, hemoglobin, GPT and GOT levels. The results of this study indicate that bus drivers needed regular exercise, and control of their body weights to prevent chronic diseases, and that their health status should be monitored as the length of their driving careers and ages increase.

PURPOSE: The purpose of the study was to identify the prevalence, risk factors and clinical meanings of geriatric foot problems and to suggest implications for the future. METHODS: One hundred eighty nine elderly aged 60 and over from institution as well as community were investigated for their foot conditions by means of a questionnaire including general characteristics, self care capacity, risk factors, foot problem checklist, X-ray, podoscopy and foot scan. Descriptive statistics and chi-square-test was performed as appropriate utilizing SPSS version 14. Less than .05 of p-value was adopted as statistical significance level. RESULTS: All subjects had at least one kind of foot problem and the most prevalent ones were nail problems, foot deformities in order. Prevalence of foot pain and edema was relatively low. CONCLUSION: Foot problem in elderly is prevalent and geriatric foot is expected to emerge as one of the most important problems in the geriatric field. Therefore strategies to deal with geriatric foot should be developed and practiced for better quality of life in later life.

This study investigated the correlation between health-related habits and the biochemical characteristics of the blood of Korean bus drivers. A total of 178 bus drivers working in the Masan area participated in this study. The general characteristics and health-related habits of the subjects were surveyed using a serf-administered questionnaire. The biochemical date were collected from regular health check-ups done through their workshop health insurance company in lune, 1999. The results were as follows : The average age of the subjects was 38 years, and 44.2% had a driving career of between 10 and 19 years. About 90% of the subjects ate irregularly, and 36% ate. health floods. Their rates of drinking, smoking and exercise were 74.6%, 83.6% and 70.8%, respectively. Their frequency of drinking was significantly increased as the length of their driving career increased (p<0.001). Their levels of total serum cholesterol, in relation to the length of their driving career was significantly higher in the over 20 year group than in the below 10 year group (p<0.05). Their systolic blood pressures, according to age, were significantly higher in the 51 to 60 age group than in the other groups (p<0.05). Their total um cholesterol levels signiscantly increased with increasing age (p<0.001). Their diastolic blood pressures (p<0.001) and blood glucose levels (p<0.05) were significantly higher among the subjects who drinking alcohol almost everyday, than among other subjects. Their systolic blood pressures who significantly increased with increasing frequency of their drinking (p<0.001). The systolic blood pressure and diastolic blood pressure were positively related to age, the amount of drinking, the frequency of drinking and the obesity index, and negatively related to exercise and the duration of exercise. The hemoglobin was positively related to age, preference for a meat diet and the obesity index. The blood glucose was positively related to the amount of drinking, and the total serum cholesterol was positively related to age, length of driving career, period of smoking and the obesity index. The results of this study indicated that bus drivers need regular exercise, moderation of their drinking and smoking, and control of their body weights so as to prevent chronic diseases.

BACKGROUND/AIMS: Metabolic syndrome is associated with an increased risk of colorectal cancer. Non-alcoholic fatty liver disease (NAFLD) is regarded as a hepatic manifestation of metabolic syndrome. Increased echogenicity suggesting NAFLD is a frequent incidental finding on ultrasound examination. This study examined whether NAFLD is related to colorectal neoplasia. METHODS: We reviewed 1,938 consecutive individuals who underwent screening colonoscopy at Changwon Fatima Hospital between Jan 2009 and Sept 2011. The individuals were divided into adenomatous polyp (Group A; n = 494) and control (Group B; n = 1,444) groups. NAFLD was diagnosed by increased echogenicity on abdominal ultrasound. RESULTS: The prevalence of NAFLD was 171 (34.6%) in group A and 336 (23.3%) in group B. Compared with normal subjects, group A subjects were more likely to be men, older, and have a higher body mass index (BMI), blood pressure, waist circumference, fasting glucose, uric acid, triglyceride, HbA1c, Hb and gammaGT levels, and a higher prevalence of NAFLD, metabolic syndrome, diabetes mellitus, and hypertension. In a multiple logistic regression analysis, older age (> or = 50 years) (OR 2.051; 95% CI 1.647-2.553), male sex (OR 2.419; 95% CI 1.837-3.184), and prevalence of NAFLD (OR 1.289; 95% CI 1.004-1.655) were associated with an increased risk of adenomatous polyps. CONCLUSIONS: NAFLD is associated with a high prevalence of colorectal neoplasia. A fatty liver on abdominal ultrasound might predict the development of colorectal adenomatous polyps and cancer.
